Purpose: The purpose of the study was to evaluate joint kinematics and tendon work of flexion (WOF) following a flexor digitorum profundus (FDP)-to-volar plate (VP) repair technique relative to a pullout button for zone I flexor tendon injuries., Methods: Fourteen digits were tested using an in vitro active finger motion simulator under 3 repaired conditions following a simulated zone I avulsion: button, FDP-VP, and "no slack" FDP-VP (corrected for additional VP length). Outcome metrics included active joint range of motion (ROM), fingertip strength, FDP and flexor digitorum superficialis tensile loads, and WOF., Results: The button and FDP-VP techniques restored WOF to the intact condition for FDP and flexor digitorum superficialis. All repairs restored distal interphalangeal joint ROM and kinematics to the intact condition. Similarly, all repairs restored WOF; however, the "no slack" FDP-VP significantly increased WOF by 10% to 12% over the simple FDP-VP repair. The button technique had similar fingertip strength to the intact condition, whereas the FDP-VP repairs significantly reduced peak fingertip strength from intact, albeit only 1-2 N compared with the button repair., Conclusion: In this in vitro cadaveric model, the button and FDP-VP techniques restored WOF and ROM to within intact levels, with no difference between these repairs in all measured outcome metrics., Clinical Relevance: Based on its initial strength and its equal biomechanical performance compared with the button repair, the FDP-VP technique may be a viable option for treating FDP avulsions., (Copyright © 2024 American Society for Surgery of the Hand. Purpose: Olecranon osteotomy is commonly used to obtain access to the distal humerus for fracture fixation. The goal of this study was to accurately describe the anatomy of the bare area to minimize articular cartilage damage while performing olecranon osteotomies., Methods: Twenty cadaveric ulnae were denuded to expose the bare area. Laser surface mapping was used to create 3-dimensional models, and the nonarticular portions of the ulnae were digitally measured., Results: The morphology of the bare area from all aspects of the proximal ulna was defined. The central bare area was consistent in its location, 4.9 ± 1.5 mm distal to the deepest portion of the trochlear notch and 23.2 ± 2.3 mm distal to the olecranon tip. The maximum chevron osteotomy apical angle to stay within the bare area averaged 110° ± 11.8°. However, there was little tolerance for error without the risk of violating the articular cartilage. With transverse osteotomy, averaging 18° ± 10.6° in the coronal plane, there is less risk of damaging the articular cartilage., Conclusions: Transverse osteotomy perpendicular to the posterior surface of the ulna aiming at the visible bare area on the medial and lateral sides of the greater sigmoid notch may reduce the chances of violating the nonvisible articular cartilage of the proximal ulna. Based on the findings of this study, if chevron osteotomy is used, a shallow apex distal angle of more than 110° is recommended., Clinical Relevance: This study provides intraoperative landmarks to guide surgeons performing olecranon osteotomies to stay within the bare area., (Copyright © 2024 American Society for Surgery of the Hand. Purpose: To compare in vitro failure loads of nerve coaptations using fibrin glue alone, a suture alone, and a combination of fibrin glue and a suture., Methods: The median, radial, and ulnar nerves of 15 fresh-frozen cadaveric upper extremity specimens (45 nerves in total) were dissected in vitro and transected 5 cm proximal to the wrist crease to simulate an injury requiring coaptation. Three coaptation techniques were used: fibrin glue alone, a suture alone, and a suture augmented with fibrin glue. The load to failure of each repair was measured using a linear servo-actuator with an in-line force sensor. The results were analyzed using 2-way repeated measures analysis of variance tests and pairwise comparisons with Bonferroni correction., Results: Both the nerve coaptation technique and the specific nerve that was repaired had a significant effect on failure load. Suture-glue repair had the highest load to failure, 11.2 ± 2.9 N, and significantly increased the load to failure by 2.9 ± 1.7 N compared with glue repair alone. There was no significant difference between suture-glue repair and suture repair alone or between glue repair alone and suture repair alone., Conclusions: In this in vitro cadaveric model, nerve injury coaptation using both a suture and fibrin glue resulted in the strongest repair. The addition of fibrin glue may provide some benefit when used to augment suture repair, but when used in isolation, it is inferior to combined suture-and-glue constructs., Clinical Relevance: Combined suture-and-glue nerve coaptations might be useful in the early postoperative period in increasing nerve repair strength and potentially reducing rupture rates., (Copyright © 2023. Purpose: The motor branch of the ulnar nerve contains fascicles that innervate the intrinsic musculature of the hand. This cadaveric study aimed to describe the organization and consistency of the internal topography of the motor branch of the ulnar nerve., Methods: Five fresh-frozen cadaveric specimens with an average age of 74 years (range, 65-88 years) were dissected. The ulnar nerve was exposed and transfixed to the underlying tissues to maintain its orientation throughout the dissection. The dorsal cutaneous branch (DCB) and the volar sensory branch were identified and reflected to expose the motor branch. The fascicles