Background/aims: H. pylori and NSAIDs are the two most important pathogenic factors in gastric ulcer. NSAIDs and H. pylori share common pathogenic characteristics, but have different effects in modulating COX-2 and local cytokines in gastric ulcer mucosa. This study was designed to explore the influence of COX-2 and local cytokine expressions in gastric ulcer mucosa induced by H. pylori infection and NSAID use., Methodology: Twenty-three patients were recruited. Sixteen subjects were infected with H. pylori. Fifteen patients used NSAIDs. Gastric biopsy specimens were obtained by endoscopy. COX-2 and local cytokine mRNA expressions were assessed by real-time RT-PCR., Results: COX-2 and local cytokines were over-expressed in gastric ulcer and were positively intercorrelated. H. pylori did not alter COX-2 and local cytokine expressions in gastric ulcer, but induced IL-8 and COX-2 in antral mucosa. NSAIDs inhibited COX-2 expression in gastric ulcer, but had no effect on other local cytokines. COX-2 inhibition by NSAIDs in gastric ulcer was compatible with the findings that NSAIDs delayed gastric ulcer healing., Conclusions: H. pylori increased IL-8 and COX-2 in the antral mucosa, but did not influence COX-2 and local cytokines in gastric ulcer. NSAIDs inhibited COX-2 in gastric ulcer and delayed gastric ulcer healing.

Background/aims: Technetium-99m methoxyisobutyl isonitrile (MIBI) has been shown to be useful in identifying several types of tumors, such as breast, lung and thyroid cancers. There are only a few reports in the literature regarding Tc-99m MIBI uptake in hepatocellular carcinoma, and the results are conflicting. The aim of this study was to investigate the relationship between Tc-99m MIBI accumulation in patients with hepatocellular carcinoma and the gene expression of P-glycoprotein-multidrug-resistance., Methodology: Twenty-two patients with hepatocellular carcinoma were enrolled in this study. Tc-99m MIBI imaging was performed 10 minutes after intravenous injection of 20 mCi Tc-99m MIBI. All patients had liver biopsy or surgery within 1 week of MIBI imaging. Immunohistochemical study of the biopsy or resected hepatocellular carcinoma specimens was performed using the anti-human P-glycoprotein antibody., Results: On Tc-99m MIBI imaging, 20 of 22 (90.9%) patients with hepatocellular carcinoma showed negative Tc-99m MIBI uptake in tumor lesions, whereas only 2 patients showed positive Tc-99m MIBI uptake in tumor lesions. P-glycoprotein expression was observed in 13 of 20 (65%) patients with negative Tc-99m MIBI uptake, but in the 2 patients who showed positive Tc-99m MIBI uptake in tumor lesions, P-glycoprotein expression was negative., Conclusions: Tc-99m MIBI SPECT is a useful noninvasive method for predicting the expression of P-glycoprotein-multidrug-resistance gene in hepatocellular carcinoma patients.

Background/aims: Hepatocellular carcinoma is one of the most common malignancies in Taiwan, a hepatitis B endemic area. In this study, we aimed to evaluate the characteristics of hepatocellular carcinoma in patients receiving hemodialysis, as this patient group is at high risk for malignancy., Methodology: From October 1991 to September 1997, thirteen patients receiving hemodialysis and diagnosed with hepatocellular carcinoma were enrolled in this retrospective study. Patients' clinical course, laboratory data, image study and treatment were evaluated., Results: Among these 13 patients, hepatitis B virus related in 6 and hepatitis C virus related in 7. There was no statistical significance in serum levels of asparate aminotransferase, alanine aminotransferase, bilirubin and alpha-fetoprotein between hepatitis B virus and hepatitis C virus related hepatocellular carcinoma. Hepatitis B virus related hepatocellular carcinoma had a shorter mean dialysis period (29.67 +/- 22.18 vs. 87.86 +/- 79.90 months, P = 0.25) and mean duration from beginning hemodialysis to diagnosis of hepatocellular carcinoma (17.16 +/- 26.94 vs. 76.08 +/- 65.69 months, P = 0.05), but there was no statistical significance. In the area of treatment, the survival curves of the treatment (hepatic resection and/or transcatheter arterial chemoembolization) group and supportive group were compared by log-rank test and there was no statistical significance for these two groups (P = 0.69)., Conclusions: Both hepatitis B virus and hepatitis C virus are equally important for hepatocellular carcinoma in hemodialysis patients in hepatitis B endemic Taiwan. The acquisition of hepatitis B virus or hepatitis C virus might be not related to hemodialysis. Periodic screening with ultrasonography and serum alpha-fetoprotein is necessary among hemodialysis patients with evidence of hepatitis B virus or hepatitis C virus infection.

