Abstract

SUMMARY: BACKGROUND In a multicenter case-control study of lung cancer in central and eastern Europe and in Liverpool, exposure to occupational agents was assessed by teams of local experts. We performed an interteam agreement study to estimate the levels of exposure misclassification and the expected attenuation of the risk estimate.METHODS Eight teams of experts and a reference rater assessed exposure to 70 putative lung carcinogens for 19 jobs. Agreement among teams was calculated through Cohen's kappa, sensitivity, and specificity.RESULTS Each team showed an overall fair to good agreement with the reference (kappa between 0.53 and 0.64). The agreement among teams in the presence of exposure was excellent for 9 agents, fair to good for 16, and poor for 29. For all agents the specificity was high (average 0.94), although sensitivity varied considerably.CONCLUSIONS This study of expert exposure assessment showed a small range in reliability among teams of experts, but large differences among agents. This paper presents the range in levels of misclassification that can be expected using experts for assessing occupational exposure to different agents, and the attenuation of the odds ratio that can be expected to result from this misclassification. 40. Cancer Mortality among ManMade Vitreous Fiber Production Workers

Abstract

We have updated the followup of cancer mortality for a cohort study of manmade vitreous fiber production workers from Denmark, Finland, Norway, Sweden, United Kingdom, Germany, and Italy, from 1982 to 1990. In the mortality analysis, 22,002 production workers contributed 489,551 personyears, during which there were 4,521 deaths. Workers with less than 1 year of employment had an increased mortality standardized mortality ratio SMR 1.45 95 confidence interval CI 1.37–1.53. Workers with 1 year or more of employment, contributing 65 of personyears, had an SMR of 1.05 95 CI 1.02–1.09. The SMR for lung cancer was 1.34 95 CI 1.08–1.63, 97 deaths among rockslag wool workers and 1.27 95 CI 1.07–1.50, 140 deaths among glass wool workers. In the latter group, no increase was present when local mortality rates were used. Among rockslag wool workers, the risk of lung cancer increased with timesincefirstemployment and duration of employment. The trend in lung cancer mortality according to technologic phase at first employment was less marked than in the previous followup. We obtained similar results from a Poisson regression analysis limited to rockslag wool workers. Five deaths from pleural mesothelioma were reported, which may not represent an excess. There was no apparent excess for other categories of neoplasm. Tobacco smoking and other factors linked to social class, as well as exposures in other industries, appear unlikely to explain the whole increase in lung cancer mortality among rockslag wool workers. Limited data on other agents do not indicate an important role of asbestos, slag, or bitumen. These results are not sufficient to conclude that the increased lung cancer risk is the result of exposure to rockslag wool however, insofar as respirable fibers were an important component of the ambient pollution of the working environment, they may have contributed to the increased risk.

41. A Case Study Comparing a MetaAnalysis and a Pooled Analysis of Studies of Sinonasal Cancer among Wood Workers

Abstract

A pooled analysis of raw data from 12 case-control studies of sinonasal cancer has recently been conducted by the International Agency for Research on Cancer; the summary odds ratio for all wood-related occupations was 2.0 (95 confidence interval = 1.6–2.5). We have conducted a meta-analysis of the published results of 14 studies on the same topic, including 11 studies from the pooled analysis, and compared the results for several categories of wood workers. Usable results were available for 12 studies: male wood workers had a summary odds ratio of sinonasal cancer of 2.6 (95 confidence interval = 2.1–3.3). The corresponding value based only on the studies that were also included in the pooled analysis was 2.4 (95 confidence interval = 1.9–3.0). When our meta-analysis was based on a relatively large number of studies, results tended to be higher than those of the pooled analysis. As compared with the meta-analysis, the pooled analysis controlled the effect of publication bias by using data from studies for which no specific result was reported, and it reduced misclassification of exposure; the resources used in the pooled analysis, however, were one order of magnitude greater than those needed for the meta-analysis. Whether a pooled analysis of raw data or a meta-analysis should be carried out depends on the research question; we propose some criteria for this decision. (Epidemiology 1998;9:518–524)
