The nutritional health and wellbeing of children, and by extension their weight, is a heated topic in contemporary discussions of food and health, particularly for low-income populations. Despite contrary understandings, there remains a dominant societal framing that parents – in particular low-income mothers – are solely responsible for the status of their children's health and wellbeing. In this paper, we examine how low-income mothers are positioned within the academic literature to reveal where responsibility for children's health and well-being is positioned. We present a meta-synthesis of 18 qualitative studies to identify how mothers' food choices and feeding are positioned, and the recommendations that researchers identify for promoting child health within this discursive terrain. We found that low-income mothers faced multiple challenges relating to cost, convenience, concerns about health and wellbeing. However, many of the recommendations made by researchers focused extensively on behavioural interventions aimed at the vulnerable mother rather than structural interventions to support mothers' feeding practices. We argue that discourses of low-income motherhood must recommend structural, and not just individual, change to counteract dominant constructions of the 'vulnerable-empowered mother'. In the past decades, public health has increasingly addressed what has been called the children's obesity 'epidemic', most notably through large-scale initiatives promoting physical activity. Through a discourse analysis the current paper critically examines such efforts in Canadian and Australian public health. Public health websites in Canada and Australia were examined for information concerning children's health, physical activity and obesity and explored for how these issues were represented in the discourse. Bacchi's (2009) 'What's the problem represented to be?' approach to discourse analysis guided our interrogation of the taken-for-granted assumptions underlying Canadian and Australian public health discourse, the ideological and political influences involved in its construction, and the knowledge base upon which it rests. This Editorial provides a critical review of the progress in social determinants of health research over the last two decades, suggesting new perspectives which may further our understanding of persistent social inequalities in health. It highlights the global significance of the Special Issue setting it in the context of the recently released WHO Commission on Social Determinants of Health Final Report (CSDH 2008). The Editorial introduces individual papers in the Special Issue and discusses how they are in line with, complement or provide feedback to, the WHO CSDH Final Report and its call for actions to close the health gap in a generation. Finally, based on the articles in this Special Issue and emerging themes of the global research and policy literature, it outlines a number of significant directions for future research and actions, from both methodological and theoretical points of view. The decline in indicators of human development linked to rising social inequalities, despite post-modern society's unprecedented economic prosperity, has been called 'modernity's paradox' (Keating and Hertzman 1999). Scholars of developmental health suggest that micro-level influences from the social, economic and psychological circumstances in early life may explain social inequalities across the lifespan. However, children's poor developmental and health outcomes are also a product of the wider contexts of their lives. This paper extends the human developmental framework by linking the proximal determinants of health and well-being with macro-level forces. It reviews recent changes in the political, economic and social environments in developed countries, to provide insights into the recent trends in poor outcomes in children and youth, which remain paradoxical given the expectations of economic prosperity and the advances in medical and other technologies. Objectives: This paper examines social inequality in children's health following the election of Evo Morales. Bolivia is marked by dramatic socioeconomic, ethnic and geographic inequality in children's health. The Morales government initiated several public health reforms designed to provide more equal access to public health services. Methods: Analysis was conducted using Bolivia Demographic and Health Survey data from 2003 to 2008. Results: The analysis shows some improvements in children's health. However, inequality in access to doctors, child mortality and child nutritional status did not diminish. In contrast, inequalities in immunization declined dramatically. Conclusions: Results suggest that aspects of health that are easier to implement such as immunization are more accessible to public policy than are more complex health problems like malnutrition and high child mortality. Breastfeeding is the optimal nutrition for infants and requires the infant and mother to spend significant time together. In July 2006 the Australian Government introduced the Family Law Amendment (Shared Parental Responsibility) Act 2006 (Cth) (the Shared Parental Responsibility Act 2006) which puts in place a legal presumption of shared parental responsibility for children after separation and which emphasises 'equal time' parenting arrangements. The expectation of 'equal time' or substantial and significant parenting arrangements becomes problematic when considering breastfed children. Decisions about parenting of children under the Family Law Act 1975 (Cth) are required to be made with the 'best interests of the child' as the paramount consideration; a central tenet of the Act which remains in place following the Shared Parental Responsibility Act 2006. There appears to be a tension in determining the best interests of the child in cases where children are breastfed and their father is seeking equal or substantial shared care arrangements. This article begins a discussion about the decisions regarding 'shared parenting' of breastfed children that do not always appear to be in the best interests of children's health and well-being. Two cases from an on-going study to investigate breastfeeding women's experiences of the implementation of the Act will be used to illustrate that the court made decisions for breastfeeding mothers are not consistent and compromise the ability of women to continue breast feeding. The paper argues that the Shared Parental Responsibility Act 2006, and the decisions made, can work at a macro-level to produce social and health disparities for these children. Further questions are raised about the best interests of children when domestic violence and/or abuse are present. The impact of this new law on the continued breastfeeding of very young children is an unacknowledged consequence and a public health concern. [ABSTRACT FROM AUTHOR]

