Background & Aims: Recent reports have suggested the mucosa of an ileal reservoir could be at risk of neoplasia. Risk factors may include the age of the pouch, chronic pouchitis, and previous colonic neoplasia. This study examined a group of such patients to determine the risk of dysplasia., Methods: From a cohort of 1221 patients with ileal pouches, 171 patients with possible risk factors were selected. Successful contact was made with 138 patients who were invited for endoscopy and multiple biopsies. Biopsy specimens were stained with H&E and p53, scored for inflammatory changes including villous atrophy, and analyzed by flow cytometry., Results: One hundred six patients took part and fell into 1 or more of the following clinical categories: chronic pouchitis (n = 34), pelvic pouch for > or =12 years (n = 42); Kock pouch for > or =14 years (n = 29), and neoplasia in colectomy specimen (n = 11). Thirty-three patients had severe villous atrophy. One patient of 106 (95% confidence interval, 0.9% +/- 1.6%) with a long-standing pouch had low-grade dysplasia that was multifocal. DNA analysis by flow cytometry showed aneuploidy in this patient and 2 others., Conclusions: These data suggest that the development of dysplasia in ileal pouches performed for ulcerative colitis is probably a rare event within 15-20 years of pouch surgery.

Background & Aims: Recurrence of Crohn's disease frequently occurs after surgery. A randomized controlled trial was performed to determine if mesalamine is effective in decreasing the risk of recurrent Crohn's disease after surgical resection is performed., Methods: One hundred sixty-three patients who underwent a surgical resection and had no evidence of residual disease were randomized to a treatment group (1.5 g mesalamine twice a day) or a placebo control group within 8 weeks of surgery. The follow-up period was a maximum of 72 months., Results: The symptomatic recurrence rate (symptoms plus endoscopic and/or radiological confirmation of disease) in the treatment group was 31% (27 of 87) compared with 41% (31 of 76) in the control group (P = 0.031). The relative risk of developing recurrent disease was 0.628 (90% confidence interval, 0.40-0.97) for those in the treatment group (P = 0.039; one-tail test) using an intention-to-treat analysis and 0.532 (90% confidence interval, 0.32-0.87) using an efficacy analysis. The endoscopic and radiological rate of recurrence was also significantly decreased with relative risks of 0.654 (90% confidence interval, 0.47-0.91) in the effectiveness analysis and 0.635 (90% confidence interval, 0.44-0.91) in the efficacy analysis. There was only one serious side effect (pancreatitis) in subjects in the treatment group., Conclusions: Mesalamine (3.0 g/day) is effective in decreasing the risk of recurrence of Crohn's disease after surgical resection is performed.

Two instruments, the time trade-off technique and direct questioning of objectives, were used to assign the worth (utility) of the perceived quality of life of patients who had ulcerative colitis requiring surgery. Utilities ranged from 0 to 1 with 0 signifying death and 1 signifying full health. Two groups of patients were studied. Group 1 consisted of 20 patients (14 male and 6 female; mean age, 34.0 +/- 10.3 years) who were studied preoperatively and 1 year postoperatively. The mean preoperative utility was 0.58 +/- 0.34 using the time trade-off technique and 0.38 +/- 0.27 using the direct questioning of objectives. Mean utilities were significantly higher postoperatively, 0.98 +/- 0.07 using the time trade-off technique and 0.88 +/- 0.19 using the direct questioning of objectives (P less than 0.05). Group 2 included 93 patients who had had surgery at least 1 year previously. Twenty-eight had conventional ileostomies, 28 had Kock pouches, and 37 had ileal reservoirs. Mean utilities for these three subgroups ranged from 0.87 +/- 0.18 to 0.97 +/- 0.6. No significant differences were detected among the three subgroups. This study suggests that quality of life is improved after surgery for ulcerative colitis and is high irrespective of the surgical procedure.
