BACKGROUND Interface pressure and stiffness characterizing the elastic properties of the material are the deciding parameters determining the dosage of compression treatment and should therefore be measured in future clinical trials. AIM The aim of this consensus paper is to provide some recommendations regarding the use of suitable methods for this indication. METHOD This paper was formulated based on the results of an international consensus meeting between a group of medical experts and representatives from the industry held in January 2005 in Vienna, Austria. RESULTS Proposals are made concerning methods for measuring the interface pressure and for assessing the stiffness of a compression device in an individual patient. BACKGROUND. This paper compares the scalp kinetics of the scalp-lifting procedures with and without an intact occipital neurovascular bundle. OBJECTIVE. The objective of this paper is to perform lateral scalp-lifts and observe if an intact occipital neurovascular bundle hampers the amount of scalp advancement. METHODS. Ten consecutive patients had lateral scalp-lifts performed without severing the occipital neurovascular bundle. Advancements were then measured at three points (mid-posterior, above the pinna, and at the temporal recession). The occipital neurovascular bundle was then divided, and the same measurements were made. RESULTS. An intact occipital neurovascular bundle impeded the amount of advancement that could be accomplished with scalp-lifting. The anterior vector was primarily affected. CONCLUSION. In light of the finding that an intact occipital neurovascular hampers the amount of advancement with scalp-lifting, the surgeon must weigh the advantages and disadvantages of both approaches (prior division of occipital neurovascular bundles versus leaving the bundles intact) when deciding upon the appropriate approach for the given patient. BACKGROUND. The upper lip is an important esthetic unit of the face and its reconstruction is a big challenge to the dermatosurgeon. OBJECTIVE. The aim of the paper is to present results of upper lip reconstruction for moderately sized defects with single island subcutaneously pedicled flaps, or in combination with additional local flaps. METHODS. Thirteen patients were operated upon mostly for malignant skin neoplasms. Defects of skin only were covered with island flaps planned individually to fit esthetic units of the face. Defects in skin and vermilion were covered with island flaps, and additionally and independently with flaps that reconstructed the vermilion. RESULTS. The healing process was in all the cases but one (partial flap necrosis) free of complications. Follow-up results were also very good functionally, and esthetically. [ABSTRACT FROM AUTHOR]

BACKGROUND. Follicular transplantation is a method of hair restoration surgery that uses hair in its naturally occurring groups called follicular units. By using the follicular unit exclusively in the transplant, the surgeon can move extensive quantities of implants in a single session to complete the hair restoration in as few sessions as possible. OBJECTIVE. To briefly review the salient features of follicular transplantation, to discuss how the patient should be evaluated for the follicular transplant procedure, and to provide guidelines for the planning of the first and subsequent transplant sessions. This paper will examine: 1) ways to anticipate future hair loss, 2) how to assess donor reserves, 3) what can be accomplished in a single transplant session, 4) how to plan for subsequent sessions, and 5) how to manage the patient with diffuse androgenetic alopecia. METHODS. Follicular transplantation is performed according to techniques detailed in a previous publication (Bernstein et al. Int J Aesthet Rest Surg 1995;3:119-32). RESULTS. Excellent cosmetic results can be achieved when there is meticulous attention to transplanting intact follicular units, when the extent of balding is accurately assessed, and when the finite nature of the donor supply is respected. CONCLUSIONS. Because of their small "physiologic" size, follicular implants enable the surgeon to transplant large numbers in a single session. Follicular transplantation will greatly increase the benefits of the hair restoration procedure when the patient is thoroughly evaluated and when the procedure is undertaken after careful short- and long-term planning. BACKGROUND. Because many men seeking hair transplantation possess insufficient donor hair, conservative strategies that create a "less-than-complete" head of hair are necessary. OBJECTIVE. This paper defines the "frontal forelock concept" and highlights its advantages. Also, the benefits from utilizing the "central density concept" in all hair transplant procedures are explained. METHODS. The author's 3-year experience with the frontal forelock in 65 men serves as the backdrop for the comments in this article. During the same period, the "central density concept" has been used in virtually all transplant procedures. RESULTS. Patient satisfaction with the forelock approach in the appropriate candidates has been excellent. Using the "central density concept" has achieved very satisfactory visual density. CONCLUSION. In men with present extensive baldness and in young men with the possibility of advancing to the same state, the frontal forelock concept utilizing "central density" is proposed as the ideal model for hair transplantation. BACKGROUND. How to organize a scientific paper and present it ala forum of one's peers is not a skill presently taught at many training programs in dermatology. OBJECTIVE. To enumerate the principles, rules, impressions, and helpful hints that will help in the preparation and delivery of a scientific talk. METHODS. An attempt has been made, from the authors experience, to assimilate and codify the requisite organizational and delivery skills necessary to accomplish this. CONCLUSIONS. Organization must begin early. The key is to keep in mind that new information is being conveyed to the audience. BACKGROUND. Skin appearance alterations as a result of dermatologic surgical procedures may produce temporary or permanent cosmetic defects. Basic camouflaging cosmetics and techniques can restore self-confidence and allow the surgical patient resume social contact. OBJECTIVE. This paper presents the dermatologic surgeon with a basic understanding of the principles and techniques employed lit cosmetic camouflaging. METHODS. BACKGROUND. Hyperthermia therapy was described previously to be effective for treating warts using a hot water technique as well as an ultrasound hyperthermia technique. Recently, two Other papers described controlled localized heat therapy for cutaneous warts. A new heat-generated application system and method has been developed for treating warts using a self-ad- ministered, self-adhesive disposable exothermic patch. OBJECTIVE. The objective of this study was to describe our clinical experience in using the exothermic patch to successfully treat patients with cutaneous recalcitrant warts who failed to respond to multiple antiwart therapy. METHODS. Two patients with recalcitrant warts that did not respond to other therapeutic modalities were treated with the exothermic patch with complete remission of their warts. Exothermic patches were used in this study that were specially constructed to provide long-lasting, continuous, and reproducible heating of the skin surface for at least 2 hours at a temperature level of 42-43°C, which will not burn or otherwise injure the skin. CONCLUSIONS. Warts are difficult to treat in part because there are no specific anti-Papillomavirus drugs or methods available. BACKGROUND. Surgical defects of the convex curve of the central nasal tip have always been difficult to repair. Most dermatologic surgeons use a full-thickness graft in this region. The pinch modification of the linear advancement flap was described by Peng et al in 1987 as a means of repairing this region. OBJECTIVE. The purpose of this paper is to discuss the effectiveness of using the Peng flap for repairs of the convex curve of the central nasal tip. METHODS. This flap was used on a series of 25 patients, all of whom had defects of the convex curve of the central nasal tip. RESULTS. Because the flap both rotates and advances, it shortens the length of the traditional advancement flap and increases flap survival rate. We found excellent cosmetic results for central nasal tip defects and had no complications of necrosis using this flap. CONCLUSION. We have found this to be an excellent flap for repairs of the convex curve of the central nasal tip, as the rotating arms of the flap conform to the three-dimensional shape and the lateral lines fit well into cosmetic lines of the nose. When repairing central nasal tip defects, we believe the Peng flap to be the flap of choice to re-create the convex curve of this region. [ABSTRACT FROM AUTHOR]

