Abstract

IL-12 is a novel cytokine with interesting features regarding its potential usefulness in peripheral blood stem cell transplantation and leukemia immunotherapy. We used cryopreserved leukemia cells of 18 patients with acute myelogenous (n= 14) or lymphocytic (n= 4) leukemia to investigate the effect of IL-12, alone or in combination with IL-2, on the cytolytic activity of NK cells against human leukemia targets. Effector cells were peripheral blood mononuclear cells from healthy donors which were depleted from CD3+ T cells by immunomagnetic separation. CD3-negative effector cells (mainly CD56+ NK cells) were treated for 24 h with various concentrations of IL-2 (100 U/ml to 1000 U/ml) and IL-12 (1 U/ml to 100 U/ml). Cytotoxicity was measured in a 4 h 51Cr-release assay. Whereas a two-fold enhancement of cytotoxic activity was observed after incubation with optimal doses of IL-2 or IL-12, the combination of both cytokines (500 U/ml IL-2, 100 U/ml IL-12) increased the lytic activity more than six-fold. This effect was accompanied by increased expression of cellular adhesion molecules (CD2, CD18) and CD25 on CD56+ effector cells. Of 18 leukemias investigated, five were completely resistant to lysis by effector cells activated with IL-2 or IL-12 alone. In three of these five cases, however, high cytolytic activity was observed after coincubation with IL-2 and IL-12. In comparison to allogeneic NK cells, autologous cells of three patients in remission demonstrated significantly lower cytotoxic activity. No killing of nonmalignant cells (PHA blasts) by allogeneic NK cells was observed. Our data demonstrate that IL-12 can enhance or even induce MHC-unrestricted cytotoxicity of IL-2-activated allogeneic natural killer cells. Since IL-12 has also been shown to have stem-cell mobilizing capacities, it could be used for the recruitment of both stem cells and antileukemic effector cells in the context of peripheral blood stem cell transplantation.

33. Alloreactivity: the Janus-face of hematopoietic stem cell transplantation

Abstract

Differences in major and minor histocompatibility antigens between donor and recipient trigger powerful graft-versus-host reactions after allogeneic hematopoietic stem cell transplantation (HSCT). The clinical effects of alloreactivity present a Janus-face: detrimental graft-versus-host disease increases non-relapse mortality, beneficial graft-versus-malignancy may cure the recipient. The ultimate consequences on long-term outcome remain a matter of debate. We hypothesized that increasing donor-recipient antigen matching would decrease the negative effects, while preserving antitumor alloreactivity. We analyzed retrospectively a predefined cohort of 32 838 such patients and compared it to 59 692 patients with autologous HSCT as reference group. We found a significant and systematic decrease in non-relapse mortality with decreasing phenotypic and genotypic antigen disparity, paralleled by a stepwise increase in overall and relapse-free survival (Spearman correlation coefficients of cumulative excess event rates at 5 years 0.964; P<0.00; respectively 0.976; P<0.00). We observed this systematic stepwise effect in all main disease and disease-stage categories. The results suggest that detrimental effects of alloreactivity are additive with each step of mismatching; the beneficial effects remain preserved. Hence, if there is a choice, the best match should be donor of choice. The data support an intensified search for predictive genomic and environmental factors of ‘no-graft-versus-host disease’.

Abstract

Highly polymorphic tandemly repetitive DNA sequences provide powerful genetic markers for the identification of individuals by restriction fragment length polymorphisms (RFLP) even in close relatives. Over a three-year period, 61 consecutive patients from a single institution undergoing allogeneic bone marrow transplantation (BMT) for various hematological diseases were grafted with unmanipulated marrow and followed for the development of hematopoietic chimerism. Three synthetic oligonucleotide probes homologous to the so-called minisatellite or variable number of tandem repeat (VNTR) sequences were evaluated in the clinical setting of BMT for their usefulness: (i) to document marrow engraftment or rejection; (ii) to elucidate the kinetics of mixed chimerism; and (iii) in providing a sensitive tool for early detection of relapse. In addition, in patients with CML karyotyping and analysis of bcr/abl gene rearrangement was performed. Using this panel of three oligonucleotide probes, informative markers specific for donor or recipient RFLP could be demonstrated in all cases. Engraftment could be documented in all patients surviving beyond day +14 after BMT. Mixed chimerism was detected in 14% of the patients in the early phase (day +14 to day +78) after BMT but only one patient turned out to become a long-term stable mixed chimera. These results support the hypothesis that lymphocytes of recipient origin surviving the conditioning regimen may considerably contribute to mixed chimerism early after BMT. Long-term stable mixed chimerism is a rare event after BMT with unmanipulated marrow. Simultaneous analysis of chimerism after BMT by VNTR-RFLP, karyotyping, and detection of bcr/abl rearrangement in patients with CML showed corresponding results in nine out of 12 patients. In three patients either one of the methods failed to detect residual recipient cells in the early phase after BMT. Therefore different methods for assessment of mixed chimerism seem to complement rather than to exclude each other. Eleven patients who all exhibited complete chimerism early after BMT relapsed from their underlying disease. In seven of these patients grafted for acute leukemia, analysis of DNA-RFLP had been performed shortly before clinical relapse (30-86 days) and failed to herald relapse. As the sensitivity for the detection of the minor cell population by analysis of DNA-RFLPs is approximately 1%, these data may indicate that relapse of acute leukemia after BMT is characterized by a sudden increase in the percentage of recipient blast cells not detectable even by frequent RFLP analyses.
