Aims Heart failure (HF) is accompanied by major neuroendocrine changes including the activation of the natriuretic peptide (NP) pathway. Using the unique model of patients undergoing implantation of the CARMAT total artificial heart and investigating regional differences in soluble neprilysin (sNEP) in patients with reduced or preserved systolic function, we studied the regulation of the NP pathway in HF. Methods and results Venous blood samples from two patients undergoing replacement of the failing ventricles with a total artificial heart were collected before implantation and weekly thereafter until post-operative week 6. The ventricular removal was associated with an immediate drop in circulating NPs, a nearly total disappearance of circulating glycosylated proBNP and furin activity and a marked decrease in sNEP. From post-operative week 1 onwards, NP concentrations remained overall unchanged. In contrast, partial recoveries in glycosylated proBNP, furin activity, and sNEP were observed. Furthermore, while in patients with preserved systolic function (n = 6), sNEP concentrations in the coronary sinus and systemic vessels were similar (all P > 0.05), in patients with reduced left-ventricular systolic function, sNEP concentration, and activity were ∼three-fold higher in coronary sinus compared to systemic vessels (n = 21, all P Conclusion The heart plays a pivotal role as a regulator of the endocrine response in systolic dysfunction, not only by directly releasing NPs but also by contributing to circulating sNEP, which in turn determines the bioavailability of other numerous vasoactive peptides.

Imperfect myocardial protection during prolonged ischaemia results in adverse changes during reperfusion. Clinical studies carried out during cardiac surgery show that: (1) Oxygen free radicals produced during reperfusion can lead to chromosomal damage in leukocytes. However, this effect seems to be prevented by the addition of allopurinol in the cardioplegic solution. (2) Polymorphonuclear leukocytes are directly implicated in situ in the genesis of free radicals responsible for reperfusion injury. (3) Pre-treatment with trimetazidine, an anti-ischaemic drug with antioxidant properties, and addition of the drug to the cardioplegic solution reduced oxygen free radical damage, as shown by a reduced release of malondialdehyde increase and of myosin; moreover, pre-treatment with trimetazidine enabled patients to undergo surgery with improved left ventricular function.

The aim of this study was to determine the specific conditions and pitfalls of immediate assessment of the reconstructed mitral valve in open chest patients. Sirty-rwo patients had a mitral reconstruction controlled by transoesophageal echocardiography and colour Doppler (TEE) from the moment when cardiac activity restarted under extracorporeal circulation (ECC), until complete rewarming of the patient and weaning off the by pass. The following conditions altered the imaging quality: (1) dried probe in the oesophagus or air bubbles, (2) nonapirated air from the stomach, (3) small or empty lefi atria, (4) invagination of the left atrial appendage, (5) trapped air in the posterior pericardium, (6) spontaneous contrast. Transient mitral regurgitation disappeared completely in 12/62 patients: five had temporary impairment of LV fwtction, two had reduced filling of the heart, one had lefi ventricular ourflow obstruction exacerbated by isoproterenol and nitroglycerine, two had ventricular ectopic rhythm, and two epicordial pacing. Only one of the 62 patients had persistent significant MR which reauired a second run o f ECC. we conclude that m i t h regurgitation after mitral valve repair is closely related to the quality of LV function. A decision to reoperate should eliminate the possible pitfalls, and take into consideration the specffic conditions of immediate postoperative cardiac function.

We report one case of successful cardiomyoplasty in a 21-year-old male and its beneficial effects as assessed by clinical examination, Doppler echocardiography, phonocardiography, haemodynamic and angiographic evaluation at 1 year. Long-term efficacy of this procedure on left ventricular function was confirmed by these techniques.