Background/aims: Hepatocellular carcinoma is part of the natural history of liver cirrhosis. Gastrointestinal bleeding and hepatic failure are the leading causes of death in hepatocellular carcinoma patients. With gastrointestinal bleeding, variceal bleeding is the most prominent, and most variceal bleeding is of esophageal origin. Gastric varices bleeding is often a massive and severe bleeding episode. The role of gastric varices among patients with hepatocellular carcinoma remains to be clarified. In this study, we aimed to evaluate the prevalence, clinical significance and prediction of gastric varices in patients with hepatocellular carcinoma., Methodology: From 1998 to 2000, we reviewed 304 patients with hepatocellular carcinoma receiving upper gastrointestinal endoscopic examinations. Patients' clinical characteristics, physical findings, laboratory data, image studies, endoscopic examinations and treatment were reviewed., Results: Among 304 patients with HCC, twenty-one (6.9%) had gastric varices among 304 patients with hepatocellular carcinoma. The location of gastric varices were the posterior wall in 12 (57%), the lesser curvature in 1 (5%), the greater curvature in 4 (19%) and the fundus in 4 (19%). Three (14%) of these 21 patients with hepatocellular carcinoma and gastric varices had clinical evidence of bleeding. One of them died due to uncontrollable bleeding. Child-Pugh classification, hepatic encephalopathy, portal vein or splenic vein dilatation, ascites, splenomegaly, albumin level, prothrombin time and platelet count were significantly different between hepatocellular carcinoma patients with gastric varices and without gastric varices under the univariate analysis. Ascites (Odds ratio: 5.45; 95% confidence interval: 2.12-14.01) and portal vein or splenic vein dilatation (Odds ratio: 4.38; 95% confidence interval: 1.77-10.86) were the two most important predictors under the stepwise logistic regression analysis., Conclusions: The prevalence of gastric varices in patients with hepatocellular carcinoma is 6.9% and the risk of bleeding is low in this study. The Predictors of gastric varices among hepatocellular carcinoma are related to liver cirrhosis, Child-Pugh classification, hepatic encephalopathy, portal vein or splenic vein dilatation, ascites, splenomegaly, albumin level, prothrombin time and platelet count.

Background/aims: To study and review the clinical manifestations, microbiology, comorbidity, and diagnosis of liver abscess after transarterial chemoembolization for malignant hepatic tumor., Methodology: We retrospectively reviewed 1374 patients who underwent 2581 transarterial chemoembolization procedures due to malignant hepatic tumors over an 8-year period., Results: 7 patients had liver abscess after transarterial chemoembolization. The incidence was 0.27% (7/2581). Hepatocellular carcinoma was diagnosed in all 7 patients, whose liver function was classified as stage A by the Child-Pugh criteria. The clinical manifestations were intermittent fever, abdominal pain, and leukocystosis. All the patients had hyperechoic spots with reverberative shadows on sonograms or low attenuation areas with different Hounsfield units on computed tomography scan, which expressed the 100% incidence (7 of 7) of gas-forming abscesses. Percutaneous drainage or aspiration was done in 6 patients. One received laparotomy with local debridement due to suspicious organ rupture. The pus culture showed Gram-negative bacteria in all patients. Blood cultures were positive in only 3 of 7 patients (43%). No patients died of liver abscess after aspiration, drainage, or debridement of abscess combined with parenteral antibiotic treatment. Biliary tract diseases, found in 4 patients, were the most common comorbidity., Conclusions: Liver abscess after transarterial chemoembolization is a very rare complication, which usually develops in patients with biliary tract disease. Gram-negative bacteria are the main pathogens. The incidence of gas formation is higher after transarterial chemoembolization than in the general population. However, the prognosis is good after adequate clearance of pus and antibiotic treatment.

