Auditory prostheses use implanted electrode arrays that permit stimulation at many sites along the tonotopic axis of auditory neurons. Psychophysical studies demonstrate that measures of implant function, such as detection and discrimination thresholds, vary considerably across these sites, that the across-site patterns of these measures differ across subjects, and that the likely mechanisms underlying this variability differ across measures. Psychophysical and speech recognition studies suggest that not all stimulation sites contribute equally to perception with the prosthesis and that some sites might have negative effects on perception. Studies that reduce the number of active stimulation sites indicate that most cochlear implant users can effectively utilize a maximum of only about seven sites in their processors. These findings support a strategy for improving implant performance by selecting only the best stimulation sites for the processor map. Another approach is to revise stimulation parameters for ineffective sites in an effort to improve acuity at those sites. In this paper, we discuss data supporting these approaches and some potential pitfalls.

Hearing thresholds for white-noise stimuli and temporal gap-detection thresholds in six Mongolian gerbils (Meriones unguiculatus) were determined in a GO/NOGO procedure using the method of constant stimuli. Gerbils were offspring of animals collected in the field and only bred in captivity for four generations or less. Hearing thresholds for 800 ms bursts of white noise ranged between -0.8 and 6.3 dB SPL. The median minimum-detectable gap centered in an 800 ms burst of white noise presented at 50 dB SPL was 2.1 ms. At levels of 40 dB SPL and above, gap-detection thresholds were independent of the sound-pressure level of the stimulus. At 30 dB SPL and below, the minimum-detectable gap increased with decreasing sound-pressure level. Near 5 dB sensation level, gap-detection thresholds ranged from 13.6 to 29.4 ms. The median threshold for the detection of gaps inserted 5 ms after the onset of an 800 ms burst of white noise of 50 dB SPL was 2.3 ms. The individuals' detection thresholds varied between 2.3 and 3.6 ms for stimuli in which the relative sound-pressure levels of the noise before and after the gap differed by up to 20 dB. The data found in the Mongolian gerbil match results from other mammal and bird species.

Temporal gap detection thresholds were obtained from six listeners using an adaptive tracking method and constant spectrum-level noises. In separate blocks of trials, the markers bounding the gap were systematically varied in their spectral overlap or separation (expressed in equivalent rectangular bandwidths, ERBs). In the same listeners, gap thresholds were also obtained for noises of the same bandwidths as those constituting the overlap in the overlap conditions (in the presence of a wideband notched noise masker: 'mask' conditions). For the spectral overlap/separation conditions, gap thresholds were a systematic, linear function of spectral dissimilarity in four of six listeners. In the mask conditions, gap thresholds were inversely related to bandwidth in all listeners. For the three-, four- and five-ERB conditions, gap thresholds in the same listeners for the spectral overlap conditions were higher than those for mask stimuli with the same available within-channel bandwidth and spectrum levels. These data suggest that the spectral dissimilarity between the markers over-rode the availability of within-channel information in the recovery of the temporal gap.

Presenting clicks according to maximum length sequences (MLSs) enables transient evoked otoacoustic emissions (TEOAEs) to be recorded at very high stimulation rates. Despite a decrease in TEOAE amplitude, the very large number of responses obtainable at high rates means that both signal to noise ratio (SNR) and detection sensitivity increase as the click rate increases. This study characterises conventional and MLS TEOAEs near threshold for a group of normally hearing adults. Stimulus presentation rates of 40 clicks/s (conventional) and 5000 clicks/s (MLS) were used. Compared to conventional recordings, the MLS technique enabled smaller responses to be detected, when averaged for the same time and to the same SNR. TEOAE amplitude recorded at detection threshold for MLS responses was 13 dB lower than that recorded conventionally. For each individual, MLS recording also produced clear, repeatable responses at stimulus levels below the detection threshold for conventional TEOAEs. The click level at TEOAE threshold was 12 dB lower for MLS compared to conventional emissions. These results suggest that TEOAE thresholds are not absolute but strongly related to the detection sensitivity of the recording system and physiological noise. The initial growth rates and the shape of input/output functions were found to be similar for the two recording techniques.

