The phylum Actinobacteria is recognized as the most promising producer of many bioactive compounds. Among Actinobacteria, the genus Streptomyces is a prolific producer of many antibiotics, for example, Streptomycin. The present study investigates the bioactivity profiles of Actinobacterium BTA 1-131 (A1205) isolated from an Indonesian marine sponge, Melophlus sarassinorum. Molecular identification using the 16S rRNA gene sequencing showed that the isolate is similar to Streptomyces kunmingensis (98.4%). The isolate was cultivated in three fermentation media (ISP2, YS, and SYP) to select the best media for producing bioactive compounds. The compounds were extracted from solid and liquid cultures using methanol as a solvent. The crude methanolic extracts were tested for antibacterial and anticancer activities. The bioactivity screening indicated potential activity of the extracts against bacterial pathogen Staphylococcus aureus ATCC 13420, invasive breast cancer MDA-MB-231 and human colorectal adenocarcinoma cells CACO-2. Profiling using LC-MS/MS indicated the probable influence of fermentation media to induce the selective production of bioactive compounds. Although further dereplication and fractionation are essential for the specific identification of bioactive compounds, the present study demonstrates the potential of Streptomyces BTA 1-131 as a producer of tetracycline and/or staurosporine. Further genomic analysis, such as whole genome sequencing, will also be beneficial to enable comprehensive exploration of the bioactive potential of BTA 1-131 via a genome-mining approach. Stress during Coronavirus disease-2019 (COVID-19) pandemic affects the physiological and immunological response to women's reproductive health. Meanwhile, Apium graveolens and Eucalyptus globulus are immunomodulators related to women's reproductive health. This investigation had a goal to examine the effectiveness of A. graveolens and E. globulus towards the expression of Heat Shock Protein-70 (HSP70) as the primary biomarker of stress, Tumor Necrosis Factor-Alpha (TNF-a) as a proinflammatory protein, along with Luteinizing Hormone (LH) and Growth Differentiation Factor 9 (GDF-9) as folliculogenesis markers. An experimental randomized controlled trial was utilized by using a pre-test and post-test control group design. Sixty women, who had stress based on DASS-21 questionnaire, were divided into two groups in Nusukan Health Center, Indonesia. The intervention group was orally administered with 300 mg A. graveolens capsules and E. globulus essential oil for 14 days, while the control group was given a placebo. Blood samples and stress levels were then evaluated before and after the intervention. No significant difference was found in the stress levels of the control and treatment groups at the pre-test. Meanwhile, the intervention group had the decreased HSP70, TNF-a, and stress levels (p<0.001). In contrast, increased LH and GDF-9 levels were displayed in the intervention group compared to the control group (p<0.001). These findings revealed that A. graveolens supplementation and E. globulus essential oil have the ability to decrease stress and are able to protect folliculogenesis markers on women's reproductive health due to stressful conditions during pandemic COVID-19. Immunostimulants are compounds that can stimulate an immune response by increasing the activity of non-specific and specific components of the immune system (humoral and cellular) against certain infections and diseases. The liverwort plant species Marchantia paleacea Bertol. has long been used as a source of nutrition and empirical medicine. However, scientifically there is still not much research data on immunomodulators in these plants. This study aims to determine the activity of immunomodulators in the ethanol extract of the herb Marchantia paleacea Bertol. in male mice of BALB/c strain. Bioactive compounds from this plant were extracted by maceration method using 96% ethanol. Extract characterization and phytochemical screening were determined according to WHO guidelines and standard procedures from previous studies. The immunomodulatory activity of the extract was tested by carbon clearance method and lymphoid organ index (non-specific responses), primary and secondary antibody titer tests (humoral specific responses), IL-2 cytokine levels and IFN-ɣ from serum secondary antibodies and delayed-type hypersensitivity reaction/DTH (cellular specific response). The results of qualitative