Amongst the other benefits conferred by the shift from traditional to digital pathology is the potential to use machine learning for diagnosis, prognosis, and personalization. A major challenge in the realization of this potential emerges from the extremely large size of digitized images, which are often in excess of 100,000 × 100,000 pixels. In this paper, we tackle this challenge head-on by diverging from the existing approaches in the literature—which rely on the splitting of the original images into small patches—and introducing magnifying networks (MagNets). By using an attention mechanism, MagNets identify the regions of the gigapixel image that benefit from an analysis on a finer scale. This process is repeated, resulting in an attention-driven coarse-to-fine analysis of only a small portion of the information contained in the original whole-slide images. Importantly, this is achieved using minimal ground truth annotation, namely, using only global, slide-level labels. The results from our tests on the publicly available Camelyon16 and Camelyon17 datasets demonstrate the effectiveness of MagNets—as well as the proposed optimization framework—in the task of whole-slide image classification. In the domain of AI-driven healthcare, deep learning models have markedly advanced pneumonia diagnosis through X-ray image analysis, thus indicating a significant stride in the efficacy of medical decision systems. This paper presents a novel approach utilizing a deep convolutional neural network that effectively amalgamates the strengths of EfficientNetB0 and DenseNet121, and it is enhanced by a suite of attention mechanisms for refined pneumonia image classification. Leveraging pre-trained models, our network employs multi-head, self-attention modules for meticulous feature extraction from X-ray images. The model's integration and processing efficiency are further augmented by a channel-attention-based feature fusion strategy, one that is complemented by a residual block and an attention-augmented feature enhancement and dynamic pooling strategy. Our used dataset, which comprises a comprehensive collection of chest X-ray images, represents both healthy individuals and those affected by pneumonia, and it serves as the foundation for this research. This study delves deep into the algorithms, architectural details, and operational intricacies of the proposed model. The empirical outcomes of our model are noteworthy, with an exceptional performance marked by an accuracy of 95.19%, a precision of 98.38%, a recall of 93.84%, an F1 score of 96.06%, a specificity of 97.43%, and an AUC of 0.9564 on the test dataset. Ultra-wide-field fundus imaging (UFI) provides comprehensive visualization of crucial eye components, including the optic disk, fovea, and macula. This in-depth view facilitates doctors in accurately diagnosing diseases and recommending suitable treatments. This study investigated the application of various deep learning models for detecting eye diseases using UFI. We developed an automated system that processes and enhances a dataset of 4697 images. Our approach involves brightness and contrast enhancement, followed by applying feature extraction, data augmentation and image classification, integrated with convolutional neural networks. These networks utilize layer-wise feature extraction and transfer learning from pre-trained models to accurately represent and analyze medical images. Among the five evaluated models, including ResNet152, Vision Transformer, InceptionResNetV2, RegNet and ConVNext, ResNet152 is the most effective, achieving a testing area under the curve (AUC) score of 96.47% (with a 95% confidence interval (CI) of 0.931–0.974). Additionally, the paper presents visualizations of the model's predictions, including confidence scores and heatmaps that highlight the model's focal points—particularly where lesions due to damage are evident. By streamlining the diagnosis process and providing intricate prediction details without human intervention, our system serves as a pivotal tool for ophthalmologists. Objectives: During deep learning model training, it is essential to consider the balance among the effects of sample size, actual resources, and time constraints. Single-arm objective performance criteria (OPC) was proposed to determine the optimal positive sample size for training deep learning models in caries recognition. Methods: An expected sensitivity (PT) of 0.6 and a clinically acceptable sensitivity (P0) of 0.5 were applied to the single-arm OPC calculation formula, yielding an optimal training set comprising 263 carious teeth. U-Net, YOLOv5n, and CariesDetectNet were trained and validated using clinically self-collected cone-beam computed tomography (CBCT) images that included varying quantities of carious teeth. To assess performance, an additional dataset was utilized to evaluate the accuracy of caries detection by both the models and two dental radiologists. Results: When the number of carious teeth reached approximately 250, the models reached the optimal performance levels. U-Net demonstrated superior performance, achieving accuracy, sensitivity, specificity, F1-Score, and Dice similarity coefficients of 0.9929, 0.9307, 0.9989, 