Osteochondroma of condyle is an exophytic lesion that arises from the cortex of bone and is capped with cartilage. It is one of the most common benign tumors of the bone. It usually appears in endochondral bones. In the mandible, it occurs most frequently at the condyle or coronoid processes. There has been ambiguity in understanding the nature of osteochondroma, whether it is neoplastic or just a reactive phenomenon. Here, we are presenting a case report of a 43-year-old male patient with a chief complaint of deviation of jaw toward the right side and progressive facial asymmetry for the past 1 year. There was an asymmetric prognathism of the lower jaw with ipsilateral open bite and contralateral crossbite. The patient was planned for surgery under general anesthesia. The tumor was dissected from its soft-tissue attachments, and then, complete excision of the tumor was done. Histopathological examination of the lesion showed a proliferation of bony and hyalinized cartilage-like tissues. Two-year postoperative follow-up was uneventful, and no second surgical intervention was required. Computed tomography scan after 2-year follow-up revealed no reoccurrence of lesion, and the patient's facial profile was significantly improved.

Intermaxillary fixation (IMF) is an integral technique utilized by maxillofacial surgeons to appropriately reduce and relate maxillary and mandibular fractures to both one another and the facial skeleton. This case report reviews the management of a comminuted mandibular fracture including inoperable bilateral condylar fractures that precluded the use of convention IMF techniques necessitating an alternative technique. This was achieved in the form of modified bony plates extending intraorally. Postoperative review showed favorable results with occlusion and range of motion comparable to the premorbid function and no unforeseen complications.

Background: The management of trauma has evolved greatly over the past many years. Various bone plating systems have been developed to provide stable fixation of mandibular fractures. The introduction of the locking plate/screw system has offered certain advantages over the conventional plating systems. This system does not require intimate adaptation of the miniplates to the underlying bone and has greater stability. This study evaluates the efficacy of locking miniplate/screw system in the treatment of mandibular fractures without maxillomandibular fixation. Materials and Methods: This was a prospective study analyzing 20 patients with undisplaced or minimally displaced mandibular fractures, who reported to Department of Oral and Maxillofacial Surgery, Government Dental College and Research Institute, Bangalore. The selected cases were treated by open reduction and internal fixation using the 2.0 mm locking plate/screw system. Results: Open reduction and internal fixation with the 2.0 mm locking plate/screw system were achieved in all the 20 cases with satisfactory stability of the fracture fragments. The system was found to be reliable and effective intraoperatively. Only two complications were noted in the study. Conclusion: The locking miniplate system was found to be reliable and effective in management of mandibular fractures without postoperative intermaxillary fixation, however further studies with more sample size is required.

Introduction: Various methods have been described for intermaxillary fixation (IMF) for treatment of faciomaxillary injuries. Many studies have been described to evaluate the efficacy of different methods. Hanger plate method has not been commonly used. The aim of the present study was to compare the advantages and disadvantages of this method over Erich arch bar in mandibular fracture. Materials and Methods: Sixty patients of only mandibular fracture presenting to trauma center requiring open reduction and internal fixation under general anesthesia were randomly allocated to Group A and Group B comprising thirty patients in each. Group A included patients who received IMF with Erich arch bar. Group B included patients who received IMF with hanger plate method. The two groups were compared for time duration of intermaxillary procedure, total duration of surgery, oral hygiene score, postoperative occlusion, and complications. Results: The average time of intermaxillary procedure, total duration of surgery, and wire prick injuries were more in Group A. Oral hygiene score was significantly better in Group B. Postoperative occlusion was comparable between the two groups. There was screw loosening in four patients in Group B, but none had tooth root injury. The cost of material for IMF was more in Group B. Conclusion: IMF with hanger plate method is more safe and efficacious compared to Erich arch bar in the treatment of mandibular fractures.

Aim: This study aimed to evaluate the efficacy of intermaxillary fixation (IMF) screws and modified arch bar. Materials and Methods: This study is a randomized clinical trial in which all participants were divided into two groups of ten in each group and designated as Group A and Group B. In Group A, IMF was achieved by the use of four to six 2×8mm stainless steel IMF screws. In Group B, IMF was achieved by modified screw arch bar. Results: In the present study, a total of twenty patients were analyzed. The average working time for Group A and Group B was 16 min and 29 min, respectively. Oral hygiene scores through modified Turesky Gilmore plaque index were calculated at immediate postoperative period and after 15 days, 30 days, and 45 days. Maximum hygiene was maintained in IMF screw group than modified arch bar group, but maximum stability was observed in the modified arch bar group than IMF screw group. Conclusion: This study emphasizes the use of IMF screws as a quick and easy method than modified arch bar. Oral hygiene maintenance was comparatively better in patients with IMF screws than those with modified arch bar. Modified arch bar was significantly stable when compared with IMF screws; therefore, for patients who require long-term IMF, modified arch bars can be a viable option, but the perforation in the original arch bar may lead to the weakening of the arch bar, and therefore the prefabricated modified arch bar would be a better option.

Context: This questionnaire survey was conducted to ascertain if the practice of the routine use of postoperative intermaxillary fixation (IMF) in mandibular trauma (of the dentate segments only) was based on evidence available in the literature. Settings and Design: This study was designed as a questionnaire survey that would be conducted among surgeons operating on mandibular fractures in the state of Gujarat. Materials and Methods: A typed questionnaire was sent to oral maxillofacial surgeons and plastic surgeons of Gujarat state for their feedback by post. Approval of the ethical committee of the university was obtained. All the feedback forms received back were included for this survey which included 25 oral and maxillofacial surgeons and 25 plastic surgeons. Results: Although majority of the surgeons use open reduction and internal fixation (ORIF), 25% of the surgeons still prefer using only IMF as the sole modality of treatment for the said group of mandibular fractures. According to our survey, the majority of surgeons use IMF routinely in the postoperative setting even after using ORIF. Occlusion seems to be the critical factor among 72% of surgeons in deciding on the use of IMF as an adjunct after ORIF. Seventy-eight percent of surgeons vary their period of IMF based on the site of fracture. Conclusion: The routine use of postoperative IMF in cases of mandibular fractures seems to find favor among surgeons despite lack of sound scientific evidence supporting its use. There is also no evidence to suggest that if IMF is not used, it would have deleterious effects on occlusion or otherwise.