phytochemical screening contained flavonoid compounds, saponins, phenolics, tannins and steroids/triterpenoids. The results of the non-specific immune response immunomodulator test showed that the dose of 52 mg/kg bw had the largest phagocytic index of 1.52 which included strong immunostimulation (K > 1.5) and the organ spleen index of 0.55 ± 0.11 which increased significantly compared to the control (p<0.05). The data on the acquisition of specific immune responses in the primary and secondary antibody titer test in the three test extracts resulted in increased titer levels compared to the control and at a dose of 52 mg/kg bw could significantly increase the levels of IL-2 cytokines in the control group (p<0,05). Meanwhile, in DTH test, doses of 13 and 26 mg/kg bw could significantly increase the thickness of the soles of mice compared to controls (p<0.05). [ABSTRACT FROM AUTHOR]

Two of Mannich bases derivatives of 4'-methoxy-substituted of asymmetrical cyclovalone analog (ACA) (2a and 2b) were synthesized. The synthesized compounds and the other two Mannich bases derivatives of 4'- methoxy-substituted ACA (2c and 2d) were evaluated for their in-vitro antiinflammatory activity preliminary by protein denaturation inhibition method using a final concentration of 1.57 µM. The study found that all the Mannich bases exhibited anti-inflammatory potential with inhibition ranging from 33.17- 42.47%. The activity of 2b (42,47%) and 2d (41.90%) was higher than that of diclofenac sodium (35.27%) and the parent compound 1 (38.16%). As a conclusion, 2b and 2d have a prospect as a potential candidate for an anti-inflammatory agent. Further study should be done using more specific methods. [ABSTRACT FROM AUTHOR]

One of the causes of injury to the elderly is due to falls. Falling can be prevented by identifying and controlling risk factors. One risk factor that can be controlled is the use of fall risk medicines including psychoactive. This study aims to identify the association between the use of psychoactive medicine and its characteristic with the risk of falls among the elderly in Indonesia. The study utilized a case-control study design for a total number of 414 elderly patients, during October until December 2018. Cases were elderly aged 60 years or above with a high risk of falling assessed using the Morse Fall Scale (MFS=45). Each case was matched with up to two randomly selected controls of the same age who are classified as low to moderate risk of falling (MFS<45). The use of psychoactive medicines was screened from a history of drug use for the past six months. Psychoactive medicine-fall risk associations were estimated via logistic regression. There were 138 cases and 276 controls. The median age of subjects was 66 years old and 54.83% was a woman. Elderly with a high risk of falling had higher psychoactive medicines use when compare with controls (31.16% vs 21.38%, p< 0.05). After adjusting for potential confounders, the use of psychoactive medicines was significantly associated with higher fall risk in elderly patients (OR 1.79 95% CI 1.10-2.90). Only the duration of psychoactive medication use over 90 days was significantly associated with a high risk of falling (AOR 3.65 95% CI 1.46- 9.14). In elderly patients, the continued use of psychoactive medicines increased the risk of fall. Curcumin has been reported can interact with multiple molecular targets involved in a large variety of diseases. Accumulated evidence indicated curcumin plays an inhibitory role against infection of numerous viruses. Some studies have been reported that curcumin can interfere the infection processes of dengue virus. In this work, a benzenesulfonyl curcumin, (3E,5E)-3,5-bis(4-methoxybenzylidene)-1-(phenylsulfonyl) piperidin-4-one (compound 2) has been synthesized by two steps of reactions. The structure of compound 2 has been established based on the interpretation of spectral data include UV, FT-IR, MS/MS, 1H and 13C NMR. Then, the in silico studies have been also performed to predict the potency of compound 2 as inhibitor for dengue virus Type 2 (DEN 2) NS2B/NS3 protease. The in silico studies showed that compound 2 has hydrogen bonding with His51 residue, and amazingly that the other catalytic triad such as Asp75 and Ser135 were also showed interactions with the ligand. Less optimized therapeutic effects constrain the use of doxorubicin as the main agent of chemotherapy for metastatic breast cancer, resistance and side effects. Therefore we