Abstract

Effects of SO2on Respiratory System of Adult Miyakejima Resident 2 Years after Returning to the Island: Satoko Iwasawa, et al. Department of Preventive Medicine and Public Health, School of Medicine, Keio University Mt. Oyama in Miyakejima Island erupted in June, 2000. All Miyake villagers were forced to evacuate from the island in September, 2000, due to continuous eruptions and emissions of unsafe amounts of volcanic gas, mainly SO2. From February, 2005, Miyake villagers returned to the island despite volcanic gas still being emitted. This study examines the 2‐yr changes in Miyake residents' respiratory systems from autumn 2004 to November 2006. The study population was 823 Miyake adult residents who participated in the health check‐up in 2006. Respiratory effects were evaluated by a questionnaire for respiratory symptoms and spirometry. SO2has been continuously monitored at 7 sampling points of the inhabited area. The mean SO2concentration from February 2005 to November 2006 was 0.031 ppm. The area was categorized into 4 areas by SO2concentration, namely, areas L, H‐1, H‐2 and H‐3, where average SO2concentrations were 0.019, 0.026, 0.032, and 0.045 ppm, respectively. The study subjects showed no deterioration in lung function. Prevalence of cough and phlegm among all participants were significantly higher in 2006 than in 2004, and age‐, sex‐ and smoking‐adjusted odds ratios of cough and phlegm were 1.75 (95%CI 1.33–2.30) and 1.44 (1.12–1.87). Prevalence of chronic bronchitis‐like symptoms among normosusceptive subjects in 2006 was 4.1% which was significantly higher than that of 2.1% in 2004 (p=0.035). Compared to area L, the frequencies of phlegm and irritation of the nose were significantly increased in areas H‐2 and H‐3. SO2exposure‐related respiratory symptoms were observed in adult Miyakejima residents after returning to the island.

Marital Status and Cardiovascular Risk Factors among Middle‐aged Japanese Male Workers: The High‐risk and Population Strategy for Occupational Health Promotion (HIPOP‐OHP) Study: Yuko Kamon, et al. Department of Health Science, Shiga University of Medical Science—Marital status is related to cardiovascular disease (CVD) risk factors in Western countries. However, few studies have addressed the relationship between marital status and CVD risk factors in other populations. We investigated lifestyle and CVD risk factors relative to marital status among middle‐aged Japanese men. We analyzed baseline data of 40–59‐yr‐old male workers who participated in the high‐risk and population strategy for occupational health promotion (HIPOPOHP) study. We compared lifestyle and CVD risk factors between men who were married (Group M; n=1,419, mean age 47.9 ± 5.1 yr) and those who had never married (Group N; n=163, mean age 46.7 ± 4.3 yr). Men in Group N were more likely to skip breakfast, work more shifts and exercise less. Current smoking rates, as well as average values of diastolic blood pressure (DBP), serum total cholesterol and fasting plasma glucose were also higher in Group N than in Group M. The proportion of participants with three or more CVD risk factors, namely smoking, hypertension, hypercholesterolemia and hyperglycemia was higher in Group N, than in Group M (12.9% vs. 5.0%, p<0.01). The difference between Groups M and N was more evident in the subgroup of living with others, than in the subgroup of living alone. Since men who have never married might be at higher risk for CVD, effort should be made to educate this population about decreasing lifestyle‐related risk factors.

