Das Kaposi-Sarkom (KS) ist eine seltene, maligne, von lymphatischen Endothelzellen ausgehende, multilokuläre Gefäßerkrankung, die vor allem Haut und Schleimhäute, aber auch das lymphatische System und innere Organe wie den Gastrointestinaltrakt, die Lunge oder die Leber befallen kann. Fünf epidemiologische Subtypen des KS mit variablem klinischem Verlauf und unterschiedlicher Prognose werden unterschieden, die in spezifischen Populationen vermehrt auftreten: (1) klassisches KS, (2) iatrogenes KS bei Immunsuppression, (3) endemisches (afrikanisches) lymphadenopathisches KS, (4) epidemisches, HIV-assoziiertes KS und mit einem Immunrekonstitutions-Inflammations-Syndrom (IRIS) assoziiertes KS und (5) KS bei Männern, die Sex mit Männern haben (MSM) ohne HIV-Infektion. Diese interdisziplinäre Leitlinie fasst aktuelle praxisrelevante Empfehlungen zu Diagnostik und Therapie der verschiedenen Formen des KS zusammen. Die in dieser Kurzleitlinie genannten Empfehlungen werden in der Langfassung der Leitlinie (Online-Version des JDDG) detaillierter ausgeführt.

Das Merkelzellkarzinom (MCC, ICD-O M8247/3) ist ein seltener, maligner, primärer Hauttumor mit epithelialer und neuroendokriner Differenzierung. Die Tumorzellen teilen viele morphologische, immunhistochemische und ultrastrukturelle Eigenschaften mit den Merkelzellen der Haut. Die Diagnose MCC wird selten klinisch gestellt. Erst histologisch und immunhistochemisch wird der Verdacht auf ein MCC bestätigt. Nachdem eine Fernmetastasierung durch Bildgebung mit Schnittbildverfahren ausgeschlossen worden ist, sollte aufgrund der hohen Zahl von okkulten Lymphknotenmetastasen eine Schildwächterlymphknotenbiopsie erfolgen. Bei Primärtumoren ohne Hinweise auf das Vorliegen von Organmetastasen soll eine vollständige chirurgische Exzision mit Sicherheitsabstand erfolgen. Die strahlentherapeutische Behandlung sollte in allen Stadien der Tumorerkrankung in Betracht gezogen werden. Für das fortgeschrittene, operative oder strahlentherapeutische nicht mehr kurativ behandelbare MCC gibt es bisher keine etablierte systemische Therapie, für die in einer prospektiv-randomisierten Studie eine Verbesserung des rezidivfreien Überlebens oder des Gesamtüberlebens demonstriert werden konnte. Eine Immuntherapie mittels PD-1/PD-L1-Blockade erscheint aber einer Chemotherapie überlegen zu sein. Aufgrund seiner Aggressivität, der in der Regel gegebenen Indikation zur Schildwächterlymphknotenbiopsie bei gleichzeitig häufiger Lokalisation im Kopf-Hals-Bereich, der zu evaluierenden Indikation einer adjuvanten Radiotherapie sowie der Komplexität der notwendigen Diagnostik zur korrekten Prognoseabschätzung soll das Vorgehen im Rahmen einer interdisziplinären Zusammenarbeit (interdisziplinäres Tumorboard) festgelegt werden.

The identification of targetable molecules in cellular signaling pathways represents a milestone in the treatment of melanoma. Selective inhibitors of these molecules, known as phosphokinases, allow for individual signaling pathways to be "switched off". This is of particular importance for tumors in which these pathways are constitutively activated by mutations in genes encoding said molecules. Especially patients with BRAF-mutated melanomas significantly benefit from kinase inhibitor therapies, with the current standard of combined BRAF and MEK inhibition providing very good long-term disease control. Such regimens have been shown to achieve a progression-free survival of more than ten months and an overall survival of more than two years, along with good quality of life. Given that the majority of patients develop secondary resistance during long-term kinase inhibitor therapy, current clinical trials are geared towards finding suitable drug combinations including inhibitors of other signaling pathways as well as immune checkpoint inhibitors. The present review highlights targeted therapies for melanoma currently available as well as potential future options presently under clinical investigation.

