Purpose: Although the existing literature has explored the link between cognitive functioning and speech recognition in noise, the specific role of fluid intelligence still needs to be studied. Given the established association between working memory capacity (WMC) and fluid intelligence and the predictive power of WMC for speech recognition in noise, we aimed to elucidate the mediating role of fluid intelligence., Method: We used data from the n200 study, a longitudinal investigation into aging, hearing ability, and cognitive functioning. We analyzed two age-matched samples: participants with hearing aids and a group with normal hearing. WMC was assessed using the Reading Span task, and fluid intelligence was measured with Raven's Progressive Matrices. Speech recognition in noise was evaluated using Hagerman sentences presented to target 80% speech-reception thresholds in four-talker babble. Data were analyzed using mediation analysis to examine fluid intelligence as a mediator between WMC and speech recognition in noise., Results: We found a partial mediating effect of fluid intelligence on the relationship between WMC and speech recognition in noise, and that hearing status did not moderate this effect. In other words, WMC and fluid intelligence were related, and fluid intelligence partially explained the influence of WMC on speech recognition in noise., Conclusions: This study shows the importance of fluid intelligence in speech recognition in noise, regardless of hearing status. Future research should use other advanced statistical techniques and explore various speech recognition tests and background maskers to deepen our understanding of the interplay between WMC and fluid intelligence in speech recognition.

Purpose The purpose of this study was to conceptualize the subtle balancing act between language input and prediction (cognitive priming of future input) to achieve understanding of communicated content. When understanding fails, reconstructive postdiction is initiated. Three memory systems play important roles: working memory (WM), episodic long-term memory (ELTM), and semantic long-term memory (SLTM). The axiom of the Ease of Language Understanding (ELU) model is that explicit WM resources are invoked by a mismatch between language input-in the form of rapid automatic multimodal binding of phonology-and multimodal phonological and lexical representations in SLTM. However, if there is a match between rapid automatic multimodal binding of phonology output and SLTM/ELTM representations, language processing continues rapidly and implicitly. Method and Results In our first ELU approach, we focused on experimental manipulations of signal processing in hearing aids and background noise to cause a mismatch with LTM representations; both resulted in increased dependence on WM. Our second-and main approach relevant for this review article-focuses on the relative effects of age-related hearing loss on the three memory systems. According to the ELU, WM is predicted to be frequently occupied with reconstruction of what was actually heard, resulting in a relative disuse of phonological/lexical representations in the ELTM and SLTM systems. The prediction and results do not depend on test modality per se but rather on the particular memory system. This will be further discussed. Conclusions Related to the literature on ELTM decline as precursors of dementia and the fact that the risk for Alzheimer's disease increases substantially over time due to hearing loss, there is a possibility that lowered ELTM due to hearing loss and disuse may be part of the causal chain linking hearing loss and dementia. Future ELU research will focus on this possibility.