to the first dorsal interosseus (FDI), flexor pollicis brevis, and abductor digiti minimi (ADM) were identified. Internal neurolysis was performed distal to proximal to identify the interfascicular arrangement of these fascicles within the motor branch. The organization of these fascicles was noted, and the branch points of the DCB, FDI, and ADM were measured relative to the pisiform using a handheld electronic caliper., Results: The internal topography of the motor branch was consistent among all specimens. Proximal to the pisiform, the arrangement from radial to ulnar was as follows: volar sensory branch, flexor pollicis brevis, FDI/intrinsic muscles, ADM, and DCB. The position of these branches remained consistent as the deep motor branch curved radially within the palm and traveled to the terminal musculature. The locations of the average branch points of the FDI, ADM, and DCB with respect to the pisiform were as follows: FDI, 4.6 cm distal (range, 4.1-4.9 cm), 4.5 cm radial (range, 4.1-4.9 cm); ADM, 0.65 cm distal (range, 0.3-1.1 cm), 0.7 cm radial (range, 0.3-1.1 cm), DCB, 7.7 cm proximal (range, 4.2-10.1 cm), and 0.4 cm ulnar (range, 0.3-0.8 cm)., Conclusions: The internal topography of the ulnar nerve motor branch was consistent among the specimens studied. The topography of the motor branches was maintained as the motor branch turns radially within the palm., Clinical Relevance: This study provides further understanding of the internal topography of the ulnar nerve motor branch at the wrist level., (Copyright © 2023 American Society for Surgery of the Hand. Purpose: The purpose of this study was to investigate the impact of repairing a zone II flexor digitorum profundus (FDP) laceration anatomically versus extra-anatomically on tendon loads and work of flexion (WOF)., Methods: Twenty digits from 5 cadaveric specimens were tested using an in vitro active finger motion simulator under 2 FDP tendon repair conditions: anatomic and extra-anatomic. Tensile loads in FDP and flexor digitorum superficialis (FDS), WOF, and total active finger range of motion (ROM) were measured using in-line load cells and electromagnetic tracking, respectively., Results: The anatomic repairs had no effect on tendon loads or WOF for either FDP or FDS. The extra-anatomic repairs increased FDP loads by 32% and decreased FDS loads by 9% compared with those in the intact condition. This pattern was similar for WOF following extra-anatomic repairs, which increased FDP WOF by 31% and decreased FDS WOF by 18%. Comparing the 2 repairs, FDP loads and WOF were 25% and 22% greater, respectively, with extra-anatomic repairs compared with anatomic repairs, with no significant change in FDS. Total active ROM was not affected by either repair., Conclusions: In this in vitro cadaveric model, extra-anatomic repairs of FDP increased tendon loads and WOF compared with anatomic repairs., Clinical Relevance: On the basis of this study, reconstitution of the anatomic relationship of FDP and FDS at the Camper chiasm during the repair of zone II flexor tendon lacerations is recommended., (Copyright © 2023 American Society for Surgery of the Hand. Purpose: Normal digital flexion relies on flexor tendon pulleys to convert linear muscular force to angular digital motion. However, there is a growing trend to vent them partially during flexor tendon repair. The objective of this study was to examine the effects of a thermoplastic ring, acting as an external pulley, on flexor tendon biomechanics and finger range of motion (ROM) after pulley venting., Methods: We tested 15 cadaveric digits using an in vitro active finger motion simulator. We measured loads induced by flexor digitorum superficialis (FDS) and flexor digitorum profundus (FDP) as well as joint ROM with sequential sectioning of the A2, A3, and A4 pulleys compared with an intact pulley condition. At each stage, external thermoplastic pulley rings were applied snugly over the proximal and middle phalanges to recreate A2 and A4 function, respectively., Results: After complete venting of the A2, A3, and A4 pulleys, proximal interphalangeal joint ROM significantly decreased by 13.4° ± 2.7° and distal interphalangeal joint ROM decreased by 15.8° ± 2.1°. Application of external rings over the proximal and middle phalanx resulted in a residual ROM decrease of 8.3° ± 1.9° at the proximal interphalangeal joint and 7.9° ± 2.1° at the distal interphalangeal joint, nearly restoring ROM. Similarly, complete pulley venting resulted in reduced FDS load by 37% and FDP load by 50% compared with intact pulleys. After application of external rings, loads were restored almost to normal, with a 9% reduction for FDS load and 9% reduction for FDP load compared with intact pulleys., Conclusions: The application of thermoplastic rings acting as external pulleys is an effective, noninvasive, and reproducible approach to restore flexor tendon biomechanics and digit ROM after pulley venting., Clinical Relevance: Thermoplastic rings may be a useful therapeutic adjunct in restoring joint ROM and flexor tendon loads after surgical venting of the pulleys., (Copyright © 2021 American Society for Surgery of the Hand. Purpose: To examine the effect of volar angulation deformities of the distal radius with and without triangular fibrocartilage complex (TFCC) rupture on forearm range of motion and the kinematics of the ulnar head at the distal radioulnar joint (DRUJ) during simulated active forearm rotation., Methods: Volar angulation deformities of the distal radius with 10° and 20° angulation from the native orientation were created in 8 cadaveric specimens using an