Masking patterns for a 2-kHz sinusoidal masker at 45, 65 or 85 dB SPL were measured for three normal-hearing subjects, using a 3-AFC method with feedback (condition 1). The patterns showed distinct irregularities, particularly at the highest masker level. In condition 2, a lowpass noise was added to mask combination tones. The noise increased thresholds mainly for the 85 dB masker, for signal frequencies of 2.3-3.0 kHz. In condition 3, a pair of high-frequency tones ('modulation detection interference (MDI) tones') was used to introduce beats at the same rate as produced by the interaction of the masker and signal. Thresholds were higher than for condition 1, particularly for signal frequencies adjacent to the masker frequency. In condition 4, the lowpass noise was presented simultaneously with the MDI tones. Thresholds were well predicted as a combination of the effects of the lowpass noise and the MDI tones. In condition 5, a pair of low-frequency MDI tones was added to the masker. The thresholds had the same overall pattern as in condition 4. We conclude that the shapes of masking patterns measured using a 2-kHz masker are influenced by the detection of beats for masker-signal frequency separations up to at least 300 Hz and by the detection of combination tones for separations between 300 and 1000 Hz.

The plot of detection thresholds vs. time for a forward-masked Gaussian-shaped 2 kHz pip (S.D.=0.5 ms) shows a noisy ‘fine structure’ that disappears with across-subject averaging. The averaging uncovers a slower threshold periodicity, that can be predicted from the output power of the auditory filter during extended ringing.

Frequency difference limens were determined as a function of reference-stimulus level for pulsatile electrical stimuli in 5 postlingually deaf human subjects with Nucleus-22 cochlear implants and for sinusoidally amplitude-modulated acoustic white noise stimuli in 4 normal-hearing humans. Subjects were tested at levels throughout the dynamic range and extending to the lowest detectable levels. Response stability was measured over the course of 10 sessions. For electrical stimulation in the deaf ears, difference limens decreased as a function of level throughout much or all of the dynamic range of hearing. This result contrasts with the case for nonspectral acoustic stimulation of normal-hearing subjects, where nonspectral frequency difference limens were strongly affected by level only near the detection threshold. These data suggest differences in the acoustic and electrical response spaces that must be considered in the design of auditory prosthesis processors.

The Derived cochlear nerve compound action potential (CAP) and the Enhanced CAP are both measures which demonstrate the ability of a forward masker to increase the amplitude of the CAP produced by a probe stimulus. Enhancement occurs whenever the amplitude of a masked CAP is larger than that of a nonmasked CAP, whereas the derived CAP is produced by the subtraction of the entire masked CAP waveform from that of the nonmasked waveform. Therefore, a derived CAP is created whenever the masker produces a difference of amplitude, latency, and/or waveform shape. The present experiments compare these two measures by observing the effects of 13 kHz maskers varying from levels of −10 to + 70 dB SPL on CAPs produced by 50 or 60 dB SPL, 13 kHz probe stimuli. Enhancement is characterized by a nonmonotonic increase of CAP amplitude (and sometimes a decrease of latency) as a function of increasing levels of the forward masker, whereas this pattern seldom occurs with the derived CAP. Enhancement is typically seen with forward making, but seldom seen with simultaneous masking, whereas the derived CAP is very similar under these two types of masking.

Forward masking can either decrease or increase the response to a subsequent stimulus. In the gerbil, frequency-intensity domains of the maskers that decrease the amplitude of the compound action potential (CAP) can be plotted as the sharply defined CAP tuning curve (TC). Regions were also found over which masking increases (enhances) the amplitude of the CAP. Center-frequency (CF) enhancement domains were found in approximately 2 3 of the animals tested, in response to maskers having frequencies very near that of the probe stimulus, at levels ranging from below the CAP detection threshold to just below the tip threshold of the TC. Approximately 2 3 of the animals showing CF enhancement also displayed low-frequency (LF) enhancement, in response to a domain which borders the low-frequency tail of the TC.

The amplitudes of evoked otoacoustic emissions (EOE) and their detection threshold were measured in 44 normal young adults and 118 patients with two categories of cochlear dysfunction, acoustic trauma and presbycusis. A different method was used for each category: detection of click EOE or of stimulus frequency emissions. A partial correlation and multivariate analysis was performed for both groups of results to investigate the relations between EOE threshold one pure tone audiometric thresholds (250 to 8000 Hz). Only one significant correlation was found, linearly relating EOE threshold and hearing threshold at 2 kHz (P less than 0.001), independently of the origin of cochlear dysfunction. It suggests that EOE threshold is not frequency-specific since the frequency of EOE at threshold was nearly always close to 1 kHz. A simple model is proposed, based on the assumption that EOE amplitudes and threshold are proportional to the total number of residual active sites in the organ of Corti, i.e. to the total length of active basilar membrane. It is shown that this model accounts for the results disclosed by the statistical analysis and fits the experimental data. It can be used for quantitatively predicting the residual cochlear activity of a patient. However, the EOE threshold is only sensitive to already important cochlear alterations and this parameter does not seem to allow a follow-up of early stages of cochlear dysfunction.

