We describe the case of a pregnant woman with an adenosinerefractory, poorly tolerated atrioventricular nodal reciprocating tachycardia (AVNRT), and successfully treated by RF ablation using an electroanatomical mapping system without radiation exposure. The procedure with the use of the navigation system associated with an irrigated catheter results in a possibility of definitive resolution of the tachycardia in absolute safety for the patient and for the fetus.

A 39 year-old man with no history of heart disease and no significant comorbidity was admitted to our department for intense palpitations with dyspnea and chest pain. The electrocardiogram revealed wide QRS tachycardia with right bundle branch block and extreme left axis deviation with ventricular rate of 230 bpm. No response to repeated bolus of adenosine and verapamil infusion. In sinus rhythm during ablation has highlighted the presence of the potential of Purkjnie located in the left inferior-septal area of left ventricle. Ablation in this site during tachycardia was effective to restore sinus rhythm without arrhythmia inducibility.
