To highlight the state of the art and the current outlook on the adherence to treatment in asthma, starting from the ‘Manifesto on Adherence to asthma treatment in respiratory allergy’ endorsed by the World Allergy Organization, Allergic Rhinitis and Its Impact on Asthma and Global Allergy, Asthma European Network, and Interasma.

Guidelines for clinical practice are expected to gather evidence-based recommendations to support optimal medical behaviours. The aim of the current review is to explore how currently available research regarding the strategy of using budesonideformoterol (BUDFORM) as maintenance and reliever therapy (Symbicort SMART) covers the items considered by the Grade of Recommendations, Assessment, Development and Evaluation (GRADE) system, through a comparative analysis of methodological approaches, clinical outcomes, patient-reported outcomes and costs, in order to highlight uncovered areas.

The aim of this review is to evaluate the factors related to the doctor and the patient that could make following the guidelines difficult. It also underlines the importance of the adherence to guidelines themselves in order to ameliorate both the control of healthcare and the patients' quality of life.
