This paper summarizes 41 reports on the height and mass of Australian children aged between 5.00 and 16.99 years between 1899 and 1999. In all, data on 644,613 children were collated, including individual data on 68,196 children. After primary data treatment to correct for methodological and statistical artifacts, regressions were calculated to quantify the rate of change of height and mass over time. Distributional analysis was used to probe for changes in skewness of mass values, indicative of differentially greater increases at higher percentiles. In addition, studies reporting skin fold measurements were analyzed to assess changes in subcutaneous adiposity since 1976. The results show that height has been increasing at a rate of about 1.02 cm.decade(-1), and mass at a rate of about 0.99 kg.decade(-1). The height and mass of children continue to increase, after a slowing down in the rate of increase between 1950 and 1980. Increases in mass at the higher percentiles have been much greater than at lower percentiles, particularly since the mid-1980s, suggesting that the incidence of obesity is increasing in Australian children. Furthermore, a steady linear increase in subcutaneous skin fold thicknesses since 1976 suggests that the overall level of fatness is increasing in Australian children. These findings indicate that Australian children are following trends becoming evident elsewhere in the developed world, and that we may see an increasingly large subset of increasingly obese children in the early years of the 21st century.

Obesity has become a public-health and policy problem in many parts of the world. Epidemiological and population studies in this field are usually based on different anthropometric measures; however, common genetic and environmental factors between these phenotypes have been scarcely studied. The objective of this article is to assess the strength of these factors on the covariation among a large set of obesity-related traits. The subject group consisted of 533 nuclear families living in the Greater Bilbao (Spain), and included 1,702 individuals aged 2-61 years. Detailed anthropometric measurements (stature, breadths, circumferences and skinfolds) were carried out in each subject. Bivariate quantitative genetic analyses were performed using a variance-components procedure implemented in the software SOLAR. The results revealed that the majority of these traits is affected by common genetic and environmental factors. All correlations were significantly different from 1 and varied from non-significant to very high (0.90, P0.0001), with clearly lower pleiotropic effects among pairs including fat-distribution traits. Despite the strong common genetic effects detected among phenotypes determining the amount of body fat and mass, there is a residual genetic influence on the local fatness measures that cannot be explained exclusively by the genetic influence on overall fatness. Moreover, the observed relationships confirm a partially different genetic control of truncal and peripheral fat. In conclusion, our findings highlight the relevance of considering different types of traits in the prevention and treatment of obesity, as well as in the search for genes involved in its development.

Thyroxine is an endocrine hormone that regulates cellular and organismic metabolism. Current research on thyroxine has primarily examined its adaptive potential and genetic inheritance patterns. To date, no studies have attempted to investigate the interaction between the genetic and environmental components of thyroxine variation. This approach is useful because hormones are on feedback regulation; thus interaction occurs between the environment and gene expression. The purposes of this research are to characterize the genetic and environmental components of thyroxine variation using univariate statistics and to estimate the genetic and cultural heritabilities through path analysis. For univariate analyses, analyses of variance are used to determine whether or not age, sex, or community affiliation are covariates of thyroxine level. Significant differences existed in thyroxine level based on sex and community affiliation (p0.05). The genetic and environmental components of thyroxine variation were partitioned through path analysis. Heritability was estimated at 0.317 +/- 0.109 for the genetic component and at 0.060 +/- 0.029 for the environmental component. The environmental variables that contributed to the variation in thyroxine level were caffeine consumption, blood calcium level, and biceps skinfold thickness.

