Objective To describe the temporal and geographical distribution of Zika virus infection and associated neurological disorders, from 1947 to 1 February 2016, when Zika became a Public Health Emergency of International Concern (PHEIC). Methods We did a literature search using the terms "Zika" and "ZIKV" in PubMed, cross-checked the findings for completeness against other published reviews and added formal notifications to WHO submitted under the International Health Regulations. Findings From the discovery of Zika virus in Uganda in 1947 to the declaration of a PHEIC by the World Health Organization (WHO) on 1 February 2016, a total of 74 countries and territories had reported human Zika virus infections. The timeline in this paper charts the discovery of the virus (1947), its isolation from mosquitos (1948), the first human infection (1952), the initial spread of infection from Asia to a Pacific island (2007), the first known instance of sexual transmission (2008), reports of Guillain-Barré syndrome (2014) and microcephaly (2015) linked to Zika infections and the first appearance of Zika in the Americas (from 2015). Conclusion Zika virus infection in humans appears to have changed in character as its geographical range has expanded from equatorial Africa and Asia. The change is from an endemic, mosquito-borne infection causing mild illness to one that can cause large outbreaks linked with neurological sequelae and congenital abnormalities. Problem In low-and middle-income countries, breast and cervical cancer have a poor prognosis, partly owing to barriers to treatment. To redress this situation, health systems must be strengthened. Approach Pink Ribbon Red Ribbon (PRRR) is an innovative partnership designed to leverage public and private investments in global health and to build on the successful United States President's Emergency Plan for AIDS Relief (PEPFAR) platform to combat cancers of the breast and cervix in sub-Saharan Africa and Latin America. By supporting a comprehensive set of country-owned and country-driven interventions, PRRR seeks to reduce deaths from cervical cancer among women screened and treated through the programme and to reduce deaths from breast cancer by promoting early detection. Local setting In its initial phase, PRRR is supporting the governments of Botswana, Zambia and other countries in expanding cervical cancer prevention, screening and treatment coverage - especially to high-risk women with human immunodeficiency virus infection - and in strengthening breast cancer education and control services. Relevant changes PRRR has introduced a diagonal strategy based on the life course and continuum of care approaches to cancer control. Its work has resulted in the delivery of the human papillomavirus vaccine to young girls in several settings and in the strengthening of prevention, screening and treatment delivery systems from the community to the tertiary level. Lessons learnt This paper outlines the approach PRRR has taken as a country-aligned public-private partnership and the preliminary lessons learnt, including the need for flexible implementation, effective country coordination mechanism and regular communication with all stakeholders. Objective To describe the different types of data sets on aid flows, what they capture and the types of questions they answer, and to explore the extent of variation in levels and trends between these data sets at the regional and country levels. Methods Data included in the database of the World Health Organization are derived from official country documents and are published annually after review by each country. In addition to such data, the authors extracted data from publicly available web sites. The data extracted covered all aid flows from all donors specified for sub-Saharan African countries (including aid for the African region as a whole or for groups of countries in the region) as being for health. Findings The variation in levels and trends in development assistance for health across the six data sets compared in this paper was substantial. Variation was greater at the country than at the regional level, partly because the different aggregates of development assistance for health have different meanings and partly because of incomplete reporting. Conclusion It is important to know what the different aggregates of development assistance for health reported in the different databases mean before deciding which ones to use to answer a particular policy question. Objective Literature on human resources for health in Africa has focused on personal health services. Little is known about graduate public health education. This paper maps "advanced" public health education in Africa. Public health includes all professionals needed to manage and optimize health systems and the public's health. Methods Data were collected through questionnaires and personal visits to departments, institutes and schools of community medicine or public health. Simple descriptive statistics were used to analyse the data. Findings For more than 900 million people, there are fewer than 500 full-time staff, around two-thirds of whom are male. More men (89%) than women (72%) hold senior degrees. Over half (55%) of countries do not have any postgraduate public health programme. This shortage is most severe in lusophone and francophone Africa. The units offering public health programmes are small: 81% have less than 20 staff, and 62% less than 10. On the other hand, over 80% of Africans live in countries where at least one programme is available, and there are six larger schools with over 25 staff. Programmes are often narrowly focused on medical professionals, but "open" programmes are increasing in number. Public health education and research are not linked. Conclusion Africa urgently needs a plan for developing its public health education capacity. Lack of critical mass seems a key gap to be addressed by strengthening subregional centres, each of which should provide programmes to surrounding countries. Objective The Global Fund to Fight AIDS, Tuberculosis and Malaria is one of the largest funders to fight these diseases. This paper discusses the programmatic contribution of Global Fund-supported programmes towards achieving international targets and Millennium Development Goals, using data from Global Fund grants. Methodology Results until June 2006 of 333 grants supported by the Global Fund in 127 countries were aggregated and compared against international targets for HIV/AIDS, tuberculosis and malaria. Progress reports to the Global Fund secretariat were used as a basis to calculate results. Service delivery indicators for antiretrovirals (ARV) for HIV/AIDS, case detection under the DOTS strategy for tuberculosis (DOTS) and insecticide-treated nets (ITNs) for malaria prevention were selected to estimate programmatic contributions to international targets for the three diseases. Targets of Global Fund-supported programmes were projected based on proposals for Rounds 1 to 4 and compared to international targets for 2009. Findings Results for Global Fund-supported programmes total 544 000 people on ARV, 1.4 million on DOTS and 11.3 million for ITNs by June 2006. Global Fund-supported programmes contributed 18% of international ARV targets, 29% of DOTS targets and 9% of ITNs in sub-Saharan Africa by mid-2006. Existing Global Fund-supported programmes have agreed targets that are projected to account for 19% of the international target for ARV delivery expected for 2009, 28% of the international target for DOTS and 84% of ITN targets in sub-Saharan Africa. Conclusion Global Fund-supported programmes have already contributed substantially to international targets by mid-2006, but there is a still significant gap. Many of the first European settlers in Africa sought refuge from the heat and diseases of the plains by moving to the cool and salubrious highlands. Although many of the highlands were originally malaria free, there has been a progressive rise in the incidence of the disease over the last 50 years, largely as a consequence of agroforestry development, and it has been exacerbated by scarce health resources. In these areas of fringe transmission where the malaria pattern is unstable, epidemics may be precipitated by relatively subtle climatic changes. Since there is little immunity against the disease in these communities, outbreaks can be devastating, resulting in a substantial increase in morbidity and death among both children and adults. We present here the results obtained using a mathematical model designed to identify these epidemic-prone regions in the African highlands and the differences expected to occur as a result of projected global climate change. These highlands should be recognized as an area of special concern. We further recommend that a regional modelling approach should be adopted to assess the extent and severity of this problem and help improve disease surveillance and the quality of health care delivered in this unstable ecosystem.

