Abstract: Objective: To assess whether a user-centred prototype clinical decision support system (CDSS) providing patient-specific advice better supports healthcare practitioners in terms of (a) types of usability problems detected and (b) effective and efficient retrieval of childhood cancer survivor's follow-up screening procedures compared to an expert-driven paper-based guideline. Methods and materials: A user-centred design (UCD) process was employed to design a prototype CDSS. Usability problems in information retrieval with the paper-based guideline were assessed by think-aloud analysis with 13 participants. Both simple and more complex tasks were applied. The analysis provided input for the UCD process of the prototype. The usability of the prototype CDSS was subsequently evaluated by think-aloud analysis with the same participants. Usability problems of the paper-based guideline and the prototype CDSS were compared by using the classification of usability problems scheme. In addition, efficiency (time to complete task) and effectiveness (completeness of retrieved screening procedures) of information retrieval of participants in the expert-driven paper-based guideline and the user-centred prototype CDSS were compared. Results: Usability problems in both the paper-based guideline and the CDSS prototype were mainly classified as ‘incongruent with participants’ mental model’. The prototype CDSS reduced this type of problem from 17 to 6 problems. The time to perform simple information retrieval tasks increased by 58s when using the prototype CDSS, however, it resulted in a 58% improvement in task completeness compared to the paper-based guideline. The time to perform complex scenarios decreased by 3:50min with the prototype CDSS, with 17% higher completeness compared to the paper-based guideline. Conclusion: Analysis showed that usability problems experienced by healthcare practitioners when using a paper-based guideline could be overcome by implementing the guideline in a user-centred CDSS design. Although different types of usability problems were experienced with the prototype CDSS, they did not inhibit effective and efficient performance of tasks in the system. The usability problem analysis of the paper-based guideline effectively supported comparison of usability problems found in the two information retrieval systems and it supported the UCD of the CDSS. We trust it is clear that JBI addresses a broader range of methods than AI, whereas the other two journals are singularly focused on AI and data science. With the growth in Artificial Intelligence in Medicine (AIM) research and the plethora of informatics journals, there is some confusion where to direct an AIM-related manuscript for peer review and possible publication. Objective: In this paper, we aim to evaluate the use of electronic technologies in out of hours (OoH) task-management for assisting the design of effective support systems in health care; targeting local facilities, wards or specific working groups. In addition, we seek to draw and validate conclusions with relevance to a frequently revised service, subject to increasing pressures.Methods and Material: We have analysed 4 years of digitised demand-data extracted from a recently deployed electronic task-management system, within the Hospital at Night setting in two jointly coordinated hospitals in the United Kingdom. The methodology employed relies on Bayesian inference methods and parameter-driven state-space models for multivariate series of count data.Results: Main results support claims relating to (i) the importance of data-driven staffing alternatives and (ii) demand forecasts serving as a basis to intelligent scheduling within working groups. We have displayed a split in workload patterns across groups of medical and surgical specialities, and sustained assertions regarding staff behaviour and work-need changes according to shifts or days of the week. Also, we have provided evidence regarding the relevance of day-to-day planning and prioritisation.Conclusions: The work exhibits potential contributions of electronic tasking alternatives for the purpose of data-driven support systems design; for scheduling, prioritisation and management of care delivery. Electronic tasking technologies provide means to design intelligent systems specific to a ward, speciality or task-type; hence, the paper emphasizes the importance of replacing traditional pager-based approaches to management for modern alternatives. Objective: The analysis of surgical motion has received a growing interest with the development of devices allowing their automatic capture. In this context, the use of advanced surgical training systems makes an automated assessment of surgical trainee possible. Automatic and quantitative evaluation of surgical skills is a very important step in improving surgical patient care.Material and Method: In this paper, we present an approach for the discovery and ranking of discriminative and interpretable patterns of surgical practice from recordings of surgical motions. A pattern is defined as a series of actions or events in the kinematic data that together are distinctive of a specific gesture or skill level. Our approach is based on the decomposition of continuous kinematic data into a set of overlapping gestures represented by strings (bag of words) for which we compute comparative numerical statistic (tf-idf) enabling the discriminative gesture discovery via its relative occurrence frequency.Results: We carried out experiments on three surgical motion datasets. The results show that the patterns identified by the proposed method can be used to accurately classify individual gestures, skill levels and surgical interfaces. We also present how the patterns provide a detailed feedback on the trainee skill assessment.Conclusions: The proposed approach is an interesting addition to existing learning tools for surgery as it provides a way to obtain a feedback on which parts of an exercise have been used to classify the attempt as correct or incorrect. Clinical trials are typically conducted over a population within a defined time period in order to illuminate certain characteristics of a health issue or disease process. Cross-sectional studies provide a snapshot of these disease processes over a large number of people but do not allow us to model the temporal nature of disease, which is essential for modelling detailed prognostic predictions. Longitudinal studies, on the other hand, are used to explore how these processes develop over time in a number of people but can be expensive and time-consuming, and many studies only cover a relatively small window within the disease process. This paper explores the application of intelligent data analysis techniques for building reliable models of disease progression from both cross-sectional and longitudinal studies. The aim is to learn disease 'trajectories' from cross-sectional data by building realistic trajectories from healthy patients to those with advanced disease. We focus on exploring whether we can 'calibrate' models learnt from these trajectories with real longitudinal data using Baum-Welch re-estimation so that the dynamic parameters reflect the true underlying processes more closely. We use Kullback-Leibler distance and Wilcoxon rank metrics to assess how calibration improves the models to better reflect the underlying dynamics. [ABSTRACT FROM AUTHOR]