Purpose Hearing loss is associated with changes in brain volume in regions supporting auditory and cognitive processing. The purpose of this study was to determine whether there is a systematic association between hearing ability and brain volume in cross-sectional data from a large nonclinical cohort of middle-aged adults available from the UK Biobank Resource ( http://www.ukbiobank.ac.uk ). Method We performed a set of regression analyses to determine the association between speech reception threshold in noise (SRTn) and global brain volume as well as predefined regions of interest (ROIs) based on T1-weighted structural images, controlling for hearing-related comorbidities and cognition as well as demographic factors. In a 2nd set of analyses, we additionally controlled for hearing aid (HA) use. We predicted statistically significant associations globally and in ROIs including auditory and cognitive processing regions, possibly modulated by HA use. Results Whole-brain gray matter volume was significantly lower for individuals with poorer SRTn. Furthermore, the volume of 9 predicted ROIs including both auditory and cognitive processing regions was lower for individuals with poorer SRTn. The greatest percentage difference (-0.57%) in ROI volume relating to a 1 SD worsening of SRTn was found in the left superior temporal gyrus. HA use did not substantially modulate the pattern of association between brain volume and SRTn. Conclusions In a large middle-aged nonclinical population, poorer hearing ability is associated with lower brain volume globally as well as in cortical and subcortical regions involved in auditory and cognitive processing, but there was no conclusive evidence that this effect is moderated by HA use. This pattern of results supports the notion that poor hearing leads to reduced volume in brain regions recruited during speech understanding under challenging conditions. Purpose: The visual Text Reception Threshold (TRT) test (Zekveld et al., 2007) has been designed to assess modality-general factors relevant for speech perception in noise. In the last decade, the test has been adopted in audiology labs worldwide. The 1st aim of this study was to examine which factors best predict interindividual differences in the TRT. Second, we aimed to assess the relationships between the TRT and the speech reception thresholds (SRTs) estimated in various conditions., Method: First, we reviewed studies reporting relationships between the TRT and the auditory and/or cognitive factors and formulated specific hypotheses regarding the TRT predictors. These hypotheses were tested using a prediction model applied to a rich data set of 180 hearing aid users. In separate association models, we tested the relationships between the TRT and the various SRTs and subjective hearing difficulties, while taking into account potential confounding variables., Results: The results of the prediction model indicate that the TRT is predicted by the ability to fill in missing words in incomplete sentences, by lexical access speed, and by working memory capacity. Furthermore, in line with previous studies, a moderate association between higher age, poorer pure-tone hearing acuity, and poorer TRTs was observed. Better TRTs were associated with better SRTs for the correct perception of 50% of Hagerman matrix sentences in a 4-talker babble, as well as with better subjective ratings of speech perception. Age and pure-tone hearing thresholds significantly confounded these associations. The associations of the TRT with SRTs estimated in other conditions and with subjective qualities of hearing were not statistically significant when adjusting for age and pure-tone average., Conclusions: We conclude that the abilities tapped into by the TRT test include processes relevant for speeded lexical decision making when completing partly masked sentences and that these processes require working memory capacity. Furthermore, the TRT is associated with the SRT of hearing aid users as estimated in a challenging condition that includes informational masking and with experienced difficulties with speech perception in daily-life conditions. The current results underline the value of using the TRT test in studies involving speech perception and aid in the interpretation of findings acquired using the test.

Purpose: We sought to examine the contribution of visual cues in audiovisual identification of consonants and vowels-in terms of isolation points (the shortest time required for correct identification of a speech stimulus), accuracy, and cognitive demands-in listeners with hearing impairment using hearing aids., Method: The study comprised 199 participants with hearing impairment (mean age = 61.1 years) with bilateral, symmetrical, mild-to-severe sensorineural hearing loss. Gated Swedish consonants and vowels were presented aurally and audiovisually to participants. Linear amplification was adjusted for each participant to assure audibility. The reading span test was used to measure participants' working memory capacity., Results: Audiovisual presentation resulted in shortened isolation points and improved accuracy for consonants and vowels relative to auditory-only presentation. This benefit was more evident for consonants than vowels. In addition, correlations and subsequent analyses revealed that listeners with higher scores on the reading span test identified both consonants and vowels earlier in auditory-only presentation, but only vowels (not consonants) in audiovisual presentation., Conclusion: Consonants and vowels differed in terms of the benefits afforded from their associative visual cues, as indicated by the degree of audiovisual benefit and reduction in cognitive demands linked to the identification of consonants and vowels presented audiovisually.

Purpose: Seeing the talker's face improves speech understanding in noise, possibly releasing resources for cognitive processing. We investigated whether it improves free recall of spoken two-digit numbers., Method: Twenty younger adults with normal hearing and 24 older adults with hearing loss listened to and subsequently recalled lists of 13 two-digit numbers, with alternating male and female talkers. Lists were presented in quiet as well as in stationary and speech-like noise at a signal-to-noise ratio giving approximately 90% intelligibility. Amplification compensated for loss of audibility., Results: Seeing the talker's face improved free recall performance for the younger but not the older group. Poorer performance in background noise was contingent on individual differences in working memory capacity. The effect of seeing the talker's face did not differ in quiet and noise., Conclusions: We have argued that the absence of an effect of seeing the talker's face for older adults with hearing loss may be due to modulation of audiovisual integration mechanisms caused by an interaction between task demands and participant characteristics. In particular, we suggest that executive task demands and interindividual executive skills may play a key role in determining the benefit of seeing the talker's face during a speech-based cognitive task.

