Nowadays, the Internet of Things (IoT) is used in every domain, especially in the healthcare system it plays a very important role in our life and made our life easier by providing various facilities in every area. On the Internet of Medical Things (IoMT), IoT devices collect real-time data from patients and send it to the cloud through the Internet. These gathered data are extorted to centralized processing and computational power. Centralization can lead to a single point of failure, data modification, snooping, security, and privacy evasion. Blockchain can resolve such issues by providing decentralized computational power and better storage capacity for IoT data. Therefore, integrating IoT and blockchain technologies can be an intelligent way to implement a decentralized IoT-based smart healthcare system. We find that integration of the blockchain and IoT is persuasive and can cause remarkable transformations across the healthcare system. We discussed blockchain and its applicability in the healthcare industry. The paper explores blockchain's dimensions, making the innovative healthcare system more secure and stable. We provide a comparative analysis of well-known recent research on IoT-based smart healthcare system security using blockchain based on different parameters such as architecture, data integrity, medical information sharing, distributed electronic health records, patient encryption key, access control, hardware implementation, etc. The paper focuses on the current security issues in IoT-based smart healthcare systems, architectures, taxonomy, and research gap and their possible solutions we point out various research challenges that should be considered before the deployment of a blockchain network in an IoT setting. With the increase in the use of the internet, there is an increase in hacking also, especially in the health-care sector, e-commerce, military, and Education. Dealing with the huge volume of information in the electronics-healthcare area requires the processing of patient's information day by day and its memory allocation. Cloud is the best option to store these health records in a distributed manner rather than on a single server. So, whatever the information gathered from it will be encoded on block chain technology and an anonymous digital ring signature will be used to authorize the intended user and maintain the privacy of patient cloud-based data distributed Electronics health record system before sending the information to the cloud storage. As healthcare data is extremely sensitive, it poses a risk of invading individuals' privacy if stored or exported without proper security measures. De-identification entails pseudonymization or anonymization of data, which are methods for temporarily or permanently removing an individual's identity. These methods are most suitable to keep user healthcare data private. Inference attacks are a commonly overlooked weakness of de-identification techniques. In this paper, I discuss a method for de-identifying Electronic Healthcare Records (EHR) using chained hashing to generate short-lived pseudonyms to reduce the impact of inference attacks, as well as a mechanism for re-identification based on information self-determination. It also removes the weaknesses of existing de-identification algorithms and resolve them by using appropriate real-time de-identification algorithm, Ephemeral Pseudonym Generation Algorithm (EPGA). While record linkage can expand analyses performable from survey microdata, it also incurs greater risk of privacy-encroaching disclosure. One way to mitigate this risk is to replace some of the information added through linkage with synthetic data elements. This paper describes a case study using the National Hospital Care Survey (NHCS), which collects patient records under a pledge of protecting patient privacy from a sample of U.S. hospitals for statistical analysis purposes. The NHCS data were linked to the National Death Index (NDI) to enhance the survey with mortality information. The added information from NDI linkage enables survival analyses related to hospitalization, but as the death information includes dates of death and detailed causes of death, having it joined with the patient records increases the risk of patient re-identification (albeit only for deceased persons). For this reason, an approach was tested to develop synthetic data that uses models from survival analysis to replace vital status and actual dates-of-death with synthetic values and uses classification tree analysis to replace actual causes of death with synthesized causes of death. The degree to which analyses performed on the synthetic data replicate results from analysis on the actual data is measured by comparing survival analysis parameter estimates from both data files. Because synthetic data only have value to the degree that they can be used to produce statistical estimates that are like those based on the actual data, this evaluation is an essential first step in assessing the potential utility of synthetic mortality data. Data derived from electronic health records (EHR) are heterogeneous with availability of specific measures dependent on the type and timing of patients' healthcare interactions. This creates a challenge for research using EHR-derived exposures because gold-standard exposure data, determined by a definitive assessment, may only be available for a subset of the population. Alternative approaches to exposure ascertainment in this case include restricting the analytic sample to only those patients with gold-standard exposure data available (exclusion); using gold-standard data, when available, and using a proxy exposure measure when the gold standard is unavailable (best available); or using a proxy exposure measure for everyone (common data). Exclusion may induce selection bias in outcome/exposure association estimates, while incorporating information from a proxy exposure via either the best available or common data approaches may result in information bias due to measurement error. The objective of this paper was to explore the bias and efficiency of these three analytic approaches across a broad range of scenarios motivated by a study of the association between chronic hyperglycemia and 5-year mortality in an EHR-derived cohort of colon cancer survivors. We found that the best available approach tended to mitigate inefficiency and selection bias resulting from exclusion while suffering from less information bias than the common data approach. However, bias in all three approaches can be severe, particularly when both selection bias and information bias are present. Unmeasured confounding undermines the validity of observational studies. Although randomized clinical trials (RCTs) are considered the "gold standard" of study