1. A 58 years old man of left tuberculous empyema with fistulas underwent open-wound thoracotomy six months ago. First, latissimus dorsal muscle was meticulously dissected for a flap. Next, pleural cavity was derided and short length of both lower ribs’ ends were removed for thoracoplasty. After omental flap was prepared, fistulas were closed by suturing with omental flap. Moreover latissimus dorsi muscle flap was tied over and the wound was closed without residual empyema space
