This qualitative study investigates health care practitioners’ approaches to early childhood obesity in Denmark and their view on their own ability to initiate processes of change in affected families, and it asks the overall question of whether perceived barriers become real through practitioners’ reluctance to intervene in families labelled as ´problematic’. The paper identifies three approaches in the practitioners’ narratives: the socially oriented, the individually oriented, and the mixed. The individually oriented approach was based on a logic resembling individualistic explanatory models of behaviour change, and was related to a positive perspective on their own ability to move families towards healthier habits by health care practitioners. The socially oriented approach borrowed elements from a sociological perspective, which seemed to lead to resigned pessimism in the face of the complexity of the problem of childhood overweight and a reluctance to address early childhood overweight in some families. In practitioners of all three types, widespread cultural essentialism was found. Non-western, ethnic minority background was seen as determining family habits and making preventive action especially difficult. Based on this, the current paper discusses whether individual actions and choices are being ascribed too much explanatory power, and the health system's biased perceptions too little. Menopause is an important health transition for women, but health-related research has largely failed to account for menopause in a meaningful way, especially when making group comparisons. Age often stands in for menopausal status; at times, it is treated as a dichotomy; however, these are clearly oversimplifications. Menopause is a process that takes place over several years, and varies in onset, length, and expression through symptoms. Defining menopause dichotomously or linking it with age obscures variability in women's experiences and overlooks patterns in physical health, mental health, and quality of life. This paper reviews research to highlight menopausal differences by age, race, and lifestyle factors in the United States. It suggests ways to incorporate a life course-based view of menopause in health research, improving our understanding of women's health outcomes on multiple dimensions. [ABSTRACT FROM AUTHOR]

Evidence-based medicine (EBM) has been one of the most important movements in clinical medicine and public health in recent years. At the heart of the EBM movement lies the Cochrane Collaboration, an infl uential organisation that produces systematic assessments of healthcare interventions known as Cochrane reviews. Although Cochrane methods were initially designed to test the efficacy of medical therapies, the desire for 'evidence-based' practice has pushed the movement far beyond its initial scope into the assessment of complex social phenomena. Through an examination of one particular Cochrane review -- Physician advice for smoking cessation -- this paper highlights the limitations of EBM 'creep', and some of the more problematic conceptions of human nature underwriting Cochrane principles and methodologies. Contemporary health policy increasingly positions responsibility for the management of health with the individual which reflects newer neo-liberal discourses of health. Such an approach can be seen as problematic in the context of men's health, with men tending to be seen as largely 'irresponsible' towards their own health. This paper addresses this question by drawing on qualitative data on how men conceptualise responsibility for health. Whilst the desire to be responsible for health was borne by most of the men in the study, this was not always reflected in practice. There was also evidence of strategies that men adopted for either divesting themselves of responsibility for health or for legitimising lack of responsibility. In some instances, acting 'irresponsibly' was what defined participants as 'real men'. The implications of these findings for men's health policy are discussed with specific reference to the recent publication of Ireland's National Men's Health Policy. [ABSTRACT FROM AUTHOR]

