A transverse force couple (TFC) functional imbalance has been demonstrated in osteoarthritic shoulders by recent 3-dimensional (3D) muscle volumetric studies. Altered rotator cuff vectors may be an additional factor contributing to a muscle imbalance and the propagation of glenoid deformity. Computed tomography images of 33 Walch type A and 60 Walch type B shoulders were evaluated. The 3D volumes of the entire subscapularis, supraspinatus, and infraspinatus–teres minor (ISP-Tm) and scapula were manually segmented. The volume masks and scapular landmarks were imported into MATLAB to create a coordinate system, enabling calculation of muscle force vectors. The direction of each muscle force vector was described in the transverse and vertical plane, calculated with respect to the glenoid. Each muscle vector was then resolved into compression and shear force across the glenoid face. The relationship between muscle force vectors, glenoid retroversion or inclination, compression/shear forces on the glenoid, and Walch type was determined using linear regression. In the transverse plane with all rotator cuff muscles combined, increasing retroversion was significantly associated with increasing posterior drag (P <.001). Type B glenoids had significantly more posterior drag than type A (P <.001). In the vertical plane for each individual muscle group and in combination, superior drag increases as superior inclination increases (P <.001). Analysis of individual muscle groups showed that the anterior thrust of ISP-Tm and supraspinatus switched to a posterior drag at 8° and 10° of retroversion respectively. The compression force on the glenoid face by ISP-Tm and supraspinatus did not change with increasing retroversion for type A shoulders (P =.592 and P =.715, respectively), but they did for type B shoulders (P <.001 for both). The glenoid shear force ratio in the transverse plane for the ISP-Tm and supraspinatus moved from anterior to posterior shear with increasing glenoid retroversion, crossing zero at 8° and 10° of retroversion, whereas the subscapularis exerted a posterior shear force for every retroversion angle. Increased glenoid retroversion is associated with increased posterior shear and decreased compression forces on the glenoid face, explaining some of the pathognomonic bone morphometrics that characterize the osteoarthritic shoulder. Although the subscapularis always maintains a posterior thrust, the ISP-Tm and supraspinatus together showed an inflection at 8° and 10° of retroversion, changing from an anterior thrust to a posterior drag. The etiology of the Walch type B shoulder remains unclear. We hypothesized that a scapulohumeral muscle imbalance, due to a disturbed transverse force couple (TFC) between the anterior and posterior rotator cuff muscles, may have a role in the pathogenesis of the type B morphology. The purpose of this study was to determine whether there is a TFC imbalance in the Walch type B shoulder using an imaging-based 3-dimensional (3D) volumetric and fatty infiltration assessment of segmented rotator cuff muscles. Computed tomography images of 33 Walch type A and 60 Walch type B shoulders with the complete scapula and humerus including the distal humeral epicondyles were evaluated. The 3D volumes of the entire subscapularis, supraspinatus, and infraspinatus–teres minor (Infra-Tm) were manually segmented and analyzed. Additionally, anthropometric parameters including glenoid version, glenoid inclination, posterior humeral head subluxation, and humeral torsion were measured. The 3D muscle analysis was then compared with the anthropometric parameters using the Wilcoxon rank sum and Kruskal-Wallis tests. There were no significant differences (P >.200) in muscle volume ratios between the Infra-Tm and the subscapularis in Walch type A (0.93) and type B (0.96) shoulders. The fatty infiltration percentage ratio, however, was significantly greater in type B shoulders (0.94 vs. 0.75, P <.001). The Infra-Tm to subscapularis fatty infiltration percentage ratio was significantly larger in patients with >75% humeral head subluxation than in those with 60%-75% head subluxation (0.97 vs. 0.74, P <.001) and significantly larger in patients with >25° of retroversion than in those with <15° of retroversion (1.10 vs. 0.75, P =.004). The supraspinatus fatty infiltration percentage was significantly lower in Walch type B shoulders than type A shoulders (P =.004). Walch type A shoulders had mean humeral retrotorsion of 22° ± 10° whereas Walch type B shoulders had humeral retrotorsion of only 14° ± 9° relative to the epicondylar axis (P <.001). The TFC is in balance in the Walch type B shoulder in terms of 3D volumetric rotator cuff muscle analysis; however, the posterior rotator cuff does demonstrate increased fatty infiltration. Posterior humeral head subluxation and glenoid retroversion, which are pathognomonic of the Walch type B shoulder, may lead to a disturbance in the length-tension relationship of the posterior rotator cuff, causing fatty infiltration. The reconstructive options for instability-related anterior glenoid bone loss include iliac crest autograft, allograft, or coracoid transfer. The use of distal clavicle autograft (DCG) has also been described. The purpose of this imaging and cadaveric study was to examine the dimensions, morphology, and bone density of the DCG and compare it with the Latarjet procedure. We used 49 computed tomography scans from patients with anterior glenoid bone loss to measure the distal clavicle dimensions and bone density. Four glenoid reconstructions were simulated to compare techniques: DCG inferior surface toward glenoid (DCG inferior), DCG superior, classic Latarjet, and congruent-arc Latarjet. In addition, the morphology of the DCG was assessed on computed tomography and confirmed in 27 cadavers. The mean width of the DCG (11 mm) was significantly greater (P <.001) than that of the classic Latarjet orientation (9 mm) but less (P =.002) than that of the congruent-arc