Background/aims: Acupuncture has been used to treat gastrointestinal symptoms in oriental countries for many years. The underlying mechanism is still not fully understood., Methodology: Fifteen healthy male volunteers were enrolled into this study. A cutaneous electrogastrography recording was obtained. Two frequencies (2 Hz and 100 Hz) of electrical stimulation were applied to Zusnali or Shousanli point in different sessions. Another non-acu-point (2 cm lateral to Zusanli) without electrical stimulation was used for a control study., Results: There was a significant increase in the percentage of normal frequency during 2 Hz of electrical stimulation on Zusnali (baseline vs. acupuncture, 82.49 +/- 12.87% vs. 93.18 +/- 8.40%, p < 0.01). The percentage of normal frequency did not change significantly, during or after acupuncture, with 100 Hz of electrical stimulation on Zusnali, or 2 Hz and 100 Hz of electrical stimulation on Shousanli. In addition, the percentage of tachygastric frequency and power ratio also changed significantly during 2 Hz of electrical stimulation on Zusnali. However, the change in the percentage of bradygastria and dominant frequency was not statistically significant among all five study sessions during each stage., Conclusions: The results of this study demonstrated that electrical stimulation, with a frequency of 2 Hz, on Zusanli might enhance the regularity of gastric myoelectrical activity. The effect of acupuncture on gastric myoelectrical activity was acu-point-specific, and that effect was observed with 2 Hz but not 100 Hz electrical stimulation.

Background/aims: Although antidepressants have been used for decades in treating patients with functional abdominal syndromes, how they influence gastrointestinal motility remains unclear. We aimed to assess the role of depression in functional dyspepsia, and the effect of antidepressants on the functional dyspepsia patients' symptoms and gastric myoelectrical activity., Methodology: We conducted an open clinical trial with 40 functional dyspepsia patients. Zung self-rating depression scale was used in evaluating the patients' depression. Cutaneous electrogastrography and evaluation of upper gastrointestinal symptoms were performed before and after administration of a one-month course of fluoxetine., Results: In the baseline study, the depressed functional dyspepsia patients had higher symptom scores than non-depressed patients (P < 0.05). The depressed functional dyspepsia patients had higher percentages of tachygastria than healthy controls (P < 0.05), but the electrogastrography parameters of depressed and non-depressed functional dyspepsia patients were not different. After one-month fluoxetine treatment, the symptom scores improved significantly in the depressed functional dyspepsia patients (P < 0.05), but not in the non-depressed patients. Electrogastrography did not improve in either group., Conclusions: Depressive functional dyspepsia patients had higher symptom scores and responded well to fluoxetine treatment. However, electrogastrography did not improve after the treatment. These findings suggest that depression is significant in the presentation of functional dyspepsia symptoms, but not correlated with gastric myoelectrical activity.

Background/aims: Combined hepatocellular-cholangiocarcinoma is a rare subtype of primary liver cancer. To clarify the significance of this tumor, we conducted the present study to investigate the clinicopathological characteristics. As well, a comparison between this tumor and cholangiocarcinoma was made to determine whether these two types of liver cancers are closely related., Methodology: Seventeen cases of combined hepatocellular-cholangiocarcinoma and 104 cases of cholangiocarcinoma were included. Patients were evaluated on the basis of age, gender, viral hepatitis markers, tumor markers, laboratory data, indocyanine green test, tumor location, histological differentiation, lymph node involvement, and organ metastasis. In addition, the presenting symptoms at admission were also recorded. Data was analyzed using the chi 2 test and Student's t test and differences between the curves were tested using the two-tailed log-rank test., Results: Combined hepatocellular-cholangiocarcinoma was found to be more prevalent in males. In addition, the combined hepatocellular-cholangiocarcinoma patients had lower serum bilirubin, ALP levels (p < 0.05), and more poor differentiation (p < 0.05). Also, the presenting symptoms of combined hepatocellular-cholangiocarcinoma patients were different from those of cholangiocarcinoma patients with the former a lower incidence of fever, chills, jaundice and a higher incidence of fatigue and weakness., Conclusions: The clinicopathological features of combined hepatocellular-cholangiocarcinoma were different from those of cholangiocarcinoma with more advanced histological differentiation, more prevalent in males, and lower levels of serum bilirubin and ALP. These characteristic features may help us in diagnosing combined hepatocellular-cholangiocarcinoma in patients with suspected cholangiocarcinoma.

Background/aims: Constipation is the most common digestive complaint. Radionuclide colon transit study is a useful tool for assessing the motility of the colon. In this study, we evaluate patients with idiopathic constipation using the radionuclide colon transit study., Methodology: We studied radionuclide colon transit in 23 patients with idiopathic constipation. Thirty healthy volunteers were included as a control., Results: Overall, patients with idiopathic constipation usually had slow or normal colon transit., Conclusions: Our study suggested that the radionuclide colon transit study may be clinically useful in evaluating and assessing the efficacy of therapy in patients with idiopathic constipation.