Rates of lung cancer in China are rising rapidly, creating an urgent need for prevention. Effective prevention measures require understanding local beliefs and perceptions about the risk for developing lung cancer. This article explores the explanations that Chinese lung cancer patients and their families give about the aetiology of their disease. Fifty-three interviews were conducted among lung cancer patients and their family members at a large tumour hospital in southern China. Participants presented a complex multifactorial explanation of lung cancer associating their disease with risks like tobacco use, occupational exposures, environmental pollution, lifestyle changes, and personal characters. While these are all standard risk factors commonly associated with lung cancer, participants presented them within a larger contextual frame of structural issues that impede their ability to change their behaviours. Using a social ecological model, we demonstrate how China's socio-cultural environment shapes assumptions about the risk of lung cancer with particular reference to work, home, social situations, and the natural environment. In Australia's Northern Territory (NT) most people who access health services are Aboriginal and most healthcare providers are non-Indigenous; many providers struggle to deliver culturally competent care. Cultural awareness training is offered however, dissatisfaction exists with the limited scope of training and the face-to-face or online delivery format. Therefore, we developed and evaluated Ask the Specialist: Larrakia, Tiwi and Yolŋu stories to inspire better healthcare, a cultural education podcast in which Aboriginal leaders of Larrakia, Tiwi and Yolŋu nations, known as the Specialists, answer doctors' questions about working with Aboriginal patients. The Specialists offer 'counterstories' which encourage the development of critical consciousness thereby challenging racist narratives in healthcare. After listening to the podcast, doctors reported attitudinal and behavioural changes which led to stereotypes being overturned and more culturally competent care delivery. While the podcast was purposefully local, issues raised had applicability beyond the NT and outside of healthcare. Our approach was shaped by cultural safety, critical race theory and Freirean pedagogy. This pilot is embedded in a Participatory Action Research study which explores strategies to improve culturally safe communication at the main NT hospital Royal Darwin Hospital. Food security is an evolving concept with various definitions and associated meanings. The way food security is understood by policy-makers impacts on how it gets addressed in public policy. This research uses an interpretive approach to uncover how food security is represented by stakeholders. Semi-structured interviews were conducted with 24 stakeholders in South Australia between June and September 2010. The findings reveal four dominant representations for food security that locate responsibility for the issue across different stakeholders, namely individuals, governments, communities and private enterprise. These representations hold different underlying assumptions about the role of government in addressing the issue; the neo-liberal perspective supports a reduced role for government whereas the social determinants of health perspective calls for greater government intervention. Food security activists may need a greater awareness of how they represent the issue in order to change public policy action in this area. Over the last two decades, childhood obesity has emerged as a major public health issue in Switzerland. Health professionals and public health organisations now frame this condition as a disease that requires medical intervention. In this article, I report the findings of an ethnographic study conducted in a Swiss hospital program for the management of childhood obesity. The study explored the impact of the medical treatment of childhood obesity on children's lives. Using a Bourdieusian approach to guide my analysis, I found that the experience of the therapy varies substantially according to children's socioeconomic position. Children with a high SES seem to encounter less difficulties in adapting their lifestyle to professionals' recommendations than low SES children, because their habitus facilitates the internalisation of health norms and they have greater access to economic, social and cultural capitals. This article examines how Indigenous women who receive intercultural healthcare programs manage to develop cooperation networks, get involved in community affairs and improve their reproductive healthcare. It concentrates on the case of the Bolivian intercultural health program 'EXTENSA' and analyzes how this program is successful at activating greater community participation in health prevention and improving the reproductive healthcare of the Indigenous women who live in the Department of Beni, Bolivia. Through an ethnographic analysis, it argues that community participation and healthy behaviors are the result of tapping into bonding social capital and fostering women's empowerment. The analysis shows that the program gives women community leaders and women networks members (who predate EXTENSA) access to different types of resources, allowing women to transform their community assets into sources of collective empowerment. This article addresses the issue of the autonomy of patients who are not treated in standard situations of hospitalisation in institutions, but who receive ambulatory care. Academic and institutional discourses both suggest that the development of out-of-hospital care delivery models promotes patient autonomy. Through a qualitative study based on semi-structured interviews of cancer patients treated using ambulatory chemotherapy in a French hospital we show that some of the constraints usually encountered by patients receiving care in institutions are also observed in patients receiving ambulatory care. They experience a form of cognitive isolation regarding access to information about their health, and their social life appears to be institutionalised. [ABSTRACT FROM AUTHOR]

Signifi cant transformations of the health care services sector over the past three decades have seen an increasing reliance on the private provision of health care services mediated through private health insurance. In countries such as Australia and the UK, private health insurance is promoted as providing a greater choice for individuals and easing the burden on the public system. While these claims, the policy contexts and the decision-making processes of individual consumers have attracted some sociological attention, little has been said about the role of private insurers. In this article we present a comparative analysis of the websites of private health insurers in Australia and the UK. Our analysis highlights adoption by private health insurers of neoliberal discourses of choice and individual responsibility, partnership and healthy lifestyles. In these respects, similarities between the discourses over-ride national differences which might otherwise be expected given their contrasting health care traditions and contexts. Sex comparisons reveal men as more likely than women to die earlier and experience debilitating injury. Historically, this trend has been positioned as somewhat inevitable, an outcome of men's 'natural' biologically charged tendencies for risk-taking and reluctance around help-seeking. More recently, gender research has emerged to describe cultural norms about masculinity and explore their relationships to men's health and illness practices. Empirically, masculinities and men's health research has revealed diverse practices that suggest some men's risky health behaviours are amenable to change. This article provides a brief review of how masculinity has been understood in men's health research before making recommendations for where we might next go in theorising social constructions of masculinities. Specifically, a vignette drawn from a study examining young men's responses to the death of a peer is used to illustrate how the communities of practice framework can be applied, and might conceptually advance future masculinities and men's health research.
