Background and Purpose: In Spain there is a lack of population data that specifically compare hospitalization for systolic and diastolic heart failure (HF). We assessed clinical characteristics, in-hospital mortality and 30-day cardiovascular readmission rates differentiating by HF type., Methods: We conducted a retrospective observational study of patients discharged with the principal diagnosis of HF from The National Health System' acute hospital during 2016-2019, distinguishing between systolic and diastolic HF. The source of the data was the Minimum Basic Data Set. The risk-standardized in-hospital mortality ratio and risk-standardized 30-day cardiovascular readmission ratio were calculated using multilevel risk adjustment models., Results: The 190,200 episodes of HF were selected. Of these, 163,727 (86.1%) were classified as diastolic HF and were characterized by older age, higher proportion of women, diabetes mellitus, dementia and renal failure than those with systolic HF. In the multilevel risk adjustment models, diastolic HF was a protective factor for both in-hospital mortality (odds ratio [OR]: 0.79; 95% confidence interval [CI]: 0.75-0.83; P<.001) and 30-day cardiovascular readmission versus systolic HF (OR: 0.93; 95% CI: 0.88-0.97; P=.002)., Conclusions: In Spain, between 2016 and 2019, hospitalization episodes for HF were mostly due to diastolic HF. According to the multilevel risk adjustment models, diastolic HF compared to systolic HF was a protective factor for both in-hospital mortality and 30-day cardiovascular readmission., (Copyright © 2023 Elsevier España, S.L.U. Background: Patients with heart failure (HF) undergoing noncardiac surgical procedures is rising worldwide. This study was aiming at analyzing the impact of heart failure (HF) on the outcomes (mortality, complications, readmissions, and length of stay) of elderly patients undergoing elective major noncardiac surgical procedures in Spain., Methods: A retrospective observational study of patients undergoing noncardiac surgery was conducted. The Minimum Basic Data Set (MBDS) was used to collect information about the demographic characteristics of patients discharged from hospitals of the Spanish National Health System (SNHS), variables related to patients' medical conditions and surgeries conducted during the episode., Results: A total of 996,986 selected procedures in the discharge record were identified in the period 2007-2015. HF was recorded as a secondary diagnosis in 22,367 discharges (2.24%). The mean age of patients was 76.6±7.27 years, with a difference in patients without and with HF: 76.5 (95% CI: 76.47-76.50) vs 82.8 (95% CI: 82.71-82.90). The number of selected surgical procedures increased by 13.2% (117,487 in 2015 vs. 103,744 in 2007), and the proportion of presence HF as a comorbidity increased by 24.4% (2.4% in 2015 v 1.9% in 2007). The proportion of women was higher in the HF group: 53.2% (95% CI: 53.18-53.22) vs 64.3% (95% CI: 64.20-64.44), with a longer average length of stay: 7.9 (95% CI: 7.9-7.9) vs 14.9 (95% CI 14.7-15.0) days, and women had a higher proportion of comorbidities. HF was found to be an independent risk factor in-hospital mortality in the multilevel risk adjustment model (OR=2.3; 95% CI: 2.2-2.4)., Conclusions: Patients with HF undergoing any of the selected surgical procedures are older; there was women predominance and there is also an important burden of comorbidities than patients without HF undergoing these surgical procedures. HF in the selected procedures, increasing in-hospital mortality, mean length of stay, and the occurrence of adverse events in the Spanish population. Background and Objectives: Pseudomonas aeruginosa bacteraemia is associated with a very high mortality, conditioned by comorbidity, source, severity of the episode and lack of adequate treatment. The aim of the study is to know the mortality and prognostic factors of bacteraemia by P.aeruginosa in our hospital., Patients and Methods: We conducted a retrospective study of P.aeruginosa bacteraemia detected between 2009 and 2014. Epidemiological, clinical and microbiological characteristics were described. A risk factor analysis for mortality was performed., Results: We analysed 110 episodes of bacteraemia, which was more frequent in men of advanced age and with a history of hospitalisation, comorbidity and immunosuppression. Most of the bacteraemias were secondary (mainly of respiratory or urinary source) and led to a significant clinical deterioration. The presence of antibiotic resistance was very high, with 27.3% of multiresistant strains. Empirical treatment was adequate in 60.0% and 92.3% for definite treatment. Overall mortality was 37.3% and attributable mortality was 29.1%. The most important prognostic factors were Charlson index ≥3, history of haematologic malignancy, neutropenia and previous use of corticosteroids, source of bacteraemia, Pitt index ≥4, renal