Die Behandlungsstrategie beim metastasierten Melanom hat sich mit der Identifizierung therapeutisch angreifbarer molekularer Zielstrukturen innerhalb zellularer Signalwege radikal geandert. Durch die Zulassung von Substanzen, die gezielt an den zentralen Schaltmolekulen, den Phosphokinasen, angreifen, konnen diese Signalwege selektiv abgeschaltet werden. Dies ist insbesondere bei denjenigen Tumoren von Interesse, deren Signalwege durch aktivierende Mutationen der fur die Schaltmolekule kodierenden Gene konstitutiv aktiviert sind. Aktuell ist diese therapeutische Strategie insbesondere fur Patienten bedeutsam, deren Melanome eine Mutation im BRAF-Gen aufweisen. Diese Patienten konnen durch eine Kombinationstherapie aus Inhibitoren der Phosphokinasen BRAF und MEK langfristig mit sehr guter Krankheitskontrolle behandelt werden. Unter dieser Kombinationstherapie wird aktuell ein progressionsfreies Uberleben von uber zehn Monaten und ein Gesamtuberleben von mehr als zwei Jahren bei guter Lebensqualitat erzielt. Da unter langerfristiger Therapie mit Kinaseinhibitoren jedoch bei einem Grosteil der Patienten eine Resistenzbildung auftritt, sind aktuelle klinische Therapiestudien auf die Suche nach geeigneten Kombinationspartnern unter Blockierung anderer Signalwege oder unter Aktivierung der T-Zell-vermittelten Immunantwort ausgerichtet. Der vorliegende Ubersichtsartikel stellt sowohl die aktuell verfugbaren als auch die in der klinischen Testung befindlichen zukunftigen Optionen der zielgerichteten Therapie des Melanoms dar.

Summary Background: Systemic treatment with pegylated liposomal doxorubicin is an established second-line treatment of advanced cutaneous T-cell lymphoma. Pegylated liposomal doxorubicin (PLD) is currently unavailable, therefore, clinical studies investigating the efficacy of non-pegylated liposomal formula (NPLD) have been analyzed. Methods: Since clinical trials comparing PLD and NPLD in CTCL do not exist, the clinical use of NPLD including safety and efficiency profile in other types of non-Hodgkin lymphoma were analyzed. Results: Clinical trials show a comparable efficacy of NPLD and PLD in non-Hodgkin lymphoma. The dosage of NPLD used in the treatment of systemic lymphoma within polychemotherapy regimens was 50 mg/m2 every three weeks. Overall response was 75–95%, including a complete remission rate of 65–80% and 2- and 3-year overall survival rates of 55–75%. These data indicate that the non-pegylated formula of doxorubicin has a similar antitumor effect as the pegylated one but shows reduced cardiotoxicity. The palmoplantar erythrodysesthesia frequently observed in PLD has not been observed with the use of the NPLD. Conclusions: The clinical use of NPLD in the treatment of CTCL is reasonable. In analogy to the clinical trials of NPLD in non-Hodgkin lymphoma a dosage of 50 mg/m2 every three weeks is recommended for the treatment of CTCL.

Summary Background: Primary cutaneous diffuse large B-cell lymphoma (PCLBCL) represents a rare subtype among primary cutaneous B-cell lymphoma exhibiting a characteristic genetic background, an aggressive clinical course and a high relapse rate under different therapeutic regimen. Therefore, PCLBCL has a rather restricted prognosis. Patients and methods: Four patients with PCLBCL were treated at our institution with age- and toxicity-adapted first-line immunochemotherapy with rituximab and modified CHOP (cyclophosphamid, vincristin, liposomal doxorubicin, prednisolon). On relapse, the same regimen with R-CHOP or different antineoplastic strategies (radiation, polychemotherapy, immunotherapy, stem cell transplantation) were applied. Toxicity, clinical response and overall survival was documented. Results: Under this regimen, clinical response to modified R-CHOP was achieved in all patients with tolerable toxicity – however, being characterized by a rapid disease progression with inconsistent response towards the subsequent therapeutic armentarium and unsecure impact on overall survival. Conclusions: So far, it is still unknown, if an extensive multimodal therapy for PBLBCL improves overall survival. Immunochemotherapy with R-CHOP currently represents the most effective treatment.

