To evaluate the mRNA expression levels of cytokines interferon-γ, tumour necrosis factor-α, interleukin IL-1β, IL-10, and the chemokine CCL2/MCP-1, CCL4, and CXCR4 in the periapical interstitial fluid from root canal infections before and after bacterial load reduction in patients undergoing haematopoietic stem cell transplantation (HSCT). The case group was composed of 10 patients undergoing HSCT, and our control group included 10 healthy patients. Clinical samples were taken from teeth with pulp necrosis. To evaluate the selenium (Se) behavior when used as an endodontic dressing in teeth with pulp necrosis. Additionally, its effects was also compared with the calcium hydroxide (C.H.), which is used globally as a root canal dressing, and the combination of the C.H. with Se (C.H. + Se). The sample consisted of 60 patients requiring endodontic treatment who were divided into groups, i.e., without intracanal medication (empty) and with medications as follows: selenium (Se), calcium hydroxide (C.H.), and calcium hydroxide + selenium (C.H. + Se) (n = 15). The purpose of the present study was to test the hypothesis that apical extrusion of the irrigant occurs during laser-driven irrigation in vivo. Three hundred human root canals, in 181 patients, were divided into two groups: the no lesion group (n = 140) and the lesion group (n = 160). All the root canals were enlarged using a crown down technique up to size 30–80 K-files, depending on the original condition of the root canal. For the final irrigation, the teeth were irrigated with a mixture of radiopaque contrast medium and NaOCl in solution. The solution was activated for 60 s in teeth with one canal or two canals and for 120 s in teeth with three or four canals. Radiopaque contrast medium was absent from the periapical tissues of all samples. No contrast medium was observed radiographically in the periapical tissues. The hypothesis that apical extrusion of root canal irrigants occur during laser cavitation was rejected It appears that the power of the laser used at 1 W for 1–2 min can drive the irrigation solution to the tip of the canal without harming the apical tissues.

The aim of this study was to compare two different thermoplastic techniques—a core-carrier technique (Thermafil) and warm vertical compaction—in terms of overextension of root canal filling in vivo. Flaring of 88 teeth was conducted using Pro Files .04 as finishing files, and the teeth were obturated using Thermafil. Flaring of 74 teeth was performed using Pro Files .06 as finishing files, and the teeth were obturated using warm vertical compaction. Seventy (80 %) of the teeth obturated using Thermafil and 31 (42 %) teeth obturated using warm vertical compaction show extruded root canal filling. In contrast to Thermafil, there is a higher rate of extruded root canal filling of teeth with more than one root canal using warm vertical compaction. Thermafil demonstrated a higher rate of extruded root canal filling compared to warm vertical compaction. Warm vertical compaction is a more predictable method of filling compared to Thermafil. Root canal filling extrusion will cause irritation of the surrounding tissue and impair repair processes. In the present in vivo study, there was a higher rate of root canal filling extrusion using Thermafil compared to warm vertical compaction.

Root canal therapy (RCT) and tooth extraction have been conventional treatment options for management of human mature teeth with irreversible pulpitis. Excellent short-term treatment outcomes of vital pulp therapy with calcium-enriched mixture cement (VPT/CEM), as a new treatment option, on postoperative pain relief was demonstrated; if intermediate- and long-term treatment outcomes of the new treatment are also non-inferior compared to RCT, then VPT/CEM may become a viable treatment option for management of mature teeth with irreversible pulpitis.In 23 healthcare centers, 407 9- to 65-year-old patients were randomly allocated into two study arms including one-visit RCT (reference treatment; n = 202) and VPT/CEM (alternative treatment; n = 205). Six- and twelve-month clinical and radiographic successes were assessed.Mean follow-up times at 6- and 12-month follow-ups were "6.70 ± 0.68 and 6.72 ± 0.71 months" and "12.96 ± 0.67 and 12.90 ± 0.66 months" in the available cases of RCT and VPT/CEM arms, respectively. Favorable clinical success rates in the two study arms did not show statistical difference; however, the radiographic success rate in the VPT/CEM was significantly greater than RCT arm at the two follow-ups (P0.001). The patients' age had no effect on the treatment outcomes (P = 0.231).Treatment outcomes of VPT/CEM may be superior to RCT in mature molars with irreversible pulpitis. The performance of biomaterials such CEM cement may assist in the shift towards more biologic treatments.VPT/CEM may be a realistic alternative treatment for human mature molar teeth with symptoms of irreversible pulpitis; the use of VPT/CEM is highly beneficial for patients as well as general dentists.

