To investigate certain aspects of hydrocephalus in patients with myelomeningocele. We retrospectively analyzed data of 1050 patients with myelomeningocele who underwent surgical treatment between June 1991 and June 2021. These patients were divided into three groups: group 1 consisted of patients who underwent surgery after the first 6 h of life, group 2 consisted of patients who underwent surgery within the first 6 h, and group 3 consisted of patients who underwent surgery during the fetal period and before 26 6/7 weeks of gestation. There were 125, 590, and 335 patients in groups 1, 2, and 3, respectively. In groups 1 and 2, 593 (83%) patients developed hydrocephalus after birth and required ventriculoperitoneal shunt placement in the maternity ward, mainly within the first 4 days of life. In contrast, in group 3, 24 (7.2%) patients required surgery to treat hydrocephalus after birth. Hydrocephalus was the primary cause of mortality in groups 1 and 2, with mortality rates of 35% and 10%, respectively. In group 3, the mortality rate was 0.8% and was not related to hydrocephalus. The onset of hydrocephalus is directly related to myelomeningocele closure in neurosurgery.

Ventriculo-subgaleal shunt is an established treatment of hydrocephalus following germinal matrix haemorrhage in low birth weight neonates. It is also used in treatment of post-infective hydrocephalus in children. We intend to emphasise the impact of its extended use in multiple clinical conditions to reduce the number of permanent shunt implantation in infants. Retrospective review of clinical cases in a single institution from medical records. VSG shunts with low-pressure valve system were useful in variety of hydrocephalus in infants (post-haemorrhagic, post-infective, post-myelomeningocele, post-shunt block, post-traumatic, hydrocephalus associated with brain tumours). A significant number of infants especially those with post-haemorrhagic and post-myelomeningocele hydrocephalus could be made free of permanent shunt placement. Ventriculo-subgaleal shunt is an effective, less risky temporary solution of hydrocephalus in infants and can be used in a variety of hydrocephalus in children and helps in avoiding shunt dependency in some of them.

While cases of acquired Chiari I malformation following ventriculoperitoneal shunting for posthemorrhagic hydrocephalus have been reported, true disproportionate cerebellar growth is rare, with no previous cases requiring posterior fossa decompression reported. We present a premature neonate who underwent ventriculoperitoneal shunt placement for suspected posthemorrhagic hydrocephalus. He subsequently developed a symptomatic Chiari I malformation with volumetric measurements demonstrating disproportionate growth of the cerebellum. He did not demonstrate thickening of the supratentorial or posterior fossa cranium. The patient underwent an extradural posterior fossa decompression, with resolution of symptoms. We review the extant literature regarding the development of Chiari malformation type I as a manifestation of craniocerebral disproportion (CCD) following shunt placement for posthemorrhagic hydrocephalus of prematurity. Most previous reports reflect a mechanism that includes underdevelopment of the intracranial posterior fossa (or supratentorial) volume. The case presented in this report, as well as one additional case, indicates that there may exist a variant mechanism, characterized by rapid growth of the cerebellum itself, in the absence of one of the rare syndromes associated with primary macrocerebellum. While this case was effectively managed with extradural posterior fossa decompression, previous reports indicate that supratentorial cranial expansion procedures are preferable in some cases. As such, pediatric neurosurgeons should be able to distinguish the patterns of craniocerebral disproportion when considering treatment options for these patients. Further investigation regarding these uncommon patients may better describe the underlying mechanisms.

There is still no standard protocol for management of patients of tubercular meningitis (TBM) with hydrocephalus in poor neurological grade. In general, a trial of external ventricular drain (EVD) is an accepted method of treatment to decide whether a particular patient will benefit from shunt surgery. However, recent studies suggest that ventriculoperitoneal (VP) shunt may be undertaken without the trial of an EVD. Our study prospectively evaluates the role of direct VP shunt placement in poor grade patients of TBM with hydrocephalus. Twenty-six consecutive pediatric patients of TBM with hydrocephalus in Palur grades III and IV underwent direct VP shunt placement, without prior placement of EVD. Outcome was assessed at the end of 3 months using Glasgow Outcome Score. The mean age of patients was 3.3 years (range, 4 months to 11 years). Twenty-one (80.8 %) patients were in grade III and five (19.2 %) were in grade IV. Good outcome and mortality in grade IV patients was 20 % (1/5) and 60 % (3/5) respectively; whereas in grade III patients, it was 71.4 % (15/21) and 9.5 % (2/21), respectively. Thirteen patients presented with focal neurological deficit at admission, which persisted in only three patients at 3 months follow up. VP shunt-related complications were observed in six (23.5 %) patients Despite poor grade at admission, 71.4 % patients in grade III and 20 % patients in grade IV had a good outcome at 3 months follow-up. Direct VP shunt placement is a safe and effective option even in poor grade patients of TBM with hydrocephalus, with a low complication rate.