Accounting for patients with multiple health conditions is a complex task that requires analysing potential interactions among recommendations meant to address each condition. Although some approaches have been proposed to address this issue, important features still require more investigation, such as (re)usability and scalability. To this end, this paper presents an approach that relies on reusable rules for detecting interactions among recommendations coming from various guidelines. It extends a previously proposed knowledge representation model (TMR) to enhance the detection of interactions and it provides a systematic analysis of relevant interactions in the context of multimorbidity. The approach is evaluated in a case study on rehabilitation of breast cancer patients, developed in collaboration with experts. Objective: More than half a million surgeries are performed every day worldwide, which makes surgery one of the most important component of global health care. In this context, the objective of this paper is to introduce a new method for the prediction of the possible next task that a surgeon is going to perform during surgery.Material and Method: We formulate the problem as finding the optimal registration of a partial sequence to a complete reference sequence of surgical activities. We propose an efficient algorithm to find the optimal partial alignment and a prediction system using maximum a posteriori probability estimation and filtering. We also introduce a weighting scheme allowing to improve the predictions by taking into account the relative similarity between the current surgery and a set of pre-recorded surgeries.Results: Our method is evaluated on two types of neurosurgical procedures: lumbar disc herniation removal and anterior cervical discectomy. Results show that our method outperformed the state of the art by predicting the next task that the surgeon will perform with 95% accuracy.Conclusions: This work shows that, even from the low-level description of surgeries and without other sources of information, it is often possible to predict the next surgical task when the conditions are consistent with the previously recorded surgeries. Abstract: Objective: While EIRA has proved to be successful in the detection of anomalous patient responses to treatments in the Intensive Care Unit, it could not describe to clinicians the rationales behind the anomalous detections. The aim of this paper is to address this problem. Methods: Few attempts have been made in the past to build knowledge-based medical systems that possess both argumentation and explanation capabilities. Here we propose an approach based on Dung''s seminal calculus of opposition. Results: We have developed a new tool, arguEIRA, which is an extension of the existing EIRA system. In this paper we extend EIRA by providing it with an argumentation-based justification system that formalizes and communicates to the clinicians the reasons why a patient response is anomalous. Conclusion: Our comparative evaluation of the EIRA system against the newly developed tool highlights the multiple benefits that the use of argumentation-logic can bring to the field of medical decision support and explanation. During the COVID-19 pandemic, the patient care delivery paradigm rapidly shifted to remote technological solutions. Rising rates of life expectancy of older people, and deaths due to chronic diseases (CDs) such as cancer, diabetes and respiratory disease pose many challenges to healthcare. While the feasibility of Remote Patient Monitoring (RPM) with a Smart Healthcare Monitoring (SHM) framework was somewhat questionable before the COVID-19 pandemic, it is now a proven commodity and is on its way to becoming ubiquitous. More health organizations are adopting RPM to enable CD management in the absence of individual monitoring. The current studies on SHM have reviewed the applications of IoT and/or Machine Learning (ML) in the domain, their architecture, security, privacy and other network related issues. However, no study has analyzed the AI and ubiquitous computing advances in SHM frameworks. The objective of this research is to identify and map key technical concepts in the SHM framework. In this context an interesting and meaningful classification of the research articles surveyed for this work is presented. The comprehensive and systematic review is based on the "Preferred Reporting Items for Systematic Review and Meta-Analysis" (PRISMA) approach. A total of 2540 papers were screened from leading research archives from 2016 to March 2021, and finally, 50 articles were selected for review. The major advantages, developments, distinctive architectural structure, components, technical challenges and possibilities in SHM are briefly discussed. A review of various recent cloud and fog computing based architectures, major ML implementation challenges, prospects and future trends is also presented. Summary: Objective:: Medical terminological systems (TSs) play an increasingly important role in health care by supporting recording, retrieval and analysis of patient information. As the size and complexity of TSs are growing, the need arises for means to audit them, i.e. verify and maintain (logical) consistency and (semantic) correctness of their contents. This is not only important for the management of TSs but also for providing their users with confidence about the reliability of their contents. Formal methods have the potential to play an important role in the audit of TSs, although there are few empirical studies to assess the benefits of using these methods. Methods and material:: In this paper we propose a method based on description logics (DLs) for the audit of TSs. This method is based on the migration of the medical TS from a frame-based representation to a DL-based one. Our method is characterized by a process in which initially stringent assumptions are made about concept definitions. The assumptions allow the detection of concepts and relations that might comprise a source of logical inconsistency. If the assumptions hold then definitions are to be altered to eliminate the inconsistency, otherwise the assumptions are revised. Results:: In order to demonstrate the utility of the approach in a real-world case study we audit a TS in the intensive care domain and discuss decisions pertaining to building DL-based representations. This case study demonstrates that certain types of inconsistencies can indeed be detected by applying the method to a medical terminological system. Conclusion:: The added value of the method described in this paper is that it provides a means to evaluate the compliance to a number of common modeling principles in a formal manner. The proposed method reveals potential modeling inconsistencies, helping to audit and (if possible) improve the medical TS. During the last decade, studies have shown the benefits of using clinical guidelines in the practice of medicine. Although the importance of these guidelines is widely recognized, health care organizations typically pay more attention to guideline development than to guideline implementation for routine use in daily care. However, studies have shown that clinicians are often not familiar with written guidelines and do not apply them appropriately during the actual care process.Implementing guidelines in computer-based decision support systems promises to improve the acceptance and application of guidelines in daily practice because the actions and observations of health care workers are monitored and advice is generated whenever a guideline is not followed. Such implementations are increasingly applied in diverse areas such as policy development, utilization management, education, clinical trials, and workflow facilitation. Many parties are developing computer-based guidelines as well as decision support systems that incorporate these guidelines. This paper reviews generic approaches for developing and implementing computer-based guidelines that facilitate decision support. It addresses guideline representation, acquisition, verification and execution aspects. Present-day healthcare witnesses a growing demand for coordination of patient care. Coordination is needed especially in those cases in which hospitals have structured healthcare into specialty-oriented units, while a substantial portion of patient care is not limited to single units. From a logistic point of view, this multi-disciplinary patient care creates a tension between controlling the hospital’s units, and the need for a control of the patient flow between units. A possible solution is the creation of new units in which different specialties work together for specific groups of patients. A first step in this solution is to identify the salient patient groups in need of multi-disciplinary care. Grouping techniques seem to offer a solution. However, most grouping approaches in medicine are driven by a search for pathophysiological homogeneity. In this paper, we present an alternative logistic-driven grouping approach.The starting point of our approach is a database with medical cases for 3603 patients with peripheral arterial vascular (PAV) diseases. For these medical cases, six basic logistic variables (such as the number of visits to different specialist) are selected. Using these logistic variables, clustering techniques are used to group the medical cases in logistically homogeneous groups. In our approach, the quality of the resulting grouping is not measured by statistical significance, but by (i) the usefulness of the grouping for the creation of new multi-disciplinary units; (ii) how well patients can be selected for treatment in the new units. Given a priori knowledge of a patient (e.g. age, diagnosis), machine learning techniques are employed to induce rules that can be used for the selection of the patients eligible for treatment in the new units. In the paper, we describe the results of the above-proposed methodology for patients with PAV