The purpose of this paper is to better characterize changes over time that occurred in psychophysical detection thresholds for electrical stimulation of the cochlea. Threshold changes observed in nonhuman primates implanted with cochlear electrode arrays can be divided into at least three types based on the patterns of change over time. Short-term increases and subsequent decreases in threshold were commonly observed during the first months after implantation and were often followed by periods of long-term threshold stability. Long-term slow increases in thresholds and more rapid increases after a period of threshold stability have also been observed. The threshold changes may be divided into at least two classes based on their dependence on the waveforms used for the threshold measurements. Some changes occurred primarily in thresholds for long phase-duration signals while other changes were equal in magnitude (in decibels) for all tested stimuli. This suggests that at least two mechanisms underlay these threshold changes. The observed changes in thresholds have implications for experimental studies of electrical stimulation and for clinical application of auditory prostheses.

We test Lowenstein’s dc bias hypothesis as an alternative mechanism for the phenomenon sometimes called ‘stochastic resonance’. Probe stimuli consisting of paired phase-locked tones at frequencies f1 and f2 (where f23f1 = 800 Hz, f1 s 4.5 kHz) and at equal intensity were used to generate synchronous 800 Hz cochlear nerve activity (envelope responses). When a background tone of the same intensity, with a frequency halfway between f1 and f2, is presented simultaneously with the probe stimulus, the envelope response amplitude typically decreases. Consistent with Lowenstein’s hypothesis, however, when the intensities of the probe and background tone are near the detection threshold of the envelope response (approximately 0^20 dB sound pressure level), the simultaneous presence of the background tone often increases the amplitude of the envelope response. At these same intensity levels, when the background tone precedes the probe stimulus, it decreases the amplitude of the response to the probe stimulus. The effects of simultaneous presentation of the probe and the background tone are frequency-dependent, becoming less pronounced or reversing as the frequency of the background tone departs from those of the probe stimuli. fl 2001 Elsevier Science B.V. Several species have been, and continue to be, used as subjects in studies of electrical stimulation of the cochlea. Few attempts, however, have been made to determine if data obtained from different species are quantitatively or qualitatively similar. The present work compares psychophysical absolute detection threshold vs. frequency functions for sinusoidal stimuli obtained from humans, nonhuman primates, cats, and guinea pigs. Threshold data for monopolar and bipolar electrode configurations from both previously published and unpublished studies are compared. In general, within all four species, significant intersubject variation in detection threshold level was found, but slopes of threshold vs. frequency functions were relatively well conserved within a species, under the conditions studied. With one exception (cat bipolar stimulation), threshold functions reached a minimum at or near 100 Hz across species and electrode configurations. In all cases, thresholds were significantly lower for monopolar, as compared with bipolar, configurations. Statistically, there were no significant differences in absolute threshold level across species. Threshold levels increased with frequency above 100 Hz at a rate of 3.0-7.9 dB/octave, depending on both electrode configuration and species. Slopes were steeper for monopolar than for bipolar configurations. When slopes were averaged between 200 and 2000 Hz, no statistically significant differences in overall slopes were found, nor was there a significant interaction between electrode configuration and species. There were, however, consistent species differences within more restricted regions of the function. Human functions for both monopolar and bipolar stimulation were steeper than all animal functions in the range of 100-300 Hz. Within this range, the differences between slopes for human and nonhuman subjects were statistically significant. In addition, differences were noted in the frequency at which slope decreased, with slopes for nonhuman subjects showing the decrease at higher frequencies than did those for human subjects. These differences may be true species differences, or may reflect the influence of confounding variables associated with each experimental-subject model.

A behavioral method for assessing auditory threshold in cats using food reward conditioning is described. The method requires that the cat press its nose against a touch panel in order to receive a stimulus and that it remove its nose when the stimulus is detected. Behavioral thresholds for pure tone stimuli of various durations are described for three laboratory-raised cats. For all frequencies tested, detection threshold varies as an exponential function of stimulus duration. For frequencies from 125 Hz to 8 kHz, integration time constant and stimulus frequency are inversely related. Response latencies are also affected by the duration of the stimulus. Results are considered in the light of similar measures obtained from human subjects.