adjustable apparatus. Active supination and pronation were performed using a forearm motion simulator. Pronation and supination range of motion was quantified with each deformity. In addition, changes in the dorsovolar position of the ulnar head relative to the radius were calculated after simulating each distal radial deformity. Testing was performed with the TFCC intact and sectioned., Results: Volar angulation deformities of 20° decreased the supination range with preservation of pronation. There was no effect of TFCC status on the range of forearm rotation. With the TFCC intact, volar angulation deformities translated the ulna slightly dorsally in pronation and volarly in supination. After sectioning the TFCC, volar angulation deformities of 10° and 20° translated the ulna dorsally throughout forearm rotation., Conclusions: Volar angulation deformities reduce supination range and alter the DRUJ kinematics. The increased tension in the intact TFCC caused by volar angulation deformities likely prevented the expected dorsovolar displacement at the DRUJ and restricted supination. Dividing the TFCC released the constraining effect on the DRUJ and allowed the ulna to translate dorsally. However, supination remained limited, presumably because of impediment from the dorsally subluxated ulna., Clinical Relevance: This study demonstrated the importance of correcting volar angulation deformities of the distal radius to less than 20° in order to maintain normal range of forearm rotation and to less than 10° to maintain normal DRUJ kinematics when the TFCC is ruptured., (Copyright © 2015 American Society for Surgery of the Hand. Purpose: To measure the effects of distal radius malalignment on loading at the distal ulna., Methods: Using an adjustable mechanism to simulate angulated and translated malalignments, clinically relevant distal radius deformities were simulated in a cadaveric model. A custom-built load cell was inserted just proximal to the native ulna head to measure the resultant force and torque in the distal ulna. Loads were measured before and after transecting the triangular fibrocartilage complex (TFCC)., Results: There was an increase in distal ulna load and torque with increasing dorsal translation and angulation. Combined conditions of angulation and translation increased force and torque in the distal ulna to a greater extent than with either condition in isolation. Transecting the TFCC resulted in a reduction in distal ulna load and torque., Conclusions: A progressive increase in load at the distal ulna was observed with increasing severity of malalignment, which may be an important contributor to residual ulnar wrist pain and dysfunction. However, no clear-cut threshold of malalignment of a dorsally angulated and translated distal radius fracture was identified. These observations suggest that radius deformities cause articular incongruity, which increases TFCC tension and distal radioulnar joint load. Cutting of the TFCC decreased distal ulna loading, likely by releasing the articular constraining effect of the TFCC on the distal radioulnar joint, allowing the radius to rotate more freely with respect to the ulna., Clinical Relevance: Anatomical reduction of a distal radius fracture minimizes the forces in the distal ulna and may reduce residual ulnar wrist pain and dysfunction., (Copyright © 2015 American Society for Surgery of the Hand. Purpose: To examine the effects of dorsal angulation deformities of the distal radius with and without triangular fibrocartilage complex (TFCC) rupture on the 3-dimensional kinematics of the distal radioulnar joint (DRUJ) during simulated active motion., Methods: Nine fresh-frozen cadaveric specimens were tested in a forearm simulator that produced active forearm rotation. Dorsal angulation deformities of the distal radius with 10°, 20°, and 30° angulation were created. Changes in the position of the ulna relative to the radius at the DRUJ as a consequence of each dorsal angulation deformity were quantified during simulated active supination in terms of volar, ulnar, and distal displacement of the ulna. Testing was performed initially with the TFCC intact and repeated after complete sectioning of the TFCC at its ulnar insertion., Results: Increasing dorsal angulation deformities of the distal radius significantly increased volar, ulnar, and distal displacement of the ulna when the TFCC was intact. Sectioning of the TFCC significantly increased volar displacement of the ulna in dorsal angulation deformities. As little as 10° of dorsal angulation significantly increased distal displacement of the ulna with the TFCC intact and resulted in a significant increase in volar, ulnar, and distal displacement of the ulna with sectioned TFCC., Conclusions: Dorsal angulation deformities of the distal radius affect the 3-dimensional kinematics of the DRUJ, especially with the TFCC sectioned., Clinical Relevance: The progressive change in DRUJ kinematics with increasing dorsal angulation may partially explain the relationship between the magnitude of dorsal angulation of distal radius fractures and functional outcomes in younger patients. The status of the TFCC should be evaluated carefully, as well as the magnitude of osseous deformity in patients with distal radius fractures and malunions, because changes in DRUJ kinematics caused by dorsal angulation are greater when the TFCC is ruptured., (Copyright © 2014 American Society for Surgery of the Hand. Purpose: The coronoid process has been recognized as a critical component in maintaining elbow stability. In the case of comminuted coronoid fractures, where