diseases. Two groupings and the accompanied classification rule sets are presented. One grouping is based on all the logistic variables, and another grouping is based on two latent factors found by applying factor analysis. On the basis of the experimental results, we can conclude that it is possible to search for medical logistic homogenous groups (i) that can be characterized by rules based on the aggregated logistic variables; (ii) for which we can formulate rules to predict to which cluster new patients belong. There has been much research effort expended toward the use of Bayesian networks (BNs) in medical decision-making. However, because of the gap between developing an accurate BN and demonstrating its clinical usefulness, this has not resulted in any widespread BN adoption in clinical practice. This paper investigates this problem with the aim of finding an explanation and ways to address the problem through a comprehensive literature review of articles describing BNs in healthcare. Based on the literature collection that has been systematically narrowed down from 3810 to 116 most relevant articles, this paper analyses the benefits, barriers and facilitating factors (BBF) for implementing BN-based systems in healthcare using the ITPOSMO-BBF framework. A key finding is that works in the literature rarely consider barriers and even when these were identified they were not connected to facilitating factors. The main finding is that the barriers can be grouped into: (1) data inadequacies; (2) clinicians' resistance to new technologies; (3) lack of clinical credibility; (4) failure to demonstrate clinical impact; (5) absence of an acceptable predictive performance; and (6) absence of evidence for model's generalisability. The facilitating factors can be grouped into: (1) data collection improvements; (2) software and technological improvements; (3) having interpretable and easy to use BN-based systems; (4) clinical involvement in the development or review of the model; (5) investigation of model's clinical impact; (6) internal validation of the model's performance; and (7) external validation of the model. These groupings form a strong basis for a generic framework that could be used for formulating strategies for ensuring BN-based clinical decision-support system adoption in frontline care settings. The output of this review is expected to enhance the dialogue among researchers by providing a deeper understanding for the neglected issue of BN adoption in practice and promoting efforts for implementing BN-based systems. One of the popular metaheuristic search algorithms is Harmony Search (HS). It has been verified that HS can find solutions to optimization problems due to its balanced exploratory and convergence behavior and its simple and flexible structure. This capability makes the algorithm preferable to be applied in several real-world applications in various fields, including healthcare systems, different engineering fields, and computer science. The popularity of HS urges us to provide a comprehensive survey of the literature on HS and its variants on health systems, analyze its strengths and weaknesses, and suggest future research directions. In this review paper, the current studies and uses of harmony search are studied in four main domains. (i) The variants of HS, including its modifications and hybridization. (ii) Summary of the previous review works. (iii) Applications of HS in healthcare systems. (iv) And finally, an operational framework is proposed for the applications of HS in healthcare systems. The main contribution of this review is intended to provide a thorough examination of HS in healthcare systems while also serving as a valuable resource for prospective scholars who want to investigate or implement this method. Objective Clinicians’ attention is a precious resource, which in the current healthcare practice is consumed by the cognitive demands arising from complex patient conditions, information overload, time pressure, and the need to aggregate and synthesize information from disparate sources. The ability to organize information in ways that facilitate the generation of effective diagnostic solutions is a distinguishing characteristic of expert physicians, suggesting that automated systems that organize clinical information in a similar manner may augment physicians’ decision-making capabilities. In this paper, we describe the design and evaluation of a theoretically driven cognitive support system (CSS) that assists psychiatrists in their interpretation of clinical cases. The system highlights, and provides the means to navigate to, text that is organized in accordance with a set of diagnostically and therapeutically meaningful higher-level concepts. Methods and materials To evaluate the interface, 16 psychiatry residents interpreted two clinical case scenarios, with and without the CSS. Think-aloud protocols captured during their interpretation of the cases were transcribed and analyzed qualitatively. In addition, the frequency and relative position of content related to key higher-level concepts in a verbal summary of the case were evaluated. In addition the transcripts from both groups were compared to an expert derived reference standard using latent semantic analysis (LSA). Results Qualitative analysis showed that users of the system better attended to specific clinically important aspects of both cases when these were highlighted by the system, and revealed ways in which the system mediates hypotheses generation and evaluation. Analysis of the summary data showed differences in emphasis with and without the system. The LSA analysis suggested users of the system were more “expert-like” in their emphasis, and that cognitive support was more effective in the more complex case. Conclusions Cognitive support impacts upon clinical comprehension. This appears to be largely helpful, but may also lead to neglect of information (such as the psychosocial history) that the system does not highlight. The results have implications for the design of CSSs for clinical narratives including the role of information organization and textual embellishments for more efficient clinical case presentation and comprehension. Background: In the last ten years, the international workshop on knowledge representation for health care (KR4HC) has hosted outstanding contributions of the artificial intelligence in medicine community pertaining to the formalization and representation of medical knowledge for supporting clinical care. Contributions regarding modeling languages, technologies and methodologies to produce these models, their incorporation into medical decision support systems, and practical applications in concrete medical settings have been the main contributions and the basis to define the evolution of this field across Europe and worldwide.Objectives: Carry out a review of the papers accepted in KR4HC in the 2009-2018 decade, analyze and characterize the topics and trends within this field, and identify challenges for the evolution of the area in the near future.Methods: We reviewed the title, the abstract, and the keywords of the 112 papers that were accepted to the workshop, identified the medical and technological topics involved in these works, provided a classification of these papers in medical and technological perspectives and obtained the timeline of these topics in order to determine interest growths and declines. The experience of the authors in the field and the evidences after the review were the basis to propose a list of challenges of knowledge representation in health care for the future.Results: The most generic knowledge representation methods are ontologies (31%), semantic web related formalisms (26%), decision tables and rules (19%), logic (14%), and probabilistic models (10%). From a medical informatics perspective, knowledge is mainly represented as computer interpretable clinical guidelines (43%), medical domain ontologies (26%), and electronic health care records (22%). Within the knowledge lifecycle, contributions are found in knowledge generation (38%), knowledge specification (24%), exception detection and management (12%), knowledge enactment (8%), temporal knowledge and reasoning (7%), and knowledge sharing and maintenance (7%). The clinical emphasis of knowledge is mainly related to clinical treatments (27%), diagnosis (13%), clinical quality indicators (13%), and guideline integration for multimorbid patients (12%). According to the level of development of the works presented, we distinguished four maturity levels: formal (22%), implementation (52%), testing (13%), and deployment (2%) levels. Some papers described technologies for specific