orientation (12 mm). The DCG had a lower bone density than the coracoid (P <.001). The mean articular surface area of the DCG-inferior orientation was 208 mm2, which was greater (P =.013) than that of the DCG-superior orientation (195 mm2) and not significantly different (P =.44) than that of the classic Latarjet orientation (214 mm2). The surface area of the congruent-arc orientation was greater (285 mm2, P <.001) than that of all other graft orientations. The DCG-inferior orientation was able to reconstruct 22% of the glenoid articular surface; DCG-superior orientation, 21%; classic Latarjet orientation, 23%; and congruent-arc orientation, 30%. Three DCG morphologies were identified: square (34%), trapezoidal (53%), and rounded (13%). The distal clavicle osteoarticular graft was able to reconstruct 22% of the glenoid face. Joint registries provide invaluable data on primary arthroplasties with revision as the endpoint; however, the revision outcomes are often excluded. Therefore, a PROSPERO registered review (CRD42015032531) of all revision studies in North America and Europe was conducted to evaluate demographics, etiologies and indications, implant manufacturer, and complications by geographic region. The MEDLINE, EMBASE, and CENTRAL databases were searched for revision arthroplasty clinical studies with a minimum mean 24-month follow-up. There were no language exclusions. Articles published in German, French, and Italian were reviewed by research personnel proficient in each language. The mean age at revision was 66 ± 5 years (male = 759, female = 1123). The male-female ratio in North American and Europeans studies was 43:57 and 34:66, respectively. The most common etiology for primary surgery in both regions was osteoarthritis or glenoid arthrosis (38%). The most common revision indication overall was rotator cuff tear, deficiency, or arthropathy (26%). The most common implant type used in revisions was a reverse shoulder arthroplasty (54%). The complication rate for all revisions was 17%. There were a total of 465 complications, and of those, 74% lead to a reoperation. Generally, shoulder arthroplasties are designed to last 10-15 years; however, revisions are being performed at a mean 3.9 years from the primary procedure, based on the published studies included in this systematic review. Additionally, of the complications, a large number (74%) went on to a reoperation. The Walch type B glenoid has the hallmark features of retroversion, joint subluxation, and bony erosion. Although the type B glenoid has been well described, the morphology of the corresponding type B humerus is poorly understood. As such, the aim of this imaging-based anthropometric study was to investigate humeral torsion in Walch type B shoulders. Three-dimensional models of the full-length humerus were generated from computed tomography data for the Walch type B group (n = 59) and for a control group of normal nonarthritic shoulders (n = 59). An anatomic humeral head-neck plane was created and used to determine humeral torsion relative to the epicondylar axis. Measurements were repeated, and intraclass correlation coefficients were calculated. The type B humeri had significantly (P <.001) less retrotorsion (14° ± 9°) than the control group (36° ± 12°) relative to the epicondylar axis. Male and female individuals within the control group showed statistically significant differences in humeral torsion (P =.043), which were not found in the type B group. Inter-rater reliability showed excellent agreement for humeral torsion (intraclass correlation coefficient, 0.962). A subgroup analysis between Walch type B2 and B3 shoulders showed no significant differences in any of the humeral or glenoid parameters. The Walch type B humerus has significantly less retrotorsion than non-osteoarthritic shoulders. At present, it is unknown whether the altered humeral retrotorsion is a cause or effect of the type B glenoid. In addition, it is unknown whether surgeons should be reconstructing type B2 humeral component version to pathologic torsion or to nonpathologic population means to optimize arthroplasty survivorship. Background The type B3 glenoid is an addition to the Walch classification. A potential etiologic theory is that it is a progression of the B2. It is characterized by uniconcavity, absent paleoglenoid, medialization, retroversion, and subluxation. The purpose of this study was to describe the morphology of B3 glenoids. Methods Fifty-two patients with B3 glenoids underwent 3-dimensional analysis of computed tomography data. Glenoid measurements (retroversion, inclination, medialization) and humeral head subluxation according to the scapular and glenoid planes were determined. The measured variables were compared between male and female patients. Results The mean B3 retroversion, inclination, and medialization were 24° ± 7°, 8° ± 6° superior, and 14 ± 4 mm, respectively. The mean posterior subluxation was 80% ± 8% and 54% ± 6% according to the scapular and glenoid planes, respectively. There were no differences in B3 characteristics between sexes ( P  > .05). A significant correlation existed between glenoid retroversion and humeral head subluxation relative to the scapular plane, with every 1° increase in retroversion translating to a 1% increase in subluxation ( P  < .001). In contrast, when referencing the glenoid plane, the humeral head remained concentric to the erosion. Conclusions The B3 is uniconcave and retroverted. As glenoid retroversion increases, posterior humeral head subluxation significantly increases as referenced to the scapular plane; however, when referenced to the glenoid plane, the head remains concentric to the erosion. This appearance of “concentricity” is acquired secondary to the wear pattern, creating a uniconcave glenoid. Therefore, surgeons should be aware that the visualized concentricity is a product of the erosion pattern and thus may conceal a greater amount of subluxation potential. [ABSTRACT FROM AUTHOR]

