Aims We evaluated the ability of a new simplified algorithm for three-dimensional echocardiography (3DE) left ventricular (LV) measurements with minimal operator interaction to be reproducible and robust, independently of the experience. Methods and results A total of 163 subjects were investigated using two-dimensional echocardiography (2DE) and 3DE. The 3D data sets were blindly analysed offline by novice investigators and experts. A subgroup of 30 patients was assessed using cardiac magnetic resonance imaging (CMRI) to compare end-diastolic volume (EDV), end-systolic volume (ESV), and ejection fraction (EF) obtained by 2DE, 3DE, and CMRI. Intra-observer and inter-observer variabilities of 2DE and 3DE measurements were evaluated according to level of experience. Mean time analysis of 3DE data was 23.2 ± 6.3s for the novice and 26.1 ± 4.1 s for the expert ( P = ns). Correlations ( r ) and mean error measurements (MEM) between 3DE analysis by experts and novices were 0.91 and −3.5 mL for EDV, 0.97 and 4.3 mL for ESV, and 0.91 and −2.6% for EF, respectively. Correlations between 3DE and CMRI were good with low variability and greater agreement when compared with those between 2DE and CMRI. For the novice, MEM was −21.3 mL for EDV, −15.0 mL for ESV, and 2.3% for EF. MEM and 95% confidence intervals were wider for 2DE vs. CMRI than for 3DE vs. CMRI in relation to both expert and novice. Conclusion This new semi-automated algorithm of LV endocardial border detection based on 3DE data appears suitable for clinical use by either expert or novice investigators with greater reproducibility and time of analysis than 2DE.

Conventionally, the evaluation of left ventricular (LV) systolic function is based on ejection fraction assessment, which may be supplemented by other echocardiographic techniques, such as tissue Doppler imaging, 3D evaluation, and speckle tracking strains. However, these imaging modalities have a high technicity and are time-consuming, while being associated with reproducibility limitations. In this context, the usefulness of simpler measurements such as sys- tolic time intervals (STI) by pulsed Doppler echocardiography must be emphasized. Methods and results In this multicentre study, left ventricular ejection fraction (LVEF), dP/dtmax, LV stroke volume, myocardial longitudinal deformation, aortic pre-ejectional period (PEP, ms), and left ventricular ejection time (LVET, ms) were prospectively investigated and compared in 134 consecutive heart failure (HF) patients and 43 control subjects. Feasibility of STI measurements was 100%. Intra-observer reproducibility was 98% for PEP, 96% for LVET, 87% for LVEF, and 93% for global longitudinal strain (GLS). By subgroup analyses, with increasingly altered LVEF or GLS, PEP significantly increased, whereas significantly LVET decreased, resulting in a significantly increased PEP/LVET ratio (P , 0.001). In the HF patients group, a correlation between LVEF and PEP/LVET was found, with r ¼ 0.55 (y ¼ 20.0083x + 0.75, P , 0.001). Based on receiver operating curve analyses, the area under the curve was 0.91 for PEP/LVET . 0.43, which allowed us to detect LVEF , 35% with a sensitivity of 87%, and a specificity of 84%. Conclusion STI can be easily and accurately measured in clinical practice, and may be used for detecting alterations in LV systolic function. Moreover, this method is likely to have potential applications in the management of HF patients.

Aims The mechanisms associated with cardiac resynchronization therapy (CRT) benefits have been elucidated in part, however little is known about the effects of biventricular pacing (BVP) on regional myocardial contractility. Methods and results Twenty patients with conventional CRT criteria were studied before and after device implantation. Speckle tracking strain analysis was performed from standard bidimensional acquisitions during different pacing configurations. Longitudinal and transverse strains were measured for the six basal left ventricular (LV) segments. Acute CRT improved LV end-diastolic and end-systolic volumes without significantly modifying LV ejection fraction. Moreover, CRT produced significant changes in regional strain. When compared with spontaneous rhythm, simultaneous BVP caused a significant increase in longitudinal septal strain (−4.8 vs. −7.3%, P = 0.003) and a significant decrease in longitudinal lateral strain (−8.2 vs. −5.9%, P = 0.03). Simultaneous BVP also induced a significant decrease in transverse lateral strain from 17.9 to 10.9% ( P = 0.004). Conclusion In this acute echocardiographic study, BVP decreased lateral wall deformation while improving septal wall contraction, thus potentially improving LV contractile pattern.