insufficiency, adequate definite treatment, empiric treatment with piperacillin/tazobactam in severe episodes and focus control., Conclusion: P.aeruginosa bacteraemia is associated with a very high mortality, possibly more related to previous comorbidity and severity of the episode than to the treatment chosen. Background and Objective: Shunt-related infection is a major complication. The effort to face up to this problem has focused on therapy, analysis of risk factors and primary prevention. The aim of this study is to identify risk factors for cerebrospinal fluid shunt infection., Material and Method: We retrospectively reviewed all patients who underwent shunt placement at our hospital during the period of study. Procedures followed for less than 90 days were excluded. One hundred and nineteen procedures were included in the final analysis., Results: Infection rate per procedure was 8.4%. Previous cerebrospinal fluid infection, previous shunt and complete substitution of the shunt were identified as risk factors for infection in the univariate analysis. However, a multivariate analysis confirmed that previous cerebrospinal fluid infection was the only independent risk factor for shunt-related infection (p=0.011)., Conclusion: Previous cerebrospinal fluid infection is an independent risk factor for shunt-related infection. Other factors such as age, gender, etiology of hydrocephalus, previous shunt or complete substitution of the shunt were not associated with increased risk of infection., (Copyright © 2010 Elsevier España, S.L. Background and Objective: The aim of this report is to describe and evaluate the results of a secondary prevention program in osteoporotic fractures in 70 Spanish hospitals. This project was started to improve the treatment of these fractures, with the inhospital implementation of software collecting epidemiological data on patients with osteoporotic fractures., Patients and Methods: A prospective case study was conducted between March 2003 and March 2006 involving a consecutive sample of 24,534 patients with established fragility fracture seen in 70 hospitals. The study exclusion criterion was secondary osteoporotic fracture. The project consists of implementing software in hospitals, designed to collect such epidemiological data among these patients. Based on this information and on the published scientific evidence, the software automatically generates drug therapy recommendations and non pharmacologic measures of prevention to be included in the discharge report., Results: The number of patients with osteoporotic fractures included in the program totalled 24,534 subjects. Of these patients, only 22.7% were receiving treatment for osteoporosis. Implementation of the program made it possible to discharge 19,033 patients (77.6%) with prescribed treatment, but in only 15,153 patients (61.8%) the treatment was correct., Conclusions: The GIOS project has proved effective in increasing the identification of fragility fractures and improve the percentage of secondary prevention measures, both pharmacological and non pharmacological, for patients with these fractures., (Copyright © 2010 Elsevier España, S.L. Background and Objective: To assess the prevalence of low glomerular filtration rate (GFR < 60 ml/min/1.73 m2) in general population of subjects aged 60 years or older of Spain, and the related factors., Patients and Method: Analysis of PREV-ICTUS, a population-based study carried out in a sample of subjects aged 60 years or more, randomly selected and stratified by Autonomic Communities according to the census of inhabitants and the habitat in each Community. Demographic data, cardiovascular risk factors and diseases were collected. GFR was calculated using the MDRD (Modification of Diet in Renal Disease) equation., Results: In 6,799 subjects (age 71.9 years-old; 53.6% women; 72.9% with arterial hypertension [HT]; 27.0% with diabetes mellitus [DM]; 24.3% with cardiovascular disease), 25.9% had low GFR (95% confidence interval, 24.8-26.9). The prevalence increased linearly, from 16.5% in subjects aged 60-64 years to 46.5% in those aged 85 or more (p < 0.001), and was higher in women (36.1% vs 14.1% in men, p < 0.001). In the multivariate analysis, low GFR was independently related to advanced age (odds ratio [OR] between 1.30 y 4.20), female gender (OR = 5.82), HT older than 7 years (OR = 1.23), uric acid (OR = 1.52 for each increment of 1 mg/dl) and cardiovascular disease (OR = 1.68). The association with DM did not reach statistical significance. In a model without cardiovascular disease, related factors remained the same, increasing the significance of HT older than 7 years (OR = 1.31) and of DM (OR = 1.19)., Conclusion: One out or 4 subjects of this sample showed low GFR. The variables directly associated with low GFR were advanced age, female gender, HT of longer evolution, cardiovascular disease, increased uric acid, and, in one model, DM.