Background: Posterior glenoid erosion is thought to be initiated by humeral head subluxation. However, it is unknown whether subluxation is entirely caused by soft-tissue instability and unbalanced muscle activity or whether osseous morphology is a contributing factor. We hypothesized that patients with posterior erosion may exhibit premorbid glenoid morphology that is inherently retroverted and inferiorly inclined compared with age-matched normal glenoids. Methods: This study examined 80 scapulae, evenly distributed between 2 groups: osteoarthritic with type B2 glenoids and age-matched normal glenoids. From 3-dimensional computed tomography reconstructions, version and inclination were measured from the anterior paleoglenoid region of the B2 glenoids, which is representative of the premorbid glenoid, and compared with measurements obtained from similar regions in the normal cohort. Results: The anterior paleoglenoid region of B2 glenoids was significantly ( P  < .001) more retroverted (−14° ± 6°) compared with similar regions in nonarthritic normal glenoids (−5° ± 5°). There were no significant differences ( P  = .166) in the glenoid inclination angle between type B2 glenoids (0° ± 6°) and nonarthritic normal glenoids (2° ± 5°). Negative values represent retroverted and inferiorly inclined glenoids. Discussion: Understanding premorbid glenoid morphologic variations may provide insight into the pathoanatomy of humeral head subluxation, osteoarthritis, and posterior glenoid erosion. The results of this study indicate that patients with type B2 osteoarthritic glenoids have significantly greater premorbid glenoid retroversion compared with nonarthritic normal glenoids, suggesting that this premorbid morphologic variation may be one contributing factor to posterior erosion. Background The purpose of this study was to evaluate and to compare the size and morphologic patterns among normal and osteoarthritic (OA) humeral heads. Methods This comparative anatomic imaging study evaluated 150 humeral heads that were separated into 3 cohorts: normal, OA with symmetric glenoid erosion, and OA with asymmetric (type B2) glenoid erosion. Three-dimensional models were created of the humeral head from computed tomography data, and point coordinates were extracted for evaluation. Parameters measured were diameter (sphere fit and circle fit), chord distance (superoinferior and anteroposterior), and humeral head height. Results The sphere-fit diameter of the humeral head for the entire OA cohort (100 patients; mean diameter, 59 ± 9 mm) was significantly greater ( P < .001) than that of the normal cohort (50 patients; mean diameter, 49 ± 5 mm). Similarly, the humeral head circle-fit diameters in the superoinferior and anteroposterior planes were significantly greater ( P < .001) in the combined OA cohorts (59 ± 9 mm and 56 ± 10 mm, respectively) compared with the normal cohort (51 ± 5 mm and 47 ± 5 mm, respectively). However, there were no significant differences ( P ≥ .099) between the symmetric and asymmetric OA cohorts in sphere-fit or circle-fit diameters. The mean values of humeral head heights were not significantly different ( P = .382) between cohorts, 19 ± 2 mm, 18 ± 2 mm, and 18 ± 2 mm for the normal, symmetric, and asymmetric cohorts, respectively. Discussion Although OA humeral head morphology varies significantly from normal, it does not vary as a function of the Walch classification between symmetric and asymmetric glenoids. Understanding of the morphologic variability of the pathologic humeral head may provide insight into the pathoanatomy of osteoarthritis and the development of various erosion patterns. Background Distal humeral hemiarthroplasty is a treatment option for elbow joint disease that predominantly affects the distal humerus, including distal humerus fractures, nonunions, and avascular necrosis. The effect of hemiarthroplasty implants on joint contact has not been reported. The purpose of this in vitro study was to quantify the effects of hemiarthroplasty and implant size on ulnohumeral joint congruency. Methods Five fresh frozen cadaveric upper extremities were mounted to a custom elbow testing system. Active and passive motion were performed in dependent, horizontal, varus, and valgus positions. A registration and interbone distance algorithm was used to quantify ulnohumeral joint congruency throughout elbow flexion. Results The optimally sized hemiarthroplasty implant demonstrated the greatest joint congruency with the ulna, followed by the oversized implant, then the undersized implant. Joint congruency was greater during active vs. passive flexion, indicating that the elbow joint is more reduced in active flexion than in passive flexion. Conclusion This study demonstrates that undersized distal humeral hemiarthroplasty implants have the lowest joint congruency compared with an optimally sized or oversized implant. Background The purpose of this computational modeling study was to compare the volume of glenoid bone removal required to implant 3 augmented component designs for management of B2 erosions. In addition, we assessed bone quality of the supporting bone directly beneath the implants by measuring bone density and porosity. Methods Three augmented component designs—full-wedge, posterior-wedge, and posterior-step—were studied by virtual implantation in a cohort of 16 patients with B2 glenoids. B2 retroversion was corrected to 0° and 10°. The outcome variables were the volume of glenoid bone removal required for implantation and the density and porosity of the bone immediately beneath the implant. Results Implant design had a significant effect on the volume of bone removal ( P < .001). When correcting to 0°, the posterior-wedge implant removed less bone than the posterior-step ( P < .001) and the full-wedge ( P = .004). At 10° retroversion, the posterior-wedge removed less bone ( P = .029) than the posterior-step but was no different than the full-wedge ( P = .143). The residual glenoid bone density with the posterior-wedge