Unlabelled: We have constructed a dedicated breast PET/CT scanner capable of high-resolution functional and anatomic imaging. Here, we present an initial characterization of scanner performance during patient imaging., Methods: The system consisted of a lutetium oxyorthosilicate-based dual-planar head PET camera (crystal size, 3 x 3 x 20 mm) and 768-slice cone-beam CT. The position of the PET heads (separation and height) could be adjusted for varying breast dimensions. For scanning, the patient lay prone on a specialized bed and inserted a single pendent breast through an aperture in the table top. Compression of the breast as used in mammography is not required. PET and CT systems rotate in the coronal plane underneath the patient sequentially to collect fully tomographic datasets. PET images were reconstructed with the fully 3-dimensional maximum a posteriori method, and CT images were reconstructed with the Feldkamp algorithm, then spatially registered and fused for display. Phantom scans were obtained to assess the registration accuracy between PET and CT images and the influence of PET electronics and activity on CT image quality. We imaged 4 women with mammographic findings highly suggestive of breast cancer (breast imaging reporting and data system, category 5) in an ongoing clinical trial. Patients were injected with (18)F-FDG and imaged for 12.5 min per breast. From patient data, noise-equivalent counting rates and the singles-to-trues ratio (a surrogate for the randoms fraction) were calculated., Results: The average registration error between PET and CT images was 0.18 mm. PET electronics and activity did not significantly affect CT image quality. For the patient trial, biopsy-confirmed cancers were visualized on dedicated breast PET/CT on all patient scans, including the detection of ductal carcinoma in situ in 1 case. The singles-to-trues ratio was found to be inversely correlated with breast volume in the field of view, suggesting that larger breasts trend toward increased noise-equivalent counting rates for all other things equal., Conclusion: Scanning of the uncompressed breast with dedicated breast PET/CT can accurately visualize suspected lesions in 3 dimensions.

The extensive development of image fusion techniques over the past 20 y has shown that the fusion of images from complementary modalities offers a more complete and accurate assessment of disease than do images from a single modality. Although software techniques have been successful in fusing images of the brain from different modalities, they have achieved rather limited success for other parts of the body. The recent introduction of technology that can acquire both anatomic and functional images in a single scan has addressed many of the limitations of software fusion. The combination of CT and PET was introduced commercially in 2001, followed by CT and SPECT in 2004. Clinical adoption of PET/CT has been surprisingly rapid, and despite continuing debate, the new technology has advanced the use of clinical molecular imaging, particularly for oncology.

Unlabelled: The National Electrical Manufacturers Association (NEMA) NU 2-2001 performance measurements were conducted on the Discovery RX, a whole-body PET/CT system under development by GE Healthcare. The PET scanner uses 4.2 x 6.3 x 30 mm lutetium yttrium orthosilicate (LYSO) crystals grouped in 9 x 6 blocks. There are 24 rings with 630 crystals per ring and the ring diameter is 88.6 cm. The transaxial and axial fields of view are 70.0 and 15.7 cm, respectively. The scanner has retractable septa and can operate in both 2-dimensional (2D) and 3-dimensional (3D) modes. 2D acquisitions use ring differences of +/-4 for direct and +/-5 for cross slices; 3D acquisitions use a ring difference of 23. The coincident window width is 6.5 ns and the energy window is 425-650 keV. Other than the detectors, the system uses the same hardware and software as a Discovery ST. The CT scanner is a 16-slice LightSpeed; the performance characteristics of the CT component are not included herein., Methods: Performance measurements of sensitivity, spatial resolution, image quality, scatter fraction and counting rate performance, and image quality were obtained using NEMA methodology., Results: The system sensitivity in 2D and 3D was measured as 1.7 cps/kBq and 7.3 cps/kBq, respectively. The transaxial resolution for 2D (3D) was 5.1 mm full width at half maximum (FWHM) (5.0 mm) at 1 cm from gantry center and the radial and tangential resolutions were 5.9 mm (5.9 mm) and 5.1 mm (5.2 mm) at 10 cm, respectively. The axial resolution for 2D (3D) was 4.8 mm FWHM (5.8 mm) and 6.3 mm (6.5 mm) at 1 cm and 10 cm from gantry center, respectively. The scatter fraction was 13.1% and 31.8% in 2D and 3D. The peak noise equivalent count rate (NECR) was 155 kcps at 92.1 kBq/mL in 2D and 117.7 kcps at 21.7 kBq/mL in 3D for a noise-free estimation of randoms. The contrast of the 22, 17, 13, and 10 mm hot spheres in the image quality phantom in 2D (3D) were 74.6% (72.4%), 56.7% (59.5%), 46.2% (44.6%), and 17.9% (18.0%), respectively., Conclusion: The Discovery RX is a scanner that possesses high NECR, low scatter fraction, and good spatial resolution characteristics.