Aim The aim of this study is to examine the survival distributions of primary root canal treatment using interval-censored data and to assess the factors affecting the outcome of primary root canal treatment, in terms of periapical healing and tooth survival. Materials and methods About one tenth of primary root canal treatment performed between January 1981 and December 1994 in a dental teaching hospital were systematically sampled for inclusion in this study. Information about the patients' personal particulars, medical history, pre-operative status, treatment details, and previous review status of the treated teeth, were obtained from dental records. Patients were recalled for examination clinically and radiographically. Treatment outcomes were categorized according to the status for periapical healing and tooth survival. The event time was interval-censored and subjected to survival analysis using the Weibull accelerated failure time model. Results A total of 889 teeth were suitable for analysis. Survival curves of both outcome measures (periapical healing and tooth survival) declined in a non-linear fashion with time. Median survival of the treated teeth was 119 months (periapical healing) and 252 months (tooth survival). Age, tooth type, pre-operative periapical status, occlusion, type of final restoration, and condition of the tooth/restoration margin were significant factors affecting both periapical healing and tooth survival. The aim of this study was to test whether the Third Molar Eruption Predictor, developed for panoramic radiographs, can also be applied to periapical radiographs. The Third Molar Eruption Predictor is a transparent device (US patent 5,816,814), not commercially available, and developed for prediction of future eruption or impaction of third molars. The material consisted of periapical radiographs of mandibular third molars taken from the lateral projection without angulation of the central beam. The radiographs taken at the mean age of 20.6 years (SD +/- 1.4 years) were retrospectively available from a four-year follow-up study of third molars carried out at the University of Copenhagen, Denmark. Initially unerupted or partially erupted third molars (n=43) in 28 dental students were analyzed. Clinical statuses of the third molars were available at baseline and four years later. The device was calibrated using the method of simple proportions and Bayes' Decision Theory. The predictions made with the calibrated device were in conformity with the final clinical outcome in 84% of the cases. It was concluded that the Third Molar Eruption Predictor may also be used on periapical radiographs after calibration of the device.

This study evaluated the presence and density of natural killer (NK) cells as well as collagen density in chronic apical periodontitis lesions and tried to find any correlations with concomitant herpesvirus infection or histopathological status of the lesion. Surgical specimens of chronic apical periodontitis lesions were surveyed for the presence and density of NK cells by immunohistochemical analysis. Collagen density in these lesions was quantified by means of histochemistry. All specimens were positive for the presence of CD57-positive cells. Topographically, CD57-positive cells were found singly or forming clusters in the granulomatous tissue, as well as subjacent and within the cystic epithelium. No significant differences in the density of CD57-positive cells were found between nonepithelialized and epithelialized lesions or between herpesvirus-positive and herpesvirus-negative lesions. Significant differences were found in volumetric density of collagen when comparing nonepithelialized and epithelialized lesions, with the latter demonstrating higher values. When no distinction of lesion type was made, there was no significant difference in collagen density between herpesvirus-positive and herpesvirus-negative lesions. When comparing the collagen density in herpesvirus-positive and herpesvirus-negative specimens from the same lesion type, a significant difference was found in nonepithelialized lesions, with herpesvirus-positive lesions showing lower values. The presence of CD57-positive cells in all chronic apical periodontitis specimens may indicate that activated NK cells play a role in the pathogenesis of this disease, possibly by participating in innate immunity events involved in the control of virus infection. Collagen density may vary in function of the type of lesion and presence of herpesvirus infection.

In order to minimize the incidence of local and/or systemic side effects, the biocompatibility of all endodontic materials should be investigated by various in vitro and in vivo tests prior to clinical application. The battery of in vitro tests includes determinations of mutagenicity, cytotoxicity, and antibacterial effects. Several reports have shown that paraformaldehyde-containing ZnO-eugenol cements in particular, such as Endomethasone and N2, are antibacterial. On the other hand, it has been found that endodontic materials with strong antimicrobial activity are frequently mutagenic, i.e., primarily those which release formaldehyde. Cell culture tests clearly show significantly different cytocompatibility of the various types of endodontic sealers: in general, formaldehyde-containing ZnO-eugenol cements are classified as highly/extremely cytotoxic, whereas most Ca(OH)2-based sealers are rated as possessing good or excellent cytocompatibility. These results were confirmed by numerous histological studies in vivo. Sealers with inferior biocompatibility, such as formaldehyde-releasing materials, should no longer be applied in practice because safer alternatives are available.
