Background Atrial fibrillation (AF) is the most prevalent heart rhythm disorder in clinical practice and it is associated with poor quality of life and increased risks of heart failure, dementia, stroke, and death. Moreover AF management is a huge cost for healthcare systems. AF is irregular, typically originates from the pulmonary veins, and as such, requires cardioversion to terminate persistent episodes. AF is not susceptible to pace-termination, however, the MINERVA trial has shown that AF may transform in slower organized rhythms such as atrial flutter or atrial tachycardia, which can often be terminated by atrial anticahycardia pacing (ATP); in particular by Reactive ATP, a specific ATP feature which can be re-armed when atrial arrhythmias get slower or more regular. The MINERVA trial showed that the combination of ATP, preventive atrial pacing algorithms and minimal ventricular pacing (MVP) was associated with lower progression to persistent and permanent AF, compared with standard DDD pacing mode and to MVP mode, in pacemaker patients with clinical history of AF. Purpose We aimed to confirm MINERVA trial results in real-world clinical practice and to evaluate whether AF prevention was associated with preventive atrial pacing or solely with ATP. Indeed in our project atrial preventive pacing algorithms were not enabled and the pacing mode (DDD or MVP) was chosen according to patients' AV conduction characteristics. Methods Consecutive dual-chamber pacemaker patients with sinus node disease and device detected AT/AF were prospectively followed by 30 Italian cardiologic centers in an observational research. Clinical and device data were collected and reviewed by expert cardiologists to assess AT/AF occurrence through in clinic visit and/or remote transmissions of device data. Results A total of 239 patients (73 years old, 56% male) wearing a dual-chamber pacemaker with Reactive ATP were included in the project, followed for a median observation period of 13 months and compared with 1145 patients included in the MINERVA trial followed for a median observation period of 34 months and programmed with DDD pacing mode (383 patients), MVP (389 patients) and MVP+Reactive ATP+preventive algorithms (373 patients). As shown in the following figure incidence of 7 consecutive days of AF in the patients treated by DDD/MVP+Reactive ATP in real-world clinical practice was very similar to that found in the MINERVA trial arm programmed with MVP+Reactive ATP+preventive algorithms. Incidence of AF longer than 7 days Conclusions Our analysis performed in a population of sinus node disease patients with dual-chamber pacemakers confirmed MINERVA trial results in terms of prevention of long-lasting AF episodes. In particular these results confirm the benefit associated with the use of Reactive ATP, rather than preventive atrial pacing algorithms.

Background Left cardiac sympathetic denervation (LCSD) is an established therapy for refractory ventricular arrhythmias (VAs) in channelopathies. A multicentric American and Indian case series suggested a greater efficacy of bilateral denervation (BCSD) in patients with structural heart disease (SHD). Purpose To describe our single-center experience with BCSD in SHD. Methods Nine patients (78% male, mean 55±18 yrs, mean LVEF 31±14%) with SHD and refractory VAs underwent BCSD. All had a Video-Assisted Thoracoscopic Surgery (VATS), in 2 cases associated with the robotic technique. The underlying cardiomyopathy (CMP) was non-ischemic (NICMP) in most cases (n=5, 55%), ischemic in 2 cases, arrhythmogenic right ventricular (ARVC) in one and related to lamin A/C deficiency in one. All patients had an ICD, 44% (n=4) a CRT-D. NYHA functional class I was present in 4 patients, the rest were in NYHA class II (n=3) or III (n=2). Three patients were candidates to heart transplant/LV assistance device. The arrhythmic burden pre BCSD included in 7 pts (78%) a history of electrical storm (ES); the median number of shocks/patient in the 12 months before BCSD was 5 (IQ range 3–18). Except for 2 patients with previous thyrotoxicosis, the remaining were either on amiodarone (n=6) or on sotalol (n=1) before BCSD. Main BCSD indications were represented by drug refractory fast VT in 7 pts (cycle Results No major complication occurred. One patient (NICMP, NYHA II), has an uneventful follow up (FU) of less than 1 month and was excluded from the efficacy analysis. The median FU in the remaining 8 patients is 10 months (IQ range 6–19), during which the median number of shocks/patients was 0.5 (IQ range 0–3). Overall, 4 patients (50%) had ICD shock recurrences. Two cases (mean LVEF 17.5%, NYHA class III) had an ES during severe hemodynamic instability and subsequently died because of cardiogenic shock respectively 1 and 7 months after BCSD. One case had three, not consecutive ICD shocks 20 months after BCSD in the setting of severe amiodarone-induced thyrotoxicosis. Finally, one patient received a single intra-hospital ICD shock 5 days after BCSD before reintroduction of full-dose beta-blockers. The figure summarizes ICD shocks burden in the 6 months before and after BCSD. Among the 5 patients with NICMP/ARVC (4 in NYHA class I), only 1 had a single ICD shock recurrence. ICD shocks pre versus post BCSD, n=8 Conclusions Our case series, although numerically small, has a good follow-up and is the first reported in Europe. The results are in agreement with the suggested remarkable efficacy of BCSD in patients with good functional capacity and fast VAs. Therefore, cardiac sympathetic denervation should always be considered in patients with SHD and refractory ventricular tachyarrhythmias, especially in case VT ablation is either not indicated or fails.