types, we often observe divergent findings between RCTs and empirical settings. We present the "L-table", a simulation-based, prior knowledge (e.g., RCTs) guided approach that estimates the true effect adjusting for the potential influence of unmeasured confounders when using observational data. Using electronic health record data from Kaiser Permanente Southern California, we compare the effectiveness of coronary artery bypass grafting (CABG) and percutaneous coronary intervention (PCI) on endpoints at 1, 3, 5, and 10 years for patients with stable ischemic heart disease. We applied the L-table approach to the propensity score adjusted cohort to derive the omitted-confounder-adjusted estimated effects. After the L-table adjustment, CABG patients are 57.6% less likely to encounter major adverse cardiac and cerebrovascular event (MACCE) at 1 year (OR [95% CI] 0.424 [0.396, 0.517]), 56.4% less likely at 3 years (OR [95% CI] 0.436 [0.369, 0.527]), and 48.9% less likely at 5 years (OR [95% CI] 0.511 [0.451, 0.538]). CABG patients are also 49.5% less likely to die by the end of 10 years than PCI patients (OR [95% CI] 0.505 [0.446, 0.582]). We found the estimated true effects all shifted towards CABG as a more effective procedure that led to better health outcomes compared to PCI. Unlike existing sensitivity tools, the L-table approach explicitly lays out probable values and can therefore better support clinical decision-making. Healthcare delivery, like manufacturing, is on the verge of a fundamental shift into the new era of smart and connected health care, termed Health Care 4.0. Sharing healthcare data is an important step in improving the healthcare system's intelligence and service quality. Healthcare data, which is a personal asset of the patient, should be owned and managed by the patient rather than being dispersed among several healthcare systems, preventing data exchange and jeopardizing patient privacy. Electronic Health Records (EHRs) assist individuals by allowing them to combine and manage their medical data. On the other hand, today's EHR systems fall short of providing patients with traceable, trustworthy, and secure ownership over their medical data, creating serious security risks. Furthermore, the most of present EHR techniques and systems are centralized, making it difficult to share medical data and increasing the danger of a single point of failure. Blockchain technology can enable dependable, transparent, and auditable computing in the healthcare industry by utilizing a decentralized network of peers and a public ledger. In this article, we propose Patient-Controlled Blockchain Enabled Electronic Health Records as a way for patients to have safe control over their data that is decentralized, immutable, transparent, traceable, and trustworthy. Limited research exists on the health of the homeless population, and often relies on large health care datasets and varied methods, with little work to understand how the choice of measurement, such as an address-based indicator versus diagnosis code, affects the estimates of demographics and comorbidity burden of this population. Using state-wide hospital discharge data, we identified homeless individuals three ways: via ICD-10 Z59.0, an administrative indicator, and those with both. From 2,427,549 discharges, 17,479 individuals were identified via only Z-code, 9183 via only the indicator, and 4578 had both. There were differences in the race, sex, payer, and admission type between the groups. Similarly, comorbidity profiles were different, with the differences reaching 11.4% for alcohol abuse. The difference in the size, composition, and comorbidities between these groups suggests that researchers, clinicians, and policy experts should be wary of current methods to identify homeless individuals and continue to improve capture of this determinant of health. Simply, if we are unable to accurately identify the group of interest and their potential health needs we are equally unable to effectively intervene and provide appropriate and relevant care. In medical product development, there has been a growing interest in utilizing real-world data which have become abundant owing to advances in biomedical science, information technology and engineering. High-quality real-world data may be utilized to generate real-world evidence for regulatory or healthcare decision-making. We discuss propensity score-based approaches for leveraging patients from a real-world data source to construct a control group for a non-randomized comparative study or to augment a single-arm or randomized prospective investigational clinical study. The proposed propensity score-based approaches leverage real-world patients that are similar to those prospectively enrolled into the investigational clinical study in terms of baseline characteristics. Either frequentist or Bayesian inference can then be applied for outcome data analysis, with the option of down-weighting information from the real-world data source. Exposures derived from electronic health records (EHR) may be misclassified, leading to biased estimates of their association with outcomes of interest. An example of this problem arises in the context of cancer screening where test indication, the purpose for which a test was performed, is often unavailable. This poses a challenge to understanding the effectiveness of screening tests because estimates of screening test effectiveness are biased if some diagnostic tests are misclassified as screening. Prediction models have been developed for a variety of exposure variables that can be derived from EHR, but no previous research has investigated appropriate methods for obtaining unbiased association estimates using these predicted probabilities. The full likelihood incorporating information on both the predicted probability of exposure-class membership and the association between the exposure and outcome of interest can be expressed using a finite mixture model. When the regression model of interest is a generalized linear model (GLM), the expectation-maximization algorithm can be used to estimate the parameters using standard software for GLMs. Using simulation studies, we compared the bias and efficiency of this mixture model approach to alternative approaches including multiple imputation and dichotomization of the predicted probabilities to create a proxy for the missing predictor. The mixture model was the only approach that was unbiased across all scenarios investigated. Finally, we explored the performance of these alternatives in a study of colorectal cancer screening with colonoscopy. These findings have broad applicability in studies using EHR data where gold-standard exposures are unavailable and prediction models have been developed for estimating proxies. [ABSTRACT FROM AUTHOR]