was significantly greater than with the posterior-step ( P = .048), with no other significant differences ( P > .05). Residual glenoid bone porosity was not significantly different between implants ( P > .262). Conclusions Augmented components can provide a bone-preserving option for B2 glenoid management. Substantial variations in the volume of bone removal and the quality of the remaining glenoid bone were found between 3 different designs of augmented implants. Background The purpose of this computed tomography–based study was to quantify erosions in B2 glenoids. We hypothesized that erosions do not occur symmetrically and that they have a predictable orientation. Methods This study evaluated 55 type B2 glenoid cases. Computed tomography data were used to create three-dimensional reconstructions, and point coordinates were extracted from each reconstruction for morphologic analysis of the osteoarthritic glenoid and humerus. Results There was a significant difference between the mean orientation angle (28° ± 11°) of the posterior glenoid line of erosion and the superoinferior axis ( P < .001), which resulted in the average erosion being directed toward the 8-o'clock position. On average, the erosion started 1.6 ± 3.4 mm posterior to the glenoid center point. In 35% of B2 cases, the line of erosion was curved. The mean surface area was 763 ± 296 mm 2 for the neoglenoid and 957 ± 276 mm 2 for the paleoglenoid, indicating that the neoglenoid occupied 44% ± 12% of the total glenoid area. In this cohort, the mean radius of the humeral head neoarticulation was 32 ± 6 mm, the neoglenoid radius was 37 ± 8 mm, and the paleoglenoid radius was 34 ± 7 mm. The radius of the humeral head was significantly less than that of the neoglenoid ( P < .001) and the paleoglenoid ( P = .009). In addition, the radius of the neoglenoid was significantly greater than the radius of the paleoglenoid ( P = .012). Discussion Type B2 glenoids have a predictable wear pattern, which is not axisymmetric to the glenoid superoinferior axis. In addition, the identified anatomic characteristics of B2 erosions will aid surgeons in the operative management of bone loss and may assist manufacturers in the design of augmented components. Background Accurate characterization of regional variations in bone density in symmetric and asymmetric (B2) glenoid erosion patterns can assist with surgical planning, reaming, and component implantation. The purpose of this study was to characterize regional bone density and porosity in symmetric and asymmetric (B2) osteoarthritic glenoids. Methods Symmetric (n = 25) and asymmetric (B2) (n = 25) erosion patterns were compared by computed tomography–based imaging software. An orthogonal coordinate system separated each glenoid into quadrants. In addition, a linear best-fit line defined the line-of-erosion between the paleoglenoid and neoglenoid in the asymmetric cohort. All glenoids were further divided into volumes at depths of 0 to 2.5 mm and 2.5 to 5 mm. Average bone density was measured in Hounsfield units. Bone voids or cysts were included to quantify regional porosity as the fraction of void volume to total glenoid volume. Results For the symmetric cohort, there were no significant differences in bone density between quadrants at either depth ( P ≥ .089). For the asymmetric cohort, bone density was significantly higher in the posterior quadrants compared with the anterior quadrants ( P < .001), especially posteroinferiorly ( P ≤ .007) at both depths. In addition, the neoglenoid had significantly higher density and lower void fraction compared with the paleoglenoid ( P < .001). There were also significant differences in void fraction between quadrants for both cohorts, at both depths ( P ≤ .004). Conclusions This study demonstrates that osteoarthritic glenoids with symmetric erosion have uniform subarticular bone density. However, asymmetric (B2) erosion patterns have potentially important regional variations in bone density and porosity, with the densest bone with the least porosity found posteroinferiorly or in the neoglenoid region. Background: Distal humeral hemiarthroplasty is a treatment option for distal humeral fractures, nonunions, and avascular necrosis. The biomechanical effects, however, have not been reported. The purpose of this in vitro study was to quantify the effects of hemiarthroplasty and implant size on elbow joint kinematics. Methods: Eight fresh-frozen cadaveric arms were mounted in an in vitro motion simulator. An electromagnetic tracking system quantified elbow kinematics. A custom distal humeral stem was implanted by use of navigation, and 3 humeral articular spools were evaluated: optimally sized, undersized, and oversized. Statistical analysis was performed with repeated-measures analysis of variance. Results: Distal humeral hemiarthroplasty altered elbow kinematics, regardless of implant size. In the valgus position, the optimally sized implant resulted in a mean increase in valgus angulation of 3° ± 1° (P = .003) as compared with the osteotomy control. In the varus position, the optimal and undersized implants both resulted in significant increases in varus angulation: 3° ± 1° (P = .01) and 3° ± 1° (P = .001), respectively. The undersized implant had the greatest alteration in kinematics, whereas the oversized implant best reproduced native elbow kinematics. Conclusion: This study showed a small but significant alteration in elbow joint kinematics with placement of a distal humeral hemiarthroplasty implant, regardless of implant size. This could be due to errors in implant positioning and/or differences in the shape of the humeral implant relative to the native elbow. Background: The optimal articular shape for distal humeral hemiarthroplasty has not been defined because of a paucity of data quantifying the morphology of the normal distal humerus. This study defines the osseous anatomy and anatomic variability of the distal humerus using 3-dimensional imaging techniques. Methods: Three-dimensional