BACKGROUND. Laser resurfacing is rapidly becoming a widely used method of skin ablation for cosmetic improvement. The technique with which the instrument is used can make the difference between obtaining the desired result with a benign post-operative period and one with unnecessary complications. OBJECTIVE. This paper describes a technique that maximizes the benefits while minimizes the risks in using this highly advanced instrument. METHODS. We have limited the areas where multiple passes are done to areas that have rhytides; for the rest of the skin a single pass is done and the final carbonized eschar is not removed. RESULTS. Faster healing and less complications. Shorter operative and anesthesia times. Greater patient acceptance and satisfactory cosmetic results. CONCLUSIONS. This article focuses on a paper that emphasizes the importance of identifying and treating perineural invasion in cutaneous malignancies. The authors appropriately elaborate on the surprising distance that tumors may extend within the low resistance cleavage plane of the perineural sheath. For squamous cell carcinoma with perineural invasion, Mohs micrographic surgery (MMS) offers the advantage of obtaining tumor-free margins with conservation of the affected nerve and uninvolved tissues, and if negative margins are obtained, MMS is generally agreed to be the surgical approach with the lowest risk for local tumor recurrence.

The purpose of this July 1998 special issue of the journal "Dermatologic Surgery" is to present some of the latest studies and innovations in the field of dermatologic surgery from a European perspective. Over the past years researchers have had the opportunity to share ideas and expertise with colleagues from around the globe at scientific meetings as well as through the medical and surgical literature. "Dermatologic Surgery" has always welcomed the strong participation and contributions of dermatologic surgery colleagues worldwide as demonstrated both by inclusion of international editors on the journal's editorial board and by the frequent publication of papers of scientific merit from countries all around the globe.

This article presents the author's comments on the article "The Knudsen Nomenclature," by dermatologists Dowling B. Stough and George L. Bondar. Although a paper supporting a standardization of terminology, seems perhaps dry and not immediately enticing, the cogently argued case put forward by Stough and Bondar deserves serious attention. In this era of large sessions of micrografts, it is not only the physicians who are confused by what different authors are proposing. The patient, who is usually being charged a fee on a per-graft basis, does not easily know how this translates into the amount of hair transplanted.

In the February 1997 issue of the periodical "Dermatologic Surgery," dermatologists report the rare malignant transformation of a benign hidradenoma or acrospiroma and attempt to draw insight from the scant literature available regarding malignancies of eccrine or folliculosebaceous-apocrine lineage. This case report describes a sweat gland carcinoma that is also referred to as a malignant hidradenona. It is appropriate that the authors end their paper with a call for better documentation of these fascinating and unusual neoplasms.

This paper talks about an excellent alternative for local resurfacing utilizing a time-honored and safe modality. It also illustrates the necessity not to overlook 88% phenol as an easy way to achieve temporary anesthesia on the skin. The recent celebration of laser resurfacing may unfairly prejudice the inexperienced reader against the benefits of phenol application, especially non-Baker's formula local phenol. The author has presented a valuable pearl that supplements the recent trend in the dermatology literature towards manual dermabrasion.

This article introduces dermatologist Ronald Moy who is the new Editor-in-Chief of the periodical "Dermatologic Surgery." Dr. Moy exemplifies the renaissance dermatologic surgeon, able to balance interests in research, teaching, and clinical practice. Dr. Moy has been the author of more than 100 scientific papers, books, and abstracts. He has served on the Board of Directors of the Association of Academic Dermatologic Surgeons, the American College of Mohs Micrographic Surgery and Cutaneous Oncology, and the American Society for Dermatologic Surgery.
