OBJECTIVE: To probe into the patterns and content of pharmaceutical care for one pregnancy with hepatic injury complicated with hepatitis B virus by clinical pharmacists. METHODS: The clinical pharmacists participated into the treatment process of one pregnancy with hepatic injury complicated with hepatitis B virus. According to the condition and the special physiological periods of the patient, tire clinical pharmacists provided pharmaceutical care on the selection of therapies, the communication with medical staff, the monitoring of the clinical effects and the education of pharmacotherapy. And the clinical pharmacists analyzed the rationality of antiviral drug and liver protecting drugs for the pregnancy through the latest evidence-based medicine. RESULTS: The clinical pharmacists provided pharmaceutical care for the patient and optimized the treatment regimen could improve the level of rational drug use and avoid potential health threats to the fetus induced by drug treatment. CONCLUSIONS: Combined with the latest evidence-based medicine, the clinical pharmacists provide pharmaceutical care from aspects of selection of therapies and adverse drug reactions can improve the safety and effectiveness of medication for special patients. OBJECTIVE ： To probe into the model and content of pharmaceutical care for the patient with liver failure treated with vitamin , so as to promote the rational drug use in clinic. METHODS ： Combined with the patients’ conditions, the clinical pharmacists participated into the treatment process of one patient with liver failure, provided pharmaceutical care for the selection of treatment regimen and clinical efficacy, and analyzed the rationality of using vitamin Kt through the evidence-based evidence. RESULTS ： If vitamin Kt was an ineffective treatment for patients with liver failure, vitamin Kt was not recommended. CONCLUSIONS ： It is necessary to attach more importance to the use of vitamin in patient with liver failure. Proper use should be given in the case of clinical evidence of vitamin K deficiency, which can ensure the medication safety and rationality. [ABSTRACT FROM AUTHOR]