Genetic and environmental correlations between measures of obesity [body mass index (BMI)] and body fat distribution [waist/hip ratio (WHR) and subscapular/triceps ratio (SSTR)] were examined in 133 monozygotic and 129 dizygotic pairs of elderly white male twins, age 59 to 70 years, participating in the third cardiovascular examination of the National Heart, Lung, and Blood Institute Twin Study. The BMI, WHR, and SSTR fat measures were significantly correlated in these twins, with BMI more closely related to WHR (r = 0.52) than to SSTR (r = 0.18), and the WHR-SSTR association intermediate (r = 0.27). Multivariate genetic analyses of the three indexes using the LISREL modeling approach indicated a significant heritable component for each fatness variable, h2 = 0.66, 0.46, and 0.25 for BMI, WHR, and SSTR, respectively, and a significant correlation between genetic influences on BMI and WHR (genetic r = 0.51). The common genetic component accounted for 54% of the observed BMI-WHR correlation, suggesting that overall obesity and abdominal adiposity distribution are mediated to some extent by similar genetic influences. The genetic correlations between SSTR and BMI and between SSTR and WHR were not significantly different from zero, suggesting that genetic influences on skinfold distribution are independent of those on abdominal body fat and overall obesity. The genetic findings support the hypothesis that the WHR and SSTR indexes do not assess the same dimensions of fat patterning.

Sibling resemblance in fatness and subcutaneous fat distribution is addressed in a sample of healthy adolescents who belong to 122 nuclear families. Relative fatness and fat distribution were estimated using anthropometric measurements, bioelectric impedance, and ultrasound images. The patterns of fat distribution were defined by indexes constructed using combinations of subcutaneous fat measurements. Adjusted Z scores were used to analyze age- and sex-related changes on fatness and fat distribution. An analysis of variance was used to calculate the resemblance within and between families. The results show that there is significant family resemblance for the level of fatness. There is also significant resemblance between sibs of like sex for the fat distribution patterns. The results also show that subcutaneous fat thickness has a strong genetic component (60-65% of the variance) and that the patterns of fat distribution are apparent from early adolescence.

A validation study of convenient indicators of obesity was undertaken in 540 male and female subjects, aged 7-14 yr. Four adiposity measures that have commonly accepted obesity classification points [relative weight, relative body mass index (BMI), sum of five skinfolds, and triceps skinfold] were derived from measures of height, weight, and five skinfold thickness measurements. Body density measures were converted to percentage of body fat using Lohman's (1986) age- and gender-specific regression equations. Using greater than or equal to 20% body fat for males and greater than or equal to 25% for females as the standard for obesity, the diagnostic utilities (sensitivity, specificity, overall accuracy, and positive and negative predictive values) of the four obesity indicators at their commonly used obesity cutoff points were determined. Preliminary analyses demonstrate that use of these indicators should not be considered independent of the gender of the subject or without reference to the purpose for classifying subjects as obese. Secondary analyses, in which the obesity cutoff point in each indicator was altered to obtain a minimum specificity level of 95%, demonstrated that a sum of skinfolds was better at identifying true obesity than all other indicators in both males and females. There is potential for inappropriate labeling with all convenient indicators of obesity, and thus they should be used with caution.

One-hundred twenty-one male and 93 female subjects, aged 18-23 years, were selected for an investigation of the proportion of subcutaneous to total fat in the whole body. Body fat mass was calculated from body density using the Siri equation. Subcutaneous fat mass was calculated by measuring skinfold thickness at 15 sites and using a modification of the equation derived by Skerjl, Brozek, and Hunt. The main modification to this equation was the introduction of a midlayer area of subcutaneous tissue that is multiplied by fat thickness to give fat volume. The outermost body surface area, which has been utilized in previous research, results in an overestimation of the true subcutaneous fat mass. The average percentages of fat situated subcutaneously (PFSSs) were calculated as 53.7% for males and 62.6% for females. This sex difference is also seen in correlation-regression analysis of PFSS and percentage of fat. In females PFSS decreases with increasing total percentage of fat, whereas in males there is no significant relationship between PFSS and total percentage of fat. This suggests that the proportion of subcutaneous to total fat distribution is negatively related to fatness in females.