To achieve a complete coverage of all Implantable Cardioverter Defibrillator (ICD) performed in Italy between 2001 and 2015, data from the National Hospital Discharge Database (SDO: “Scheda di Dimissione Ospedaliera”) were analyzed. Frequencies and implant rate of ICD and Cardiac Resynchronization Therapy/Defibrillator (CRTD) according to the Italian population were computed. Data were analyzed for the following age classes: 80. Results Total number of ICD increased from 3141 in 2001 (implant rate 54/million inhabitants) to 23.540 in 2015 (387/million). From 2009 to 2015 CRT-D increased from 2916 in 2009 (49/million) to 8245 in 2015 (136/million), while the proportion of CRT/total ICD from 17% to 35%. Mean age (from 67±12 in 2001 to 68±12 in 2015) and the proportion of males (from 80% to 79%) remained unchanged. Ischemic heart disease was the most frequent diagnosis (55.6% in 2001, 45.4% in 2015). ICD for secondary prevention have been reducing from 55.6% to 13.6% (ventricular tachycardia, VT) and from 18.1% to 5.9% (ventricular fibrillation, VF). The proportion of patients with heart failure (from 23.9% to 47.9%), hypertension (from 11.2% to 16.2%), diabetes mellitus (6.5% to 11.6%) and renal insufficiency (from 4.4% to 8%) increased. The proportion of CRT/ICD remained constant (except for the 2009, where a possible underdiagnosis is very likely), between 43% and 39%. In patients In patients >80 years old the number of ICD increased from 195 (82/million) in 2001 to 3902 (981/million) in 2015; ischemic heart disease was the most frequent diagnosis (53.8% in 2001, 43.5% in 2015). ICD for secondary prevention reduced from 50.3% to 14,8% (VT) and from 16.4% to 4.1% (VF), with an increase of the proportion of patients with heart failure (from 22.6% to 46.3%), hypertension (from 11.8% to 17.1%), diabetes mellitus (5.6% to 9.9%) and renal insufficiency (from 7.2% to 12.1%). Conclusions There was a steady increase of ICD and CRTD implantation during the last fifteen years, especially in those >80 years; the proportion of patients treated for secondary prevention dramatically reduced since early 2000; an increase of co-morbidities was observed.

Aims Risk stratification in individuals with type 1 Brugada electrocardiogram (ECG) pattern (type 1 ECG) for primary prevention of sudden death (SD). Methods and results Three hundred and twenty patients (258 males, median age 43 years) with type 1 ECG were enrolled. No patient had previous cardiac arrest. Fifty-four per cent of patients had a spontaneous and 46% a drug-induced type 1 ECG. One-third had syncope, two-thirds were asymptomatic. Two hundred and forty-five patients underwent electrophysiologic study (EPS) and 110 patients received an implantable cardiac defibrillator (ICD). During follow-up [median length 40 months (IQ20-67)], 17 patients had major arrhythmic events (MAE) (14 resuscitated ventricular fibrillation (VF) and three SD). Both a spontaneous type 1 ECG and syncope significantly increased the risk (2.6 and 3.0% event rate per year vs. 0.4 and 0.8%). Major arrhythmic events occurred in 14% of subjects with positive EPS, in no subjects with negative EPS and in 5.3% of subjects without EPS. All MAE occurred in subjects who had at least two potential risk factors (syncope, family history of SD, and positive EPS). Among these patients, those with spontaneous type 1 ECG had a 30% event rate. Conclusion (1) In subjects with the Brugada type 1 ECG, no single clinical risk factor, nor EPS alone, is able to identify subjects at highest risk; (2) a multiparametric approach (including syncope, family history of SD, and positive EPS) helps to identify populations at highest risk; (3) subjects at highest risk are those with a spontaneous type 1 ECG and at least two risk factors; (4) the remainder are at low risk.