clinical issues or diseases, mainly cancer (22%) and diseases of the circulatory system (20%). Chronicity and comorbidity were present in 10% and 8% of the papers, respectively.Conclusions: KR4HC is a stable community, still active after ten years. A persistent focus has been knowledge representation, with an emphasis on semantic-web ontologies and on clinical-guideline based decision-support. Among others, two topics receive growing attention: integration of computer-interpretable guideline knowledge for the management of multimorbidity patients, and patient empowerment and patient-centric care. Abstract: Objective: Large health care datasets normally have a hierarchical structure, in terms of levels, as the data have been obtained from different practices, hospitals, or regions. Multilevel regression is the technique commonly used to deal with such multilevel data. However, for the statistical analysis of interactions between entities from a domain, multilevel regression yields little to no insight. While Bayesian networks have proved to be useful for analysis of interactions, they do not have the capability to deal with hierarchical data. In this paper, we describe a new formalism, which we call multilevel Bayesian networks; its effectiveness for the analysis of hierarchically structured health care data is studied from the perspective of multimorbidity. Methods: Multilevel Bayesian networks are formally defined and applied to analyze clinical data from family practices in The Netherlands with the aim to predict interactions between heart failure and diabetes mellitus. We compare the results obtained with multilevel regression. Results: The results obtained by multilevel Bayesian networks closely resembled those obtained by multilevel regression. For both diseases, the area under the curve of the prediction model improved, and the net reclassification improvements were significantly positive. In addition, the models offered considerable more insight, through its internal structure, into the interactions between the diseases. Conclusions: Multilevel Bayesian networks offer a suitable alternative to multilevel regression when analyzing hierarchical health care data. They provide more insight into the interactions between multiple diseases. Moreover, a multilevel Bayesian network model can be used for the prediction of the occurrence of multiple diseases, even when some of the predictors are unknown, which is typically the case in medicine. Abstract: Objective: This paper describes a methodology which enables computer-aided support for the planning, visualization and execution of personalized patient treatments in a specific healthcare process, taking into account complex temporal constraints and the allocation of institutional resources. To this end, a translation from a time-annotated computer-interpretable guideline (CIG) model of a clinical protocol into a temporal hierarchical task network (HTN) planning domain is presented. Materials and methods: The proposed method uses a knowledge-driven reasoning process to translate knowledge previously described in a CIG into a corresponding HTN Planning and Scheduling domain, taking advantage of HTNs known ability to (i) dynamically cope with temporal and resource constraints, and (ii) automatically generate customized plans. The proposed method, focusing on the representation of temporal knowledge and based on the identification of workflow and temporal patterns in a CIG, makes it possible to automatically generate time-annotated and resource-based care pathways tailored to the needs of any possible patient profile. Results: The proposed translation is illustrated through a case study based on a 70 pages long clinical protocol to manage Hodgkin''s disease, developed by the Spanish Society of Pediatric Oncology. We show that an HTN planning domain can be generated from the corresponding specification of the protocol in the Asbru language, providing a running example of this translation. Furthermore, the correctness of the translation is checked and also the management of ten different types of temporal patterns represented in the protocol. By interpreting the automatically generated domain with a state-of-art HTN planner, a time-annotated care pathway is automatically obtained, customized for the patient''s and institutional needs. The generated care pathway can then be used by clinicians to plan and manage the patients long-term care. Conclusion: The described methodology makes it possible to automatically generate patient-tailored care pathways, leveraging an incremental knowledge-driven engineering process that starts from the expert knowledge of medical professionals. The presented approach makes the most of the strengths inherent in both CIG languages and HTN planning and scheduling techniques: for the former, knowledge acquisition and representation of the original clinical protocol, and for the latter, knowledge reasoning capabilities and an ability to deal with complex temporal and resource constraints. Moreover, the proposed approach provides immediate access to technologies such as business process management (BPM) tools, which are increasingly being used to support healthcare processes. Abstract: Objective: In the modern healthcare system, rapidly expanding costs/complexity, the growing myriad of treatment options, and exploding information streams that often do not effectively reach the front lines hinder the ability to choose optimal treatment decisions over time. The goal in this paper is to develop a general purpose (non-disease-specific) computational/artificial intelligence (AI) framework to address these challenges. This framework serves two potential functions: (1) a simulation environment for exploring various healthcare policies, payment methodologies, etc., and (2) the basis for clinical artificial intelligence – an AI that can “think like a doctor”. Methods: This approach combines Markov decision processes and dynamic decision networks to learn from clinical data and develop complex plans via simulation of alternative sequential decision paths while capturing the sometimes conflicting, sometimes synergistic interactions of various components in the healthcare system. It can operate in partially observable environments (in the case of missing observations or data) by maintaining belief states about patient health status and functions as an online agent that plans and re-plans as actions are performed and new observations are obtained. This framework was evaluated using real patient data from an electronic health record. Results: The results demonstrate the feasibility of this approach; such an AI framework easily outperforms the current treatment-as-usual (TAU) case-rate/fee-for-service models of healthcare. The cost per unit of outcome change (CPUC) was $189 vs. $497 for AI vs. TAU (where lower is considered optimal) – while at the same time the AI approach could obtain a 30–35% increase in patient outcomes. Tweaking certain AI model parameters could further enhance this advantage, obtaining approximately 50% more improvement (outcome change) for roughly half the costs. Conclusion: Given careful design and problem formulation, an AI simulation framework can approximate optimal decisions even in complex and uncertain environments. Abstract: Objectives: Evidence-based decision making is becoming increasingly important in healthcare. Much valuable evidence is in the form of the results from clinical trials that compare the relative merits of treatments. In this paper, we present a new framework for representing and synthesizing knowledge from clinical trials involving multiple outcome indicators. Method: The framework generates and evaluates arguments for claiming that one treatment is superior, or equivalent, to another based on the available evidence. Evidence comes from randomized clinical trials, systematic reviews, meta-analyses, network analyses, etc. Preference criteria over arguments are used that are based on the outcome indicators, and the magnitude of those outcome indicators, in the evidence. Meta-arguments attacks arguments that are based on weaker evidence. Results: We evaluated the framework with respect to the aggregation of evidence undertaken in three published clinical guidelines that involve 56 items of evidence and 16 treatments. For each of the three guidelines, the treatment we identified as being superior using our method is a recommended treatment in the corresponding guideline. Conclusions: The framework offers a formal approach to aggregating clinical evidence, taking into account subjective criteria such as preferences over outcome indicators. In the evaluation, the aggregations obtained showed a good correspondence with published clinical guidelines. Furthermore, preliminary computational studies indicate that the approach is viable for the size of evidence tables normally encountered in practice. [Copyright &y& Elsevier]