Aims Systolic pulmonary artery pressure (sPAP) cannot always be assessed from Doppler-detected tricuspid regurgitation (TR), especially when sPAP is normal. The right ventricular isovolumic relaxation time (rIVRT) is related to sPAP, and assessment of rIVRT by tissular Doppler imaging (rIVRT 0 ) has recently been proposed as an alternative method for estimating sPAP in patients with pulmonary artery hypertension (PAH). We evaluated here its usefulness in everyday clinical practice. Methods and results We conducted a prospective Doppler vs. catheterization study in 26 patients. TR was undetectable in 6 patients (32%) with normal sPAP and in one patient (14%) from those with PAH. rIVRT 0 was recordable in all patients. We found a strong correlation between rIVRT 0 and sPAP (r ¼ 0.87; P , 0.0001). rIVRT 0 had a high sensitivity in detecting PAH, and a rIVRT 0 of 40 ms or less excluded PAH with a negative predictive value of 100%. We also found that a prolonged rIVRT 0 is not specific to PAH and that the rIVRT 0 -evaluated sPAP did not agree well with the catheter-evaluated value. Conclusion Measurement of rIVRT 0 can help estimate sPAP in the absence of TR: A normal rIVRT 0 excludes PAH with a high negative predictive value. - Abstract
AIMS: Functional ischaemic mitral regurgitation (IMR) is common in patients with ischaemic left ventricular dysfunction undergoing coronary artery bypass surgery. Although the presence of IMR negatively affects prognosis, the additional benefit of valve repair is debated, particularly with mild IMR at rest. Exercise echocardiography may help identify a subset of patients at higher risk of cardiovascular events by revealing the dynamic component of IMR. METHODS: A large prospective, multicentre, non-randomized registry is designed to evaluate the effects of surgery on IMR at rest and on its dynamic component at exercise (z). SIMRAM will enrol approximately 550 patients with IMR in up to 17 centres with clinical and exercise follow-up for 1 year. Three sets of outcomes will be prospectively assessed and several hypotheses will be tested including determinants of adverse outcome and progressive left ventricular remodeling, efficacy of treatment and role of ischaemia on the dynamic consequences of IMR. - Abstract
Objectives: Myocardial fibrosis in noninfarcted myocardium is emerging as a principal phenotype of vulnerability to adverse events such as mortality and hospitalization for heart failure (HHF), but its optimal noninvasive measurement remains uncertain despite consistently robust histologic validation data for extracellular volume fraction (ECV). We therefore compared ECV, native T1, post contrast T1, the gadolinium contrast partition coefficient (lambda), and the presence of nonischemic scar in their associations with mortality and HHF outcomes. Method: To quantify of myocardial fibrosis, we performed T1 mapping (MOLLI) in basal and mid short axis slices with cardiovascular magnetic resonance (CMR) before contrast and 12-30 minutes post contrast bolus in 1185 consecutive patients without amyloidosis, hypertrophic or stress cardiomyopathy. We assessed associations with outcomes using Kaplan-Meier plots and chi square values from univariable Cox regression models. All standard T1 mapping parameters were obtained: native and post contrast myocardial T1, the partition coefficient lambda, and ECV. ECV = (1-hematocrit) · [ΔR1myocardium]/[ΔR1bloodpool], where R1 = 1/T1 Late gadolinium enhancement imaging with phase sensitive reconstruction identified nonischemic scar. Results: Over a median of 1.7 years, 111 individuals experienced events after CMR: 55 HHF events and 74 deaths. ECV yielded better separation of Kaplan-Meier curves in a dose dependent fashion (Figure) and also stronger associations with the combined endpoint of death or HHF. The ECV chi square (77.3, p < 0.001) was at least twice as large as the Native T1 chi square (37.5, p < 0.001), the lambda chi square (34.8, p < 0.001) and nonischemic scar (chi square = 20.5, p<0.001). Post-contrast T1 was not associated with outcomes, even when adjusting further for time after contrast bolus, renal function, and patient weight (chi square <3, p >0.10). Conclusion: Analogous to histologic previously published validation data, quantitative ECV myocardial fibrosis measures associated with outcomes far stronger than other surrogate measures outcome measures such as native T1, post contrast T1 and nonischemic scar on LGE images. - Abstract