Aims To investigate the efficacy and safety of a cardiac resynchronization therapy with cardioverter–defibrillator (CRT-D) device with simplified ventricular tachycardia management in patients with non-ischaemic heart failure (HF) and primary prevention implantable cardioverter defibrillator (ICD) indication. Methods and results Prospective, controlled, parallel, multicentre, non-randomized study enrolling 324 primary prevention non-ischaemic HF patients implanted with CRT-D devices from 2004 to 2007: Protect group, 164 patients implanted with a Medtronic Insync III Protect device and Control group, 160 patients utilizing other Medtronic CRT-D devices. Efficacy was assessed by computing appropriate and inappropriate detections and therapies during follow-up; safety compared hospitalizations and syncopal events between groups. Ninety per cent of both ventricular and supraventricular tachyarrhythmias terminated within the 13–29 beat detection interval with the Protect algorithm. The Protect group showed a significantly better event-free survival to first delivered therapy for total ( P = 0.0001), appropriately treated ( P = 0.002), and inappropriately treated episodes ( P = 0.017). The total number of delivered shocks was significantly lower in the Protect group (22 vs. 59, P < 0.0001). In the Protect group, a significantly reduced HF hospitalization (hazard ratio 0.38, 95% CI 0.15–0.98, P = 0.044) was observed without any increase of syncope or death. Conclusion A simplified CRT-D device with fixed long detection reduced overall ICD therapy burden and HF hospitalizations without entailing any additional adverse events in primary prevention non-ischaemic HF patients.

Atrial fibrillation is a very common arrhythmia in patients with structural heart disease, but it also occurs in patients without underlying heart disease. Acute therapy for paroxysmal atrial fibrillation is very dependent on the clinical condition of the patient. Direct current cardioversion is usually the first choice whenever the arrhythmia precipitates heart failure or severe angina, while more stable patients are normally treated with drugs. Most episodes of atrial fibrillation eventually convert to sinus rhythm even in the absence of treatment. Antiarrhythmic drugs can be used to control the ventricular response or to restore sinus rhythm, and several have been tested to assess their ability to convert recent onset atrial fibrillation. The success rate has varied, but generally flecainide and propafenone appear the most effective. Digitalis and calcium channel blockers do not increase the likelihood of reversion but they reduce ventricular rate. Amiodarone has been tested as a possible alternative to flecainide and propafenone. The pros and the cons of these and other drugs in the setting of paroxysmal atrial fibrillation will be discussed. In particular, special emphasis will be given to the differences in the design and in patient selection of the trials that tested antiarrhythmic drugs in paroxysmal atrial fibrillation.

Aims To assess prospectively the value of cardiac magnetic resonance imaging in patients with apparently idiopathic premature contractions arising from the right ventricular outflow tract. Methods We compared magnetic resonance imaging scans in 19 patients (13 males and six females, mean age 44 years) with frequent (>100 per hour), monomorphic (left bundle branch block and inferior axis morphology) extrasystoles, and in 10 volunteers (four males and six females, mean age 36·7 years) without structural heart disease. Magnetic resonance imaging studies (1 or 1·5 Tesla) included spinecho and gradient-echo sequences in the standard planes. The presence of structural and dynamic abnormalities of the right and left ventricles, such as reduced wall thickness, systolic bulging, and decreased systolic thickening, were evaluated. In addition, end-diastolic diameters of the right ventricular outflow tract were measured in the transverse plane. Results The dimensions of the right ventricular outflow tract were wider in patients with extrasystoles compared to the control group. Purpose: Atrial fibrillation (AF) is a common arrhythmia in patients wearing implantable cardiac devices. AF has been associated with higher risk of stroke, but, when asymptomatic and intermittent, it may remain clinically unknown, thus reducing the chances of implementing stroke prevention strategies, like oral anticoagulation therapy (OAC). Aim of our project was to measure the clinical and organizational value of a web-based application, the AFinder, in improving AF detection and OAC management in patients wearing implantable pacemakers or defibrillators. Methods: 7 Italian cardiological centres prospectively followed 472 patients via in-hospital and remote visits from January 2011 to November 2012. Remote monitoring was performed by expert nurses and cardiologists who reviewed device data transmitted via patient monitors and classified clinically relevant events as AF occurrence. A new web-based application, AFinder, was exploited to identify patients with AF through a systematic scan of device data transmitted via remote monitors. To evaluate the clinical and organizational value of AFinder, we compared the number of patients found in AF during the observation period by the remote monitoring and by AFinder application and counted the number of patients in whom the OAC therapy was implemented as a consequence of new AF documentation. Results: 472 patients (76% male, 69±10 years old) were included; 184 (39%) had AF history with a mean CHADS2 score ≥2; 208 (44%) were on OAC. During a mean remote monitoring period of 12±8 months, AF occurred in 159/472 (34%) patients, in particular 58/109 pacemaker patients and 101/363 ICD patients. AFinder application found AF also in 54 patients who were not identified as AF patients during the remote monitoring observation. Out of these 54 patients, 15 had AF history as documented by patient medical records and were already treated by OAC, 6 patients were on OAC because they had valvular prostheses or suffered thromboembolic complications, while, importantly, 33 patients had no previous history of AF and were therefore not protected by OAC; in 22/33 patients AF was not considered clinically relevant due to short duration – less than 1 hour in most patients – while OAC therapy was proposed to 11/33 patients who were considered to have clinically relevant AF and high stroke risk. Conclusions: AF is a frequent comorbidity in both pacemaker and ICD patients. AFinder application enabled us to discover additional AF patients, as compared with standard remote monitoring and to improve OAC use in accordance with available guidelines for stroke risk reduction.