0.9590, and 0.9435, respectively. The three models exhibited greater accuracy in caries recognition compared to dental radiologists. Background: Diagnosing lung diseases accurately is crucial for proper treatment. Convolutional neural networks (CNNs) have advanced medical image processing, but challenges remain in their accurate explainability and reliability. This study combines U-Net with attention and Vision Transformers (ViTs) to enhance lung disease segmentation and classification. We hypothesize that Attention U-Net will enhance segmentation accuracy and that ViTs will improve classification performance. The explainability methodologies will shed light on model decision-making processes, aiding in clinical acceptance. Methodology: A comparative approach was used to evaluate deep learning models for segmenting and classifying lung illnesses using chest X-rays. The Attention U-Net model is used for segmentation, and architectures consisting of four CNNs and four ViTs were investigated for classification. Methods like Gradient-weighted Class Activation Mapping plus plus (Grad-CAM++) and Layer-wise Relevance Propagation (LRP) provide explainability by identifying crucial areas influencing model decisions. Results: The results support the conclusion that ViTs are outstanding in identifying lung disorders. Attention U-Net obtained a Dice Coefficient of 98.54% and a Jaccard Index of 97.12%. ViTs outperformed CNNs in classification tasks by 9.26%, reaching an accuracy of 98.52% with MobileViT. An 8.3% increase in accuracy was seen while moving from raw data classification to segmented image classification. Techniques like Grad-CAM++ and LRP provided insights into the decision-making processes of the models. Conclusions: This study highlights the benefits of integrating Attention U-Net and ViTs for analyzing lung diseases, demonstrating their importance in clinical settings. Computed tomography (CT) scans have recently emerged as a major technique for the fast diagnosis of lung diseases via image classification techniques. In this study, we propose a method for the diagnosis of COVID-19 disease with improved accuracy by utilizing graph convolutional networks (GCN) at various layer formations and kernel sizes to extract features from CT scan images. We apply a U-Net model to aid in segmentation and feature extraction. In contrast with previous research retrieving deep features from convolutional filters and pooling layers, which fail to fully consider the spatial connectivity of the nodes, we employ GCNs for classification and prediction to capture spatial connectivity patterns, which provides a significant association benefit. We handle the extracted deep features to form an adjacency matrix that contains a graph structure and pass it to a GCN along with the original image graph and the largest kernel graph. We combine these graphs to form one block of the graph input and then pass it through a GCN with an additional dropout layer to avoid overfitting. Our findings show that the suggested framework, called the feature-extracted graph convolutional network (FGCN), performs better in identifying lung diseases compared to recently proposed deep learning architectures that are not based on graph representations. The proposed model also outperforms a variety of transfer learning models commonly used for medical diagnosis tasks, highlighting the abstraction potential of the graph representation over traditional methods. Deep learning (DL) models for medical image classification frequently struggle to generalize to data from outside institutions. Additional clinical data are also rarely collected to comprehensively assess and understand model performance amongst subgroups. Following the development of a single-center model to identify the lung sliding artifact on lung ultrasound (LUS), we pursued a validation strategy using external LUS data. As annotated LUS data are relatively scarce—compared to other medical imaging data—we adopted a novel technique to optimize the use of limited external data to improve model generalizability. Externally acquired LUS data from three tertiary care centers, totaling 641 clips from 238 patients, were used to assess the baseline generalizability of our lung sliding model. We then employed our novel Threshold-Aware Accumulative Fine-Tuning (TAAFT) method to fine-tune the baseline model and determine the minimum amount of data required to achieve predefined performance goals. A subgroup analysis was also performed and Grad-CAM++ explanations were examined. The final model was fine-tuned on one-third of the external dataset to achieve 0.917 sensitivity, 0.817 specificity, and 0.920 area under the receiver operator characteristic curve (AUC) on the external validation dataset, exceeding our predefined performance goals. Subgroup analyses identified LUS characteristics that most greatly challenged the model's performance. Grad-CAM++ saliency maps highlighted clinically relevant regions on M-mode images. We report a multicenter study that exploits limited available external data to improve the generalizability and performance of our lung sliding model while identifying poorly performing subgroups to inform future iterative improvements.