Lack of Relationship between Occupational Exposure to Carbon Disulfide and Endocrine Dysfunction: A Six‐year Cohort Study of the Japanese Rayon Workers: Toru Takebayashi, et al.Department of Preventive Medicine and Public Health, School of Medicine, Keio University A six‐year prospective cohort study was conducted to clarify whether the current carbon disulfide (CS2) exposure level is low enough to prevent subclinical health impairment and/or to ameliorate health effects due to previous high exposure. This paper describes the follow‐up results, exposure assessment, and effects on the endocrine system. The study subjects were 432 male workers exposed to CS2at one of the 11 rayon factories in Japan and 402 referent workers from the same factories, all of whom were examined in 1992–93. 251 CS2exposed, 140 former‐exposed and 359 referent workers participated in the follow‐up survey (follow‐up rate 89.9%) in 1998–99. Urinary 2‐thiothiazolidine‐4carboxylic acid (TTCA) at the end of a shift as internal exposure index and the time‐weighted average CS2concentration as external exposure index were measured twice a year from 1992–1993. By using various blood indices, we evaluated effects on thyroid, hypophysis, and gonad function, as well as effects on glucose metabolism. Information on potential confounding factors, such as age, smoking and alcohol drinking, were collected and adjusted for. When the individual exposure level was represented by arithmetic mean for TTCA and CS2concentration, geometric means (SD) of CS2workers were 1.61 (1.91) mg/g.crea and 5.02 (1.84) ppm, respectively. There were no exposure‐related differences in glucose metabolism or any of the endocrinological indices. Conclusions—No biologically significant effects of CS2were found on endocrine function in Japanese rayon manufacturing workers when the average internal and external exposure levels were maintained below the current occupational exposure limit.

Urinary Excretion of TTCA after Intake of brassicaVegetables: Yuriko Kikuchi, et al.Department of Preventive Medicine and Public Health, School of Medicine, Keio University—Aim: Recent studies have made it clear that brassicavegetables contain 2-thiothiazolidine-4-carboxylic acid (TTCA), which is the most widely used biological monitoring index of exposure to carbon disulfide (CS2). This study aimed to assess the time-course of TTCA excretion in urine (TTCA-U) after eating brassicavegetables. Methods: After a 1-d break from eating brassicavegetables, ten volunteers (6 males and 4 females) ingested 100 grams of chopped raw cabbage containing 4.3 mg/kg of TTCA, and the TTCA concentration in urine samples was determined over 24 h. TTCA concentrations in brassicavegetables purchased from a local supermarket were also measured. Results: TTCA-U reached peak concentrations 3-9 h after cabbage intake, gradually decreased, and was below the detection limit (<0.1 mg/I) in 8 of 10 volunteers in the last urine samples. The total amount of TTCA excreted in 24 h ranged from 0.19 to 0.42 mg, and half of the total TTCA was excreted within 6.5 h on average (range: 4.5-10.1). The excretion profiles of young and middle-aged volunteers seemed to differ, but not those of young males and young females. TTCA was detected in both raw and boiled cabbage, Japanese radish, turnip, and broccoli, but was not detected in Chinese cabbage or chingentsuai. Conclusion: TTCA-U may be overestimated as an index of CS2exposure when brassicavegetables are ingested within approximately 24 h before collection of the urine sample.

24. Acute and Recurrent Hepatitis Induced by 2,2-Dichloro-1,1,1-trifluoroethane (HCFC-123)

Abstract

Acute and Recurrent Hepatitis Induced by 2,2-Dichloro-1,1,1-trifluoroethane (HCFC-123): Kazuyuki Omae, et al.Department of Preventive Medicine and Public Health, School of Medicine, Keio University—A 49-yr-old female cleaning worker first visited a hospital because of acute hepatitis. On admission, laboratory examination revealed severe hepatic damage (AST 2125 IU/L, ALT 2391 IU/L, LDH 1279 IU/L, t-Bil 8.5 mg/dl), but failed to disclose any evidence of viral infection or morphological lesions. She recovered day by day without any specific treatment for hepatitis and was discharged one month later. On the 11th day after restarting her job, she was readmitted due to similar symptoms and signs. Serum biochemistry data again showed severe liver damage (AST 1354, ALT 1604, LDH 1119 t-Bil 2.6). Histopathological diagnosis based on hepatic needle biopsy was chronic active hepatitis with diffuse infiltration of lymphocyte-dominant inflammatory cells, bridging necrosis, intercellular fibrosis in a limited region, and with ballooning, degeneration, and/or necrosis of the hepatic cells. A job-related cause was suspected and 2,2-dichloro-1,1,1-trifluoroethane (HCFC-123), which has been proven to cause acute hepatitis in humans, was detected in a dry-cleaning solvent by gas chromatography-mass spectrometry analysis. A job simulation experiment suggested that the concentration of the patient’s exposure to HCFC-123 exceeded 1,000 ppm during the busiest work period, which was sufficiently high to induce severe liver damage.
