In the context of dementia care, Artificial Intelligence (AI) powered clinical decision support systems have the potential to enhance diagnosis and management. However, the scope and challenges of applying these technologies remain unclear. This scoping review aims to investigate the current state of AI applications in the development of intelligent decision support systems for dementia care. We conducted a comprehensive scoping review of empirical studies that utilised AI-powered clinical decision support systems in dementia care. The results indicate that AI applications in dementia care primarily focus on diagnosis, with limited attention to other aspects outlined in the World Health Organization (WHO) Global Action Plan on the Public Health Response to Dementia 2017–2025 (GAPD). A trifecta of challenges, encompassing data availability, cost considerations, and AI algorithm performance, emerges as noteworthy barriers in adoption of AI applications in dementia care. To address these challenges and enhance AI reliability, we propose a novel approach: a digital twin-based patient journey model. Future research should address identified gaps in GAPD action areas, navigate data-related obstacles, and explore the implementation of digital twins. Additionally, it is imperative to emphasize that addressing trust and combating the stigma associated with AI in healthcare should be a central focus of future research directions. • This paper examines the trajectory of AI-based CDSS application in dementia care. • It focuses on GAPD's seven action areas, highlighting AI's role in improving diagnosis and care. • It identifies limitations in data volume and cost as barriers to their development and usage in practice. • A model utilising digital twin technology is presented as a potential approach to addressing these identified limitations. Medical use cases for machine learning (ML) are growing exponentially. The first hospitals are already using ML systems as decision support systems in their daily routine. At the same time, most ML systems are still opaque and it is not clear how these systems arrive at their predictions. In this paper, we provide a brief overview of the taxonomy of explainability methods and review popular methods. In addition, we conduct a systematic literature search on PubMed to investigate which explainable artificial intelligence (XAI) methods are used in 450 specific medical supervised ML use cases, how the use of XAI methods has emerged recently, and how the precision of describing ML pipelines has evolved over the past 20 years. A large fraction of publications with ML use cases do not use XAI methods at all to explain ML predictions. However, when XAI methods are used, open-source and model-agnostic explanation methods are more commonly used, with SHapley Additive exPlanations (SHAP) and Gradient Class Activation Mapping (Grad-CAM) for tabular and image data leading the way. ML pipelines have been described in increasing detail and uniformity in recent years. However, the willingness to share data and code has stagnated at about one-quarter. XAI methods are mainly used when their application requires little effort. The homogenization of reports in ML use cases facilitates the comparability of work and should be advanced in the coming years. Experts who can mediate between the worlds of informatics and medicine will become more and more in demand when using ML systems due to the high complexity of the domain. Hospital-acquired pressure injuries (HAPIs) constitute a significant challenge harming thousands of people worldwide yearly. While various tools and methods are used to identify pressure injuries, artificial intelligence (AI) and decision support systems (DSS) can help to reduce HAPIs risks by proactively identifying patients at risk and preventing them before harming patients. This paper comprehensively reviews AI and DSS applications for HAPIs prediction using Electronic Health Records (EHR), including a systematic literature review and bibliometric analysis. A systematic literature review was conducted through PRISMA and bibliometric analysis. In February 2023, the search was performed using four electronic databases: SCOPIS, PubMed, EBSCO, and PMCID. Articles on using AI and DSS in the management of PIs were included. The search approach yielded 319 articles, 39 of which have been included and classified into 27 AI-related and 12 DSS-related categories. The years of publication varied from 2006 to 2023, with 40% of the studies taking place in the US. Most studies focused on using AI algorithms or DSS for HAPIs prediction in inpatient units using various types of data such as electronic health records, PI assessment scales, and expert knowledge-based and environmental data to identify the risk factors associated with HAPIs development. There is insufficient evidence in the existing literature concerning the real impact of AI or DSS on making decisions for HAPIs treatment or prevention. Most studies reviewed are solely hypothetical and retrospective prediction models, with no actual application in healthcare settings. Context: Several different computer-assisted management systems of computer interpretable guidelines (CIGs) have been developed by the Artificial Intelligence in Medicine community. Each CIG system is characterized by a specific formalism to represent CIGs, and usually provides a manager to acquire, consult and execute them. Though there are several commonalities between most formalisms in the literature, each formalism has its own peculiarities.Objective: The goal of our work is to provide a flexible support to the extension or definition of CIGs formalisms, and of their acquisition and execution engines. Instead of defining "yet another CIG formalism and its manager", we propose META-GLARE (META Guideline Acquisition, Representation, and Execution), a "meta"-system to define new CIG systems.Method and Materials: In this paper, META-GLARE, a meta-system to define new CIG systems, is presented. We try to capture the commonalities among current