Abstract: Objective: Extensive efforts have been made in both academia and industry in the research and development of smart wearable systems (SWS) for health monitoring (HM). Primarily influenced by skyrocketing healthcare costs and supported by recent technological advances in micro- and nanotechnologies, miniaturisation of sensors, and smart fabrics, the continuous advances in SWS will progressively change the landscape of healthcare by allowing individual management and continuous monitoring of a patient''s health status. Consisting of various components and devices, ranging from sensors and actuators to multimedia devices, these systems support complex healthcare applications and enable low-cost wearable, non-invasive alternatives for continuous 24-h monitoring of health, activity, mobility, and mental status, both indoors and outdoors. Our objective has been to examine the current research in wearable to serve as references for researchers and provide perspectives for future research. Methods: Herein, we review the current research and development of and the challenges facing SWS for HM, focusing on multi-parameter physiological sensor systems and activity and mobility measurement system designs that reliably measure mobility or vital signs and integrate real-time decision support processing for disease prevention, symptom detection, and diagnosis. For this literature review, we have chosen specific selection criteria to include papers in which wearable systems or devices are covered. Results: We describe the state of the art in SWS and provide a survey of recent implementations of wearable health-care systems. We describe current issues, challenges, and prospects of SWS. Summary: Objective: In this paper, we extend a preliminary proposal and discuss in a deeper and more formal way an approach to evaluate temporal similarity between clinical workflow cases (i.e., executions of clinical processes). More precisely, we focus on (i) the representation of clinical processes by using a temporal conceptual workflow model; (ii) the definition of ad hoc temporal constraint networks to formally represent clinical workflow cases; (iii) the definition of temporal similarity for clinical workflow cases based on the comparison of temporal constraint networks; (iv) the management of the similarity of clinical processes related to the Italian guideline for stroke prevention and management (SPREAD). Background: Clinical processes are composed by clinical activities to be done by given actors in a given order satisfying given temporal constraints. This description means that clinical processes can be seen as organizational processes, and modeled by workflow schemata. When a workflow schema represents a clinical process, its cases represent different instances derived from dealing with different patients in different situations. With respect to all the cases related to a workflow schema, each clinical case can be different with respect to its structure and to its temporal aspects. Clinical cases can be stored in clinical databases and information retrieval can be done evaluating the similarity between workflow cases. Methodology: We first describe a possible approach to the conceptual modeling of a clinical process, by using a temporally extended workflow model. Then, we define how a workflow case can be represented as a set of activities, and show how to express them through temporal constraint networks. Once we have built temporal constraint networks related to the cases to compare, we propose a similarity function able to evaluate the differences between the considered cases with respect to the order and duration of corresponding activities, and with respect to the presence/absence of some activities. Results: In this work, we propose an approach to evaluate temporal similarity between workflow cases. The proposed approach can be used (i) to query clinical databases storing clinical cases representing activities related to the management of different patients in different situations; (ii) to evaluate the quality of the service comparing the similarity between a (possibly synthetic) case, perceived as the good one with respect to a given clinical situation, and the other clinical cases; and (iii) to retrieve a particular class of cases similar to an interesting one. Summary: Objective: This paper studies a problem of efficiently discovering risk patterns in medical data. Risk patterns are defined by a statistical metric, relative risk, which has been widely used in epidemiological research. Methods: To avoid fruitless search in the complete exploration of risk patterns, we define optimal risk pattern set to exclude superfluous patterns, i.e. complicated patterns with lower relative risk than their corresponding simpler form patterns. We prove that mining optimal risk pattern sets conforms an anti-monotone property that supports an efficient mining algorithm. We propose an efficient algorithm for mining optimal risk pattern sets based on this property. We also propose a hierarchical structure to present discovered patterns for the easy perusal by domain experts. Results: The proposed approach is compared with two well-known rule discovery methods, decision tree and association rule mining approaches on benchmark data sets and applied to a real world application. The proposed method discovers more and better quality risk patterns than a decision tree approach. The decision tree method is not designed for such applications and is inadequate for pattern exploring. The proposed method does not discover a large number of uninteresting superfluous patterns as an association mining approach does. The proposed method is more efficient than an association rule mining method. A real world case study shows that the method reveals some interesting risk patterns to medical practitioners. Conclusion: The proposed method is an efficient approach to explore risk patterns. It quickly identifies cohorts of patients that are vulnerable to a risk outcome from a large data set. The proposed method is useful for exploratory study on large medical data to generate and refine hypotheses. The method is also useful for designing medical surveillance systems. Summary: Objective: Patients emerging from cardiac surgery can display varying degrees of cardiovascular instability arising from potentially complex, multi-factorial and interlinked causes. Stabilization and control of the cardiovascular system are currently managed by healthcare experts using experiential knowledge, and, in some centers, manually inputted decision pathway algorithms. This paper describes a clinical trial undertaken to determine the basic functioning of a clinical decision support system (CDSS) designed and constructed by the authors to facilitate the control of the major cardiovascular components in the early post-operative phase. Part II follows Part I''s description of the software and simulation testing of the CDSS, and describes the hardware setup of a patient monitoring and CDSS. The system is evaluated on three post-cardiac surgery intensive care patients whom had all undergone cardio-pulmonary bypass. Methods: The study was approved by the Sheffield Teaching Hospitals National Health Service (NHS) Foundation Trust Research Ethics Committee and conducted at the North Trent cardio-thoracic surgical unit and cardiac intensive care unit (CICU), Northern General Hospital, Sheffield (UK). Patients considered as ‘very likely’ to require active intervention to support the cardiovascular function following routine cardiac surgery were recruited during pre-operative surgical and anesthetic assessment, giving written informed consent when admitted for their operation. These patients underwent routine induction and maintenance of anesthesia by a non-study consultant anesthetist and the operation performed. There were no restrictions placed on the types of invasive monitoring used, on the use of trans-oesophageal echocardiography, drug selection, or the anesthetic agents selected by the clinicians performing the operations. All patients had full, routine invasive and non-invasive monitoring applied, including electrocardiography, central venous and peripheral arterial catheterisation, urine outputs and central temperature. After chest closure the patients were transferred to the CICU, sedated and ventilated, and the study commenced by the study anesthetist (1st author). The patients were in a clinically stable condition when admitted to the