Fear of movement (kinesiophobia) seems to play an important role in the development of chronic pain. However, for temporomandibular disorders ( TMD), there is a scarcity of studies about this topic. The Tampa Scale for Kinesiophobia for TMD ( TSK/ TMD) is the most widely used instrument to measure fear of movement and it is not available in Brazilian Portuguese. The purpose of this study was to culturally adapt the TSK/ TMD to Brazilian Portuguese and to assess its psychometric properties regarding internal consistency, reliability, and construct and structural validity. A total of 100 female patients with chronic TMD participated in the validation process of the TSK/ TMD-Br. The intraclass correlation coefficient ( ICC) was used for statistical analysis of reliability (test-retest), Cronbach's alpha for internal consistency, Spearman's rank correlation for construct validity and confirmatory factor analysis ( CFA) for structural validity. CFA endorsed the pre-specified model with two domains and 12-items (Activity Avoidance - AA/Somatic Focus - SF) and all items obtained a loading factor greater than 0·4. Acceptable levels of reliability were found (ICC > 0·75) for all questions and domains of the TSK/ TMD-Br. For internal consistency, Cronbach's α of 0·78 for both domains were found. Moderate correlations (0·40 < r < 0.60) were observed for 84% of the analyses conducted between TSK/ TMD-Br scores versus catastrophising, depression and jaw functional limitation. TSK/ TMD-Br 12 items and two-factor demonstrated sound psychometric properties (transcultural validity, reliability, internal consistency and structural validity). Objective: To identify assessment tools used to evaluate patients with temporomandibular disorders (TMD) considered to be clinically most useful by a panel of international experts in TMD physical therapy (PT)., Methods: A Delphi survey method administered to a panel of international experts in TMD PT was conducted over three rounds from October 2017 to June 2018. The initial contact was made by email. Participation was voluntary. An e-survey, according to the Checklist for Reporting Results of Internet E-Surveys (CHERRIES), was posted using SurveyMonkey for each round. Percentages of responses were analysed for each question from each round of the Delphi survey administrations., Results: Twenty-three experts (completion rate: 23/25) completed all three rounds of the survey for three clinical test categories: 1) questionnaires, 2) pain screening tools and 3) physical examination tests. The following was the consensus-based decision regarding the identification of the clinically most useful assessments. (1) Four of 9 questionnaires were identified: Jaw Functional Limitation (JFL-8), Mandibular Function Impairment Questionnaire (MFIQ), Tampa Scale for Kinesiophobia for Temporomandibular disorders (TSK/TMD) and the neck disability index (NDI). (2) Three of 8 identified pain screening tests: visual analog scale (VAS), numeric pain rating scale (NRS) and pain during mandibular movements. (3) Eight of 18 identified physical examination tests: physiological temporomandibular joint (TMJ) movements, trigger point (TrP) palpation of the masticatory muscles, TrP palpation away from the masticatory system, accessory movements, articular palpation, noise detection during movement, manual screening of the cervical spine and the Neck Flexor Muscle Endurance Test., Conclusion: After three rounds in this Delphi survey, the results of the most used assessment tools by TMD PT experts were established. They proved to be founded on test construct, test psychometric properties (reliability/validity) and expert preference for test clusters. A concordance with the screening tools of the diagnostic criteria of TMD consortium was noted. Background: Cervical spine disorders are frequently associated with temporomandibular disorders (TMDs). Although headache is common in TMD, few studies have considered headache regarding the functional performance of the cervical spine in TMD., Objectives: To evaluate TMD subjects with and without self-reported headache, assess the active and passive range of motion (ROM) of the upper cervical spine (C1-C2) and performance of the deep cervical flexors, and evaluate the correlation of these outcomes with neck disability and temporomandibular pain., Methods: In this cross-sectional study, we assessed 57 women (40 with TMDs and 17 without TMDs [controls]). The TMD group was divided into 25 and 15 patients with and without self-reported headache, respectively. We assessed the active and passive upper cervical ROMs using the flexion-rotation test (FRT) and muscle performance using the craniocervical flexion test (CCFT). The neck disability index questionnaire and visual analog scale were used to assess self-reported neck disability and temporomandibular pain, respectively., Results: Compared to controls, all TMD subjects showed reduced cervical mobility on flexion/extension movements, reduced FRT findings and worse performance in CCFT (P < .05 all). Additionally, the CCFT and FRT findings were correlated with temporomandibular pain (r = .4 and .5, respectively) and neck disability (r = .3 and .4, respectively)., Conclusion: Women with TMDs, regardless of self-reported headaches, showed limited flexion/extension ROM, limited upper cervical spine (C1-C2) mobility and poor deep cervical flexor performance.
