Objectives: To analyze a possible association between glutathione-S-transferase T1 (GSTM1) and/or glutathione-S-transferase M1 (GSTM1) polymorphisms and chronic or aggressive forms of periodontitis in a Caucasian ethnic group. Design and methods: Sixty-nine chronic, 14 aggressive periodontitis and 61 controls, deeply analyzed from a clinical point of view, were studied for their GSTM1 and GSTT1 polymorphisms by allelic specific PCR techniques. As a second control group, 64 blood donors were also included. Results: A significant association was found between GSTM1-null genotype and both chronic and aggressive periodontitis. The aggressive forms were associated with the double null GSTM1 and GSTT1 combination. These results were independent of the patients' age, gender, hygienic habits and smoke (evaluated as tobacco smoking yes/no, cigarettes/day and pack years) as confirmed by multivariate logistic regression analysis. Conclusions: The GSTM1-null variant is statistically associated with the two forms of periodontitis, while the aggressive one also presents a second null variant: GSTT1.

- Abstract
Objective: The usefulness, the methods and the criteria of intra-operative monitoring of the parathyroid hormone (ioPTH) during parathyroidectomy (PTX) for renal secondary hyperparathyroidism (rSHPT) in patients on chronic hemodialysis remain still matter of debate. The present study aimed to evaluate the ability of a low cost central-laboratory second generation PTH assay to predict an incomplete resection of parathyroid glands (PTG)., Methods: The ioPTH decay was determined In 42 consecutive patients undergoing PTX (15 subtotal and 27 total without auto-transplant of PTG) for rSHPT. The ioPTH monitoring included five samples: pre-intubation, post-manipulation of PTG and at 10, 20 and 30min post-PTG excision. The patients with PTH exceeding the normal value (65pg/ml) at the first postoperative week, 6 and 12months were classified as persistent rSHPT., Results: The concentrations of ioPTH declined significantly over time in patients who received total or subtotal PTX; however, no difference was found between the two types of PTX. Irrespective of the type of PTX and the number of PTG removed, combining the absolute and percentage of ioPTH decay at 30min after PTG excision, we found high sensitivity (100%), specificity (92%), negative predictive value (100%) and accuracy (93%) in predicting the persistence of rSHPT., Conclusions: The monitoring of the ioPTH decline by a low cost central-laboratory second generation assay is extremely accurate in predicting the persistence of disease in patients on maintenance hemodialysis undergoing surgery for rSHPT., (Copyright © 2016 The Canadian Society of Clinical Chemists. - Abstract
Objectives: RI-PTH measurements are a prerequisite for minimally invasive parathyroidectomy, providing guidance regarding the removal of hyper-functioning tissue. Different criteria of PTH decrease, concentration and clearance were analyzed in order to predict surgical treatment., Design and Methods: Blood samples at pre-incision, manipulation, 5, 10 and 20 min after resection, were collected from 145 patients presenting unambiguous, pre-surgical "single adenoma" diagnosis., Results: The meeting of Irvin criterion would have permitted the identification of 28% uncured cases leading to 4% unnecessary neck exploration. On the contrary, we would have identified all of the uncured patients, to the detriment of 7% unnecessarily prolonged procedure by taking into account PTH drop, concentration and clearance shape at 20 min., Conclusions: The 20' end-point plays a key role in the correct determination of surgical outcome, strongly improving the possibility of adequate patient treatment. However, since the high success rate of traditional parathyroidectomy, yet not provided by RI-PTH, the utmost improvement to hyper-parathyroidism surgical treatment by RI-PTH could be achieved in pre-operative equivocal glands localization or multiglandular disease selected population to quickly guide and confirm the complete removal of all hyper-secreting tissue.
