Introduction: Primary carcinosarcoma is a rare neoplasm of the gallbladder, and is characterized by collision elements of the adenocarcinomatous and sarcomatous components. The tumours comprise both malignant epithelial and mesenchymal elements, and are usually very aggressive, conferring poor prognosis. Adenocarcinomas are the most common neoplasms of the gall bladder, while carcinosarcoma are extremely rare, with a global prevalence rate of less than 1%. The low incidence of carcinosarcoma greatly hinders the understanding of its aetiology., Presentation of Case: Here, we report the surgical outcome of 3 rare cases of gallbladder carcinosarcomas, two of which had a IVB stage disease and in one case the staging was II. Following surgery, the survival in the former was less than 1 year, while in the latter, it extended up to 7 years (86 months), and the patient is alive till date. This study aimed to elucidate the surgical outcomes and prognosis after curable resection for carcinosarcomas of the gallbladder., Discussion: The findings suggest that carcinosarcomas are associated with a poor prognosis and short survival, particularly in those with tumours of >5 cm. In the absence of effective adjuvant treatment, surgery remains the mainstay of treatment., Conclusion: Complete surgical resection may improve prognosis and extend survival in patients with this rare and aggressive form of gallbladder cancer., (Copyright © 2019 The Author(s). Introduction: Ampullary adenocarcinoma is a rare entity during pregnancy. It accounts for 0.5% of all gastrointestinal malignancies. The best treatment modality for resectable tumors with the best reported outcome is surgical resection in the form of pancreaticoduodenectomy (Whipple procedure). In this case report, we discuss the challenges in the management of ampullary adenocarcinoma in a pregnant patient., Case Presentation: We report a case of 22 years old pregnant women who presented with vague abdominal pain and jaundice at the 28th week of gestation. Endoscopic retrograde cholangiopancreatography (ERCP) showed a stricture around the ampulla and distal common bile duct (CBD). The CBD was stented and a biopsy was taken. Histopathology revealed: an invasive adenocarcinoma. The patient was managed by elective pancreaticoduodenectomy after induction of vaginal delivery at the 34th week of gestation., Conclusion: Diagnosing and managing ampullary adenocarcinoma in pregnant patients in their 3rd trimester is challenging. Yet, a delayed viable delivery followed by a definitive surgery in the form of pancreaticoduodenectomy offers the best outcomes for both the mother & fetus in case of early stage disease., (Copyright © 2019 The Authors. Introduction: Primary squamous cell carcinoma of the gallbladder is extremely rare, and accounts for about 3% of all malignant gallbladder neoplasms., Presentation of Case: We report the case of a 58-year-old woman who presented with acute onset epigastric pain radiating to the back. The initial diagnosis, based on radiological images, was an incidental gallbladder mass with multiple gallstones. A staging laparoscopy was performed, followed by exploratory laparotomy with radical cholecystectomy. Segments 4b and 5 of the liver and the first part of the duodenum with the transverse colon were also resected. Histopathology of the gallbladder mass revealed invasive moderately differentiated squamous cell carcinoma with infiltration of liver segments 4b and 5, the first part of the duodenum, and two pericaval lymph nodes (with lymphovascular and perineural invasion). The primary tumour was scored as pT3, pN2, M1, stage IVB, based on the American Joint Committee on Cancer classification, version 7. The patient was discharged postoperatively and started adjuvant chemotherapy., Discussion: The best option for treating early-stage gallbladder cancer is radical surgery, while adjuvant chemo-radiation can also be beneficial. Our patient did not exhibit the typical symptoms of gallbladder cancer, and radiography was required for her diagnosis. Thus, additional work is needed to improve the detection of squamous cell carcinoma to improve the prognosis of patients like our own., Conclusion: Clinicians must be alert to the possibility of squamous cell gallbladder carcinoma, and gallbladder neoplasms should be among the possibilities considered during the differential diagnosis of symptoms related to the gallbladder., (Copyright © 2018 The Author(s). Introduction: Rapid weight loss following bariatric surgery is associated with high incidence of gallstones and complications that may need bilioenteric diversion. This presents a specific challenge in the management of this group of patients., Case Presentation: A 37 years old female underwent a Roux-en-Y gastric bypass (RYGB) in 2008 for morbid obesity. In 2009 she presented with obstructive jaundice and was diagnosed with choledocholithiasis successfully managed by open cholecystectomy and choledochoduodenostomy. In the following years, she developed recurrent attacks of fever, chills, jaundice, and right upper quadrant pain and her weight loss was not satisfactory. Imaging of the liver showed multiple cholangitic abscesses. Reflux at the choledochoduodenostomy site was suggestive of sump syndrome as a cause of her recurrent cholangitis and a definitive surgical treatment was indicated. Intraoperative findings confirmed sump at the choledochoduodenostomy site and also revealed the presence of a large superficial accessory duct arising from segment four of the liver with separate drainage into the duodenum distal to the choledochoduodenostomy site. A formal hepaticojejunostomy was done after ductoplasty. The Roux limb was created by transecting the jejunum 40cm distal to the foot anastomosis of the RYGB. The gastric limb was lengthened as part of this procedure which afforded the patient the additional benefit of weight loss., Conclusion: Choledochoduodenostomy should be avoided in patients with RYGB due to the risk of sump syndrome which requires conversion to a formal hepaticojejunostomy., (Copyright © 2016 The Authors.