surface models were created from computed tomography scans obtained from 50 unpaired human cadaveric elbows. Geometric centers of the capitellum and the trochlear groove defined the anatomic flexion-extension axis. A coordinate system was created, and the distal humerus was sectioned into 100 slices along this axis. The C line was defined as the line of best fit connecting the geometric centers of each of the slices. Results: The anatomic flexion-extension axis of the distal humerus was found to be an average of 1° ± 1° from the C line (range, 0°-3°) in the coronal plane and 2° ± 1° (range, 0°-7°) in the transverse plane. The average trochlear width was 22 ± 3 mm, and the average trochlear height was 18 ± 2 mm. The mean width of the capitellum was 17 ± 2 mm; the height was 23 ± 2 mm (P < .001). Conclusions: The difference in the capitellum width and height demonstrates that the capitellum is ellipsoid, not spherical. A data bank of humeral dimensions may be used for the development of future distal humeral hemiarthroplasty implants. A more anatomic implant may optimize kinematics and maximize contact area, thus minimizing contact stresses on the native ulna and radius. [Copyright &y& Elsevier]

Background: There is no validated method to determine the correct diameter of a radial head implant when the radial head is too comminuted to function as a template or during revision surgery when the radial head has been previously excised. The purpose of this study was to determine if ipsilateral capitellar dimensions could be used to predict the diameter of the radial head; and hence to assist with implant selection. Methods: Computer tomography scans of 50 normal elbows were used to generate 3D models. Measurements of the radial head included the maximum (Dmax) and minimum (Dmin) outer diameters and the maximum (Dishmax) and minimum (Dishmin) articular dish diameters. Measurements of the humerus included the width of the capitellum (CAPwidth), and the width from the lateral aspect of the capitellum to the lateral trochlear ridge (CAP-TROCHridge). Relationships were determined with Pearson bivariate coefficients. Results: The mean radial head dimensions were Dmax = 24.7 ± 2.3 mm, Dmin = 23.5 ± 2.3 mm, Dishmax = 18.2 ± 1.9 mm and Dishmin = 16.8 ± 1.7 mm. The mean capitellar measurements were CAPwidth (18.4 ± 1.4 mm) and CAP-TROCHridge (23.0 ± 2.1 mm). The most significant correlations were found between Dmax and CAP-TROCHridge (R = .90, P < .001) and Dmin and CAP-TROCHridge (R = .90, P < .001). Discussion: Radiologic measurements of the capitellum are useful in the estimation of native radial head diameter. The CAP-TROCHridge measurement was very strongly correlated with both the maximum and minimum diameters of the radial head. This suggests that CAP-TROCHridge may be useful to accurately predict the native radial head diameter. Introduction: Little information exists on radial head implant diameter sizing methods. When the native head is absent due to extensive comminution or previous excision, the lesser sigmoid notch may be a useful landmark for sizing. We evaluated the reliability of native radial head measurements, and the lesser sigmoid notch, as landmarks for radial head implant diameter sizing. Methods: We examined 27 fresh frozen ulnae and their corresponding radial heads. The maximum, minimum, and dish diameters of the radial heads were measured. A radial head implant diameter was selected based on the congruency of the trial implants with the radius of curvature of the lesser sigmoid notch. Intraobserver and interobserver reliability for all measurements and implant selection were assessed using intraclass correlation coefficients (ICC). Correlations between the native radial head measurements and the selected radial head implant diameter or the lesser sigmoid notch radius of curvature were assessed using the Pearson correlation coefficient (PCC). Results: Radial head diameter measurements demonstrated strong to excellent intraobserver (ICC ? 0.75) and interobserver reliability (ICC ? 0.82). The lesser sigmoid notch sizing method showed poor interobserver reliability (ICC = 0.34). Only a moderate correlation was found between the native radial head and the lesser sigmoid notch (PCC ? 0.80) or the selected radial head implant size (PCC ? 0.59). Conclusion: Radial head diameter measurements showed excellent reliability, suggesting that the excised radial head, when available, should be used to select the implant diameter. The reliability of using the lesser sigmoid notch for sizing the diameter of radial head implants was only moderate, suggesting this is an unreliable landmark for implant diameter sizing. Background: This biomechanical study evaluated the effects of 3 remplissage techniques on shoulder stability and motion in a Hill-Sachs (HS) instability model. Materials and methods: Cadaveric forequarters were tested on an active shoulder simulator. Three remplissage techniques were performed for 15% and 30% HS defects. Testing conditions included intact and 15% and 30% HS defects, and the 3 remplissage techniques: T1, anchors in the defect valley; T2, anchors in humeral head rim; and T3, anchors in valley with medial suture placement. Outcomes included stability, internal–external rotation range of motion (IE-ROM), and joint stiffness. Results: All remplissage techniques improved shoulder stability. In 15% HS defects tested in adduction, T3 significantly reduced IE-ROM (P = .037), whereas T1 and T2 did also (mean IE-ROM reductions: T1, 14°; T2, 11°; T3, 21°), but not to significance (P ? .088). In abduction, no significant reductions in IE-ROM occurred (P ? .060). In 30% HS defects tested in adduction (mean reduction IE-ROM: T1, 11°; T2, 19°; T3, 28°) and abduction (mean reduction: T1, 9°; T2, 15°; T3, 21°), all techniques significantly reduced IE-ROM (P ? .046). All techniques increased joint stiffness from 100% to 320% beyond the Bankart repair alone. A significant increase in joint stiffness was observed