unit, and were attended by the treating clinicians until the handover to the study anesthetist occurred. The LiDCOplus ® (lithium dilution cardiac output) monitor (LiDCO Limited, Flowers Building, Granta Park, Cambridge CB1 6GU, United Kingdom) was calibrated after attachment to the patient''s arterial line, and the patient''s beat-to-beat hemodynamic data transferred to the host laptop computer. The CDSS graphical interface displays the patient''s clinical details and specific cardiovascular data and prompts the anesthetist to input the target ranges for each parameter, and select a suitable advisor frequency. This is the frequency with which the therapeutic advice is displayed on screen with an audible prompt for a control inputs from the anesthetist. In each case this was selected to be 30s. When the study anesthetist agreed with the CDSS advice (administration of fluid, commencing a drug, altering the drug infusion rate) the syringe motif on the “Advisor Infusion Rates” panel of the graphical interface was ‘clicked’ on and the infusion rate immediately and manually inputted to Graseby® 3400 pumps. If any disagreement between the anesthetist and the computer''s advice arose, the syringe motif on the “Expert Infusion Rates” panel of the preferred drug was ‘clicked’ on and the expert''s therapeutic decision (e.g. infusion rate) was entered in the corresponding data field and then applied to the pump. During all trials, data was stored for off-line analysis. Results: The CDSS successfully selected suitable drug therapies for each case and advised reasonable and appropriate infusion rates such that the study anesthetist did not have to override the suggested CDSS instructions and infusion rates. Under differing clinical conditions the system was able to maintain clinically appropriate and stable control of the cardiovascular system (CVS), with good profiles under noisy physiological measurements, and was readily able to regain control following transient deterioration of the patient hemodynamic parameters (coughing, or during blood sampling). Summary: Objective: Survival probability predictions in critically ill patients are mainly used to measure the efficacy of intensive care unit (ICU) treatment. The available models are functions induced from data on thousands of patients. Eventually, some of the variables used for these purposes are not part of the clinical routine, and may not be registered in some patients. In this paper, we propose a new method to build scoring functions able to make reliable predictions, though functions whose induction only requires records from a small set of patients described by a few variables. Methods: We present a learning method based on the use of support vector machines (SVM), and a detailed study of its prediction performance, in different contexts, of groups of variables defined according to the source of information: monitoring devices, laboratory findings, and demographic and diagnostic features. Results: We employed a data set collected in general ICUs at 10 units of hospitals in Spain, 6 of which include coronary patients, while the other 4 do not treat coronary diseases. The total number of patients considered in our study was 2501, 19.83% of whom did not survive. Using these data, we report a comparison between the SVM method proposed here with other approaches based on logistic regression (LR), including a second-level recalibration of release III of the acute physiology and chronic health evaluation (APACHE, a scoring system commonly used in ICUs) induced from the available data. The SVM method significantly outperforms them all from a statistical point of view. Comparison with the commercial version of APACHE III shows that the SVM scores are slightly better when working with data sets of more than 500 patients. Conclusions: From a practical point of view, the implications of the research reported here may be helpful to address the construction of cheap and reliable prediction systems in accordance with the peculiarities of ICUs and kinds of patients. Understanding patient opinions expressed towards healthcare services in online platforms could allow healthcare professionals to respond to address patients' concerns in a timely manner. Extracting patient opinion towards various aspects of health services is closely related to aspect-based sentiment analysis (ABSA) in which we need to identify both opinion targets and target-specific opinion expressions. The lack of aspect-level annotations however makes it difficult to build such an ABSA system. This paper proposes a joint learning framework for simultaneous unsupervised aspect extraction at the sentence level and supervised sentiment classification at the document level. It achieves 98.2% sentiment classification accuracy when tested on the reviews about healthcare services collected from Yelp, outperforming several strong baselines. Moreover, our model can extract coherent aspects and can automatically infer the distribution of aspects under different polarities without requiring aspect-level annotations for model learning. Summary: Objective: This paper describes the Web-based decision support system (TELEDERM) that provides medical services to rural and remote general practitioners (GPs), and presents qualitative results on the usefulness and usability of the system obtained from trials in the GoldFields, Western Australia. Methods: Diagnostic methods and other functions were developed to assist medical practitioners, who may not be familiar with aspects of decision support systems, to diagnose patients with dermatological problems. GPs in rural and remote areas took part in a trial to assess the usefulness of TELEDERM in clinical situations. In evaluating and assessing a number of the system’s characteristics, questionnaires, seminars and interviews were used. Results and conclusions: Feedback concerning the usefulness of TELEDERM shows that 67% of the GPs found the system useful or very useful, and 25% found the system somewhat useful. In terms of its usability, 83% found the system easy to use. The responses concerning the user interface and interactivity of TELEDERM are encouraging as 92% of the GPs found the text easy to read, 84% were happy with the appearance of the interface, and 84% found the system easy to navigate. 89% indicated that they are likely to use the system again. An issue with such systems is the reluctance of GPs to use them, even given that the development was quite sensitive to the needs of the GPs and the consultants, e.g. by streamlining the query process and emphasising that the system is a support tool and not a replacement. Summary: Objective: As a complementary medical system to Western medicine, traditional Chinese medicine (TCM) provides a unique theoretical and practical approach to the treatment of diseases over thousands of years. Confronted with the increasing popularity of TCM and the huge volume of TCM data, historically accumulated and recently obtained, there is an urgent need to explore these resources effectively by the techniques of knowledge discovery in database (KDD). This paper aims at providing an overview of recent KDD studies in TCM field. Methods: A literature search was conducted in both English and Chinese publications, and major studies of knowledge discovery in TCM (KDTCM) reported in these materials were identified. Based on an introduction to the state of the art of TCM data resources, a review of four subfields of KDTCM research was presented, including KDD for the research of Chinese medical formula, KDD for the research of Chinese herbal medicine, KDD for TCM syndrome research, and KDD for TCM clinical diagnosis. Furthermore, the current state and main problems in each subfield were summarized based on a discussion of existing studies, and future directions for each subfield were also proposed accordingly. Results: A series of KDD methods are used in existing KDTCM researches, ranging from conventional frequent itemset mining to state of the art latent structure model. Considerable interesting discoveries are obtained by these methods, such as novel TCM paired drugs discovered by frequent itemset analysis, functional community of related genes discovered under syndrome perspective by text mining, the high proportion of toxic plants in the botanical family Ranunculaceae disclosed by statistical analysis, the association between M-cholinoceptor blocking drug and Solanaceae revealed by association rule mining, etc. It is particularly inspiring to see some studies connecting TCM with biomedicine, which provide a novel top–down view for functional genomics research. However, further developments of KDD methods are still expected to better