Background and Objective: We aimed to estimate the prevalence of the Metabolic Syndrome (MS) in rural and urban areas of the Province of Segovia, Spain, according to the ATPIII criteria (National Cholesterol Education Program's Adults Treatment Panel III report) modified., Subjects and Method: Cross-sectional design. Randomized and representative sample of 809 individuals (46% males) aged 35-74 years. Residents in urban and rural areas of the Province of Segovia (Spain). Period of study from January 2000 to January 2003., Results: The age/sex standardized prevalence of the MS was 17%; 15.7% in males and 18.1% in females. No significant differences in MS prevalence between rural and urban areas were found. The most frequent combination of individual components of MS was abdominal obesity, hyperglycemia and arterial hypertension in males and females. MS was associated with age and obesity in an adjusted logistic regression model. In a second model, abdominal obesity was more common in those individuals with a BMI over 30 kg/m2, secondary education level, age over 45 years, and in female residents in rural areas., Conclusions: The sex/age adjusted prevalence was lower than that reported in other studies using the ATPIII criteria in Spain. Abdominal obesity was the most frequent single component of MS in females whereas it was arterial hypertension in males.

Background and Objective: Stroke is a major cause of morbidity and mortality in hypertensive population. The aim of this study is to estimate the 10-year risk of a first stroke in Spanish hypertensive population using the Framingham scale., Patients and Method: Cross-sectional multicenter study in primary care environment, on hypertensive patients aged 55 or more (10 consecutive patients each physician). Blood pressure (BP) was measured according to standardized rules. Stroke and coronary risk were estimated using the Framingham scale., Results: 16,129 patients (mean age 67.7 years; 57.1% women; 30.4% with diabetes mellitus; 26.6% with previous history of cardiovascular disease) were analyzed. 20.3% showed electrocardiographic left ventricular hypertrophy (more prevalent in males, diabetics, with systolic BP > or = 140 mmHg; p < 0.001). The goal of BP control was reached in 30.1% of non-diabetic patients and only in 5.9% of diabetics. The estimated 10-year risk of a first stroke (standard deviation) was estimated to be 19.9% (16.8), and the coronary risk 19.2% (14.0). While males had an estimated risk of coronary disease above the stroke risk, women had an estimated risk of a first stroke greater than the estimated risk of coronary disease., Conclusions: The estimated risk of stroke among Spanish hypertensive population is high, and the main risk factors (age, systolic BP, diabetes and left ventricular hypertrophy) tend to aggregate. In women estimated risk of stroke in greater than estimated coronary risk. The risk of stroke must be taken into account in the assessment and treatment of hypertensive patients.

Background: Hospital mortality related to infections acquired in the hospital setting has not been well studied in Spain. We carried out a study of seven hospitals in order to assess and quantify the problem., Methods: The study period included three months of observations (between November 1, 1989 and January 31, 1990), and data pertaining to all deaths of patients hospitalized for a minimum of 24 hours were collected. The number of people admitted within the study period was 16,025, and the number of deaths registered and included in our study was 488 (3%). The data were obtained from the patient's medical history one week after death as well as from the hospital physicians on the case. In order to quantify the interobserver variability derived from the classification criteria, the simple kappa index was calculated and averaged to form an ordinal scale., Results: 216 (44.3%) of 488 deaths included in our study had no infection, 138 (28%) had an infection no-hospital-associated, and 134 (27%) had nosocomial infection (50-10%--"causally related to death", 59-12%--"contributing to death", and 25-5%--"not related to death"). The lower respiratory tract infections, bacteremias and surgical wound infections were the most related to cause of death. Staphylococcus aureus was the pathogen most frequently associated with the infections found at the time of death., Conclusions: Those patients admitted with non-fatal diseases made up the greatest percentage (39.9%) of deaths from nosocomial infections. The infection was considered the direct cause of death in 18.8% of these cases, although the differences found had no statistical significance.

The epidemiological surveillance provides opportunities to know the magnitude and determinants of nosocomial infection and permits, at the same time, the planning, implementation and evaluation of prevention and treatment activities in order to approach the rates of infection in the hospital, as near as possible, to the irreducible minimum. We show data collected by surveillance system Guadalajara General Hospital from 1982 to 1987. In order to analyze general trends, seasonality, accidental variations and endemic levels, we have used statistical methods as linear regression, chi 2, equality proportions and built of an endemic channel with confidence intervals of 95%. We have observed a decreasing trend, locating endemic levels around to 4% of monthly accumulated incidence. There is a possible seasonal influence concurring with holidays periods and someone accidental variation over that we expected, it was related with a deteriorated situation about medical care.