need a combination of more than one chemopreventive agent which has different molecular targets to solve that problem. The aims of this study is to prove the inhibitory effect of ethanolic extract of rhizome of Curcuma xanthorrhiza (ECx), fruit of Brucea javanica (EBj), leave of Ficus séptica (EFs) and doxorubicin (Dox) alone and its combination on migration and invasion of a highly metastatic 4T1 breast cancer cell line. Cytotoxic activity of single and combination treatment was evaluated by MTT assay, followed by an experiment of apoptosis induction by using flow cytometry. The inhibitory effect on migration was observed by the scratch wound-healing assay. Furthermore, the observation of the activity of matrix metalloproteinase-9 (MMP-9) was analyzed by gelatin zymography. The results showed that ECx, EBj, EFs, and Dox has cytotoxic activity on 4T1 cells with the value of IC50 respectively 49.7±1.53/ig/mL, 59.9±1.79/ig/mL, 15.2±2.12/g/mL and 1.2±0.23/M. Furthermore, the combination of ECx-EBj-Dox and ECx-EBj-EFs revealed a synergistic effect on 4T1 cells and decrease cell viability through the induction of apoptosis and necrosis. Based on wound healing assay, 24h incubation of this combination inhibited 4T1 cells migration compared to single treatment. Gelatin zymography analysis showed that this combination also inhibited the activity of MMP-9 greater than a single use. Curcuma xanthorrhiza, Brucea javanica, and Ficus séptica may have potential to be developed as a combination with or without doxorubicin for metastatic breast cancer treatment. Brown macroalgae (BM) which contain fucoxanthin exhibited high antioxidant activity. This study was performed to examine antioxidant, anti-collagenase, anti-elastase, antityrosinase activities, and effect on cell viability of Human Dermal Fibroblast adult (HDFa) of BM, Turbinaria decurrens Bory. T. decurrens dried powder were macerated by ethanol to obtain extract (ETD) and was fractination by column chromatography to obtain fraction (FTD). Fucoxanthin content was determined using HPLC. The antioxidant activities, anti-collagenase, anti-elastase, and tyrosinase inhibitory assay were performed. The effect of ETD and fucoxanthin standard on cell viability were conducted on HDFa cell-induced by hydrogen peroxide (H2O2). The HPLC analysis showed that ETD and FTD contain fucoxanthin of 284.9±3.3μg/g and of 653.4±30.6μg/g dry-weight, respectively. The antioxidant assay showed that ETD and FTD produced high antioxidant activity by ferric reducing antioxidant power (FRAP) and β-carotene bleaching (BCB) methods that were comparable to fucoxanthin. ETD exhibited significantly higher tyrosinase inhibitory than kojic acid (p<0.01), while FTD had a comparable effect to kojic acid. The result also revealed that ETD and FTD produced anti-elastase and anti-collagenase (matrix metalloproteinase-1 (MMP-1). Fucoxanthin and ETD were able to maintain cell viability on HDFa cell-induced H2O2. This study suggests that T. decurrens may be effective to prevent skin aging and wrinkle formation, possibly through the antioxidant activity and maintain cell viability of fibroblast. Tuberculosis (TB) is an acute infectious disease that primarily affects the lungs. Probiotics supplementation can increase the number and activity of NK cell in peripheral blood by modulation of interleukin-12 (IL-12), thus increasing interfenon-γ (IFN-γ) production by T-helper cells type 1 (Th1) response. Vitamin B1 acts on macrophages and affects neutrophil motility. Vitamin B6 is associated with the release of cytokines and the responsiveness of NK cells, while vitamin B12 affects to lymphocytes, Tcell proliferation, CD4+ ratios, and NK cell activity. This study aim to analyze the effects of probiotics and vitamin B1, B6, B12 supplementation on IFN-γ and IL-12 levels during intensive phase of antituberculosis treatment. The study was pre-post test randomised control by time series. The control group was TB patients with standard therapy of antituberculosis and vitamin B6, while the intervention group was TB patients receiving therapy plus once daily probiotics and vitamin B1, B6, B12 supplementation during the intensive phase. Blood samples were withdrawn at baseline, one month, and two months after therapy to measure plasma IFN-γ and IL-12 levels using the ELISA method. Twenty two patients were divided equally into two groups. There was a tendency to greater increase of IFN-γ in the first month of the intervention group, followed by