repair is not possible or has failed, a prosthesis may be beneficial in restoring the osseous integrity of the elbow joint. The hypothesis of this in vitro biomechanical study was that a coronoid prosthesis would restore stability to the coronoid-deficient elbow., Methods: An anatomically shaped metallic coronoid prosthesis was designed and developed based on computed tomography-derived measurements and optimized to account for average cartilage thickness. Elbow kinematics and stability were determined for 8 cadaveric arms in active and passive elbow flexion in the varus, valgus, horizontal, and vertical positions using an elbow motion simulator. Varus-valgus angulation and internal-external rotation of the ulna relative to the humerus were quantified in the intact state, after collateral ligament sectioning and repair (control state), after a simulated 40% transverse coronoid fracture, and after implantation of the coronoid prosthesis., Results: Internal rotation of the ulna increased with a 40% coronoid fracture in the horizontal and varus positions. Increases in varus angulation after coronoid fracture were also observed in the horizontal and varus positions, during active and passive flexion, respectively. Following implantation of the coronoid prosthesis, elbow kinematics were restored similar to control levels in all elbow positions., Conclusions: Our findings support our hypothesis that an anatomically shaped coronoid prosthesis would be effective in restoring stability to the coronoid-deficient elbow., Clinical Relevance: This study provides evidence that the use of an anatomical implant restores stability to the coronoid-deficient elbow and rationale for further study and development of this method. Purpose: Osteochondritis dissecans (OCD) of the capitellum most commonly affects adolescent pitchers and gymnasts, who present with pain and mechanical symptoms. Patients with larger lesions have poorer outcomes, possibly related to increased contact pressures on the surrounding articular surface with or without instability. The purpose of this in vitro study was to determine whether displaced OCD lesions of the capitellum lead to altered kinematics and stability of the elbow., Methods: We mounted 9 fresh-frozen cadaveric arms in an upper extremity joint testing system, with cables attaching the tendons of the major muscles to motors and pneumatic actuators. An electromagnetic receiver on the ulna enabled quantification of the kinematics of the radius and ulna with respect to the humerus. We used 3-dimensional computed tomography scans and computer-assisted techniques to navigate sequential osteochondral defects ranging in size from 12.5% to 100% of the capitellum. The arms were subjected to active and passive flexion in both the vertical and valgus positions with the forearm in both pronation and supination., Results: We found no significant differences in valgus angulation or ulnar rotation between any of the OCD lesions and the intact elbow during flexion, regardless of arm position or forearm rotation., Conclusions: Osteochondritis dissecans lesions of the capitellum, both small and large, did not alter the ulnohumeral kinematics and stability with intact collateral ligaments. Therefore, excision of unfixable osteochondral fragments of the capitellum in the setting of intact collateral ligaments can be considered without the risk of creating instability., (Copyright © 2011. Purpose: Many patients develop distal radioulnar joint (DRUJ) pain and loss of forearm rotation after distal radial fractures. Residual distal radial deformity is one potential cause of DRUJ dysfunction; however, the parameters of distal radial fracture alignment that lead to an acceptable functional outcome are poorly defined in the literature., Methods: We used 8 fresh-frozen cadaveric specimens in this in vitro study to examine the effect of simulated distal radius fracture misalignment on forearm rotation. A distal radial osteotomy was performed just proximal to the DRUJ and a custom-made, 3-degrees-of-freedom modular implant designed to simulate distal radius fracture deformities was secured in place. This allowed for accurate simulation of dorsal angulation, dorsal translation, and radial shortening, both independently and in combination. We examined the effects of distal radius deformity in the setting of both an intact and sectioned triangular fibrocartilage complex., Results: Pronation was not significantly affected until dorsal angulation reached 30 degrees . Dorsal translation of up to 10 mm or radial shortening up to 5 mm had no effect on forearm rotation. Combined deformities had a greater effect on forearm motion than isolated malpositions. Dorsal angulation of > or =20 degrees combined with 10 mm of dorsal translation or 20 degrees of angulation with 2.5 mm of radial shortening resulted in a significant decrease in forearm pronation. There was no effect of distal radial deformities, either isolated or combined, on the magnitude of forearm rotation after sectioning the triangular fibrocartilage complex., Conclusions: This study demonstrates that a broad range of distal radius fracture malpositions can be tolerated before a notable loss in forearm range of motion is evident. Combined deformities are more likely to result in a clinically important loss of forearm rotation, and this should be considered when choosing the optimal management of patients with displaced distal radial fractures. Disruption of the triangular fibrocartilage releases the tether on the DRUJ, allowing for preservation of forearm motion even in the setting of marked osseous deformities.

