Good evidence that oil‐soluble contrast media (OSCM) enhances pregnancy rates when used to assess fallopian tube patency by hysterosalpingogram has prompted rapid clinical uptake by some fertility doctors and imaging specialists in Australia and New Zealand. The ACCEPT group met in July 2019 to develop a consensus document outlining the indications for and safe use of OSCM, to inform and guide clinicians interested in offering procedures using this media to couples with infertility. Objectives The aim of this study was to determine the proportion of patients with advanced ovarian and related cancers ( EOC+ RC), treated with neoadjuvant chemotherapy and interval debulking surgery ( NACT - IDS), and to determine if there was any relationship with optimal cytoreduction rates and overall survival ( OS) in a state-wide gynaecologic oncology service over time. Methods A retrospective review was undertaken using a population-based database of patients with stages 3 and 4 EOC+ RC treated from 1982 till 2013 at the Queensland Centre for Gynaecological Cancer ( QCGC). The proportion of patients treated with NACT - IDS compared with primary debulking surgery ( PDS) was determined and compared with debulking rates and with the moving five-year OS probability. Results From 1982-2013, 2601 patients with advanced EOC+ RC were managed at QCGC. No patients received NACT - IDS till 1995 when the first two patients received this treatment, rising to 55% of patients in 2013. Surgical cytoreduction rates to no macroscopic residual (R0) were achieved 32% of the time by 2006, rising to 48% in 2009, and 62% in 2013. Despite the increase in utilisation of NACT - IDS, our unit has noted a continued rise in the OS probability at five years to 45%. Conclusions The increasing utilisation of NACT - IDS in the setting of a large centralised clinical service has been associated with increasing rates of optimal cytoreduction and survival rates have continued to rise in excess of those achieved in the trials reported to date. The diagnosis of adnexal torsion can be difficult to make, especially in isolated fallopian tube torsion (FTT). Only small series and several case reports on isolated FTT have been published in the literature. To demonstrate symptom presentations, objective findings and surgical outcomes in women with isolated FTT over a 12-year period at a tertiary hospital. Using the International Classification of Disease, Ninth Revision, and Clinical Modification, we reviewed the clinical records of women with isolated FTT during 1996 and 2008 at our institute. Seventeen women, age 13-50 (mean 32) with surgically proven isolated FTT, were included. Five women (29.4%) had tubal ligation history. None of these cases were diagnosed before operation. Pain characteristics were variable; the onset was sudden in 10 (58.8%) and 17 (100%) complained of lower abdominal pain, but only six (35.3%) had peritoneal signs. Other clinical manifestations were as follows: nausea or vomiting in seven women (41.2%), lower urinary tract symptoms in four (23.5%) and fever in three (17.6%). All the women had a cystic adnexal mass on ultrasound. An enlarged tubal mass (5-14 cm) was found in all the cases at surgery. Eleven women (64.7%) underwent laparoscopy, and six (35.3%) laparotomy for salpingectomy. The diagnosis of isolated FTT is often not made before surgical intervention because clinical features are non-specific. A case study of a 59 year old woman presented with an asymptomatic adnexal mass is presented. A discussion of investigative diagnostic examinations which were conducted on the woman and resulted in her receiving a diagnosis of primary clear cell carcinoma of a paratubal cyst is presented. Treatment which was given to the woman is discussed.

The article discusses the unusual case of a 51-year-old with torsion hydrosalpinx after hysterectomy. The woman was admitted because of lower abdominal discomfort and difficulty in micturition. She has a history of vaginal hysterectomy for prolapse with no tubal interruption. The diagnosis found that hydrosalpinx was the cause of her fallopian tube torsion. A case that usually present in the reproductive age group.

The article presents the case of a woman with a large symptomatic granulosa cell tumour of the ovary and fallopian tube adenocarcinoma. Results of computed tomography and laparotomy are described. The patient received cycles of adjuvant systemic chemotherapy and external beam pelvic and para-aortal radiotherapy.