adapt to the features of TCM. Conclusions: Existing studies demonstrate that KDTCM is effective in obtaining medical discoveries. However, much more work needs to be done in order to discover real diamonds from TCM domain. Summary: Objective: This article delineates a relatively unknown path in the history of medical philosophy and medical diagnosis. It is concerned with the phenomenon of vagueness in the physician''s “style of thinking” and with the use of fuzzy sets, systems, and relations with a view to create a model of such reasoning when physicians make a diagnosis. It represents specific features of medical ways of thinking that were mentioned by the Polish physician and philosopher Ludwik Fleck in 1926. The paper links Lotfi Zadeh''s work on system theory before the age of fuzzy sets with system-theory concepts in medical philosophy that were introduced by the philosopher Mario Bunge, and with the fuzzy-theoretical analysis of the notions of health, illness, and disease by the Iranian-German physician and philosopher Kazem Sadegh-Zadeh. Material: Some proposals to apply fuzzy sets in medicine were based on a suggestion made by Zadeh: symptoms and diseases are fuzzy in nature and fuzzy sets are feasible to represent these entity classes of medical knowledge. Yet other attempts to use fuzzy sets in medicine were self-contained. The use of this approach contributed to medical decision-making and the development of computer-assisted diagnosis in medicine. Conclusion: With regard to medical philosophy, decision-making, and diagnosis; the framework of fuzzy sets, systems, and relations is very useful to deal with the absence of sharp boundaries of the sets of symptoms, diagnoses, and phenomena of diseases. The foundations of reasoning and computer assistance in medicine were the result of a rapid accumulation of data from medical research. This explosion of knowledge in medicine gave rise to the speculation that computers could be used for the medical diagnosis. Medicine became, to a certain extent, a quantitative science. In the second half of the 20th century medical knowledge started to be stored in computer systems. To assist physicians in medical decision-making and patient care, medical expert systems using the theory of fuzzy sets and relations (such as the Viennese “fuzzy version” of the Computer-Assisted Diagnostic System, Cadiag, which was developed at the end of the 1970s) were constructed. The development of fuzzy relations in medicine and their application in computer-assisted diagnosis show that this fuzzy approach is a framework to deal with the “fuzzy mode of thinking” in medicine. Summary: Objective: In the present paper, we describe an application of case-based retrieval to the domain of end stage renal failure patients, treated with hemodialysis. Materials and methods: Defining a dialysis session as a case, retrieval of past similar cases has to operate both on static and on dynamic features, since most of the monitoring variables of a dialysis session are time series. Retrieval is then articulated as a two-step procedure: (1) classification, based on static features and (2) intra-class retrieval, in which dynamic features are considered. As regards step (2), we concentrate on a classical dimensionality reduction technique for time series allowing for efficient indexing, namely discrete Fourier transform (DFT). Thanks to specific index structures (i.e. k –d trees), range queries (on local feature similarity) can be efficiently performed on our case base, allowing the physician to examine the most similar stored dialysis sessions with respect to the current one. Results: The retrieval tool has been positively tested on real patients’ data, coming from the nephrology and dialysis unit of the Vigevano hospital, in Italy. Conclusions: The overall system can be seen as a means for supporting quality assessment of the hemodialysis service, providing a useful input from the knowledge management perspective. Summary: Objective: Alzheimer’s disease affects a growing population of elderly people today. The predictions about the course of the disease is a key component of health care decision making for patients with Alzheimer’s. The physician’s prognosis and predicted trajectory of cognitive decline often form the basis of treatment and health care decisions taken by patients and their families. These predictions are difficult to make because of the high variability and non-linearity exhibited by individual patterns of cognitive decline. This paper presents a new method of predicting the course of a disease using longitudinal data collected through multiple clinic visits. Longitudinal databases are similar to temporal databases, with some important differences—data is collected at irregular time intervals that are patient specific and also a varying number of observations are made for each patient, depending upon the number of times the patient visited the clinic. We propose a new type of neural network called the mixed effects neural network (MENN) model that can incorporate this type of longitudinal information. Material and methods: We have used longitudinal data on 704 subjects enrolled at the Layton aging and research center (LAARC) at Oregon Health and Science University. A back-propagation algorithm, modified for longitudinal data is used to obtain the weight parameters of the MENN. The modified back-propagation algorithm is further embedded in an iterative procedure that estimates the noise variance and the parameters that capture the longitudinal (temporal) correlation structure. Results: We have compared the performance of the MENN with linear mixed effects models and standard neural networks (NN). MENN show better performance (misclassification rate = 0.13 and relative MSE = 0.35) as compared to standard NN (misclassification rate = 0.34 and relative MSE = 2.74) and linear mixed effects models (misclassification rate = 0.14 and relative MSE = 0.4). Conclusion: The results show that this method can be a useful tool for predicting non-linear disease trajectories and uncovering significant prognostic factors in longitudinal databases. Summary: Objectives: This paper presents current work in case-based reasoning (CBR) in the health sciences, describes current trends and issues, and projects future directions for work in this field. Methods and material: It represents the contributions of researchers at two workshops on case-based reasoning in the health sciences. These workshops were held at the Fifth International Conference on Case-Based Reasoning (ICCBR-03) and the Seventh European Conference on Case-Based Reasoning (ECCBR-04). Results: Current research in CBR in the health sciences is marked by its richness. Highlighted trends include work in bioinformatics, support to the elderly and people with disabilities, formalization of CBR in biomedicine, and feature and case mining. Conclusion: CBR systems are being better designed to account for the complexity of biomedicine, to integrate into clinical settings and to communicate and interact with diverse systems and methods. The purpose of this paper is to investigate the suitability of boosted decision trees for the case-mix adjustment involved in comparing the performance of various health care entities.First, we present logistic regression, decision trees, and boosted decision trees in a unified framework. Second, we study in detail their application for two common performance indicators, the mortality rate in intensive care and the rate of potentially avoidable hospital readmissions.For both examples the technique of boosting decision trees outperformed standard prognostic models, in particular linear logistic regression models, with regard to predictive power. On the other hand, boosting decision trees was computationally demanding and the resulting models were rather complex and needed additional tools for interpretation.Boosting decision trees represents a powerful tool for case-mix adjustment in health care performance measurement. Depending on the specific priorities set in each context, the gain in predictive power might compensate for the inconvenience in the use of boosted decision trees. [Copyright &y& Elsevier]