for T3 compared with the 30% HS group (P = .004), whereas T2 trended toward an increase (P = .078). There was no significant increase in joint stiffness with T1 (P = .249). Conclusions: All remplissage techniques enhanced shoulder stability, restricted ROM, and increased joint stiffness. No significant differences were found between anchors placed in the valley (T1) vs those placed in the humeral head rim (T2). Introduction: Glenohumeral instability with glenoid bone loss is commonly treated with the Latarjet procedure. The procedure involves transfer of the coracoid and conjoint tendon, which is thought to provide a stabilizing sling effect; however, its significance is unknown. This study evaluated the effects of the Latarjet procedure, with and without conjoint tendon loading, on shoulder stability and range of motion (ROM). Materials and methods: A custom simulator was used to evaluate anterior shoulder stability and ROM in 8 cadaveric shoulders. Testing conditions included intact, 30% glenoid defect, and Latarjet with and without conjoint loading. Unloaded and 10-N loaded states were tested in adduction and 90° abduction. Outcome variables included dislocation, stiffness (neutral and 60° external rotation), and internal–external rotational ROM. Results: All 30% defects dislocated in abduction external rotation. The loaded Latarjet prevented dislocation in all specimens, whereas the unloaded Latarjet stabilized 6 of 8 specimens. In abduction external rotation, there were no significant differences in stiffness between loaded and unloaded transfers (P = .176). In adduction, there were no significant differences between the intact and the loaded Latarjet (P ? .228); however, in neutral rotation, the unloaded Latarjet (P = .015) and the 30% defects (P = .011) were significantly less stiff. Rotational ROM in abduction was significantly reduced with the loaded Latarjet (P = .014) compared with unloaded Latarjet, and no differences were found in adduction. Conclusions: These findings indicate that glenohumeral stability is improved, but not fully restored to intact, with conjoint tendon loading. Background: The management of engaging Hill-Sachs defects (HSD) is controversial. The purpose of this study was to biomechanically compare 3 treatment strategies. Materials and methods: Eight specimens were tested on a shoulder simulator. The protocol involved testing 2 unrepaired HSD (30% and 45%), which were then treated with remplissage, humeral head allograft (HHA), and partial resurfacing arthroplasty (PRA). Stability (defect engagement and glenohumeral stiffness) and range of motion (ROM) were measured. Results: All 30% and 45% HSDs engaged and dislocated. Remplissage and HHA effectively prevented engagement in all specimens; however, 62% of PRA engaged. No repair exhibited stiffness significantly greater than intact, but 30% and 45% remplissage produced a 74% and 207% increase, respectively, and were significantly greater than the unrepaired states (P ≤ .047). Stiffness results for HHA and PRA closely matched those of intact. In adduction, remplissage reduced internal-external ROM compared with both defects (P ≤ .01), but only 30% remplissage caused a significant decrease compared with intact (P = .049). In abduction, all repairs reduced ROM compared with HSD (P ≤ .04), but none compared with intact (P ≥ 0.05). In extension, remplissage had significantly less ROM than either HHA or PRA (P ≤ .02). Conclusion: All procedures improved stability; however, unlike remplissage, results from HHA and PRA closely resembled intact. Remplissage (30% and 45%) improved stability and eliminated engagement but caused reductions in ROM. HHA and PRA re-established intact ROM, but PRA could not fully prevent engagement. Hypothesis: Aseptic loosening is one of the leading causes of failure in total elbow arthroplasty. Incorrect implant positioning and alignment in other joints such as the knee have been found to lead to excessive loading and wear. Although similar alignment difficulties exist in the elbow, the effect of implant malalignment on wear-inducing loads is not yet known. This in vitro study determined the effect of anterior malpositioning and varus-valgus and internal-external malrotations on humeral stem loading in total elbow arthroplasty. Methods and materials: Computer-navigated linked elbow arthroplasty was conducted in 8 cadaveric elbows. A modular, instrumented humeral component was used to measure loading during simulated elbow motion while the position of the ulna relative to the humerus was recorded. Results: Loading increased for all malaligned implant positions tested (P < .05). During simulation of implant malpositioning, combinations of internal-external and varus-valgus malrotations that tended to preserve the line of action of the elbow flexors had lower loads than combinations that did not. Discussion: This in vitro study showed that loading does increase after humeral component malalignment; however, further studies are required to determine the long-term effects on polyethylene wear and component loosening. Background: When repair of comminuted coronoid fractures is not possible, prosthetic replacement may restore elbow stability. The purpose of this biomechanical study was to determine whether a coronoid implant with an extended tip would improve elbow stability compared with an anatomic prosthesis in the setting of collateral ligament insufficiency. Materials and methods: Passive elbow extension was performed in 7 cadaveric arms in the varus and valgus positions and active and passive extension in the horizontal position by use of an elbow motion simulator. Varus-valgus laxity of the ulna relative to the humerus was quantified with a tracking system with a native coronoid, a 40% coronoid deficiency, an anatomic prosthesis, and an extended prosthesis, with the collateral ligaments sectioned and repaired. Results: Laxity significantly increased after a 40% coronoid deficiency with both repaired and sectioned collateral