Introduction: Granuloma faciale is a rare dermatosis classified within the group of eosinophilic dermatoses, which presents a persistent challenge in both diagnosis and treatment. Objective: Presentation a case of granuloma faciale along with a comprehensive discussion of the pathophysiology, clinical presentation, and treatment of the disease. Case report: A 68-year-old man presented to a dermatologist with nodular and plaque-like lesions on his face that had been present for 6 months. Initially, based on histopathological findings, fixed drug eruption was diagnosed, and the patient was instructed to discontinue the medications he had been taking, including acetylsalicylic acid and non-steroidal anti-inflammatory drugs. Nevertheless, the skin lesions persisted. Following extended differential diagnosis (including dermoscopy and repeated histopathological examination), the diagnosis of granuloma faciale was established and, consequently, cryotherapy with liquid nitrogen was administered, resulting in a significant improvement in the patient's skin condition. Conclusions: Despite being typically located in the specific areas, granuloma faciale poses diagnostic challenges. Cryotherapy seems to be an effective and safe therapeutic approach in patients who fail to respond to topical medications. [ABSTRACT FROM AUTHOR]

Introduction: Pityriasis rubra pilaris is a rare, chronic dermatosis presenting with erythema and papular eruptions as well as characteristic perifollicular hyperkeratosis and ‘islands of sparing’ (normal appearing skin). In the literature, there are reports of cases where symptoms of infection preceded the onset of pityriasis rubra pilaris. Objective: Presentation of the case of a 67-year-old patient with type I pityriasis rubra pilaris to highlight the possible association between pityriasis rubra pilaris with infectious agents. Case report: A 67-year-old patient was referred to hospital because of rapidly progressive erythematous papular skin rash preceded by symptoms of an upper respiratory tract infection. On physical examination, attention was drawn to perifollicular papules covered with cap-like scales as well as confluent erythema and features of hyperkeratosis on the hands and feet. Based on the clinical presentation and histopathological findings, type I pityriasis rubra pilaris was diagnosed and treatment with acitretin was initiated. Conclusions: Infectious agents have the potential to be a trigger for pityriasis rubra pilaris. Acitretin continues to demonstrate efficacy in the treatment of pityriasis rubra pilaris. Currently pellagra is usually the subject of dermatological or non-dermatological case reports. Since the first description of the disease, its cause has remained unknown for many years. There were no known methods of prevention and treatment. For two centuries, pellagra was an incurable disease and caused several hundred thousand deaths in Europe and North America. Research by clinicians and scientists has made it possible to refute the common theory of the infectious etiology of the disease and link the disease epidemic to maize, which was then the primary source of food for the poor. Consumption limited to this grain resulted in an extreme deficiency of niacin (vitamin B3) and symptoms of pellagra. Niacin supplementation caused the lesions to regress, and became crucial to combat the epidemic. The article presents the first descriptions, the greatest outbreaks of the epidemic and the stormy process of discovering data on the etiology of pellagra, as well as typical clinical features of the diseases. Introduction: Glomus tumor is a painful, subcutaneous tumor. The lesion may occur in various locations. In the case of the nail apparatus, it affects both the matrix area and the nail bed. Case report: We present a 71-year-old patient who presented to our clinic with a painful nodule lasting for about 15 years, located on the fifth finger of his left hand. After surgical removal, a histological examination was performed, which confirmed the clinical suspicion of glomus tumor. Intraoperative contact onychoscopy revealed a gray-blue nodule with linear vessels at the periphery. Conclusions: In the differential diagnosis of glomeroma, the following should be considered: pyogenic granuloma, subugual viral wart, melanoma, hemangioma, leiomyoma, sweat gland tumor, gout, neuroblastoma and hemangioma. Dermoscopy can be useful in the differential diagnosis, but the final diagnosis is based on a histopathological examination. Surgical removal of the lesion is considered the treatment of choice. Introduction: The diagnosis of cutaneous lymphomas is extremely difficult because of their morphological diversity and clinical similarity to many dermatoses. Objective: Presentation of the case of a 70-year-old woman diagnosed with lichen sclerosus in whom the evolution of skin lesions raised suspicions of lymphocytic hyperplasia. Case report: The patient diagnosed with lichen sclerosus was hospitalized three times due to exacerbation of skin lesions with accompanying mycosis fungoides-mimicking infiltration. Cutaneous lymphoma was excluded in biopsy specimens collected from skin lesions. In the following months, a new nodular lesion appeared on the patient's face that turned out to be marginal zone lymphoma. After increasing the dose of methotrexate to 20 mg/week, a rapid remission of skin lesions was achieved. Conclusions: The presented case shows that the diagnosis of primary cutaneous lymphoma is a significant challenge in everyday dermatological practice and requires repeated biopsy. Introduction. The