Purpose: To analyze the risk of dental implant failure for patients with a history of antidepressant use through a systematic review and meta-analysis., Materials and Methods: An electronic search was performed up to June 2023 in three databases including PubMed/MEDLINE, Web of Science, and Cochrane Central Register of Controlled Trials; data on the comparison of implant failure rate for patients with and without the use of antidepressants were included. Meta-analyses for the risk ratio of implant failure rate at patient level and implant level were performed., Results: Overall, 11 clinical studies were selected for inclusion in this review. The meta-analyses showed a risk ratio of 2.44 (95% CI &#61; 1.75 to 3.39; P < .0001) and 2.44 (95% CI &#61; 1.73 to 3.46; P < .0001) for the implant failure at patient level and implant level, respectively. The comparisons presented a low heterogeneity for the patient-level analysis and a moderate heterogeneity for the implant-level analysis among the pooled studies. Subgroup analyses also revealed that patients who received only selective serotonin reuptake inhibitors (SSRIs) or SSRIs with other types of antidepressants had a higher risk of implant failure than those who were not on any antidepressants., Conclusions: The current review demonstrates that the use of antidepressants such as SSRIs may increase the risk of dental implant failure at both patient level and implant level. Although limited evidence suggests that a certain type of SSRI (sertraline) may have more influence on implant failure than other SSRIs, future studies are needed to confirm this finding.

Purpose: To assess site-related features of peri-implantitis occurring adjacent to teeth and its association with the proximal periodontal bone level., Materials and Methods: Periapical radiographs were collected from partially edentulous patients exhibiting peri-implantitis adjacent to teeth. The following variables were quantified: intrabony defect width (DW), implant marginal bone loss (MBLi), tooth marginal bone loss (MBLt), implant-tooth distance (ITd), intrabony defect angulation (DA), adjacent periodontal bone peak height (ABPh), and implant-tooth angulation (ITa). A correlation matrix using the Spearman correlation coefficient was created to explore the dependence of these variables. Univariate linear regression analysis was carried out by means of generalized estimating equations (GEE), using MBLt as dependent variable., Results: Overall, 61 patients and 84 implants were included in this study, consisting of a total of 105 implant sites facing adjacent teeth. This resulted in 515 linear and 194 angular measurements. A total of 11 different statistically significant associations were demonstrated between the different variables analyzed. Moreover, the univariate regression analysis revealed significant positive associations between MBLt and MBLi (P = .013) and between MBLt and periodontitis (PD) (P = .014). These associations were confirmed in the multivariate model., Conclusions: Teeth adjacent to untreated peri-implantitis lesions are associated with proximal loss of periodontal support. This finding is more remarkable in scenarios that display short implant-tooth distance.

Purpose: To study the effect of BMP-2 on the volumetric and histometric changes in peri-implant defect treatments in animal models. Materials and Methods: An electronic search of four databases and a manual search of peer-reviewed journals for relevant articles were performed. Animal studies with data that compared the volumetric and/or histometric outcomes of peri-implant defect treatment with and without the use of BMP-2 were included. Meta-analyses were performed to analyze the weighted mean difference (WMD) and confidence interval (CI) for the recorded variables. Results: After completing the search process, 21 randomized controlled trials were included. The results of the meta-analyses showed that the WMD of bone-to-implant contact (%BIC) with 4 to 8 weeks and 12 to 24 weeks of follow-up was 15.50% (95% CI = 3.28% to 27.72%, P = .01) and 16.17% (95% CI = 11.17% to 21.16%, P < .00001), respectively, favoring the BMP-2 group. The WMD for the percentage of defect fill with 4 to 8 weeks and 12 to 24 weeks of follow-up was 15.88% (95% CI = 3.90% to 27.86%, P = .009) and 10.48% (95% CI = 0.95% to 20.02%, P = .03), respectively, favoring the BMP-2 group. The WMD for the vertical bone gain with 8 to 16 weeks of follow-up was 1.63 mm (95% CI = 0.58 to 2.67 mm, P = .002), also favoring the BMP-2 group. Conclusion: This review demonstrated that the use of BMP-2 in treating peri-implant defects showed better clinical and histometric outcomes than defects not treated with BMP-2 in animal models.

