The Ebola virus disease (EVD) outbreak, which was declared as such in West Africa in March 2014, has become the largest EVD epidemic to date. It is the first time that EVD has been responsible for cases imported to the US as well as locally-acquired cases in Europe. Research on pathophysiology and treatment has been considerably accelerated, and more precise descriptions of various forms of ocular involvement have been obtained. Conjunctival hyperemia is often present during the acute phase, and it may contribute to the diagnosis of EVD in an epidemic context. During convalescence, ocular inflammatory manifestations may develop and can be caused by viral persistence in ocular tissue. Eye care providers need to be aware of the ophthalmic manifestations of EVD, sometimes several weeks after the acute infection, in order to recognize them and take appropriate safety precautions., (Copyright © 2015 Elsevier Masson SAS. Purpose: To determine the prevalence of high intraocular pressure (HIOP) in 103 patients., Methods: One hundred and three consecutive patients referred to our department for uveitis were included. Files were retrospectively analyzed for age at time of presentation, gender, type of uveitis (as defined by International Uveitis Study Group) and etiology, time of HIOP (primary or secondary to treatment), and associated filtering surgical procedure. Intraocular pressure (IOP) was measured using Goldmann applanation tonometry and HIOP was defined as intraocular pressure 21 mm Hg or higher., Results: Among these patients, 36% of uveitis cases were related to a concomitant infectious disease (herpetic disease in 20% of cases and herpes zoster in 4%). No significant difference in age or gender was found between groups. At least one episode of HIOP was found during the acute phase of the disease in 27 patients (26.2%): anterior uveitis accounted for 74% of patients. Most hypertensive cases were associated with herpetic disease (37%), whereas steroid-induced HIOP was suspected in only two patients (7.4%). HIOP was controlled by topical hypotensive treatment in 74.1% of patients, and a filtering surgical procedure was found necessary in seven patients (25.9%)., Conclusion: This retrospective study confirms that HIOP is a major complication of uveitis, especially in those involving the anterior chamber of the eye and/or related to viruses. Purpose: To investigate the possible association between cytomegalovirus (CMV) infection and Posner-Schlossman syndrome using PCR. To evaluate the clinical effect of valganciclovir treatment., Patients and Methods: Patients with anterior uveitis associated with suspected Posner-Schlossman syndrome had their aqueous humor prospectively analyzed for viral deoxyribonucleic acid by HXFL4 gene detection using quantitative real-time PCR., Results: Seven patients (two females and five males from 15 to 56 years old) with clinical signs of Posner-Schlossman syndrome (anterior nongranulomatous hypertensive uveitis with anterior chamber cells, central keratic precipitates, anisocoria, without iris atrophy) were tested. Examinations of the aqueous humor by polymerase chain reaction demonstrated CMV-DNA in five patients and were negative for other herpes viruses in all patients. Two patients were treated with oral antiviral therapy (valganciclovir). Relapses occurred in all cases after cessation of therapy., Discussion: Aqueous humor analysis by PCR is useful in differentiating between CMV and other herpes viruses and in making a positive etiological diagnosis in anterior hypertensive uveitis. Valganciclovir may be effective in treating CMV anterior uveitis, but its exact role should be determined in larger studies with a longer follow-up., Conclusion: Cytomegalovirus detection in aqueous humor using polymerase chain reaction is useful in the management of patients with Posner-Schlossman syndrome., (Copyright © 2010 Elsevier Masson SAS. Human herpesvirus 6 is a ubiquitous Herpesviridae infecting patients during childhood. Its role in ocular disorders is mostly unknown. We report the case of a 47-year-old woman with a history of rheumatoid arthritis (treated with etanercept) and tuberculosis, who presented with sudden unilateral panuveitis. The patient was initially treated with ganciclovir, as the polymerase chain reaction in the aqueous humor was positive for HHV6, and with pyrimethamine and sulfadiazine because a toxoplasmic co-infection was highly suspected, which was biologically confirmed. Management of HHV6 in a nervous system disorder is challenging because its replication has been proved but its role remains unclear. Viral eye diseases are common and associated with different well-known forms of uveitis. However, experimental models and clinical observations have led to an infectious, in particular a viral etiology in different autoimmune conditions. The use of molecular techniques is particularly informative, not only to characterize the previously well-known subgroup of presumed viral uveitis, but also to define the role of these agents or emerging viruses in atypical forms of autoimmune uveitis resistant to conventional therapy. PCR detection of viral DNA in patients with uveitis is a rapid, sensitive and accurate procedure. Therefore, aqueous humor could be analyzed when uveitis is unresponsive to anti-inflammatory molecules, in order to exclude a viral condition and dramatically modify the therapeutic management. Several new viral entities have recently been identified such as cytomegalovirus-associated chronic anterior uveitis and non-necrotizing herpetic retinopathies in immunocompetent hosts. Systemic antiviral drugs should be proposed rapidly in order to control viral replication before the use of corticosteroids. Maintenance therapy based on low-dose antivirals can reduce the rate of recurrence and should be considered.

Introduction: This study aimed to describe the clinical characteristics of uveitis presenting de novo in the elderly. The study design was a description of a retrospectively identified case series., Patients: The records of 193 patients with uveitis referred to Bicêtre Hospital's department of ophthalmology between January 1995 and January 2000 were reviewed. Among these patients, the records of 57 patients with uveitis de novo beginning after age 60 were analyzed., Results: Idiopathic uveitis accounted for the majority of cases. Whereas herpes viruses were the most frequent specific diagnosis, presumed sarcoidosis and birdshot choroidopathy were also identified as diagnostic entities of uveitis presenting for the first time in the elderly. Only three cases of masquerade syndrome were identified, two cases of intraocular lymphoma, and one metastasis of a visceral melanoma., Conclusion: Masquerade syndromes are not the leading cause of uveitis in the elderly. Idiopathic uveitis and herpes viruses are the most common etiology found.

Purpose: Evaluation of the frequency of each aetiology, comparison with recent and previous publications, focus on less classical pathologies, and interest of some complementary investigations., Methods: Prospective study of one hundred and ten cases of uveitis, submitted to aetiological investigations, including, when feasible or necessary, an anterior chamber paracentesis and a vitreous paracentesis., Results: An aetiology was identified in 65% of the cases. Toxoplasmosis still remained the most frequent diagnosis (one out of four), followed by viral infection (15%), especially herpes simplex (9%), a notion as yet not mentioned in large statistical surveys, including recent ones. Tuberculosis allergy and rhumatismal diseases (ankylosing spondylitis) represent the two other large groups (5% each). The rest is divided among the other classical causes. These results, particularly toxoplasmosis and viruses, are always based on biological criteria, on the Witmer-Desmonts ratio in aqueous humour, or even the same ratio in vitreous., Conclusion: This study confirms the predominant role of anterior chamber paracentesis, the only means of establishing a firm diagnosis for certain categories of uveitis (toxoplasmosis and viruses); it is difficult to conceive how such an investigation can be questioned, to the benefit of a purely biomicroscopic diagnosis, leading to blatant errors, as demonstrated by the results of anterior chamber paracentesis. This study also shows, due to this paracentesis, the increasing role of viruses in the aetiology of uveitis, emphasizing the risks taken when any uveitis is systematically submitted to corticotherapy.
