In relation to treatment-related research in the United States, there is relatively little systematic effort focused on the combination of risk and prevention for anxiety pathology. This article broadly discusses risk factor research and prevention program development for anxiety psychopathology. The authors also specifically discuss papers in this special issue that are focused on these topics. Risk factor research should be used by clinical researchers to inform prevention programs, and reciprocally, prevention knowledge should be effectively utilized to drive new, clinically focused risk factor research. This study investigated the effects of the Good Behavior Game (GBG) on classwide off-task behavior in two ninth-grade basic algebra resource classes. Ten students with a variety of disabilities, in two classrooms, and their special education resource teacher participated in this study. A reversal design was employed, in which the special education teacher implemented GBG compared to typical practice-algebra readiness instruction. Results showed that classwide off-task behavior decreased in the GBG conditions compared to the baseline and reversal conditions. Fidelity measures indicated that the teacher implemented GBG with fidelity. Students and the teacher rated GBG favorably. Overall findings support the use of GBG for reducing classwide off-task behavior. Implications for practice and future research directions are presented. [ABSTRACT FROM PUBLISHER]

A motivating operation (MO) alters both the effectiveness of a stimulus as a reinforcer and the current frequency of all behavior that has been reinforced by that particular stimulus. This article reviews studies that have manipulated a MO during interventions with school-age participants with intellectual disabilities and problem behavior. A systematic review was conducted using the following major electronic databases: PsychInfo, Education Resources Information Center, Science Direct, Blackwell, SAGE, and Medline. A total of 31 published articles representing 55 participants was examined. Findings from this study suggest that the modification of a MO usually has an effect on the problem behavior by either establishing or abolishing its motivation. Furthermore, a relationship was found between the type of MO and the behavioral function identified. The theoretical and practical implications of these findings, limitations of this study and potential issues for future research are discussed. [ABSTRACT FROM PUBLISHER]

In this article, the authors evaluated functional communication training (FCT) and a chained schedule of reinforcement for the treatment of challenging behavior exhibited by two individuals diagnosed with Asperger syndrome and autism, respectively. Following a functional analysis with undifferentiated results, the authors demonstrated that challenging behavior was occasioned by interruptions of ongoing activities and maintained by terminations of interruptions. Next, they demonstrated the effectiveness of a treatment consisting of FCT with a chained schedule of reinforcement. Last, they modified the chained schedule procedure to increase ease of implementation and promote toleration of activity interruptions, and academic tasks were incorporated into the treatment. The feeding behaviors of two children who maintained failure to thrive diagnoses and displayed food refusal are assessed in their homes. Descriptive assessments are used to identify schedules of consequence provided by each child's care providers for bite acceptance and food refusal behaviors. Assessments reveal rich schedules of praise and access to social interaction and preferred activities for bite acceptance and escape for food refusal. These schedule arrangements result in hypotheses that modifications to the schedule of praise and access to social interaction and preferred activities for bite acceptance would result in little to no effect and that modifications to the schedule of escape for food refusal would be necessary for treatment success. Successful interventions are subsequently implemented by manipulating the existing schedules of escape for food refusal by each child's care providers. A randomized, controlled trial was conducted to examine the cost-effectiveness of cognitive-behavioral treatment (CBT) for panic disorder with agoraphobia. A total of 100 participants were randomly assigned to standard (n = 33), group (n = 35), and brief (n = 32) treatment conditions. Results show significant clinical and statistical improvement on standard symptom measures and quality of life from baseline to posttreatment and 3-month follow-up, with no significant differences between treatment conditions. Compared with standard CBT, brief and group CBT incurred lower treatment costs and had a superior cost-effectiveness ratio, suggesting the potential of these alternative treatment conditions in increasing access to effective treatment. Acceptance and commitment therapy (ACT) has a small but growing database of support. One hundred and one heterogeneous outpatients reporting moderate to severe levels of anxiety or depression were randomly assigned to traditional cognitive therapy (CT) or to ACT. To maximize external validity, the authors utilized very minimal exclusion criteria. Participants receiving CT and ACT evidenced large, equivalent improvements in depression, anxiety, functioning difficulties, quality of life, life satisfaction, and clinician-rated functioning. Whereas improvements were equivalent across the two groups, the mechanisms of action appeared to differ. Changes in ‘observing’ and ‘describing’ one's experiences appeared to mediate outcomes for the CT group relative to the ACT group, whereas ‘experiential avoidance’ ‘acting with awareness,’ and ‘acceptance’ mediated outcomes for the ACT group. Overall, the results suggest that ACT is a viable and disseminable treatment, the effectiveness of which appears equivalent to that of CT, even as its mechanisms appear to be distinct. This article reviews the concepts of positive and negative perfectionism and the dual process model of perfectionism outlined by Slade and Owens (1998). The authors acknowledge that the dual process model represents a conceptual advance in the study of perfectionism and that Slade and Owens should be commended for identifying testable hypotheses and future research directions. However, the authors take issue with the notion that there are two types of perfectionism, with one type of perfectionism representing a ‘normal’ or ‘healthy’ form of perfectionism. They suggest that positive perfectionism is motivated, at least in part, by an avoidance orientation and fear of failure, and recent attempts to define and conceptualize positive perfectionism may have blurred the distinction between perfectionism and conscientiousness. Research findings that question the adaptiveness of positive forms of perfectionism are highlighted, and key issues for future research are identified. [ABSTRACT FROM AUTHOR]