Purpose: The purpose of this cone beam computed tomography (CBCT) study was to investigate the probability of using straight screw-channel screw-retained restorations following an immediate implant placement and provisionalization protocol in maxillary anterior teeth utilizing implant planning software., Materials and Methods: A retrospective review of CBCT images was done. The midsagittal images of maxillary anterior teeth (right canine to left canine) were created in implant planning software, screen-captured, and transferred to a presentation program. The digital implant templates were created for 3.5-mm-diameter (used for central and lateral incisors) and 4.5-mm-diameter (used for central incisors and canines) tapered implants with lengths of 13, 15, and 18 mm. The frequency percentages of immediate implant placement and provisionalization and immediate implant placement and provisionalization with straight screw-channel screw-retained restorations were recorded and compared among all maxillary anterior teeth., Results: CBCT images from 1,200 teeth were evaluated. The overall frequency percentages of immediate implant placement and provisionalization and immediate implant placement and provisionalization with straight screw-channel screw-retained restoration were 84% (range = 74% to 92%) and 14% (range = 10% to 24%), respectively., Conclusion: Although the probability of being able to employ immediate implant placement and provisionalization with a straight screw-channel screw-retained restoration in the esthetic zone is low, the use of smaller-diameter implants can substantially increase the probability.

Purpose: To evaluate the current scientific evidence on estimating cumulative risk for biologic complications relating to dental implants and to develop a patient-centered risk assessment tool for establishing aggregate risk., Materials and Methods: A review of the scientific literature on risk indicators relating to dental implants was completed with the goal of identifying and weighting individual risk indicators so aggregate biologic risk could be estimated. Three authors completed independent reviews of the literature, identifying 31 systematic reviews on risk indicators for biologic complications with dental implants, from which 24 potential risk indicators were considered. Due to inconclusive scientific data on risk indicators, a Delphi process was used to gather structured expert opinion to supplement findings from the literature. Eleven Delphi participants with expertise in prosthodontics or periodontics participated in two email exchanges and one face-to-face meeting to comment and debate on the initial identification and weighting of risk indicators, propose the addition or removal of risk indicators, and provide recommended clinical management for each risk indicator., Results: After three rounds of debate, literature review, and additions and removals of various risk indicators, consensus (defined as 95% or more in agreement) was achieved on 20 risk indicators. The Delphi group concluded that the risk indicators of smoking, diabetes, antiresorptive agents, and cemented restorations should include subcategories to more accurately identify and represent patient-specific risk. Clinical recommendations based on individual and aggregate risk were established by consensus., Conclusion: The literature on risk indicators for biologic complications was conflicting and inconclusive. The Delphi method was used to identify and establish the weighting of individual risk indicators, resulting in a risk assessment tool for estimating aggregate risk.

Purpose: The aim of this study was to systematically analyze the effect of growth factors, particularly recombinant human platelet-derived growth factor-BB (rhPDGF-BB) and bone morphogenetic proteins (BMPs), on volumetric and histomorphometric changes after socket augmentation in comparison with the natural healing sockets., Materials and Methods: An electronic search of four databases (1965 to February 2017) and a hand search of peer-reviewed journals for relevant articles were performed. Human clinical trials that reported quantitative and qualitative outcomes of soft and hard tissues in socket augmented sites with the use of rhPDGF-BB or BMPs, with a minimum five samples per group, were included., Results: Eight studies, including six randomized controlled trials and two case series, were selected. Five of them used BMPs, and three used rhPDGF-BB. Regarding linear bone width change, the weighted mean difference (WMD) between the sites with and without the use of BMPs was 1.66 mm (95% confidence interval = 0.29 to 3.02 mm, P = .02), favoring the BMP group. In terms of histomorphometric outcome, the WMD of the percentage of vital bone between the sites with the use of rhPDGF-BB and with grafting materials alone was 2.16% (95% confidence interval = -4.61% to 8.93%, P = .53)., Conclusion: This systematic review revealed that the use of BMPs in socket augmentation yields better ridge width in comparison with a natural healing socket. However, more studies are needed to warrant the effectiveness when using rhPDGF-BB in socket augmentation procedures.