Households are important health contexts, providing social, emotional, financial and material support, but little is known about the role of household composition in the social etiology of Indigenous health. Our research is framed by an Indigenous standpoint, using eight waves of data from the Longitudinal Study of Indigenous Children. We investigated whether household composition and change in household composition were associated with the self-reported general health of Indigenous children and their mothers, adjusting for socioeconomic, household structure and social support factors. Our measure of household composition comprised eight groups differentiating lone and couple parents, living with and without other children and adults. Study children in couple households with other children and adults were 16% less likely to have excellent health and mothers in these same households were 7% less likely to report excellent health than children and mothers in couple households. We find little evidence that mothers in lone parent households have poorer health than mothers in couple households, after adjustment for covariates. Change in household composition was positively associated with health for both children and mothers. The results caution against presuming a direct translatability of research findings from non-Indigenous to Indigenous Peoples. [ABSTRACT FROM AUTHOR]

This article draws upon research involving in-depth interviews with 60 mothers of young children about the home healthcare in which they engage when promoting their children's health and dealing with their illnesses, allergies or developmental problems. The study found that a series of often interconnected discourses were evident in the women's accounts. These included the discourses of health states as controllable, good health as an outcome of good management and the child's body as vulnerable. Other discourses were related to the concept of the 'good mother'. The interviewees also employed the discourses of home healthcare as emotionally distressing and as hard work and of children's illness as a mother's loss of control. As this suggests, such caring was often an intensely embodied and negative emotional experience for the mothers, particularly if they felt as if they had lost control over their children's bodies. This study explored Aboriginal perspectives of child health and wellbeing in an urban area in partnership with Aboriginal people and organisations. In depth interviews were conducted with 25 grandparents, parents, aunties or uncles of Aboriginal children. Interviews were transcribed and thematically analysed. A major conceptual theme was related to social, historical, and political factors seen by participants as influencing urban Aboriginal child health and wellbeing. This theme was called 'Strengths and Challenges: Harder for Koori Kids/Koori Kids Doing Well'. Increased challenges to achieving good health and wellbeing faced by Aboriginal children due to factors in their social, historical and political environment were emphasised. Many of these factors can be related back to historical and contemporary forms of racism. On the other hand, there was also a clear call to recognise and celebrate that many Aboriginal children were doing well in the context of these added challenges. [ABSTRACT FROM AUTHOR]

Health and wellbeing is defined internationally as a multi-dimensional and holistic concept, particularly within Indigenous worldviews. However, in Australia there is a lack of detailed frameworks supporting such definitions that are founded on Aboriginal knowledges. Tins study aimed to explore Aboriginal perspectives of child health and wellbeing in an urban setting. Qualitative interviews with 25 care-givers of Aboriginal children living in Melbourne, Australia were conducted. Aboriginal people and community controlled organisations were collaborative partners in all stages of the research. A conceptual fiamework of Aboriginal child health and wellbeing in an urban setting was developed comprising four main themes: Strong Culture; Strong Child; Strong Environment; and Strengths and Challenges. Aboriginal conceptions of culture are considered central to Aboriginal child health and wellbeing in an urban context. A holistic framework that privileges Aboriginal knowledge of child health and wellbeing has not previously been available. Further exploration of socio-ecological models within Aboriginal child health and wellbeing contexts is needed. The high incidence of sexual abuse of Indigenous children in remote Australia prompted the Australian Government to implement an emergency response of health and welfare measures (the Intervention) in 2007 to protect children and communities from harm. In this article we seek to assess the measures employed in the Intervention against the body of evidence of strategies likely to be effective in reducing Indigenous health disadvantage. Our view is that the emergency response may not have long-term benefit because of the dearth of basic primary health services in remote Aboriginal communities to continue the effort that the Intervention has begun. We conclude that Intervention measures are not sufficiently well aligned with evidence-based health improvement strategies to bring about long-term health gain in Indigenous communities. Governments must reassess the benefit of externally-imposed approaches to reducing health disparity and move to actively build capacity in Aboriginal communities to deliver culturally-relevant health and social services that meet community need. This article engages with a common model of 'pathway' in longitudinal health research. National studies of child health and development are taken as 'signature' research into how life pathways can be tracked by social scientists. The work of the philosopher Lingis is used to show how contemporary social theory can enlarge our propositions about life pathways and how they are shaped. No specific longitudinal health studies are critiqued, rather the analysis is at a conceptual level aiming to highlight new ways of thinking about events in time, forces, and lived experience.