nail apparatus is a skin appendage which, among other functions, stabilises the fingertips and protects the distal parts of fingers or toes from injuries. It may become dystrophic following chemotherapy. Diverse nail symptoms require a complex differential diagnosis. Objective. Presentation of a case of Lindsay's nails in a patient treated with paclitaxel for breast cancer. Case report. A 65-year-old woman reported to a dermatology outpatient clinic with changes to her fingernails and toenails. Six months earlier, she had undergone left-sided mastectomy for breast cancer and then commenced chemotherapy with paclitaxel in combination with trastuzumab. A few weeks after starting this chemotherapy regimen, the patient noticed pronounced haemorrhages in the region of the toes, followed by matt whitening of the proximal part of the fingernail apparatus, sharply demarcated from the brown-coloured distal part. In addition, the patient experienced neuropathic symptoms in the fingers. The clinical picture of the lesions and the patient's disease history suggested a variant of leukonychia, i.e. Lindsay's nails, induced by chemotherapy. Conclusions. The presented case of Lindsay's nails ("half-and-half" nails) is unique, as it was never before reported in the literature in association with paclitaxel therapy. [ABSTRACT FROM AUTHOR]

Introduction. Subcorneal pustular dermatosis is a rare and chronic dermatosis which often has a recurrent course. The differential diagnosis of the condition should include above all pustular psoriasis and IgA pemphigus. Objective. Presentation of a case of subcorneal pustular dermatosis successfully treated with acitretin. Case report. A 28-year-old male patient was admitted to the Department with extensive erythematous eruptions arranged in a polycyclic pattern with small pustules on the periphery. The findings of histopath-ological examination confirmed the diagnosis of subcorneal pustular dermatosis. After introducing treatment with acitretin a remission of skin lesions was achieved. Conclusions. The reported case and the literature data show that the differential diagnosis of subcorneal pustular dermatosis continues to be a clinical and histopathological challenge. Because of therapeutic differences associated with the disease entities in question, the establishment of correct diagnosis of subcorneal pustular dermatosis is especially important, while the improvement seen in the patient's condition confirms the efficacy of acitretin in the treatment of the disease. Verruca is a general term for lesions that are clinical manifestations of an infection with human papillomavirus (HPV). A particularly difficult (both diagnostically and therapeutically) site of verrucae is the nail apparatus within which anatomical conditions significantly hinder successful treatment of these common skin infections. The aim of the article is to present a series of three cases of patients with verrucae within the nail apparatus, including one case with a secondary involvement of the vermillion boarder due to biting and chewing the nail apparatus area. Diagnostics and treatment of nail apparatus verrucae constitute a special challenge in everyday dermatological practice due to its unique and hard-to-reach anatomical site as well as the risk for secondary HPV infections and development of nail unit squamous cell carcinoma. Introduction. Pigmented purpuric dermatoses (purpura simplex; inflammatory purpura without vasculitis) constitute a heterogenic group of inflammatory skin diseases. Objective. To present a patient with extensive skin lesions secondary to pigmented purpuric dermatosis, Schamberg's disease type. Case report. A 34-year-old man patient was admitted to the Dermatology Clinic in Krakow, Poland, due to disseminated pigmented purpuric lesions. Schamberg's disease was diagnosed on the basis of clinical, dermoscopic and histopathological examinations. The treatment involved preparations improving venous microcirculation, antihistamine drugs, and a series of UVB 311 treatment sessions - partial improvement was achieved. Conclusions. An overview of literature and the presented case show that diagnosing pigmented purpuric dermatoses constitutes a challenge in everyday dermatological practice. The patients require a deepend diagnostic analysis and follow-up also because of possible coexistence or risk of future development of mycosis fungoides. Achieving a therapeutic effects is difficult. Atopic dermatitis is a skin disease frequently coexisting with other disorders of the atopic diathesis (bronchial asthma, allergic rhinitis, food allergy). At present, it is the most commonly occurring inflammatory skin disease. Atopic dermatitis is a chronic condition with diverse clinical presentations. It is characterized by complex, heterogeneous pathophysiology with a wide spectrum of clinical phenotypes. Patients with atopic dermatitis may benefit from personalized medicine which is focused on improved selection of specific therapy, risk prediction and design of disease-modifying management strategies. There is an urgenat need to identify biomarkers to support clinical diagnostic criteria in approaching the disease from the perspective of precision medicine. Atopic dermatitis is one of the most common inflammatory skin diseases. The clinical hallmark of the disease, providing the most important clue for diagnosis, is intense itching of the skin. The history of dermatology is one of the most interesting elements in the history of medicine. Atopic dermatitis is a disease whose "medical entity" has been established relatively recently. A