Ren syndrome is a common developmental-neurologic disorder that has been reported almost exclusively in females. Recent work has improved recognition of this condition and helped to clarify the management of this disorder for affected individuals. The primary-care physician can become a major source of support and advocacy for the family of a girl with Rett syndrome. Many other resources are available to the primary care giver and the families of children with Rett syndrome; these may help to provide early diagnosis, psychological support, and preventive medical care for these individuals. Chronic recurrent multifocal osteomyelitis (CRMO) is an unusual inflammatory process involving multiple osseous sites, often recurrently. The authors recently diagnosed CRMO in an 8-year-old girl who had complained of toe pain for several weeks. A number of other involved, asymptomatic areas were discovered by bone scan. Biopsy of the lesions did not reveal any pathogens. Antibiotics and inflammatory agents were not used, and the patient recovered. A review of previously reported cases of CRMO showed 1) that this entity affects children almost exclusively, 2) that from two to 18 sites, usually in the extremities, can be affected at one time, and 3) that about 20 percent of the patients develop a pustular rash on their palms and soles. Other than an elevated erythrocyte sedimentation rate, there are no consistent laboratory abnormalities, and cultures of affected bone are negative. Histopathology typically shows a chronic inflammatory response, sometimes with granulomas. Antiinflammatory agents and antibiotics offer little consistent beneficial effect. The disease is self-limited, and patients usually recover fully, although exacerbations may occur. In the proper clinical setting, CRMO should he considered, since recognition of this entity avoids costly and potentially harmful diagnostic and therapeutic interventions. [ABSTRACT FROM AUTHOR]

A case is reported of isolated cryptococcal osteomyelitis in an 11-year-old girl without any detectable underlying disease. The patient was successfully treated for six weeks with intravenous amphotericin B and oral 5-flucytosine. There is only one other case of cryptococcal osteomyelitis reported in the English medical literature in a child. Goodpasture's syndrome is characterized by pulmonary hemorrhage and rapidly progressive glomerulonephritis. The disease results from circulation antiglomerular basement membrane antibodies that react with the basement membrane of both the renal glomeruli and the pulmonary alveoli. It is most commonly a disease of young adult males, and reports in young females in the pediatric age group are uncommon. This paper presents the clinical history of an adolescent female who recovered from life-threatening pulmonary hemorrhage with treatment that included aggressive cardiopulmonary support combined with plasma exchange and immunosuppressive therapy.

Primary spontaneous cerebrospinal fluid (CSF) rhinorrhea is a rare entity, especially in children. Various methods now exist for establishing its diagnosis. This paper presents the case of a twelve-year-old girl in whom CSF rhinorrhea was documented after she experienced two episodes of meningitis. The disease was associated with a cribriform plate defect that was subsequently closed surgically with complete cure.

This article cites a study determining whether a relationship exists between perineal erythema and urinary infection in girls. For this study, 75 girls with urinary infection and 1,652 normal girls, all under 12 years of age, in a middle- to upper-middle class Caucasian population near Los Angeles, California, had urine cultures taken and were studied for perineal erythema. Each child's perineal area was examined for erythema by two independent observers. It was obtained that 15 out of a consecutive series of 75 girls who had urinary infection also had perineal erythema at the time of diagnosis. The urinary organisms recovered from the 15 girls with perineal erythema and urinary infection were: ten, Escherichia coli, four, Proteus, and one Staphylococcus aureus. Observations of this study suggest that a urinary infection should be considered in the differential diagnosis of all infants with persistent "diaper rash" and of all older girls with perineal erythema.

The author describes the medical case of an eight-year-old Caucasian girl who was evaluated for possible leukemia. The girl was clearly retarded and had a facial appearance almost suggestive of mongolism. With a working diagnosis of idiopathic acquired aplastic anemia, the child was started on a therapeutic program of prednisolone and testosterone propionate. The case illustrates the problem that may occur in differentiating a hematologic problem in a mongol from Fanconi's anemia or from idiopathic aplastic anemia. Fanconi's anemia generally responds to androgen therapy but relapses when it is withdrawn. Idiopathic aplastic anemia responds to androgen therapy about 25 per cent of the time and does not relapse with withdrawal of therapy. The passage of time confirmed the diagnosis of aplastic anemia with unknown cause.

The author describes the medical case of a four-year-old girl with jaundice and an enlarged spleen and who was diagnosed with hemolytic anemia of unknown etiology. The child was given a blood transfusion because of rapidly falling hemoglobin levels and was discharged at the request of the parents. The child was brought back three months later for evaluation. The blood smears taken were studied because of the slight increase in erythrocyte fragility and showed many spherocytes. The father also had spherocytes in his blood smears. The diagnosis of hereditary spherocytic anemia was made. The lesson to be learned from the case is that hereditary spheroctosis must always be thought of, whenever there is a suggestion of a hemolytic process.