Purpose: To correlate the link between natural teeth located in esthetic regions of the arches before extraction with their definitive dental implant restorations, and to propose a new and contemporary esthetic index based on the natural dentition (EIND) preoperatively., Materials and Methods: Patients possessing a high smile line with teeth requiring implant replacement therapy in the maxillary anterior region were included. Clinical intraoral photographs and periapical digital radiographs taken before extraction and 10 ± 1.5 months (range: 9 to 12 months) after the delivery of implant-supported permanent restorations were used to make the measurements and evaluations according to the proposed index. Pink esthetic score (PES) parameters were analyzed. Statistical analyses of preimplant and postimplant parameters between tooth sites and implant sites were carried out with a computer software program., Results: The study included 51 patients (35 women, 16 men), with a mean age of 39.6 years, with a total of 83 dental implants placed in the maxillary anterior region. Of these, there were 6 right canines, 22 right lateral incisors, 11 right central incisors, 22 left central incisors, 17 left lateral incisors, and 5 left canines. Of the 83 included implants, 67 were immediately and 16 were delayed placed. Statistical analyses between tooth sites and implant sites showed significant differences in tissue contour (P = .001), texture (P = .001), alveolar deficiency (P = .001), and total PES (P = .007). No statistically significant difference was detected in mesial papilla, distal papilla, tissue margin, tissue biotype, and the amount of keratinized mucosa. Additionally, R2 value presented a higher variance and lower predictable values of peri-implant tissue parameters compared with those of the natural dentition., Conclusion: This study analyzed various periodontal and peri-implant soft and hard tissue parameters, and a new index system, EIND, was proposed. This index could be used to collect the esthetic-related parameters for pre-extraction treatment planning and provide valuable information for esthetic risk assessment.

Purpose: The rate of developing soft tissue complications that accompany guided bone regeneration (GBR) procedures varies widely, from 0% to 45%. The present review was conducted to investigate the rate for resorbable versus nonresorbable membranes and the timing of soft tissue complications., Materials and Methods: Electronic and manual literature searches were conducted by two independent reviewers using several databases, including MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, and Cochrane Oral Health Group Trials Register, for articles published through July 2015, with no language restriction. Articles were included if they were clinical trials aimed at demonstrating the incidence of soft tissue complications following GBR procedures., Results: Overall, 21 and 15 articles were included in the qualitative and quantitative synthesis, respectively. The weighted complication rate of the overall soft tissue complications, including membrane exposure, soft tissue dehiscence, and acute infection/abscess, into the calculation was 16.8% (95% CI = 10.6% to 25.4%). When considering the complication rate based on membrane type used, resorbable membrane was associated with a weighted complication rate of 18.3% (95% CI: 10.4% to 30.4%) and nonresorbable membrane with a rate of 17.6% (95% CI: 10.0% to 29.3%). Moreover, soft tissue lesions were reported as early as 1 week and as late as 6 months based on the included studies., Conclusion: Soft tissue complications after GBR are common (16.8%). Membrane type did not appear to significantly affect the complication rate, based on the limited number of data retrieved in this study. Technique sensitivity (ie, soft tissue management) may still be regarded as the main component to avoid soft tissue complications and, hence, to influence the success of bone regenerative therapy.

Purpose: This systematic review and meta-analysis was aimed at evaluating the longitudinal effect of platform switching on implant survival rates as well as on soft and hard tissue outcomes., Materials and Methods: An electronic search of the databases of the National Center for Biotechnology Information, PubMed, Ovid (MEDLINE), EMBASE, Web of Science, and Cochrane Collaboration Library was conducted in February 2015. Studies published in English with at least 10 human participants and a 12-month post-loading follow-up were included. Random effects meta-analyses of selected studies were applied to compare the primary and secondary outcomes of platform-switched (PS) and regular-platform (RP) implants, as well as the experimental designs and clinical outcomes., Results: A total of 26 studies involving 1,511 PS implants and 1,123 RP implants were evaluated. Compared to RP implants, PS implants showed a slight increase in vertical marginal bone loss (VMBL) and pocket depth reduction (weighted mean differences were -0.23 mm and -0.20 mm, respectively). The PS implants had a mean VMBL of 0.36 ± 0.15 mm within the first year of service. The meta-regression suggested a trend of decreased bone resorption at sites with thick soft tissues at baseline., Conclusion: This study suggested that platform switching may have an indirect protective effect on implant hard tissue outcomes.

