During a two-year study of a developmental surveillance programme covering all children under 5 in a Large general practice in the south of England, 2157 children were examined, including 382 newborn babies seen at home. Suspected disorders--excluding those found during non-routine consultations--were discovered in 232 children (15% of boys and 11% of girls), of whom 171 (104 boys and 67 girls) were referred to specialist agencies.
The number and nature of the disorders show that routine surveillance on the lines proposed by the Court Committee is worth while. Nevertheless, such programmes could not be started on a national scale without increased resources for the specialist services to which more children would need to be referred.
In this paper we indicate the rate of suspected (not necessarily confirmed) abnormalities detected by our examinations, and discuss the implications for the work load of the specialist services. In three trials conducted by the Medical Research Council on acute lymphoblastic leukaemia in childhood the incidence of testicular infiltration in relation to presenting features and treatment was analysed. Initial severe thrombocytopenia was associated with an increased incidence. Testicular infiltration was occasionally found during treatment in patients with high blood leucocyte counts at diagnosis but the main incidence in patients with all types of disease occurred within one year of stopping treatment. The incidence may be increased when "second-line" drugs, especially cyclophosphamide, have been used. A reappraisal of the value of additional drugs in maintenance treatment of patients with good prognosis is needed.

The radiological prevalence of Paget's disease among migrants from the United Kingdom to Australia was higher than in a comparable Australian resident population but lower than in a British resident population. A probable explanation is that the migrants carry with them the higher UK risk of developing the disease.

Since 1950 the consumption of alcohol per head of population in Britain has almost doubled. On the basis of known correlations the number of heavy drinkers is likely to have quadrupled. Over the same period the number of licensed motor vehicles has also quadrupled. The association between alcohol and driving is therefore now far more likely to occur, and our road accident statistics confirm that alcohol-related accidents have been increasing. The association may occur in a wide variety of drivers, ranging from the occasional drinker, through those with ordinary patterns of social drinking, to the regular taker of large quantifies of alcohol and alcoholics. A review of the effects of using bromocriptine in Parkinson's disease showed that it rarely helps patients not primarily improved by levodopa. Patients who show late failure with levodopa and whose response to treatment is declining are helped by combining the two drugs. High cost and severe psychosis are the main disadvantages of bromocriptine, and, although it is not recommended for patients who are doing well on levodopa, it is the best available drug for hospital use in patients who show late failure with levedopa.
Bromocriptine is of value in treating several endocrine disorders; more recently it has been used in Parkinsonism. It has produced definite improvements in objective measurements of rigidity, tremor, bradykinesia, gait disorder, and total functional disability. The method of selecting abstracts for an international congress at which only 15% of submitted papers could be accepted entailed a panel of 12 assessors using their specialised knowledges but presentation of the abstract was also important in selection. There was only a limited agreement between assessors in arranging abstracts in order of merit, so that a single assessor would be unacceptable.
Use of the full panel to grade all abstracts was very expensive, but it could be replaced, without unacceptable injustice, by dividing the work randomly among groups of three selectors. [ABSTRACT FROM AUTHOR]

In an earlier paper one of us (AB) described his personal experience of multiple sclerosis (MS).[1] Over the last two years we have taken part, singly and together, in the counselling of other sufferers from MS and close relatives. This voluntary work has been informal and much of it has taken place in our home. We discuss here some of the emotional difficulties created by MS and our observations derived from counselling and from participating in self-help groups stimulated by CRACK (the young arm of the Multiple Sclerosis Society). Intermittent claudication is the commonest symptom of an increasingly common disorder: obliterative arterial disease affecting the legs. Hitherto, despite the wide pharmaceutical range offered, there has been little medical treatment of proved value to prescribe and, in particular, vasodilator drugs have proved disappointing. This paper describes a double-blind controlled trial of naftidrofuryl (Praxilene), a new drug that is believed to facilitate oxygen exchange and enhance metabolism in ischaemic tissues.
The tendency for symptoms of claudication to improve during a period of observation is well known to those interested in vascular disease and probably accounts for the gross overprescribing of vasodilator or "vasoactive" drugs. This improvement occurred despite the fact that patients with apparently stable claudication were studied and that specific instructions on diet and smoking were withheld. In effect, by observing their own symptoms regularly, the patients were undertaking a programme of exercise similar to that commonly recommended for the condition.
We have been unable to show a benefit in our patients with claudication from oral naftidrofuryl. The administration of naftidrofuryl in more advanced vascular disease and by alternative routes may have different effects and requires separate study. [ABSTRACT FROM AUTHOR]