Purpose: The role of the posterior bundle of the medial collateral ligament in stability of the elbow remains poorly defined. The purpose of this study was to determine the effect of sectioning the posterior bundle of the medial collateral ligament on the stability of the elbow., Methods: Varus and valgus gravity-loaded passive motion and simulated active vertical motion were performed on 11 cadaveric arms using an in vitro elbow motion simulator. Varus/valgus angle and internal/external rotation of the ulna with respect to the humerus were recorded using an electromagnetic tracking system in varus, valgus, and vertical orientations. Testing was performed on the intact elbow and after sectioning of the posterior bundle of the medial collateral ligament., Results: With active flexion in the vertical position, the varus/valgus kinematics were unchanged after sectioning of the posterior bundle of the medial collateral ligament. However, in pronation, there was an increase in internal rotation after sectioning of the posterior bundle of the medial collateral ligament compared with that of the intact elbow. This rotational difference was not detected with the forearm in supination. During supinated passive flexion in the varus position, sectioning of the posterior bundle of the medial collateral ligament resulted in increased varus angulation at all flexion angles. In pronation, varus angulation and internal rotation both increased. In supination, sectioning of the posterior bundle of the medial collateral ligament had no effect on maximum varus-valgus laxity or maximum internal rotation. However, in pronation, the maximum varus-valgus laxity increased by 3.5 degrees (30%) and maximum internal rotation increased by 1.0 degrees (29%)., Conclusions: These results indicate that isolated sectioning of the posterior bundle of the medial collateral ligament causes a small increase in varus angulation and internal rotation during both passive varus and active vertical flexion. This study suggests that isolated sectioning of the posterior bundle of the medial collateral ligament may not be completely benign and may contribute to varus and rotation instability of the elbow. In patients with insufficiency of the posterior bundle of the medial collateral ligament, appropriate rehabilitation protocols (avoiding forearm pronation and shoulder abduction) should be followed when other injuries permit.

Purpose: To measure the interfragmentary compression generated across a simulated fracture in cadaveric scaphoids by 3 different headless compression screws., Methods: A transverse osteotomy was made through the waist of each scaphoid and a load cell to measure compression was interposed between the fragments, which were then fixed internally retrograde with either an Acutrak Standard (n = 10), Acutrak Mini (n = 12), or Bold (n = 10) screw. The surgeon was blinded to the measured compression, which was recorded during screw insertion and for the following 5 minutes. As a measure of scaphoid bone quality the modulus of elasticity of the trabecular bone of each specimen was then calculated from uniaxial compression tests., Results: The mean interfragmentary compression generated by the Acutrak Standard screw was significantly greater and more consistent than the Bold screw or the Acutrak Mini screw. The compression achieved by the Acutrak Standard screw was also more consistent than that obtained by either the Bold or the Acutrak Mini screws as reflected by the lower standard deviation. The mean modulus of elasticity of the scaphoid trabecular bone was similar for each screw group., Conclusions: The interfragmentary compression generated by the Acutrak Standard screw was significantly greater and more consistent than that generated by either the Bold or Acutrak Mini screws. The compression generated by the Acutrak Standard and Mini screws was significantly better sustained over time than that generated by the Bold screw; however, these differences were small and may not be clinically important. The optimal compression required to promote scaphoid fracture union remains unknown and clinical trials are needed to further evaluate the outcome from using these devices.
