Abdominal field blocks are commonly used as part of multimodal analgesia for post-operative pain relief in patients undergoing abdominal surgery. Long QT syndrome (LQTS) is an arrhythmogenic cardiac disorder resulting from the malfunction of cardiac ion channels. Patient with LQTS may present with syncope, seizures or sudden cardiac death secondary to polymorphic ventricular tachycardia (VT) or torsades de pointes. Patient may be asymptomatic in the pre-operative period but may develop VT for the first time in operation theatre. The article presents a case study of a 10-year-old child with right mainstem endobronchial tumour with collapse-consolidation of the underlying lung. The patient was set for the right postlateral thoracotomy and pneumonectomy and was induced with general anaesthesia. A difficulty in ventilation of the patient was experienced during surgery, causing the patient to became haemodynamically unstable with bradycardia. Common intraoperative issues encountered during thoracic surgery are noted.

The clinical features of hypocalcaemia can often be subtle and may manifest in the presence of associated factors. Hypoparathyroidism, an important cause of hypocalcaemia, often presents as soft tissue calcification (ostosis). Ligamentum favum ostosis can present with compressive myelopathy requiring laminectomy. We report a case of ligamentum favum ostosis and subclinical hypocalcaemia due to hypoparathyroidism, who went undetected pre-operatively resulting in significant post-operative morbidity.