Current research supports the effectiveness of self-monitoring strategies for addressing academic and behavioral challenges within educational settings. Although variations in procedures exist, frequently implementation of self-monitoring involves some form of adult feedback as a method of establishing accurate self-monitoring. To date, however, researchers have not systematically evaluated whether adult feedback is a necessary component for self-monitoring to be effective. In the current investigation, the influence of adult feedback on the effectiveness of self-monitoring was analyzed. The participant was a 13-year-old student receiving educational services in a special education school at a residential facility for youth with conduct problems. The effectiveness of self-monitoring with and without adult feedback was compared. Results suggest that adult feedback may be an important component for establishing self-monitoring as an effective intervention for behavior problems exhibited in academic settings. As we begin to apply functional assessment procedures in mainstream educational settings, there is a need to explore options for identifying behavior function that are not only effective but efficient and practical for school personnel to employ. Attempts to simplify the functional assessment process are evidenced by the development of informant assessment measures (e.g., interviews, rating scales). In this study, the agreement (i.e., on relative rankings and primary function) across sources of information regarding behavior function was examined for 19 students in a middle school setting. These measures included teacher ratings, student ratings, student interviews, observer ratings, and conditional probabilities. In addition, for 1 student, whether information obtained through these sources was consistent with that obtained through a brief analog analysis of function was examined. Respiration is a complex physiological system affecting a variety of physical processes that can act as a critical link between mind and body. This review discusses the evidence for dysregulated breathing playing a role in three clinical syndromes: panic disorder, functional cardiac disorder, and chronic pain. Recent technological advances allowing the ambulatory assessment of end-tidal partial pressure of CO[sub 2] (PCO[sub 2]) and respiratory patterns have opened up new avenues for investigation and treatment of these disorders. The latest evidence from laboratories indicates that subtle disturbances of breathing, such as tidal volume instability and sighing, contribute to the chronic hypocapnia often found in panic patients. Hypocapnia is also common in functional cardiac and chronic pain disorders, and studies indicate that it mediates some of their symptomatology. Consistent with the role of respiratory dysregulation in these disorders, initial evidence indicates efficacy of respiration-focused treatment. [ABSTRACT FROM AUTHOR]

A review of the panic disorder literature strongly suggests subtypes of panic attacks, including a respiratory subtype. This study empirically tested several aspects of Ley's panic subtype theory, measuring end-tidal carbon dioxide (ETCO2) levels at baseline, during psychologic and respiratory stressors, and at recovery. As predicted, Type 1 (classic or respiratory) panickers had significantly lower resting ETCO2 compared to Type 3 (cognitive) and to controls. Type 3 panickers did not differ from controls. Physiologic findings support the existence of respiratory and other subtypes of panic attacks in panic disorder. More complex measures of respiration and other physiology are likely required to elicit full subtype profiles. Distinguishing between chronic (compensated) hyperventilators and acute hyperventilators will likely be useful in clarifying the subtypes. Recognizing the need for differential diagnosis of panic attacks can facilitate developing more specific treatment plans and interventions (e.g., restoration of normal ETCO2 in Type 1), improving treatment success rates. Hyperventilation is often conceived of as part of a fight-or-flight response, triggered by situations with high arousal and negative valence. However, a previous study using emotional imagery found hyperventilation responses during imagery of high-arousal scenes regardless of their valence. Those imagery scripts contained suggestions of autonomic activity, which may have partly induced or enhanced the hyperventilatory responsivity. The present study used four emotional scripts--depicting relaxing, fearful, depressive, and pleasant situations--without suggestions of autonomic or respiratory responses. After each imagery trial, participants rated their imagery for valence, arousal, and vividness. Fractional end-tidal carbon dioxide (FetCO[sub 2]), inspiratory and expiratory time, tidal volume, and pulse rate were measured in a non-intrusive way. Results showed significant FetCO[sub 2] drops during the fearful and pleasant scripts. However, this effect was much smaller compared to imagery scripts with autonomic response propositions. Participants imagining scripts without autonomic response information found it harder to imagine the scripts vividly and reported lower levels of subjective arousal. [ABSTRACT FROM AUTHOR]

According to recent research, deliberate suppression of unwanted thoughts may result in a paradoxical increase in their frequency. Recent cognitive-behavioral theories of obsessive-compulsive disorder now consider active thought suppression to be a crucial factor in the pathogenesis of obsessions. However, there are considerable inconsistencies in the results of studies investigating the thought suppression paradox that make it difficult to apply findings to obsessional problems. Furthermore, many studies have low ecological validity for obsessional problems. This article reviews the thought suppression research, discusses methodological issues relevant to application of the thought suppression paradox to obsessional problems, and offers recommendations for future research. From the literature, it is increasingly apparent that dentally anxious individuals are not a homogeneous group and that the development and maintenance of dental anxiety are complex phenomena. In this study, individuals who had avoided dental treatment in the past due to fear but were no longer fearful were compared with others who had avoided and were still fearful on a number of demographic, experiential, and attitudinal variables under the assumption that avoidance is likely to prevent the emotional processing of dental anxiety. The groups differed markedly in their experience of dentistry; anxious avoiders were more likely to have experienced more invasive and less noninvasive dental treatment; they feared pain more and were more negative about dentistry than those whose anxiety had remitted. In addition, the remitted avoiders were less likely to claim frightening or embarrassing dental experiences as well as being less reactive to these experiences than the others.