Background: Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia encountered in clinical practice and has been recognized as a cause of mortality, morbidity and quality of life impairment. Purpose of our research was to assess the occurrence and clinical outcomes of AF in patients with implantable cardioverter defibrillators with cardiac resynchronization therapy (CRT-ICD). Methods: 1404 CRT-ICD patients (1118 males, 67±10 years) were prospectively followed up in 74 Italian cardiology centres. Device diagnostics allowed to detect AF occurrence and duration and its association with ventricular rate (VR), biventricular pacing and inappropriate ICD therapies. Clinical data, such as AF-related hospitalizations were collected during in-hospital scheduled and unscheduled visits. Results: In a median follow-up of 31 months, new onset AF, lasting >10 minutes, occurred in 319 patients while recurrences of a previously known AF occurred in 237 patients. Fifty-eight patients (29 M, 29 F, mean age 60.8 +/- 16 years) with unexplained syncope at the end of a complete clinical and electrophysiological evaluation, were followed for a mean period of 36.6 +/- 20.5 months (median: 30.5 months). Structural heart disease was present in 32 patients (55.2%). The standard ECG was normal in 24 (41.4%) and showed sinus bradycardia (greater than 40 m-1) and/or first degree AV block and/or intraventricular conduction disturbances in 29 patients (50%). During follow-up, recurrences of syncope were observed in 11 of 43 untreated patients (25.6%), three of seven electrically treated patients (42.9%) and two of eight pharmacologically treated patients (25%). The cause of these recurrences was cardiac in one (1.7%), non-cardiac in 10 (17.2%) and remained undetermined in five (8.6%). Sudden death occurred in only one patient (1.7%), who was receiving chronic amiodarone therapy. These results indicate that (1) syncopal recurrences may occur in an appreciable percentage of patients with unexplained syncope and a negative electrophysiologic study during a relatively long-term follow-up, (2) syncopal recurrences, when they occur, are generally due to a non-cardiac cause, (3) sudden death is an occasional and rare event in this patient population and (4) empirical prophylactic treatment with a permanent pacemaker or antiarrhythmic drugs does not usually prevent complications during the follow-up.

The aim of this study was the electrophysiological evaluation of bisoprolol, a new highly cardioselective beta blocker, void of intrinsic sympathomimetic activity and without significant membrane stabilizing action. 10 patients (4 males and 6 females, mean age 67.5 years) with cardiac arrhythmias or syncopes of undetermined origin underwent His bundle recording with incremental as well as programmed atrial and ventricular pacing in an electrophysiological study. Electrophysiological parameters were measured at basal conditions and at 15 and 30 minutes after the infusion of bisoprolol 5 mg (for patient 1 to 5) and 10 mg (for patient 6 to 10). All electrophysiological parameters were within normal range at basal evaluation. Bisoprolol prolongs significantly sinus cycle length, corrected sinus node recovery time, AH interval, cycle length inducing AV-node block, effective and functional refractory periods of the AV-node. All these parameters remained within normal limits and were modified to about the same extent by 5 and 10 mg of the drug. No drug-related changes of QT interval were observed. Bisoprolol have been confirmed to have strictly beta-blocking properties.