Purpose: The purpose of this systematic review was to evaluate long-term outcomes of regenerative procedures for treating peri-implantitis., Materials and Methods: Electronic searches of five databases and hand searches of journals were performed to identify human trials that had treated more than 10 periimplantitis lesions using a regenerative approach with a follow-up period of at least 36 months. To meet the inclusion criteria, studies had to report at least one of the following parameters: radiographic bone fill (RBF), probing depth (PD) reduction, clinical attachment level gain, bleeding on probing reduction, and mucosal level gain. The pooled weighted mean and the 95% confidence interval (CI) of each variable were estimated., Results: The searches yielded 1,412 records, and after evaluating titles, abstracts, and full texts, 5 case series and 1 controlled trial were included for quantitative data synthesis. Meta-analysis of the studies for the amount of RBF revealed a weighted mean of 2.41 mm (range, 1.46 to 3.30 mm) with 95% CI. For PD reduction, the weighted mean was 3.06 mm (range, 1.24 to 5.21 mm)., Conclusion: There is limited evidence in the literature reporting long-term results of the regenerative approach for treating peri-implantitis. Within the limits of this meta-analysis, regenerative treatment of peri-implantitis resulted in a mean radiographic defect fill of 2.41 mm after a minimum healing time of 36 months. However, this finding must be interpreted with caution, since it is difficult to discern between grafting material and newly formed bone.

Purpose: Multiple bone augmentation techniques are available to allow implant placement in the atrophic maxilla. However, questions remain, regarding which methods are most predictable and have the best dental implant survival rate (SR) in grafted bone. The aim of this systematic review was to evaluate literature from the last 30 years to determine predictability of bone grafting of the edentulous maxilla for implant placement as well as for implant SR., Materials and Methods: A systematic review was performed of studies conducted during the period 1980 to 2014, specifically focusing on the edentulous maxilla and bone grafting. Surgical techniques discussed in the publications included were guided bone regeneration (GBR), sinus augmentation, onlay bone grafting, nasal floor grafting, and Le Fort I interpositional grafting. All identified articles were evaluated and screened to meet strict inclusion criteria of at least 10 patients, complete maxillary edentulism, 1-year follow-up, and information regarding implant SR. A total of 974 articles were identified with electronic and manual searches. On further evaluation of the titles and abstracts, 44 articles were excluded. Full texts of the articles that met the inclusion criteria were reviewed, of which 40 articles were included in the systematic review., Results: For onlay bone grafting, 16 studies were included and analyzed, and the weighted mean implant SR was 85.2%. For the GBR technique, two studies were included, with a reported SR ranging from 96.1% to 100%. For Le Fort I interpositional grafting, 11 studies were included, with a weighted mean SR of 89.6%. For the sinus augmentation technique, 12 studies were investigated and the weighted mean SR was 91.5%. For the combination technique, six studies were analyzed and the weighted mean SR was 93.6%., Conclusions: All five treatment modalities discussed-onlay bone grafting, GBR, Le Fort I interpositional grafting, maxillary sinus augmentation, and/or nasal floor inlay grafting or the combination approach-can be successfully used to augment edentulous maxillary ridge with high implant SRs.

Purpose: Many techniques have been proposed to overcome the limitations displayed by maxillary atrophy. The aim of this systematic review was to assess the predictability, dimensional changes, and associated factors to successfully perform the alveolar ridge split (ARS) technique of augmentation., Materials and Methods: An electronic and manual literature searches was conducted by two independent reviewers in several databases, including Medline, Embase, Cochrane Central Register of Controlled Trials, and Cochrane Oral Health Group Trials Register, for articles written in English up to February 2014. A manual search was also performed to ensure a thorough screening process. Based on the PICO (problem, intervention, comparison, outcome) model, the chief question of this study was: Can patients with horizontal ridge deficiency be successfully treated with the ARS technique and implant therapy?, Results: Overall, 17 articles met the inclusion criteria, and a subsequent meta-analysis was performed. A Cohen kappa interagreement rate of 0.82 was reached. The implant survival rate of the included studies was 97.0% (range, 94.4% to 100%) with the full-thickness flap (FTF) approach and 95.7% (range, 86.6% to 100%) with the partial-thickness flap (PTF) approach. The weighted mean (WM) of horizontal bone width gain was calculated for included studies using FTF for the ARS technique. Four studies that had data were included in the meta-analysis. The WM ± standard deviation of bone width gain was 3.19 ± 1.19 mm (range, 2.00 to 4.03 mm). For studies using PTF for ARS, only one study provided mean and standard deviation of horizontal bone width gain (4.13 ± 3.13 mm); hence, meta-analysis could not be performed. Buccal wall fracture represented the most frequent postoperative complication, followed by postoperative ridge resorption., Conclusion: In selected scenarios, the ARS technique might represent a predictable approach as demonstrated by a high implant survival rate, adequate horizontal bone gain, and minimal intra- and postoperative complications. Further research is needed to determine the influence of the grafting materials inserted and flap tissue biotype, as well as the anatomical characteristics on final bone augmentation outcomes.