We aim to correlate the frequency of infections after ventriculoperitoneal (VP) shunt placement in neonates with myelomeningocele (MMC) who did not receive prophylactic antibiotics to the timing of VP shunt placement and the frequency of cerebrospinal fluid (CSF) leakage at the MMC wound.Fifty-nine newborns with MMC underwent VP shunt insertion in the period 1983-2007. We reviewed retrospectively all records.After MMC closure, 24 out of 59 newborns had an infection. The relative risk (RR; 95%) of having an infection is significantly higher [RR = 4,69 (1.145397-19.23568; P = .03761817)], and neuroinfection showed a tendency towards RR = 3.5 (.7067445-17.03112; P = .15414095) in newborns without symptomatic hydrocephalus at birth when we had a wait-and-watch policy (late shunt placement) compared with newborns with prompt shunt placement. The RR (95%) of having an infection [RR = 6,8 (3.314154-13.95228; P = 1.235e-07)] and also neuroinfections [RR = 4,76 (2.043019-11.09025; P = .00044478)] was highly significant if the child presented with MMC wound with CSF leakage before VP shunt insertion (Table 3).Centers with a conservative antibiotic policy should be even more careful to avoid CSF leakage before shunt placement as this gives a highly significant increased risk of both infections in total and neuroinfections, and they should reconsider this conservative policy in newborns with MMC due to the significantly high infection rate.

The outcome of endoscopic third ventriculostomy (ETV) is worse in children younger than 2 years old and especially in infants, and controversies still exist whether ETV might be superior to shunt placement in this age group. We retrospectively analyzed the data of 23 patients younger than 6 months of age treated with ETV and assessed its feasibility as a first choice of treatment for hydrocephalus.Between 1994 and 2008 in our clinic, 23 patients younger than 6 months having presented with obstructive hydrocephalus were treated endoscopically. The etiology of hydrocephalus was congenital aqueduct stenosis in 11 patients, posthemorrhagic obstruction in six patients, myelomeningocele in two patients, postmeningitis in two patients, Chiari I malformation in one patients, and Dandy walker variant in one patient. ETV was considered successful when no shunt operation was needed in the patient.ETV was successful in eight patients with regression of intracranial hypertension. In the remaining 15 patients, ventriculoperitoneal shunt implantation was necessary. Total success rate in our group of patients was 34.8%. In patients younger than 3 months of age (n=12), success rate was 25.0%. In patients from 3 to 6 months of age (n=11), success rate was 45.5%. Complication included intraventricular hemorrhage in one patient, meningitis and cerebrospinal fluid leak in one patient, and meningitis in one patient.Based on our experience, ETV could be the first method of choice for hydrocephalus in children younger than 6 months of age, especially in patients older than 3 months of age.

Significant morbidity from ventricle shunt overdrainage at 6-7 years after initial shunt placement for hydrocephalus is increasingly recognized as due to excessive gravity-flow of shunted CSF when upright. Shunts are designed primarily to control high ICP. Shunts should also mimic normal upright ICP. Normal upright ICP is -65 mm of water (vertex reference), indicating that a level of zero ICP exists at 65 mm below the brain vertex, with negative ICP above and positive ICP below that level. This normal zero ICP level must be maintained by CSF shunts to mimic normal upright ICP. This will prevent and correct CSF shunt overdrainage. The zero ICP shunt (ZIPS) by design controls this zero level with a zero pressure device (ZPD; siphon control device) installed at the normal vertical level of zero ICP (cm/mm) below the vertex (65 mm). The shunt thus prevents excessive gravity-induced CSF shunt flow. Successful use of ZIPS in 56 patients is reported (low ICP group: n = 42; high ICP group: n = 14). Follow-up is up to 4.5 years. Results show that: (1) adjustability of ZPD level can achieve the desired clinical results; (2) the level of ZPD installed correlates within 4 mm of upright ICP attained; (3) the optimal level of ZPD installation to produce normal upright ICP is 65 mm below the vertex; (4) CT ventricle size, both slit ventricles and large ventricles, may or may not normalize when normal upright ICP is attained in this group of complex, previously shunted patients.

The learning ability of congenitally hydrocephalic HTX rats in which hydrocephalus had been arrested by the insertion of a V-P shunt 7 days after birth (early shunt) was assessed by means of the light-darkness discrimination test when the animals reached maturity. Early shunt placement resulted in marked reduction in size of abnormally enlarged ventricles and the prevention of both decreasing spine density and decay of synaptic vesicle protein (SVP-38) in the affected cerebral cortex. The learning ability of such animals was also found not to be impaired compared with that of animals in a sham-operation group. On the basis of these investigations, it is concluded that early shunt placement may have a beneficial role in preventing not only impairment of synaptogenesis of the brain by progressing hydrocephalus, but also learning disability. Recent biochemical investigation of the developing brains of hydrocephalic HTX rats revealed problems that cannot be resolved by early shunt insertion, and these are also discussed.

A technique is presented for posterior fossa cyst shunt placement from a supratentorial location utilizing intraoperative ultrasonography.

The optimal management of patients with myelomeningocele and hydrocephalus is facilitated by a constant review of the patients with the aid of the sonogram and CT scan. Six infants treated with simultaneous shunt placement and myelomeningocele repair were compared with six other neonates treated with conventional sequential myelomeningocele repairs requiring a second separate procedure for shunt placement. Infants operated upon simultaneously experienced no increase in morbidity or mortality and appeared to benefit substantially.