CIG approaches, by providing (i) a general manager for the acquisition, consultation and execution of hierarchical graphs (representing the control flow of actions in CIGs), parameterized over the types of nodes and of arcs constituting it, and (ii) a library of different elementary components of guidelines nodes (actions) and arcs, in which each type definition involves the specification of how objects of this type can be acquired, consulted and executed. We provide generality and flexibility, by allowing free aggregations of such elementary components to define new primitive node and arc types.Results: We have drawn several experiments, in which we have used META-GLARE to build a CIG system (Experiment 1 in Section 8), or to extend it (Experiments 2 and 3). Such experiments show that META-GLARE provides a useful and easy-to-use support to such tasks. For instance, re-building the Guideline Acquisition, Representation, and Execution (GLARE) system using META-GLARE required less than one day (Experiment 1).Conclusions: META-GLARE is a meta-system for CIGs supporting fast prototyping. Since META-GLARE provides acquisition and execution engines that are parametric over the specific CIG formalism, it supports easy update and construction of CIG systems. [ABSTRACT FROM AUTHOR]

Motivation: Emergency Departments' (ED) modern triage systems implemented worldwide are solely based upon medical knowledge and experience. This is a limitation of these systems, since there might be hidden patterns that can be explored in big volumes of clinical historical data. Intelligent techniques can be applied to these data to develop clinical decision support systems (CDSS) thereby providing the health professionals with objective criteria. Therefore, it is of foremost importance to identify what has been hampering the application of such systems for ED triage.Objectives: The objective of this paper is to assess how intelligent CDSS for triage have been contributing to the improvement of quality of care in the ED as well as to identify the challenges they have been facing regarding implementation.Methods: We applied a standard scoping review method with the manual search of 6 digital libraries, namely: ScienceDirect, IEEE Xplore, Google Scholar, Springer, MedlinePlus and Web of Knowledge. Search queries were created and customized for each digital library in order to acquire the information. The core search consisted of searching in the papers' title, abstract and key words for the topics "triage", "emergency department"/"emergency room" and concepts within the field of intelligent systems.Results: From the review search, we found that logistic regression was the most frequently used technique for model design and the area under the receiver operating curve (AUC) the most frequently used performance measure. Beside triage priority, the most frequently used variables for modelling were patients' age, gender, vital signs and chief complaints. The main contributions of the selected papers consisted in the improvement of a patient's prioritization, prediction of need for critical care, hospital or Intensive Care Unit (ICU) admission, ED Length of Stay (LOS) and mortality from information available at the triage.Conclusions: In the papers where CDSS were validated in the ED, the authors found that there was an improvement in the health professionals' decision-making thereby leading to better clinical management and patients' outcomes. However, we found that more than half of the studies lacked this implementation phase. We concluded that for these studies, it is necessary to validate the CDSS and to define key performance measures in order to demonstrate the extent to which incorporation of CDSS at triage can actually improve care. Introduction: Thanks to improvement of care, cancer has become a chronic condition. But due to the toxicity of treatment, the importance of supporting the quality of life (QoL) of cancer patients increases. Monitoring and managing QoL relies on data collected by the patient in his/her home environment, its integration, and its analysis, which supports personalization of cancer management recommendations. We review the state-of-the-art of computerized systems that employ AI and Data Science methods to monitor the health status and provide support to cancer patients managed at home.Objective: Our main objective is to analyze the literature to identify open research challenges that a novel decision support system for cancer patients and clinicians will need to address, point to potential solutions, and provide a list of established best-practices to adopt.Methods: We designed a review study, in compliance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, analyzing studies retrieved from PubMed related to monitoring cancer patients in their home environments via sensors and self-reporting: what data is collected, what are the techniques used to collect data, semantically integrate it, infer the patient's state from it and deliver coaching/behavior change interventions.Results: Starting from an initial corpus of 819 unique articles, a total of 180 papers were considered in the full-text analysis and 109 were finally included in the review. Our findings are organized and presented in four main sub-topics consisting of data collection, data integration, predictive modeling and patient coaching.Conclusion: Development of modern decision support systems for cancer needs to utilize best practices like the use of validated electronic questionnaires for quality-of-life assessment, adoption of appropriate information modeling standards supplemented by terminologies/ontologies, adherence to FAIR data principles, external validation, stratification of patients in subgroups for better predictive modeling, and adoption of formal behavior change theories. Open research challenges include supporting emotional and social dimensions of well-being, including PROs in predictive modeling, and providing better customization of behavioral interventions for the specific population of cancer patients.