ligaments (P ≤ .01). With the ligaments repaired, there was no significant difference in laxity between the native coronoid, the anatomic implant, or the extended implant. Ligament sectioning alone produced severe instability, with a mean laxity of 42.75° ± 11.54° (P < .01). With insufficient ligaments, the anatomic prosthesis produced no change in laxity compared with the native coronoid, whereas the extended implant significantly reduced laxity by 21.56° ± 17.70° (P = .02). Conclusions: An anatomic coronoid implant with ligament repair restores stability to the coronoid-deficient elbow to intact levels. In the setting of ligament insufficiency, an extended implant improves stability relative to an anatomic implant, but the elbow remains significantly less stable than an intact elbow. Introduction: Although interest in capitellar arthroplasty is increasing, the morphology of the capitellum has not been fully characterized. Our purpose was to quantify the anthropometric features of the capitellum with an interest in arthroplasty design. We hypothesized that the shape is more complex than originally believed, and cannot be accurately modeled as a spherical structure. Methods: Fifty cadaveric human elbows underwent helical computer tomography scans. After reconstruction and establishment of a coordinate system for the distal humerus, circle-fits were applied to each of the 1-mm-thick slices. Sagittal radii of curvature were calculated every 10° of flexion around each circle (0-130° of flexion). A single transverse radius was calculated at 60° of flexion. The surface of the capitellum was described by sagittal and transverse radii of curvature and the footprint by height and width. These pairs of parameters were correlated to determine their strength of association. Results: The average height was 23.2 ± 2.9 mm (range, 18.3-29.5), while the average width was 13.9 ± 2.3 (range, 9-19). The sagittal radius of curvature was 11.6 ± 1.4 mm (range, 8.7-14.8), and the transverse radius was 14.0 ± 3.0 mm (range, 9.6-20.9). Correlations of height and width and sagittal and transverse radii were significant (R = .547, .705) (P < .01). Sagittal and transverse radii and height and width were significantly different (P < .001 for each pair). Conclusion: The capitellum does not have a spherical surface or a circular footprint. There is substantial variability in the relationship between the height and width, and between the surface radii, that may be difficult to replicate with an off-the-shelf implant. Background: Coronal shear fractures of the distal humerus can include some or all of the cartilaginous and bony surface. Fixation is preferred, but severe comminution, nonunion, and avascular necrosis may mandate excision. The amount of distal humerus that is safe to excise is unknown. This study examined the effect of excision of the capitellum and trochlea on elbow kinematics and stability with intact collateral ligaments. Methods: Eight cadaveric arms were mounted in an upper extremity joint testing system. Electromagnetic receivers on the radius and ulna enabled quantification of ulnohumeral and radiocapitellar kinematics. The distal humeral articular surface was sequentially excised to replicate clinically relevant coronal shear fractures, leaving the collateral ligaments undisturbed. The arms underwent simulated active flexion in vertical and valgus-loaded positions, and passive forearm rotation in the vertical position. Results: In the vertical position, sequential excision of the articular surface increased valgus angulation during active flexion (P ≤ .04), and excision of the entire articular surface increased ulnar external rotation compared to the intact elbow (P ≤ .02). In the valgus position, excisions involving the trochlea increased valgus angulation for active flexion (P ≤ .04). The radial head moved distal, posterior, and medial on the capitellum with some or all of the trochlea excised (P ≤ .02). Discussion: While the capitellum alone does not contribute to elbow stability, the trochlea has an important role. Excision of the trochlea resulted in multiplanar instability of the ulnohumeral and radiocapitellar joints. Therefore, excision of an irreparable capitellum fracture may be considered if collateral ligaments are intact, while excision of some or all of the trochlea may not. [Copyright &y& Elsevier]

Background: Glenoid replacement is challenging due to the difficult joint exposure and visualization of anatomical reference landmarks. Improper positioning of the glenoid component or inadequate correction of the retroversion using currently available instrumentation may lead to early failure. The objective of this study was to evaluate a computer-assisted technique to achieve a more accurate placement of the glenoid component compared to traditional techniques. Methods: Sixteen paired cadaveric shoulders were randomized to either traditional or computer-assisted glenoid implantation. Preoperative planning consisting of CT scanning with 3-dimensional image modeling of the shoulder specimens and intraoperative tracking with real-time feedback provided to the surgeon was employed in the computer-assisted group. A validated, previously published, standardized protocol for tracking the orientation of the glenoid in space using 3 glenoid surface landmarks was employed. All phases of glenoid implantation (initial guide pin insertion, reaming, drilling of the peg holes, and final component implantation) were tracked and recorded by the computer. A post-implantation CT scan was performed in both groups to compare how accurately the implants were placed. Results: The computer-assisted technique was more accurate in achieving the correct version during all phases of glenoid implantation and as measured on the post-implantation CT scan (P < .05). The largest errors with traditional glenoid implantation were observed during drilling and, more so, during reaming. The trend was to overly retrovert the glenoid. Purpose: This study