Background/aims: Endoscopic mucosal resection and laparoscopic wedge resection have become more common in the treatment of early gastric cancer. However, lymph node metastasis is a major poor prognostic factor influencing tumor recurrence and survival. To predict the risk of lymph node metastasis in early gastric cancer, the authors conducted a study to investigate the clinicopathologic characteristics of early gastric cancer with lymph node metastasis., Methodology: From 1982 to 1998, 181 patients of early gastric cancer underwent primary surgery and were included in the study. Patient data was postoperatively reviewed regarding age, gender, tumor size, depth of invasion, histologic differentiation, macroscopic classification and anatomic level of lymph node metastasis. The chi 2 test or Student's t test was used for statistical analysis. Logistic regression analysis was used to evaluate the independent risk factors for lymph node metastasis., Results: Lymph node metastasis was observed in 19 cases (11%). Early gastric cancer with size larger than 4 cm (P < 0.05), with submucosal invasion (P < 0.01), and with poor differentiation (P < 0.05) was associated with higher risk of lymph node metastasis. The macroscopic classification had no predictive value. Multivariate analysis showed that submucosal invasion correlated best with lymph node spread (OR 10.25, 95% CI: 2.10-49.96), followed by tumor size larger than 4 cm (OR 4.99, 95% CI: 1.46-17.05), and poorly differentiated histological subtype (OR 3.31, 95% CI: 1.16-9.45)., Conclusions: Poor differentiation, submucosal invasion and large tumor size were independent risk factors for lymph node metastasis in early gastric cancer. Macroscopic classification was not correlated with lymph node metastasis.

Background/aims: We compared ultrasound and Xenon-133 hepatic retention ratio to evaluate the accuracy of ultrasound in diagnosing fatty liver., Methodology: One hundred and forty-six patients with suspected fatty liver were included. All patients underwent an ultrasound study followed by a determination of the Xenon-133 hepatic retention., Results: Using the Xenon-133 hepatic retention ratio as the diagnostic standard, the overall accuracy of ultrasound in detecting fatty liver was 57.5%, with 100% sensitivity and 6.0% specificity., Conclusions: Ultrasound can be used as a first-line screening imaging modality for fatty liver. If the result of ultrasound is equivocal, then Xenon-133 hepatic retention determination should be performed for the diagnosis and management of fatty liver.

Background/aims: Chronic hepatitis C has been recognized to be associated with thrombocytopenia. The use of indium (In-111) oxine-labeled platelets has provided an important tool for classifying the mechanism of thrombocytopenia. In this study, we utilized In-111 oxine-labeled platelet scintigraphies to evaluate patients with chronic hepatitis C (with and without thrombocytopenia)., Methodology: Eleven patients with chronic hepatitis C were included in this study. Six patients (3 male, 3 female) were diagnosed with thrombocytopenia (< 140 x 10(9)/L) (group A) and five patients (3 male, 2 female) had normal platelet counts (> 140 x 10(9)/L) (group B). Spleen to heart and spleen to liver ratios were calculated from 24-hour platelet scintigraphic images., Results: The spleen to heart ratios in group A and group B were 5.18 +/- 2.09 and 5.17 +/- 2.17, respectively (P = 0.465). The spleen to liver ratio of group A and B were 3.08 +/- 1.60 and 2.38 +/- 0.87, respectively (P = 0.870)., Conclusions: Our study revealed that patients with hepatitis C infection may have thrombocytopenia independent of hypersplenism.

Background/aims: Calcium channel blocking agents have been reported to increase the risk of gastroesophageal reflux. However, whether felodipine, a newer calcium channel blocker, increases reflux episodes and decreases esophageal acid clearance in patients with gastroesophageal reflux disease has never been studied. Therefore, the aim of this study was to evaluate whether felodipine increases the incidence of gastroesophageal reflux in patients with gastroesophageal reflux disorder., Methodology: Nine patients with gastroesophageal reflux disease, 6 men and 3 women, with a mean age of 62.6 +/- 14.4 years (range: 37-80 years) were studied. They received ambulatory esophageal pH monitoring for 45.7-48 hours (mean: 47.1 +/- 0.8 hours). Various pH parameters were evaluated during a similar interval of monitoring time before and after receiving 5 mg of felodipine., Results: No significant difference was noted in any pH parameter by the Wilcoxon signed Ranks test, including reflux episodes (P = 0.552), reflux episodes longer than 5 min (P = 0.683), esophageal acid clearance (P = 0.663) and fraction time of pH < 4 (P = 0.752) before and after the use of felodipine., Conclusions: Felodipine does not increase reflux episodes or impair esophageal acid clearance in patients with gastroesophageal reflux disease.