Health care effectiveness and efficiency are under constant scrutiny especially when treatment is quite costly as in the Intensive Care (IC). Currently there are various international quality of care programs for the evaluation of IC. At the heart of such quality of care programs lie prognostic models whose prediction of patient mortality can be used as a norm to which actual mortality is compared. The current generation of prognostic models in IC are statistical parametric models based on logistic regression. Given a description of a patient at admission, these models predict the probability of his or her survival. Typically, this patient description relies on an aggregate variable, called a score, that quantifies the severity of illness of the patient. The use of a parametric model and an aggregate score form adequate means to develop models when data is relatively scarce but it introduces the risk of bias.This paper motivates and suggests a method for studying and improving the performance behavior of current state-of-the-art IC prognostic models. Our method is based on machine learning and statistical ideas and relies on exploiting information that underlies a score variable. In particular, this underlying information is used to construct a classification tree whose nodes denote patient sub-populations. For these sub-populations, local models, most notably logistic regression ones, are developed using only the total score variable. We compare the performance of this hybrid model to that of a traditional global logistic regression model. We show that the hybrid model not only provides more insight into the data but also has a better performance. We pay special attention to the precision aspect of model performance and argue why precision is more important than discrimination ability. [Copyright &y& Elsevier]

The use of multi-agent systems (MAS) in health-care domains is increasing. Such agent-mediated medical systems can manage complex tasks and have the potential to adapt gracefully to unexpected events. However, in these kinds of systems the issues of privacy, security and trust are particularly sensitive in relation to matters such as agents’ access to patient records, what is acceptable behaviour for an agent in a particular role and the development of trust both between (heterogeneous) agents and between users and agents.To address these issues we propose a formal normative framework, deriving from and developing the notion of an electronic institution. Such institutions provide a framework to define and police norms that guide, control and regulate the behaviour of the heterogeneous agents that participate in the institution. These norms define the acceptable actions that each agent may perform depending on the role or roles it is playing, and clearly specifies the data it may access and/or modify in playing those roles.In this paper, we present the formalization of Carrel, a virtual organization for the procurement of organs and tissues for transplantation purposes, as an electronic institution using the ISLANDER institution specification language as formalizing languages. We demonstrate aspects of the formalization of such an institution, example fragments in the language used for the textual specification, and how such formalization can be used as a blueprint in the implementation of the final agent architecture, through techniques such as skeleton generation. In this paper, the current published knowledge about smart and adaptive engineering systems in medicine is reviewed. The achievements of frontier research in this particular field within medical engineering are described. A multi-disciplinary approach to the applications of adaptive systems is observed from the literature surveyed. The three modalities of diagnosis, imaging and therapy are considered to be an appropriate classification method for the analysis of smart systems being applied to specified medical sub-disciplines. It is expected that future research in biomedicine should identify subject areas where more advanced intelligent systems could be applied than is currently evident. The literature provides evidence of hybridisation of different types of adaptive and smart systems with applications in different areas of medical specifications. [Copyright &y& Elsevier]

No comprehensive review of Bayesian networks (BNs) in healthcare has been published in the past, making it difficult to organize the research contributions in the present and identify challenges and neglected areas that need to be addressed in the future. This unique and novel scoping review of BNs in healthcare provides an analytical framework for comprehensively characterizing the domain and its current state. A literature search of health and health informatics literature databases using relevant keywords found 3810 articles that were reduced to 123. This was after screening out those presenting Bayesian statistics, meta-analysis or neural networks, as opposed to BNs and those describing the predictive performance of multiple machine learning algorithms, of which BNs were simply one type. Using the novel analytical framework, we show that: (1) BNs in healthcare are not used to their full potential; (2) a generic BN development process is lacking; (3) limitations exist in the way BNs in healthcare are presented in the literature, which impacts understanding, consensus towards systematic methodologies, practice and adoption; and (4) a gap exists between having an accurate BN and a useful BN that impacts clinical practice. This review highlights several neglected issues, such as restricted aims of BNs, ad hoc BN development methods, and the lack of BN adoption in practice and reveals to researchers and clinicians the need to address these problems. Data-driven healthcare uses predictive analytics to enhance decision-making and personalized healthcare. Developing prognostic models is one of the applications of predictive analytics in medical environments. Various studies have used machine learning techniques for this purpose. However, there is no specific standard for choosing prediction models for different medical purposes. In this paper, the ISAF framework was proposed for choosing appropriate prediction models with regard to the properties of the classification methods. As one of the case study applications, a prognostic model for predicting cardiac arrests in sepsis patients was developed step by step through the ISAF framework. Finally, a new modified stacking model produced the best results. We predict 85 % of heart arrest cases one hour before the incidence (sensitivity> = 0.85) and 73 % of arrest cases 25 h before the occurrence (sensitivity> = 0.73). The results indicated that the proposed prognostic model has significantly improved the prediction results compared to the two standard systems of APACHE II and MEWS. In clinical settings, a lot of medical image datasets suffer from the imbalance problem which hampers the detection of outliers (rare health care events), as most classification methods assume an equal occurrence of classes. In this way, identifying outliers in imbalanced datasets has become a crucial issue. To help address this challenge, one-class classification, which focuses on learning a model using samples from only a single given class, has attracted increasing attention. Previous one-class modeling usually uses feature mapping or feature fitting to enforce the feature learning process. However, these methods are limited for medical images which usually have complex features. In this paper, a novel method is proposed to enable deep learning models to optimally learn single-class-relevant inherent imaging features by leveraging the concept of imaging complexity. We investigate and compare the effects of simple but effective perturbing operations applied to images to capture imaging complexity and to enhance feature learning. The automatic extraction of a patient's natural history from Electronic Health Records (EHRs) is a critical step towards building intelligent systems that can reason about clinical variables and support decision making. Although EHRs contain a large amount of valuable information about the patient's medical care, this information can only be fully understood when analyzed in a temporal context. Any intelligent system should then be able to extract medical concepts, date expressions, temporal relations and the temporal ordering of medical events from the free texts of EHRs; yet, this task is hard to tackle, due to the domain specific nature of EHRs, writing quality and lack of structure of these texts, and more generally the presence of redundant information. In this paper, we introduce a new Natural Language Processing (NLP) framework, capable of extracting the aforementioned elements from EHRs written in Spanish using rule-based methods. We focus on building medical timelines, which include disease diagnosis and its progression over time. By using a large dataset of EHRs comprising information about patients suffering from lung cancer, we show that our framework has an adequate level of performance by correctly building the timeline for 843 patients from a pool of 989 patients, achieving a precision of 0.852.