determined whether elbow stability could be restored with open reduction and internal fixation (ORIF) of type II coronoid fractures and evaluated the role of collateral ligament repair. Methods: Passive varus and valgus and simulated active vertical motion were performed using an in vitro elbow motion simulator. Varus/valgus angle and internal/external rotation were measured with the coronoid intact, with 50% removed, and after ORIF. Testing was performed with the collateral ligaments detached and repaired. Results: Vertical: stability was normal when both the lateral collateral ligament (LCL) and medial collateral ligament (MCL) were repaired, irrespective of the coronoid state. Kinematics were altered with a repaired LCL, incompetent MCL, and type II coronoid fracture (P < .05). Varus: LCL repair restored coronal stability but did not restore internal rotation (P < .05). Conclusions: These findings suggest that repair of type II coronoid fractures and injured collateral ligaments should be performed where possible. Over-tensioning the LCL, in the setting of MCL and coronoid deficiency, may contribute to instability. Level of evidence: Basic science study. [Copyright &y& Elsevier]

Summary: A better understanding of the morphology of the proximal ulna should permit the development of ulnar component designs which have an improved fit to the native bone, thus leading to more accurate implant positioning. Computed-tomography (CT) scans of 31 cadaveric proximal ulnae were analyzed using computer aided design software to determine the shape of the medullary canal relative to the articular surface. The diameter, curvature, cross-sectional centroid position, and coronal and sagittal angulation of the ulnar canal were all calculated with respect to the center of the greater sigmoid notch. Posterior and lateral offsets increased distally from the articulation center, and the mean diameter of the canal was larger in males than in females (P < .05). The average valgus angulation was 8.0 ± 4.0° for males and 7.2 ± 3.1° for females (P = .6). Longer stemmed ulnar implants may require a modular design to meet anatomic constraints during implant positioning. [Copyright &y& Elsevier]

The purpose of this study was to describe the extramedullary humeral morphology in paired humeri to determine whether geometric differences exist from side to side in the same individual. The anatomic characteristics of 28 paired, dry cadaveric humeri were measured by use of an electromagnetic tracking system. Of the characteristics examined, only the humeral head height was significantly different between right and left humeri in the same individual (P < .005). Most of the characteristics had excellent intra-specimen repeatability. In conclusion, there are few significant differences between contralateral humeral anatomic characteristics. Therefore, the uninjured contralateral humerus can provide a reasonable approximation to the native geometry of the fractured humerus and should be a reliable model for measuring parameters related to implant geometry and optimal positioning during hemiarthroplasty for the treatment of proximal humeral fractures. Introduction: Capitellar hemiarthroplasty is proposed as a reconstructive option for isolated capitellar deficiency, but there is limited data on its effect on elbow biomechanics. This study assessed the effect of capitellar excision with and without replacement on elbow kinematics and stability, and evaluated 2 different implant surface shapes. Materials and methods: Ten cadaveric arms were tested with an upper extremity joint simulator. Each arm underwent computer tomography scanning for implant sizing and computer-assisted implantation. Kinematic data were obtained using an electromagnetic tracking system during elbow flexion, with the arm oriented in the valgus, varus, and vertical positions. Implants were placed through an extended lateral epicondylar osteotomy using computer-assisted techniques. A repeated-measures design compared 2 implants (anatomical and spherical) to the native capitellum control and capitellar excision states. Outcomes were maximum varus-valgus laxity and rotation of the ulna with respect to the humerus. Results: Excision of the capitellum increased the varus-valgus laxity up to 3.1° in active elbow flexion, with the forearm in pronation but not in supination. Both capitellar implant designs maintained normal varus-valgus laxity in both active and passive elbow flexion. Excision of the capitellum increased external ulnar rotation during active flexion in the vertical and valgus positions up to 1.5°, while both implants restored normal ulnar rotation. The kinematics and stability of the elbows were similar for both implant designs. Conclusion: The capitellum appears to have a role as a valgus and external rotational stabilizer of the ulnohumeral joint. A computer-assisted technique was developed for treatment of 4-part proximal humeral fractures via a hemiarthroplasty and tuberosity fixation. This was compared with a standard traditional method in 7 pairs of cadaveric shoulders. The computer-assisted technique used preoperative computed tomography data and computer simulations of anatomic characteristics of the contralateral humerus. This allowed accurate anatomic reconstruction by use of an electromagnetic tracking system and real-time intraoperative feedback. Various anatomic measurements were used to quantify the accuracy of the reconstruction. The differences between the intact and reconstructed values were improved with the computer-assisted technique for 5 of 7 characteristics. However, this was statistically significant only for humeral head offset (P < .05). With further investigation and refinement, this technique should allow for a more anatomic reconstruction of the proximal humerus, potentially resulting in improved patient outcomes. The technique may also prove to be a valuable resource for the laboratory training of inexperienced surgical trainees. [Copyright &y& Elsevier]
