A 35-year-old diabetic man was referred for nonresolving chronic cough of 3 months duration. Computed tomography scan revealed linear opacities in right upper lobe. A flexible bronchoscopy demonstrated a mass in the right upper lobe bronchus. An endobronchial biopsy of the lesion revealed granulation tissue and an actinomycotic infection. The patient did not respond to treatment for actinomycosis. He was subjected to thoracoscopic surgery and a small nonorganic foreign body covered in thick coat of actinomycosis was removed. The patient then responded to penicillin treatment. We report a case of nonorganic foreign body-induced endobronchial actinomycosis secondary to pen end cap aspirated during childhood.

Tracheobronchial foreign body aspiration is a major cause of morbidity and mortality. The incidence of foreign body aspiration decreases significantly with increasing age, resulting in atypical presentations in adults. A high index of suspicion is required in adults presenting with respiratory symptoms. We present a rare case of a foreign body (pill) aspiration in an elderly female mimicking an obstructing endobronchial lesion.

Foreign body (FB) aspiration is a relatively common problem in children, particularly during the first 3 years of life. It is an emergency condition and the removal of the FB by bronchoscopy is the primary treatment. Children with undiagnosed retained foreign bodies may present with respiratory symptoms including recurrent or persistent wheezing, with or without respiratory failure. Spontaneous expectoration of a FB is a rare occurrence. Herein, we present a case that was diagnosed with FB aspiration during investigation for persistent wheezing and who expectorated part of a sunflower seed 2 months after aspiration.

Aspiration of ferrous sulfate tablets is a rare and potentially serious condition that can lead to permanent airway stenosis. Diagnosis may be difficult, as presentation often includes nonspecific symptoms. Disease progression and treatment courses have been detailed in a limited number of publications. Herein, we report a case of severe bronchial stenosis that developed following aspiration of a ferrous sulfate tablet. To the best of our knowledge, this is the first reported attempt of laser fulguration to correct ferrous sulfate-induced bronchial stenosis.