More than 20% of total health expenditure in 48% of the 46 countries in the WHO African Region is provided by external sources. Issues surrounding aid effectiveness suggest that these countries ought to implement strategies for weaning off aid dependency. This paper broaches the following question: what are some of the strategies that countries of the region can employ to wean off donor funding for health? Five strategies are discussed: reduction in economic inefficiencies; reprioritizing public expenditures; raising additional tax revenues; increased private sector involvement in health development; and fighting corruption.

Health care is a basic service essential in any effort to combat poverty, and is often subsidized with public funds to help achieve that aim. This paper examines public spending on curative health care in several African countries and finds that this spending favours mostly the better-off rather than the poor. It concludes that this targeting problem cannot be solved simply by adjusting the subsidy allocations. The constraints that prevent the poor from taking advantage of these services must also be addressed if the public subsidies are to be effective in reaching the poor.

This paper discusses five parasitic diseases: American trypanosomiasis (Chagas disease), dracunculiasis, lymphatic filariasis, onchocerciasis and schistosomiasis. The available technology and health infrastructures in developing countries permit the eradication of dracunculiasis and the elimination of lymphatic filariasis due to Wuchereria bancrofti. Blindness due to onchocerciasis and transmission of this disease will be prevented in eleven West African countries; transmission of Chagas disease will be interrupted. A well-coordinated international effort is required to ensure that scarce resources are not wasted, efforts are not duplicated, and planned national programmes are well supported.

The biogeographical examination of rodent faunas associated with arenaviruses reveals two distinct patterns. Lymphocytic choriomeningitis (LCM) virus is associated primarily with a single murid species, Mus musculus, although it is also known to cause laboratory infections in other species. On the other hand, the arenaviruses from the Western hemisphere are associated exclusively with a large and diverse group of cricetid rodents. Studies to date, although limited, have not demonstrated their association with any other rodent groups, although in South America alone at least twelve other rodent families are known. Evidence at the present time indicates that Lassa virus is only associated with a common African rodent, Mastomys natalensis. From this limited evidence it is as yet difficult to determine whether Lassa virus will follow the pattern of the South American arenaviruses, most of which are known from several species of rodents, or that of LCM virus, which appears to be associated with only a single rodent species. In this paper, the history and structure of South American, Eurasian, and African rodent faunas are described.

The recent discovery that Simulium damnosum in Africa is not a single species but a complex of sibling species has important implications for the epidemiology and control of onchocerciasis. This paper sets out the current situation with regard to classification within the S. damnosum complex and the smaller S. neavei group. The nomenclature of the S. damnosum complex and current morphological and enzyme electrophoretic methods of differentiating among sibling species are discussed. Finally, recommendations are made on priorities for future research.
