Despite robust vaccination schedules and high vaccination rates, many countries, including the U.S., have seen large pertussis outbreaks with a shift in recent years in the distribution of disease burden towards adolescents and young adults. Areas covered: This perspective covers problems related to the increased incidence of pertussis among adolescents. Because the Tdap vaccine only protects against pertussis for 1-2 years after vaccination, we propose a new strategy which aims to optimize the benefit of Tdap in adolescents. Expert commentary: Current pertussis vaccination schedules are based on age and have not been effective at protecting adolescents and teenagers from pertussis outbreaks. An alternative to the current practice would be to take advantage of the cyclical nature of pertussis outbreaks. Rather than immunizing children and adolescent solely based on age regardless of risk of pertussis at that moment, perhaps we should consider a 'timed' Tdap. The goal would be to administer Tdap to susceptible adolescents and young adults during periods when there is a greater risk of being exposed to pertussis. This approach would optimize the use of an effective, but short-lived vaccine by maximizing protection at the time of increased risk.

Pertussis or whooping cough is a respiratory disease caused by Bordetella pertussis or Bordetella parapertussis that are only known to infect humans. This severe and acute respiratory disease presents epidemic cycles and became a vaccine-preventable disease in the 1940s/1950s when developed countries introduced vaccination. The first type of vaccine developed against this disease was a whole-cell pertussis (wP) vaccine containing inactivated B. pertussis bacteria. Most developed countries produced their own vaccine and given the pediatric nature of the disease at the time of licensure, infants and toddlers were the primary targets and were thus massively vaccinated. The characterization of few virulence factors produced by B. pertussis enabled the development of second-generation pertussis vaccines called the acellular pertussis (aP) vaccines. These only contain 1-5 purified, detoxified B. pertussis proteins and were first introduced in Japan around 30 years ago. Australia, Europe and North America introduced aP vaccines approximately 15 years later, which replaced wP vaccines since then.

Objective: To evaluate the attitudes of pediatric healthcare workers (HCWs) toward pertussis vaccination. Method: A survey was conducted among pediatric HCWs, using an anonymous questionnaire, at a university children's hospital. Results: Of 298 eligible employees, 121 (40.6%) completed the questionnaire. Lack of information regarding the personal risk of acquiring a pertussis infection and fear of side effects were frequently associated with the refusal of getting immunized. Conclusion: Misconception about pertussis vaccination was prevalent, especially among nursing staff, who are in even closer contact with at-risk children than physicians. Intervention strategies should be designed with focus on nursing staff. Our findings confirm the importance of comprehensive approaches for more education on HCWs to improve acceptance of vaccination.

Conventional wisdom suggests that if there is a vaccine that is effective in preventing a disease, vaccine uptake will increase when the disease risk is high. Recent evidence, however, suggests that this may not always be the case. In a study we conducted in Washington State, we found no population-level increase in pertussis vaccination of infants during a pertussis epidemic. In this paper, we aim to review what is known about the history of vaccine uptake during epidemics of vaccine-preventable disease, the challenges facing public health campaigns responding to these epidemics, and how the effect of a vaccine-preventable disease epidemic on vaccine uptake can be studied.

Economic studies of vaccines, including vaccine development and delivery issues, are increasingly needed to inform policy recommendations and programmatic decisions in the USA. This need arises from the increasing costs of vaccines, the complexity of the US healthcare system and the limited number of vaccine manufacturers in the market. We have developed a national research agenda in domestic and global vaccine economics by conducting key informant interviews with 42 experts and inviting ideas from an additional 128 experts. To assess priorities among the 129 ideas that were generated, we asked 15 experts representing a broad range of perspectives to rank the ideas and we analyzed their votes. The highest-ranking domestic research ideas included evaluating: the costs of vaccine shortages, the cost-effectiveness of potential human papillomavirus vaccination and adult and adolescent pertussis vaccination programs and the cost-effectiveness of universal vaccine purchase programs for adults as well as children. The highest-ranking globally-oriented ideas included developing a resource allocation model to support the best vaccination program decisions with limited funds and assessing the cost-effectiveness of HIV, rotavirus, meningococcal and malaria vaccines in developing countries. To optimize the usefulness of vaccine economics research, conceptual issues, such as how to set values for the prevention of illness and how to maximize social equity through investments in vaccines, must be addressed.