Unlabelled: PET and MRI are powerful imaging techniques that are largely complementary in the information they provide. We have designed and built a MR-compatible PET scanner based on avalanche photodiode technology that allows simultaneous acquisition of PET and MR images in small animals., Methods: The PET scanner insert uses magnetic field-insensitive, position-sensitive avalanche photodiode (PSAPD) detectors coupled, via short lengths of optical fibers, to arrays of lutetium oxyorthosilicate (LSO) scintillator crystals. The optical fibers are used to minimize electromagnetic interference between the radiofrequency and gradient coils and the PET detector system. The PET detector module components and the complete PET insert assembly are described. PET data were acquired with and without MR sequences running, and detector flood histograms were compared with the ones generated from the data acquired outside the magnet. A uniform MR phantom was also imaged to assess the effect of the PET detector on the MR data acquisition. Simultaneous PET and MRI studies of a mouse were performed ex vivo., Results: PSAPDs can be successfully used to read out large numbers of scintillator crystals coupled through optical fibers with acceptable performance in terms of energy and timing resolution and crystal identification. The PSAPD-LSO detector performs well in the 7-T magnet, and no visible artifacts are detected in the MR images using standard pulse sequences., Conclusion: The first images from the complete system have been successfully acquired and reconstructed, demonstrating that simultaneous PET and MRI studies are feasible and opening up interesting possibilities for dual-modality molecular imaging studies.

Unlabelled: This study evaluates the 2-dimensional (2D) and 3-dimensional (3D) performance characteristics of a newly developed PET/CT scanner using the National Electrical Manufacturers Association (NEMA) NU 2-1994 (NU94) and NEMA NU 2-2001 (NU01) standards. The PET detector array consists of 10,080 individual bismuth germanate crystals arranged in 24 rings of 420 crystals each. The size of each crystal is 6.3 x 6.3 x 30 mm in the axial, transaxial, and radial dimensions, respectively. The PET detector ring diameter is 88.6 cm with axial and transaxial fields of view (FOVs) of 15.7 and 70 cm, respectively. The scanner has a uniform patient port of 70 cm throughout the PET and CT FOV, and the PET scanner is equipped with retractable septa to allow 2D and 3D imaging., Methods: Spatial resolution, scatter fraction, sensitivity, counting rate, image quality, and accuracy as defined by the NEMA protocols of NU94 and NU01 for 2D and 3D modes are evaluated. The 2D mode data were acquired with a maximum ring difference of 5, whereas the 3D mode acquisition used ring differences of 23. Both 2D and 3D mode data were acquired with an energy window of 375-650 keV. Random estimation from singles counting rate was applied to all relevant analysis. In addition, images from 2 clinical whole-body oncology studies acquired in 2D and 3D modes are shown to demonstrate the image quality obtained from this scanner., Results: The 2D NU94 transaxial resolution is 6.1-mm full width at half maximum (FWHM) 1 cm off center and increases to 6.9 mm tangential and 8.1 mm radial at a radius (R) of 20 cm. NU01 2D average transaxial (axial) FWHM resolution measured 6.1 (5.2) mm at R = 1 cm and 6.7 (6.1) mm at R = 10 cm. The NU94 scatter fraction for 2D (3D) was 13% (29%), whereas the NU01 scatter fraction gave 19% (45%). NU01 peak 2D (3D) noise equivalent counting rate (T(2)/[T + R + S]) was 90.2 (67.8) kilocount per second (kcps) at 52.5 (12) kBq/mL. Total 2D (3D) system sensitivity for true events is 8 (32.9) kcps/kBq/mL for NU94 and 1.95 (9.2) kcps/Bq for NU01., Conclusion: The results show excellent system sensitivity with relatively uniform resolution throughout the FOV, making this scanner highly suitable for whole-body studies.

