Success of any bone augmentation procedure is dependent on several factors. Because complications occur in some cases, the aims of this study are to analyze adverse events associated with placement of fresh-frozen bone allografts (FFBAs) during alveolar ridge augmentation and to assess 1-year survival of dental implants placed in reconstructed sites.Fifty-eight consecutive patients (15 males and 43 females, aged 38 to 76 years; mean age: 58 ± 9.2 years) requiring maxillary bone reconstruction prior to implant placement were enrolled in this study. A total of 268 implants was subsequently placed in sites reconstructed with FFBAs. There were 22 posterior grafted sites, 19 anterior, and 17 full-arch sites. After a 4- to 6-month integration period, all patients received an implant-supported fixed prostheses. Complications occurring during treatment and the 12-month follow-up period were recorded and evaluated.Thirteen of 58 (22.41%) patients experienced some kind of complication in the receptor site. Infection occurred in six (10.34%) individuals, dehiscence in five (8.62%), and mucosal perforation in seven (12.07%). Adverse outcomes categorized as partial and total graft loss occurred in four (6.90%) and three (5.17%) patients, respectively. Implant failure rate was 16 (5.97%) of the 268 fixtures placed in 12 (20.70%) of 58 patients.Infection and suture dehiscence are significantly correlated with graft loss in a maxillary FFBA augmentation. Patients with full-arch grafting reconstructions lost significantly more implants. Early diagnosis and prompt management of adverse events seem to be of great importance in prevention of total graft loss.

Background: As patient demand increases for more natural restorations in the esthetic zone, clinicians must have the highest level of skill and knowledge to maintain or reform the interdental papilla between teeth, between implants and teeth, and between adjacent implants. To date, there are no reports that have measured the distance from the contact point to the bony crest between implants. One reason for this may be the fact that, with two adjacent implants, the contact point of the crown can be established at any distance from the gingival margin according to the restorative dentist's specifications. Therefore, in this study, the height of the soft tissue to the crest of bone was measured between two adjacent implants independent of the location of the contact point. The purpose of this study was to determine the range and average height of tissue between two adjacent implants. Methods: A total of 136 interimplant papillary heights were examined in 33 patients by eight different examiners in five private dental offices. After administration of appropriate local anesthesia, a standardized periodontal probe was placed vertically from the height of the papilla to the crest of bone. The measurements were rounded off to the nearest millimeter. Results: The mean height of papillary tissue between two adjacent implants was 3.4 mm, with a range of 1 mm to 7 mm. Conclusions: Clinicians should proceed with great caution when placing two implants adjacent to each other in the esthetic zone. In most cases, only 2, 3, or 4 mm of soft tissue height (average 3.4 mm) can be expected to form over the interimplant crest of bone. These results showed that modification of treatment plans may be necessary when esthetics are critical for success.

This study was designed to determine whether the distance from the base of the contact area to the crest of bone could be correlated with the presence or absence of the interproximal papilla in humans. A total of 288 sites in 30 patients were examined. If a space was visible apical to the contact point, then the papilla was deemed missing; if tissue filled the embrasure space, the papilla was considered to be present. The results showed that when the measurement from the contact point to the crest of bone was 5 mm or less, the papilla was present almost 100% of the time. When the distance was 6 mm, the papilla was present 56% of the time, and when the distance was 7 mm or more, the papilla was present 27% of the time or less.

Twenty consecutive hospital patients who were to have periodontal surgery were assigned to one of two groups in a double blind clinical study. Ten were placed on tetracycline 250 mg, 6 times per day for 5 days. The other ten were given an identical appearing placebo 6 times per day for 5 days. The capsules were placed in a brown envelope so that neither the patient nor the periodontist knew which medication was being administered. The patients were randomly assigned to one of the two groups and were evaluated subjectively and objectively on the 3rd day and the 7th day. Statistically there were no differences between the two groups. In both groups there was some inflammation on the 3rd day but on the 7th day, it had subsided. Tetracycline is not recommended for routine prophylactic use in periodontal surgery.

The current classifications of teeth with furcation involvements refer mainly to the horizontal component of bone loss. This article presents a subclassification that measures the probeable vertical depth from the roof of the furca apically. The subclasses being proposed are: A, B and C. "A" indicates a probeable vertical depth of 1-3 mm, "B" 4-6 mm and "C" 7 or more mm of probeable depth from the roof of the furca apically. Furcations would thus be classified as IA, IB, IC, IIA, IIB, IIC and IIIA, IIIB, IIIC.