Background/aims: Chronic gastric Helicobacter pylori infection is common in patients with dyspeptic symptoms. The effect of H. pylori infection on gastric emptying, in cirrhotic patients with dyspeptic symptoms, has never been studied. Therefore, we investigated the incidence of H. pylori infection and its relationship with gastric emptying in cirrhotic patients with dyspepsia., Methodology: A solid-phase gastric emptying study and 14C urea breath test were performed in 80 cirrhotic patients with dyspepsia. The severity of cirrhosis was assessed according to Child-Pugh's classification., Results: The overall incidence of delayed gastric emptying was 75%. Delayed gastric emptying incidences according to severity of cirrhosis were 71.4% for Child-A, 73.1% for Child-B, and 80.8% for Child-C. The differences were not significant. The incidence of H. pylori infection was 52.5% overall. H. pylori infection rates were 46.4% for Child-A, 42.3% for Child-B, and 69.2% for Child-C. Although there was a tendency for the infection rate to increase with the severity of liver cirrhosis, the difference was not significant. In addition, there were no significant differences in the incidences of H. pylori infection among patients with normal and delayed gastric emptying., Conclusions: Delayed gastric emptying is common in cirrhotic patients with dyspepsia. However, the status of H. pylori infection does not seem to play a role in delayed gastric emptying in these patients.

Background/aims: In Taiwan, most cases of hepatocellular carcinoma (HCC) are hepatitis B virus (HBV) or hepatitis C virus (HCV) related. The serum alpha-fetoprotein (AFP) level is an important factor in the diagnosis of HCC. There have been many studies discussing the role of AFP in diagnosing HBV-related HCC, but only few concerning HCV-related HCC. In this study, we aimed at analyzing the distribution of AFP levels in anti-HCV positive patients with and without HCC and evaluating the effectiveness of serum AFP levels in screening HCV-related HCC., Methodology: From 1993-1996, we collected the AFP data of 205 HCC patients retrospectively, who were anti-HCV positive. For comparison, 131 randomized anti-HCV positive patients without evidence of HCC served as the control group. We analyzed the AFP distribution in both groups over the following ranges: < or = 5 ng/ml, > 5-20 ng/ml, > 20-50 ng/ml, > 50-100 ng/ml, > 100-200 ng/ml and > 200-400 ng/ml, and > 400 ng/ml., Results: The distributions of AFP levels in anti-HCV positive patients with HCC were 13.2%, 21.5%, 11.2%, 4.9%, 4.4%, 7.3%, and 37.6%. The distributions in anti-HCV positive patients without evidence of HCC were 34.3%, 55.0%, 8.4%, 1.5%, 0.8%, 0%, 0%., Conclusions: We found the differences in AFP to be statistically significant between anti-HCV positive patients with and without HCC. A serum AFP level of more than 200 ng/ml highly indicates HCC. However, there is a large overlap between these 2 groups. Thus, in anti-HCV positive patients, AFP level is not a good single reference for diagnosis of HCC. Anti-HCV positive patients should be routinely screened for HCC by image studies along with serum AFP level.

Background/aims: Adding an acid secretion inhibitor to anti-H. pylori regimens may be potentially valuable for enhancing the effectiveness of antimicrobials that exhibit markedly reduced activity at low pH. This study was conducted to evaluate intragastric acidity as a factor in H. pylori eradication with bismuth-based triple therapy., Methodology: Forty patients with duodenal ulcer and H. pylori infection were included. The patients were divided into 2 groups--normacid (n = 20) and hyperacid (n = 20)--based on the amount of time that 24-hour intragastric pH took to reach the level pH > or = 3. All patients received bismuth subsalicylate (600 mg 3 times daily), metronidazole (500 mg 3 times daily) and amoxicillin (500 mg 3 times daily) for 2 weeks. Then, all patients continued treatment with ranitidine (150 mg twice daily) for 8 weeks prior to the follow-up examination. Blood samples were collected before treatment for measurement of fasting gastrin and pepsinogen-I., Results: Nine patients (45%) in the normacid group and 8 patients (40%) in the hyperacid group reported side effects. However, there were only 2 patients (10%) in each group who withdrew from the study due to intolerance of side-effects. There was no difference in the H. pylori eradication rate between the normacid and hyperacid groups (16/18, 88.9% vs. 15/18, 83.3%)., Conclusions: Without co-administration of anti-secretary agents, intragastric acid is not a significant factor in the effectiveness of H. pylori eradication with bismuth-based triple therapy.
