Our objective was to compare the rate of native tissue repair (NTR) versus sacrocolpopexy (SCP) and reconstructive (RECON) versus obliterative repair (OBR) for the treatment of pelvic organ prolapse (POP), evaluating for health care disparities based on race, socioeconomic, and geographic factors.The National Inpatient Sample database was queried for patients older than 18 years undergoing POP surgery from 2008 to 2018. Baseline demographics, comorbidity index, socioeconomic, and hospital variables were extracted. The weighted t test, Wilcoxon test, and χ2 test were used to compare the rate of (1) NTR versus SCP and (2) RECON vs OBR. Multivariate weighted logistic regression was used to compare while controlling for confounders. Reference groups were White race, Medicare patients, northeast region, small hospital size, and rural location.Of 71,262 patients, 67,382 (94.6%) underwent RECON. Patients undergoing OBR were older and had a higher comorbidity score. Multivariate analysis showed the following: (1) Black, Hispanic, and other races; (2) Medicaid patients; (3) patients at urban teaching hospitals are less likely to receive RECON. Patients in the midwest were more likely to receive RECON. Among 68,401 patients, 23,808 (34.8%), and 44,593 (65.19%) underwent SCP and NTR, respectively. Hysterectomy was more common in the NTR group. Multivariate analysis showed the following:(1) Black, Hispanic, and "other" races; (2) uninsured and Medicaid patients; (3) patients in the midwest, south, and west were at higher odds of receiving NTR. Patients in large and urban hospitals were less likely to undergo NTR.Racial, socioeconomic, and geographic disparities exist in surgical management for POP warranting further study to seek to eliminate these disparities.

Objective We hypothesized that instruments of pelvic floor dysfunction would yield similar responses on web-based and smartphone administration compared with paper. Methods Subjects presenting with pelvic floor disorders were prospectively enrolled at 5 sites and invited to complete 4 validated pelvic floor disorder questionnaires (Pelvic Floor Distress Inventory 20, Pelvic Floor Impact Questionnaire 7, Pelvic Organ Prolapse/Urinary Incontinence Sexual Function Questionnaire 12, Bristol Stool Scale) on both paper and electronic formats, 2 weeks apart, with the order of administration being randomized. Participants completed the questionnaires electronically on the internet via REDCap or using the PelvicTrack App on a smartphone or tablet. Results Two hundred thirty-four subjects were enrolled, and 132 subjects (56%) completed both sets of questionnaires with no intervening treatment. This group was 58 (±15) years old with body mass index 28 (±6) kg/m and parity 2 (1, 3) and was 77% white, 6% African American, 7% Asian, and 10% other. Presenting complaints were classified as 58% urinary, 37% prolapse, and 5% defecatory. There was no difference in overall demographic information between those who completed the second round of questionnaires and those who did not. There was no difference in age between those who chose to complete the questionnaires via REDcap and those who chose to complete the questionnaires via smartphone. Correlation coefficients between questionnaire administration range from 0.5 to 0.8. There was no significant difference in the responses for each total scale and individual scale between the first or second administration. Conclusions We demonstrated moderate to strong reliability between scales of pelvic floor dysfunction administered electronically compared with paper version. Our results strongly suggest that it is feasible and reliable to administer pelvic floor questionnaires in an electronic format on REDCap and on smartphones.

Background Spontaneous perioperative otorrhagia is an extremely rare entity with only 4 cases reported in the literature to date, all of which were recognized after the termination of the procedure. Case We describe the first reported case of otorrhagia recognized intraoperatively causing abortion of the procedure in a 60-year-old woman undergoing laparoscopic sacrocolpopexy. Otoscopy by the otolaryngology service revealed multiple intracutaneous hematomas and bleeding in bilateral external auditory canals. Computed tomography scan of the head revealed no intracranial hemorrhage. She underwent postoperative drainage by the otolaryngology service with no permanent ear-related sequelae. Conclusions We present the fifth reported case of spontaneous perioperative otorrhagia, the first of which to be noticed intraoperatively and cause premature termination of the procedure. The etiology is postulated to be increased arterial and venous pressures causing rupture of subcutaneous capillaries. In our case, several factors may have contributed to this event, including steep Trendelenburg patient positioning, intraperitoneal carbon dioxide insufflation from laparoscopy, and an intraoperative hypertensive episode.

OBJECTIVES : The purpose of this study was to explore resident opinions about vaginal hysterectomy training and to evaluate future practice patterns and impediments to continuing vaginal hysterectomies after graduation. METHODS : An anonymous survey was e-mailed to fourth-year gynecology residents before graduation. The survey assessed opinions on vaginal hysterectomy training and future practice plans. RESULTS : One hundred eighty-three residents responded from 41 states. Most residents (75%) think vaginal hysterectomy training has been adequate to prepare them for future practice. Residents were more likely to respond that their training was adequate if they had performed more than 20 vaginal hysterectomies and if they had learned morcellation techniques. Only 42% of residents report vaginal route as their preferred route for hysterectomy. CONCLUSIONS : Most residents feel that training in vaginal hysterectomy has been adequate to prepare them for their future practice, although they are more likely to feel prepared if they have performed more than 20 vaginal hysterectomies.