particularly fascinating aspect is the way in which the disease has been recognized as one of the greatest plagues of modern medicine. The article presents selected issues in the evolution of knowledge on the pathogenesis and treatment of atopic dermatitis. The profiles of the most famous dermatologists who have made a particularly valuable contribution to understanding the phenomenon of atopic dermatitis are outline: Jean-Louis Alibert, Ernest Henri Besnier, Ferdynand von Hebra, Marion Baldur Sulzberger. Introduction. Dermatitis bullosa pratensis (phytophotodermatitis) is an acute phototoxic reaction caused by photosensitizing substances present in different kinds of plants, activated by ultraviolet radiation exposure. An acute phototoxic reaction manifests as erythema, vesicles and even blisters with morphology of burns. Objective. We present a case report of phytophotodermatitis due to Heracleum sosnowskyi. Heracleum sosnowskyi was discovered in 1772 and described as a separate species in botanical literature in 1944 by I. P. Mandenova. Case report. In our case, skin lesions coexisted with erythema multiforme. Implemented therapy - corticosteroids, antihistamines and topical medications - was successful. After clinical resolution post-inflammatory hyperpigmentations were observed. Conclusions. Appropriate diagnosis of phytophototoxic reaction may constitute a challenge in everyday practice. Introduction. Erythema ab igne (EAI) is a rare skin disease (since the common use of central heating) characterized by reticular, erythematous and hyperpigmented patches resulting from chronic exposure to infrared radiation. Although the condition is generally benign, chronic heat exposure can induce dysplasia and, rarely, squamous cell carcinoma. The pathogenesis of the disease is not yet fully understood.**Objective. Presentation of the diagnostic and therapeutic difficulties in a patient with EAI.**Case report. The patient had erythema ab igne that developed on the shins, due to the effect of a heating device used in the flower shop. The clinical presentation, differential diagnosis and available treatment options of the disease are described.**Conclusions. Zbigniew Oszast was born on 11th March 1903 in Lublin. His scientific career started in 1929 and was constantly connected with the Department of Dermatology Jagiellonian University in Krakow. For many years professor Oszast was especially interested in radiotherapy of skin diseases. He was educated in many prestigious European institutions - among others: Finsen Institute, Electropathology Institute under professor Jelinek's supervision, Sauerbruch's and Hermansdorfer's Clinic. Extensive experience was very helpful to create a radiotherapy ward which was part of the Dermatology Department. During World War II it was the greatest radiotherapy institution in Poland. Prof Oszast was also involved in research on: ultrashort waves treatment of skin diseases, coagulation of skin lesions, therapy of skin tuberculosis and radiotherapy of skin cancers connected to tuberculosis luposa. He was also involved in development of physical therapy in Poland. Throughout the period of World War II prof Oszast was fully responsible for the Dermatology Department as well as the dermatology and radiotherapy ward in Saint Lazarus Hospital. He was a person with a versatile mind - a great scientist, excellent teacher and talented organiser. Besides research work he wrote numerous biographical articles about many famous physicians. Introduction. Syphilis is a sexually transmitted disease (STD), caused by the spirochete Treponema pallidum. The disease occurs only in humans. In the course of syphilis several stages are distinguished, which reflect the gradual spread of bacteria in the organism. Variety of sexual practices (including anal and oral sex), and widespread empirical use of antibiotics, prescribed for reasons other than STD (e.g. upper respiratory tract infections) not having treponemicidal action, seem to lead to changes in the clinical course of syphilis and its characterization by sparse symptoms. Objective. The aim of this study was to analyse the clinical manifestation of secondary syphilis in patients treated in the Department of Dermatology at Jagiellonian University, School of Medicine in Cracow. Material and methods. Data obtained from 217 patients including those of sociodemographic status, sexual orientation and HIV infection were analysed. Results. The analysed group consisted of 23 women (10.6%) and 194 men (89.4%), of whom 107 declared they were homosexuals. In 153 patients due to the absence of clinical signs of infection early latent syphilis was diagnosed. The most common clinical symptom of secondary syphilis was macular rash (43.7%), followed by maculopapular changes on the palms and soles (36.8%), generalized lymphadenopathy (33.3%), syphilitic alopecia (21.8%), syphilitic angina (14.9%) and condylomata lata (6.9%). 58.2% of patients had only one clinical symptom of the disease. The most common signs of infection were generalized lymphadenopathy and maculopapular rash on the soles and palms (11.5%). Conclusions. It seems that in the past two decades the clinical manifestation of secondary syphilis has not changed significantly. However, in comparison to data available from the literature different incidence of individual symptoms of secondary syphilis was observed, which may be explained by locally contingent diversity of sexual practices, use of empiric antibiotic therapy for reasons other than sexually transmitted infections and different accessibility to health care.