Summary Inhibitors of the epidermal growth factor receptor (EGFR) are increasingly usedin the treatment of various entities of malignant tumors. Patients treated withEGFR inhibitors very likely develop cutaneous side effects. The development of a papulopustular, follicular exanthema during the first weeks of therapy correlates with therapeutic benefit. However, this exanthema and other cuta-neous side effects can impair the quality of life of the patient and might limitthe therapy with the EGFR inhibitor.For an optimal therapeutic benefit and quality of life an adequate managementof cutaneous side effects is necessary. Summary Immunotherapy has assumed increasing importance in the therapy of malignant melanoma. The main reason is the high immunogenicity of the tumor itself, so that an immune response against the tumor often exists even without immune stimulation. The goal of modern immunotherapeutic approaches is to augment these anti-tumoral immune reactions to fight the tumor. Despite multiple successes, the ultimate breakthrough in the therapy of malignant melanoma has not yet been achieved. This overview summarizes the reasons for this lack of success and highlights future strategies for more successful therapy of malignant melanoma. Zusammenfassung Immuntherapien haben in der Therapie des malignen Melanoms in den letzten Jahren zunehmend an Bedeutung gewonnen. Der Grund hierfur liegt vor allem in der hohen Immunogenitat des Tumors selber, der auch ohne Immunstimulation bereits Immunantworten gegen Melanomzellen erkennen lasst. Ziel der immunologischen Therapieansatze war es daher, diese korpereigene Immunantwort zu verstarken und dadurch den Tumor zu bekampfen. Trotz zahlreicher Erfolge ist der grose Durchbruch in der Behandlung vor allem des metastasierten Melanoms bislang ausgeblieben. Dieser Artikel fasst die Ursachen hierfur zusammen und erlautert neue Strategien zur erfolgreicheren Therapie des Melanoms.

Summary Background: Scleromyxoedema is a rare disease of unknown aetiology that is characterized by deposition of mucin and sclerotic induration of the skin; it is associated with paraproteinaemia. Patients suffer from progressive disability due to immobilization and cosmetic disfigurement. Treatment of scleromyxoedema is a therapeutic challenge. The antimalarial hydroxychloroquine has a rapid and reliable effect in reticular erythematous mucinosis. Patients and methods: Four consecutive patients (two women, two men; median age: 50 years) with scleromyxoedema, three of them with IgG λ paraprotein, were treated with hydroxychloroquine. Treatment was initiated with 600 mg p. o. for 10 days, followed by 400 mg for at least 4 weeks, and 200 mg thereafter. Results: Complete remission of skin manifestations was achieved in one patient, whereas three patients achieved a partial remission of 61+, 5 and 25 months' duration. Notably, three patients felt increased mobility and reduced firmness of skin during the first week of treatment, which was reflected in a rapid reduction in dermal thickness. In one patient, dysphagia was reverted as evidenced by normalization of oesophageal clearance. Paraproteinaemia was not influenced at all. Side effects included one case of electroretinogram abnormalities after 19 months of therapy and one case of leucopenia after 3 months. Conclusion: Hydroxychloroquine is an effective form of therapy for scleromyxoedema, leading to rapid and prolonged alleviation of symptoms. Zusammenfassung Hintergrund: Das Skleromyxodem ist eine seltene Erkrankung unbekannter Atiologie, die durch eine Muzineinlagerung und Sklerosierung der Haut charakterisiert ist; haufig ist eine Paraproteinamie assoziiert. Die Patienten leiden unter zunehmender Versteifung und kosmetischer Beeintrachtigung. Das Skleromyxodem stellt eine therapeutische Herausforderung dar. Das Antimalaria-Mittel Hydroxychloroquin hat einen schnellen und zuverlassigen Effekt bei der retikularen erythematosen Muzinose. Patienten und Methodik: Vier konsekutive Patienten (zwei Frauen, zwei Manner, medianes Alter 50 Jahre) mit Skleromyxodem, das in drei Fallen mit einer IgG-λ-Paraproteinamie assoziiert war, wurden mit Hydroxychloroquin behandelt. Die Initialdosis war 600 mg per os fur 10 Tage, in den folgenden vier Wochen 400 mg und danach 200 mg taglich. Ergebnisse: Ein Patient kam in komplette Remission, die anderen drei Patienten zeigten eine partielle Remission von 61+, 5 und 25 Monaten Dauer. Bemerkenswerterweise trat bei drei Patienten eine subjektiv bessere Mobilitat und verminderte Verhartung der Haut schon in der ersten Behandlungswoche auf, die durch eine schnelle Reduktion der Dermisdicke objektiviert wurde. Bei einem Patienten besserte sich eine Dysphagie parallel zu einer Normalisierung der oesophagealen Clearance. Die Paraproteinamie wurde nicht beeinflusst. Als Nebenwirkungen traten bei einer Patientin nach 19 Monaten Therapie Elektroretinogramm-Veranderungen auf, bei einer Patientin wurde eine Leukopenie nach drei Monaten beobachtet. Schlusfolgerung: Hydroxychloroquin stellt eine effektive Behandlungsmoglichkeit des Skleromyxodems dar, die zu schneller und dauerhafter Symptomlinderung fuhrt.
