1. Les femmes enceintes : des voyageuses comme les autres ?

Introduction Peu de donnees sont disponibles concernant les femmes enceintes qui voyagent. Nous rapportons ici les caracteristiques des femmes qui voyagent pendant leur grossesse. Materiels et methodes Les dossiers des femmes âgees de 18 a 49 ans qui se sont presentees au Centre des vaccinations internationales (CVI), entre le 29 fevrier 2016 et le 4 avril 2021, ont ete etudies. Les donnees concernant la destination, la duree, le motif et les modalites de sejour sur place, ainsi que la prise de prophylaxie antipaludique, ont ete recueillies. Nous avons compare les caracteristiques des femmes enceintes a celles qui ne l’etaient pas dans la meme tranche d’âge. Resultats Dans la periode, 178 342 personnes se sont presentees au CVI, representant 187 147 voyages. Parmi ces voyageurs, 58 275 etaient des femmes âgees de 18 a 49 ans dont 818 etaient enceintes soit 0,46 % de l’ensemble des voyageurs. Les voyageuses enceintes ont un âge median de 32 ans contre 29 ans pour leurs homologues non enceintes. En dehors de la grossesse, le premier continent de destination est l’Afrique pour 47,7 %, suivi de l’Asie (27 %), puis de l’Amerique Latine (19 %) contre respectivement 56,2 %, 23,6 % et 18,5 % chez les femmes enceintes. Les femmes enceintes avaient prevu un sejour multi-destinations dans 0,8 % des cas contre 4,2 % des voyageuses non enceintes. La duree de sejour prevue par les femmes enceintes est inferieure a 30 jours (j) dans 85 % des cas et superieure a 90 j dans 5,2 % des cas comparativement a 72 % et 16,8 % chez les autres. La mediane de sejour est de 16 j pour les femmes enceintes et de 20 j pour celles qui ne l’etaient pas. Un tiers (36,1 %) des femmes enceintes qui voyagent ont eu une consultation medicale au CVI, en sus de l’avis vaccinal, contre 7 % pour les autres. Parmi les 296 femmes enceintes ayant eu une consultation medicale, le motif etait renseigne dans 275 cas : 24 % des femmes allaient visiter leurs proches alors que 60 % effectuaient un voyage d’agrement, et 15 % se deplacaient pour le travail. Le sejour etait prevu dans une zone d’endemie palustre dans 49,8 % des voyages et la prophylaxie prescrite etait l’atovaquone/proguanil dans 94,6 % des cas. Parmi les 4079 femmes de 18 a 49 ans ayant eu une consultation, le motif etait renseigne dans 1562 cas : 14,1 % allaient visiter leurs proches, 63,4 % faisaient un voyage d’agrement, et 22,3 % un deplacement professionnel. Lorsqu’une chimioprophylaxie antipaludique est indiquee (66 % des cas), l’atovaquone/proguanil etait l’association preferentiellement prescrite (85 %) devant la doxycycline (13,7 %). Conclusion Les femmes enceintes consultant en CVI voyagent globalement vers les memes destinations que les autres mais ont plus souvent recours a une consultation medicale. Elles sejournent y compris dans des pays a haut niveau de risque sanitaire, en zone rurale comme urbaine comme leurs homologues non enceintes, mais pour des durees moins longues. Meme si elles se deplacent plus pour visiter leurs proches, 60 % partent pour un sejour d’agrement, autant qu’hors grossesse. Elles se rendent dans la moitie des cas dans des zones d’endemie palustre, ou l’atovaquone/proguanil leur est majoritairement prescrite.
