Backache is a significant source of disability and suffering in our society. The treatment modalities need continued enhancement in order to achieve the desired goals of lowering morbidity and financial losses while improving the response of the patient.This prospective comparative study was conducted at the department of Orthopaedics and Spine Surgery, Khyber Teaching Hospital Peshawar from July 2013 to June 2015. Two interventional groups were designated; Group 1 was comprised of 54 patients who were injected with epidural bupivacaine plus methylprednisolone while Group 2 included 55 patients who were injected with bupivacaine only. Outcome was assessed using the visual analogue scale and Oswestry disability index (ODI).Fifty-five female and 54 male patients with mean age 49.37 years±10.46 SD, Mean symptoms duration was 15.01 months±9.32 SD. Common presenting symptoms were backache (77.1%), lower limbs pain (66.1%), dermatomal paresthesias (54.1%) and neurogenic claudication in 57.8% patients. The mean visual analogue score (VAS) after injection was 3.18±1.29 while mean ODI after injection was 23.615. There was a statistically significant reduction in VAS scores (2-sided p=0.003, OR =4.03, 95% CI: 1.535-10.60) following the injection.An epidural spinal injection is a viable option for achieving relief of painimproves functioning in individuals with radicular backache. However, further research is advised in order to clarify the role of ESI for long-term relief.

Supracondylar fracture of humerus is the second most common fracture in children which account for 60-75% of all fractures around the elbow. There are various treatment modalities for type-III fracture, i.e., closed reduction and casting, skeletal traction, close reduction and percutaneous pinning and open reduction and internal fixation. This study was conducted to see the outcome of open reduction and internal fixation after failed closed reduction.This study was conducted in the Orthopaedics Departments of Khyber Teaching Hospital Peshawar and Ayub Teaching Hospital Abbottabad from February 2007 to Nov 2007 on 30 children. Patients included were of either gender with age range from 5-12 years with displaced supracondylar fracture (type-III) after failed closed reduction. All fractures were fixed with two cross K-wires by open reduction and internal fixation. The patients were assessed both clinically and radiologically and results were tabulated according to Flynn criteria.Twenty-eight patients had excellent results while two had good results according to Flynn criteria. None of the patients had either fair or poor result.Open reduction and internal fixation is a good and reliable method after failed closed reduction and gives stable fixation with anatomical alignment.

Many hospitals in our country lack surgical expertise and operating room facilities like image intensifier and fractures table to carry out closed interlocking nails (ILN) in femoral shaft fractures. But availability of Surgical Implant Generation Network (SIGN) interlocking nailsnailing instrumentation have made open ILN of these fractures possible at very basic level of our health care system. We have carried out open SIGN nailing in patients with closed fractures of shaft femur without the use of image intensifier. Result for fracture union was evaluated both clinically and radio-logically, and graded at 8 months (32 weeks) after treatment by Thoressen's criteria.An experimental study of open SIGN nailing was carried out on 47 patients with fractures shaft of femur who had been admitted to our tertiary care hospital from January 2006 to December 2007. Inclusion criteria were adult patients older than 16 years with closed fractures of the shaft femur, and have presented within a week of the injury, and have not had any previous surgical treatment for the fracture. Malnourished patients and patients with open, pathological fractures and non union cases were excluded from the study. A standard protocol was followed on all patients, which is describing below. The data obtained was analysed using SPSS.The union rate was 97.83% in open nailing at 32 weeks after surgery and the Mean +/- SD time to union was 19.65 +/- 5.19 weeks (ranges from 16-32 weeks).We obtained excellent results in 39 patients (83.33%), good in 4 patients (8.50%), fair in 3 patients (6.38%) and poor in one (2.12%).The open SIGN nailing, without the use of image intensifier, for treatment of closed fractures of shaft femur achieves excellent result in term of fracture union. Results obtained are comparable to the results of closed interlocking nailing, requires less expertise and resources, and its use is recommended for long bones fracture care at the very level of our health care system.

Bone defect in long bones is a problem in orthopaedic which needs proper treatment to restore limb length and function of the limb. There are various treatment modalities available to fill the detect like bone grafting, vascularised bone graft, allograft and bone transport through fixators (Ring or Uniplanner). This study was done to determine the outcome of locally manufactured External Fixator (Naseer and Awais ) in the management of tibial bone defect due to trauma.This descriptive study was conducted on 20 patients in orthopedic department in Lady Reading Hospital Peshawar and Ayub Teaching Hospital Abbottabad, from Nov 1997 to Nov 2002. Patients from either gender aged between 20 to 50 years, having tibial bone defect of more than five centimeter due to trauma were included in the study. Follow up was done for two years. Outcome measures were adopted according to the classification of the Association for the Study and Application of the Method of llizarov (ASAMI) based on clinical (functional) and radiological findings (defect filling).Out of 20 patients 14 (70%) were males and 6 (30%) were female patients. Right tibia was involved in 12 patients (60%) and left tibia in 8 patients (40%). Bony defects due to firearm injury were in 14 patients (70%) and road traffic accidents were in 6 patients (30%). Radiological (bony) results were excellent in 12 patients (60%), good in 2 patients (10%), fair in 3 patients (15%) and poor in 3 patients (15%). Clinical (functional) results were excellent in 7 patients (35%), good in 8 patients (40%), fair in 4 patients (20%,) and poor in 1 patients (5%,). Post operative complications after distraction osteogenesis were pin tract infections in 8 patients (40%), Axial deviations in 6 patients (30%), foot equinus in 4 patients (20%) and refracture in 1 patient (5%).Uniplanner locally manufactured (NASEERAWAIS) External Fixator for distraction osteogenesis is cheap, easy to apply and gives excellent and comparable results in tibial bone defects.

Humeral diaphyseal fracture usually heals with closed methods but when non union develops then it needs surgical intervention in the form of plating and bone grafting, intramedulary nailing (open or closed simple or interlocking nails) and external fixators (circular or one plane fixator). In our unit we treated non union humeral diaphyseal fracture with plating and bone grafting and shortening of fracture ends up to 4 to 5 cm when needed.This study was conducted at Orthopaedic Department of Ayub Teaching Hospital Abbottabad from January 2002 till December 2003. We included 15 cases with atrophic non-union in 9 (60%) and hypertrophic non-union in 6 (40%) patients. All atrophic non-union were treated with plating, bone shortening by transverse osteotomy and bone grafting, while hypertrophic non-union were treated with decortications of non-union ends and fixation with compression plates, with bone grafting in old age. Follow up measures were based on clinical (range of joints motion) and radiological (healing) findings. Follow up was done for up to 6 months.Out of 15 patients the age range was 20-80 years, 12 (80%) were male and 03 (20%) female. Right humerus involved in 5 (33.33%) while left humerus in 10 (66.66%) patients. In 9 (60%) patients with atrophic non union bone shortening by transverse cut osteotomy was done while in remaining patients with hypertrophic non-union plating was done in 2 (13.33%) cases and plating with bone grafting in 4 (26.66%) patients. Union was achieved in all patients after 16 to 20 weeks of surgery. In one patient (6.66%) of 75 years age with hypertrophic non-union implant was loosened after 03 months of surgery. At that time healing (Union) was evident on X-rays and humeral brace was applied for further 03 months. Two patients (13.33%) got neuropraxia of radial nerve which resolved with in 3 months time. 02 patients (13.33%) developed shoulder stiffness which resolved after exercise.In Non Union of Humerus shortening by transverse osteotomyrigid fixation with plates give excellent results in selected cases.
