Long wait times for access to Nuclear Magnetic resonance imaging (MRI) examinations are a concern and for this reason the project "Appropriateness of referrals for MRI examinations" has been launched in Latium (Italy). The aim of this preliminary study was to describe the main characteristics of MRI referrals in the region. Findings highlight a large variation in referral rates across the region, with 80% of MRI referrals being ordered by general practitioners and family pediatricians. The latter points to the possibility of inappropriate referrals for MRI imaging in Latium.

Atrial fibrillation (AF) is the most common chronic cardiac arrhythmia and is an important risk factor for mortality and morbidity related mainly to an increased risk of cerebrovascular events and heart failure.An observational cross-sectional study was performed to evaluate the use of healthcare resources (including hospital and outpatient care) by patients with AF in the Lazio region (central Italy), from 1 January 2006 to 31 December 2008.Atrial fibrillation is an important source of healthcare resource utilization because of repeated emergency room visits, hospital admissions, outpatient consultations and procedures and extensive use of laboratory tests and pharmacological treatments.Results show that 55% of costs are attributable to hospital admissions and Emergency Room visits, 37% to pharmacological treatment and the remaining 8% to outpatient care. These results are consistent with the international literature.The impact of AF in terms of cost is not negligible and it is therefore desirable to implement an organizational scheme that safeguards the appropriateness of care, taking charge of the patient as early as possible. The aims of early diagnosis of AF are to improve the appropriateness of care and optimize the use of specialized tests, thereby reducing hospital admissions for complications or recurrences of AF.

A set of indicators for assessing healthcare needs and evaluating healthcare services are needed. This paper presents a set of indicators for evaluating healthcare districts, defined by a local health authority in the city of Rome. The set of indicators was developed by selecting 11 areas of interest and assigning to each area indicators that would allow an evaluation of its main functional aspects. Overall, 99 indicators were chosen and listed under one of two sections: basic and specific indicators. Annual measurements of the indicators is performed and a study will be conducted during 2010 to evaluate the system of indicators.

In this study, a set of 15 indicators that are used to provide information on waiting times for medical services and procedures were evaluated. Each waiting time indicator was evaluated with respect to 14 criteria and given a score. The indicator "waiting time estimated through past performance" reached the highest score with respect to all others. This indicator is calculated by dividing the total number of individuals waiting for a medical procedure by the number of procedures performed in the facility in the previous month. The result is multiplied by thirty and expressed in days. This indicator can be used even in non-computerized facilities. It allows easy data accounting (at all levels, i.e service, facility, healthcare unit, area, regional and national level) and provides useful information for citizens/users.

Previous studies have shown that the quality of medical records is not uniform across Italian hospitals. Between March 2002 and January 2003, we assessed the quality of 6,215 medical charts regarding admissions to 11 different Rome hospitals in the year 2001. The hospitals were located within the territory of the ROMA "A" and ROMA "C" Local Health Units. The following were evaluated: 1) organization, format and lay-out of medical records; 2) medical chart and discharge abstract compilation; 3) ICD-9-CM coding accuracy. The quality of medical records and of discharge abstracts, as judged especially by their format and by the completeness, accuracy, congruency and clearness of the reported information, varied extensively among the different wards and hospitals. The main problems were identified and possible solutions are discussed. We suggest that improved quality of medical records and discharge abstracts may be promoted through: 1) use of a standardized format for medical records in all wards of the same hospital; a supplement may eventually account for the characteristic requirements of specialist wards; 2) adoption of guidelines for medical record-keeping and of a code of conduct for discharge abstract coders; 3) use of a new discharge abstract form that favours more accurate ICD-9-CM coding and that allows more effective gathering of clinical data for epidemiological purposes.

Long-waiting lists to undergo preliminary medical examinations and treatments are a tough problem for western countries. Many actions and programs have been taken to offer more health services, which initially shortened the waiting lists. However, after this initial alleviation, the later demand for further diagnostic examinations inevitably creates subsequent increases in diagnostic service waiting times. In this article the authors suggest some solutions to manage this problem: creating organized procedures for case management, homogeneous waiting-list reporting systems, outpatient procedures for white triage and diagnostic services opening overtime during non-working days. An alternative form of reimbursement is also suggested.