a significant decline after two-month therapy (p <0.05). In both groups there was a rise in IL-12 levels after one month followed by a decrease in the second month (p>0.05). However, the percentage was higher in the supplementation group. To analyze the differences in the effect of dual antiplatelet post PCI on the percentage of aggregation in myocardial infarction patients with DM and non DM. Percentage of aggregation were analyzed using light transmission aggregometry (LTA) before loading dose, after PCI, and after maintenance dose of dual antiplatelet (aspirin 100mg and clopidogrel 75mg). Total 22 patients were participated in this study divided into 10 and 12 patients in diabetic and non diabetic group. Percentage of aggregation after taking dual antiplatelet maintenace dose decrease significantly in both group (p=0.006 in diabetic group and p=0.002 in non diabetic group). Mean reduction of percentage of aggregation in diabetic group (3.30±2.91%) is less than non diabetic group (6.83±5.97%). Statistical analysis shows that the mean reduction of percentage of aggregation between two groups were not significantly different (p>0.05). Mean percentage of aggregation after dual antiplatelet maintenance dose was higher in diabetic group and mean reduction of percentage of aggregation was higher in non diabetic group, although statistically in both group it is not significantly different. Patient adherence is a key factor that determines the outcome of diabetes mellitus therapy. This study was conducted to determine the effect of a medication reminder chart on level adherence and clinical outcome in patients with Type 2 diabetes mellitus. The experimental study used pretest-posttest design with control group was conducted during February until April of 2016 at Sleman Hospital in Yogyakarta. Patients were categorized into two groups of different subjects, who received of medication reminder chart tools were 40 patients (Code A) and the other 40 patients were assigned to control group (Code B). Morisky Medication Adherence Scale of 8-item (MMAS-8) was used to measure adherence. Clinical outcome were measured based on FBG decrease (mg/dl). Two independent sample t-test was using to determine the mean differences of the average score of the adherence level before and after using the medication reminder chart. The results of this study showed that Medication Reminder Chart compared control groups improved adherence (p=0.000) but did not improve clinical outcome (p=0.411). Chronic Renal Failure (CRF) is a slow progressive loss of kidney function over a period of several years. One of its complications that determines the patient's quality of life is anemia. Blood transfusion can be used to treat anemia in patient with CRF. The purpose of this study are to measure the average costs per patient, assess the therapy outcomes, and determine the factors that influence the cost of blood transfusion in anemia treatment for patients with CRF in A, B, and C hospitals. This study was a descriptive non-experimental research. Cost analysis was based on the perspective of the hospital. Data were obtained retrospectively from medical records and payment details of inpatient with CRF who experienced anemia in A (tertiery hospital in Yogyakarta province), B (tertiary hospital in Central Java province), and C (private hospital in the province of Yogyakarta) hospitals. Data was analysed by calculating the average costs per patient, while therapy outcome was measured by calculating the percentages of patients who achieved the target of hemoglobin level. Furthermore, the factors that influenced the amount of the total cost were analysed using Oneway ANOVA and Independent T-Test for data with normal distribution and the Kruskal-Wallis and Mann-Whitney for data with non-normal distribution. The results showed that the average costs per patient for anemia treatment in patient with CRF who had blood transfusion therapy were Rp 8,873,243.39±4,417,825.83 in A hospital, Rp 5,249,464.97±4,283,655.41 in B hospital; and Rp 7,961,088.00±6,105,501.80 in C hospital. Most of the patients in A, B, and C hospitals had achieved the target of hemoglobin level (>7g/dL) with the percentages of 95.56%, 68.57% and 87.50% respectively. The factors that mainly influenced the cost of anemia treatment in CRF are length of hospitalization for A hospital, age, gender, payment models, and length of hospitalization for B hospital, whereas age and length of hospitalization for C hospital.