Purpose: The reliability of oral rehabilitation by four zygomatic implants with no anterior support remains to be determined. The aim of this systematic review was to assess the predictability of this approach in regard to implant survival, technical and biologic complications, and quality of life., Materials and Methods: An electronic literature search was conducted from September 2000 to November 2013. Human clinical trials in which oral rehabilitation was achieved by the use of four zygomatic implants with no additional placement of standard implants were included. The primary outcome was the survival rate of the zygomatic implants. In addition, random effects meta-analyses of the selected studies were applied to avoid potential bias caused by methodologic differences among studies., Results: Zygomatic implant survival rate weighted mean (WM) was 96.7% (range, 95.8% to 99.9%), with a 95% confidence interval (CI) of 92.5% to 98.5%. Only a limited number of surgical complications were reported, with orbital perforation the most significant. Similar results were obtained for prosthetic complications (few occurrences). Additionally, patient satisfaction levels were shown to be high, approaching that of the general population., Conclusion: Data from the present systematic review suggest that maxillary rehabilitation by four zygomatic implants with no anterior support is a reliable approach.

Purpose: The aim of this study was to conduct a systematic review and meta-analysis to evaluate the influence of cantilevers upon implant-supported fixed partial dentures on marginal bone loss (MBL) and prosthetic-related complications., Materials and Methods: An electronic literature search was conducted in the PubMed database by two reviewers (LTM and AM) for articles written in English from June 2003 to January 2013 that were prospective human clinical trials with the clear purpose of appraising the effect of implant-supported fixed partial prostheses on peri-implant bone level and prosthetic complications. Data from the selected studies were extracted to carry out the statistical analysis., Results: Following the method described earlier, from initial research of 643 studies, 4 human clinical studies met the inclusion criteria and provided enough data to include them in the present meta-analysis. For the overall data, the pooled weighted mean (WM) of the MBL was 0.72 mm (range, 0.49 to 1.10 mm), with a 95% confidence interval (CI) of 0.36 to 1.08 mm. For the chi-square test, P = .60, representing a low heterogeneity among studies. MBL around implant-supported restorations with and without cantilevers was not found to be significant between both groups. The weighted mean difference (WMD) was 0.10 mm (favoring the non-cantilever group), with a 95% CI = -0.18 to 0.39 mm (P = .47). For the chi-square test, P = .97, also indicating a low degree of heterogeneity between the studies., Conclusion: The dearth of scientific evidence in this matter does not permit clear conclusions to be drawn. However, within the limitations, marginal bone loss does not seem to be influenced by the presence of cantilever extensions. Moreover, minor technical complications were found when a cantilever was present when compared to the control groups.

Purpose: While the ability of various grafting materials to preserve extraction socket morphology has been adequately reviewed, the quality of the grafted bone in the socket is not as well understood. This systematic review aimed to compare the proportion of vital bone and connective tissue between grafted and naturally healed sockets., Materials and Methods: An electronic search of five databases (from 1965 to November 2011) and a hand search of peer-reviewed journals for relevant articles were performed. Human clinical trials that compared histologic components of soft and hard tissues in augmented sockets and naturally healed sites, with at least five samples per group, were included., Results: Eight studies (five randomized controlled trials and three controlled clinical trials) were included. The mean percentages of vital bone and connective tissue in natural healing sockets were 38.5% ± 13.4% and 58.3% ± 10.6%, respectively. Limited evidence (one to two articles for each material) implied that vital bone fraction was not different with demineralized allografts and autografts and increased by 6.2% to 23.5% with alloplasts in comparison to nongrafted sites. Four studies investigating the effect of xenografts were available, with equivocal results. The difference in the mean percentage of vital bone ranged from -22.2% (decrease) to 9.8% (increase). Connective tissue content decreased with the use of the aforementioned bone substitutes. Considerable residual hydroxyapatite and xenograft particles (15% to 36%) remained at a mean of 5.6 months after socket augmentation procedures., Conclusions: Based on a limited number of prospective comparative studies, the use of grafting materials for socket augmentation might change the proportion of vital bone in comparison to sockets allowed to heal without grafting. Whether these changes in bone quality will influence implant success and peri-implant tissue stability remains unknown.