Purpose: The purpose of the present study was to evaluate the new Cognitive Spare Capacity Test (CSCT), which measures aspects of working memory capacity for heard speech in the audiovisual and auditory-only modalities of presentation., Method: In Experiment 1, 20 young adults with normal hearing performed the CSCT and an independent battery of cognitive tests. In the CSCT, they listened to and recalled 2-digit numbers according to instructions inducing executive processing at 2 different memory loads. In Experiment 2, 10 participants performed a less executively demanding free recall task using the same stimuli., Results: CSCT performance demonstrated an effect of memory load and was associated with independent measures of executive function and inference making but not with general working memory capacity. Audiovisual presentation was associated with lower CSCT scores but higher free recall performance scores., Conclusions: CSCT is an executively challenging test of the ability to process heard speech. It captures cognitive aspects of listening related to sentence comprehension that are quantitatively and qualitatively different from working memory capacity. Visual information provided in the audiovisual modality of presentation can hinder executive processing in working memory of nondegraded speech material.

Purpose: In this research, the authors aimed to increase the analogy between Text Reception Threshold (TRT; Zekveld, George, Kramer, Goverts, & Houtgast, 2007) and Speech Reception Threshold (SRT; Plomp & Mimpen, 1979) and to examine the TRT's value in estimating cognitive abilities that are important for speech comprehension in noise., Method: The authors administered 5 TRT versions, SRT tests in stationary (SRT(STAT)) and modulated (SRT(MOD)) noise, and 2 cognitive tests: a reading span (RSpan) test for working memory capacity and a letter-digit substitution test for information-processing speed. Fifty-five adults with normal hearing (18-78 years, M = 44 years) participated. The authors examined mutual associations of the tests and their predictive value for the SRTs with correlation and linear regression analyses., Results: SRTs and TRTs were well associated, also when controlling for age. Correlations for the SRT(STAT) were generally lower than for the SRT(MOD.) The cognitive tests were correlated to the SRTs only when age was not controlled for. Age and the TRTs were the only significant predictors of SRT(MOD). SRT(STAT) was predicted by level of education and some of the TRT versions., Conclusions: TRTs and SRTs are robustly associated, nearly independent of age. The association between SRTs and RSpan is largely age dependent. The TRT test and the RSpan test measure different nonauditory components of linguistic processing relevant for speech perception in noise.

Purpose: To investigate whether working memory capacity (WMC) modulates the effects of to-be-ignored speech on the memory of materials conveyed by to-be-attended speech., Method: Two tasks (reading span, Daneman & Carpenter, 1980; Rönnberg et al., 2008; and size-comparison span, Sörqvist, Ljungberg, & Ljung, 2010) were used to measure individual differences in WMC. Episodic long-term memory of spoken discourse was measured by requesting participants to listen to stories masked either by normal speech or by a rotated version of that speech and to subsequently answer questions on the content of the stories., Results: Normal speech impaired performance on the episodic long-term memory test, and both WMC tasks were negatively related to this effect, indicating that individuals with high WMC are less susceptible to disruption. Moreover, further analyses revealed that size-comparison span (a task that requires resolution of semantic confusion by inhibition processes) is a stronger predictor of the effect than is reading span., Conclusions: Cognitive control processes support listening in adverse conditions. In particular, inhibition processes acting to resolve semantic confusion seem to underlie the relationship between WMC and susceptibility to distraction from masking speech.

Purpose: To test the relationship between degree of hearing loss and different memory systems in hearing aid users., Method: Structural equation modeling (SEM) was used to study the relationship between auditory and visual acuity and different cognitive and memory functions in an age-hetereogenous subsample of 160 hearing aid users without dementia, drawn from the Swedish prospective cohort aging study known as Betula (L.-G. Nilsson et al., 1997)., Results: Hearing loss was selectively and negatively related to episodic and semantic long-term memory (LTM) but not short-term memory (STM) performance. This held true for both ears, even when age was accounted for. Visual acuity alone, or in combination with auditory acuity, did not contribute to any acceptable SEM solution., Conclusions: The overall relationships between hearing loss and memory systems were predicted by the ease of language understanding model (J. Rönnberg, 2003), but the exact mechanisms of episodic memory decline in hearing aid users (i.e., mismatch/disuse, attentional resources, or information degradation) remain open for further experiments. The hearing aid industry should strive to design signal processing algorithms that are cognition friendly.

The present case study of MM, who acquired both sign language and spoken language in her early preschool years-and then reached the normal milestones of development in each language--revealed that her speechreading expertise is associated with cognitive functions such as high working memory capacity and phonological skills. Her cognitive profile is in accord with previous case studies of extremely good speechreading skill. MM's enhanced right prefrontal/frontal cerebral blood flow activation during speechreading seems to be indicative of efficient visual scanning, but it is also possibly due to her strategy for phonological decoding of visual speech.