Purpose: Although delayed-enhancement magnetic resonance imaging (DEMRI) is essential for diagnosis of cardiac sarcoidosis (CS), the test was not available when pacemaker was implamted. Recently, MR-conditional pacemaker has become avilable and we hypothesized that this device would be useful for diagnosis and management of CS. The aim of this study was to assess the diagnostic ability of MR-conditional pacemaker about CS in patients with advanced A-V nodal block (AAVB). Methods: Twenty-seven AAVB patients (14 men, 13 women; mean age, 69 ± 11 years) who were implanted MR-conditional pacemaker were studied. DEMRI was performed 6 weeks after implantation of permanent pacemaker. In patients with positive for DE, additional examinations like echocardiography, radioisotope imaging, biopsy, and coronary computed-tomography were performed due to confirm the diagnosis of CS and exclude coronary artery disease. Results: DE was observed in 12 patients (44 %). Out of 12 patients, 2 patients were excluded for having prior myocardial infarction. Seven of 10 (70 %) patients were diagnosed of CS by the consensus criteria. Compared with non-CS group, CS group had significantly lower age (61 ± 12 years vs. 72 ± 9 years p = 0.017). There was no significant difference about sex, angiotensin-converting enzyme, brain natriuretic peptide, and left ventricular ejection fraction between 2 groups. Six patients had started corticosteroid therapy and 5 patients (83%) recovered A-V nodal conduction. Conclusion: MR-conditional pacemaker was useful for diagnosis and management of patients with AAVB caused by CS.

- Abstract
Background: Recent studies have demonstrated that longitudinal septal deformation in patients with left ventricular (LV) dysfunction and left bundle branch block (LBBB) predict LV remodeling after CRT. However, the importance of septal deformation patterns for prediction of long-term survival is unknown. Methods: From 2 centers a total of 193 CRT candidates with LBBB (NYHA II-IV, EF≤35, and QRS≥120ms) underwent echocardiography before CRT. Longitudinal 2-D strain analysis in the apical four-chamber view identified four patterns based on previously defined criteria: double-peaked systolic pattern (type 1), early pre-ejection shortening peak followed by prominent systolic stretch (type 2), shortening with one systolic peak inside 70% of ejection phase (type 3) and normal septal peak timing outside early 70% (type 4) . Outcome was pre-defined as freedom from death, left ventricular assist device or heart transplantation over 4 years. Results: Thirty-six patients (19%) had early septal deformation pattern type 1, 49(25%) type 2, 44(23%) type 3 and 64(33%) pseudonormal septal contraction type 4. There were 35 deaths, 4 transplantations, and 6 left ventricular assist device implantations over 4 years. The event rate was 0.05% (2/36) for type 1, 14% (7/49) for type 2, 25% (11/44) for type 3, 39% (25/64) for type 4. Patients with type 1 and 2 patterns had a more favourable event-free survival HR 0.26, CI 0.09-0.81 (P=0.005) than type 3 HR 1.17, CI 0.06-2.2 (P=0.65). Patients with type 4 septal contraction showed poor event-free survival HR 2.8, CI 1.6-5.1 (<0.001). (Figure 1) Conclusions: In LBBB-patients, septal deformation strain patterns predict long-term survival after CRT. Pattern 1 and 2 are highly associated with long-term survival while patients with pattern 4 have a poor prognosis.