Unlabelled: Accurate anatomic localization of functional abnormalities seen with PET is known to be problematic. Even though nonspecific tracers such as 18F-FDG visualize certain normal anatomic structures, the spatial resolution is generally inadequate for localization of pathology. Combining PET with a high-resolution anatomic imaging modality such as CT can resolve the localization issue, as long as the images from the two modalities are accurately coregistered. However, software-based registration techniques have difficulty accounting for differences in patient positioning and involuntary movement of internal organs, often necessitating labor-intensive nonlinear mapping that may not converge to a satisfactory result. Acquiring both CT and PET images in the same scanner obviates the need for software registration and routinely provides accurately aligned images of anatomy and function in a single scan., Discussion: A CT scanner positioned in tandem with a PET scanner and with a common patient couch and operating console has recently been explored as a solution to anatomic and functional image registration. Axial translation of the couch between the two modalities enables both CT and PET data to be acquired during a single imaging session. In addition, the CT images can be used to generate noiseless attenuation correction factors for the PET emission data. By minimizing patient movement between the CT and PET scans, and after accounting for the axial separation of the two modalities, accurately registered anatomic and functional images can be obtained. Since the introduction of the first PET/CT prototype a little over 5 years ago, several thousand cancer patients have been scanned on combined PET/CT devices. In the past 3 years, a number of commercial designs have become available featuring multidetector spiral CT scanners and high-performance PET devices. Initial experience has demonstrated an increased level of accuracy and confidence in the interpretation of the combined study compared with separate readings, particularly in the ability to distinguish pathology from normal physiologic uptake and to precisely localize abnormal foci., Conclusion: Combined PET/CT scanners represent an important evolution in technology that is helping to bring molecular imaging to the forefront in cancer diagnosis, staging, and therapy monitoring.

In the past decade, the integration of anatomic imaging and functional imaging has emerged as a new and promising diagnostic tool. Developments in software provided methods to integrate various modalities, such as PET, CT, MRI, and MR spectroscopy. The introduction of combined PET/CT scanners has boosted image fusion in this specific field and raised high expectations. Image fusion can be performed at 3 different levels: visual fusion, software fusion, and hardware fusion, each having strengths, weaknesses, and issues inherent to technique. Visual fusion is the traditional side-by-side reviewing of 2 separate modalities. Software image fusion provides evaluation of 2 modalities in 1 integrated image set. True hardware fusion of PET and CT does not exist at present. Currently, hardware fusion refers to a PET/CT scanner that consists of separate scanners, which positioned in line at a fixed distance, with projection of the PET image over the CT image. The suggested superiority of hardware fusion with these so-called hybrid PET/CT scanners over software fusion has sparked debate. Because scientific data that unequivocally show that state-of-the-art software fusion is less accurate than hardware fusion (as provided in hybrid PET/CT scanners) are unavailable, the primacy of a combined PET/CT scanner over stand-alone PET and CT is more a matter of belief than of science. Further research comparing the overall performance of PET/CT scanners with that of separate scanners with software for image fusion is much needed. The continuous development of better software for image fusion and respiratory and cardiac gating is also needed, not only for PET and CT imaging but also for fusion of PET with MRI and CT with MRI.