Thirty-eight skinfold calipers were compared in terms of dial accuracy, downscale force, and pressure at jaw openings of 10, 20, 30, 40, and 50 mm. Dynamic measures were made on four thicknesses of foam rubber. Foam rubber was used to simulate the compressibility of skinfolds and to eliminate subject and tester variability. Five caliper types were tested: 10 Harpenden (HRP), 9 Lange (LNG), 12 Slim Guide (85SG), 4 Skyndex (SKN), and 3 Lafayette (LF). Within-type variation was small and consistent across openings for dial accuracy, force, pressure, and foam rubber compressed thickness. Between-type pressures differed (p less than 0.01), with the HRP and LNG calipers having the highest but similar pressures and the SKN, LF, and 85SG calipers having the lowest but similar pressures. LF and LNG calipers (which differed from each other) had higher compressed foam rubber means than HRP, SKN, and 85SG calipers (p less than 0.01), which were similar to each other at three of four thicknesses. When average pressure was used as the covariate, differences between types were still large, with 97.5-99.3% of the variation explained by type. Compressibility of foam rubber was shown to be caliper specific. It is concluded that there are static and dynamic differences between the five major types of skinfold calipers. When measurer error is eliminated, the HRP, 85SG, and SKN calipers are similar to each other, but they give lower values than the LNG or LF calipers by approximately 50-100%, depending on compressed thickness. These differences should be noted when comparing skinfolds measured with dissimilar calipers.

Overweight and obese children demonstrate inferior motor performance for strength- and power-related activities requiring support or lifting of body weight. Our purpose here was to determine whether the inferior performance could be attributed to a lower strength to muscle area ratio in the obese. Eleven nonobese (16.6% fat) and 13 obese (35.5% fat) boys (9-13 years old) volunteered for the study. Peak torque was measured during voluntary isometric and isokinetic elbow flexion and knee extension at four joint angles and four velocities, respectively. The contractile properties, twitch torque, time to peak torque, and half-relaxation time were evoked for the elbow flexors by percutaneous stimulation. Elbow flexor and knee extensor cross-sectional areas (CSA) were determined by computed axial tomography taken at the mid-upper arm and mid-thigh, respectively. Isometric and isokinetic elbow flexion and knee extension strength normalized for body weight were significantly (p less than 0.05) higher in the nonobese compared to the obese boys. There were no significant (p greater than 0.05) differences, however, between groups for elbow flexor and knee extensor CSA or for absolute and relative (normalized for muscle CSA or the product of muscle CSA and height, the latter accounting for differences in moment arm length) isometric, isokinetic, or evoked twitch torque for elbow flexion or knee extension. Likewise, there were no differences between groups for the time-related contractile properties, time to peak torque, or half-relaxation time. These findings suggest that there is no difference in the intrinsic strength or contractile properties of the elbow flexor and knee extensor muscles between obese and nonobese pre-adolescent boys and that other factors, such as the handicapping effect of excess fat mass, probably account for the reduced motor performance of the obese child.

The heritability of centrally and peripherally deposited subcutaneous body fat, as measured by thickness of subscapular and triceps skinfolds respectively, was examined in 173 monozygotic and 178 dizygotic pairs of white male twins, ages 54 to 65 years, who participated in the second examination of the National Heart, Lung, and Blood Institute's Twin Study. The heritability of two indices of body fat distribution (subscapular/triceps ratio and subscapular-triceps difference) and two indices of overall obesity (body mass index and sum of skinfolds) were also assessed. Evidence for a genetic influence on central deposition of body fat was suggested in that the classical estimate of heritability for subscapular skinfold thickness was 0.77 (p less than 0.0001). After adjusting subscapular skinfold for the overall level of obesity, heritability was reduced but remained highly significant (0.40, p = 0.003). Heritability estimates for triceps skinfold thickness and for the two fat distribution indices were substantially lower and were not statistically significant after adjustment for overall obesity. High classical estimates of heritability were also observed for both measures of overall obesity: 0.70 for BMI and 0.73 for sum of skinfolds. However, these two estimates were biased upward because of lower total variances among monozygotic compared to dizygotic twins in this sample. The more conservative and unbiased among-component estimates also suggested substantial heritability for each measure (0.35, p = 0.08 and 0.53, p = 0.01, respectively). The heritability of overall obesity emphasizes the importance of adjusting measures of fat distribution for overall obesity before assessing its heritability.
